The technique of the arthroscopic total meniscectomy of discoid meniscus is very difficult. Ikeuchi(1982) reported a technique of arthroscopic total meniscectomy of the lateral discoid meniscus by removal of 2/3 of the anterior part of the meniscus first and then by morselization. The purpose of this paper is to report a technique of arthroscopic total meniscectomy of the lateral discoid meniscus as a whole. During the period from October 1982 to October 1984, we had 12 experiences of arthroscopic total meniscectomy of the discoid meniscus and found good results, that is, little postoperative pain, short period of hospitalization and early restoration of joint motion without physical therapy. Complications are 3 cases of symptomless slight lateral instability and 4 cases of hemarthrosis.

This report describes a patient with lipoma arborescens presenting as hemarthrosis of a knee with intraosseous lesions. A 44-year old woman complained of pain and swelling of left knee. Arthrocentesis resulted in bloody effusion. Magnetic resonance images showed villous projecting frondlike fatty soft tissue proliferation in the suprapatellar pouch and ovoid intraosseous lesions in proximal tibia. The soft tissue mass was removed through arthroscopic synovectomy and the biopsy of the mass confirmed the diagnosis. The intraosseous lesions were not considered as a cause of hemarthrosis. We failed to find the exact cause of hemarthrosis. The patient remained asymptomatic after removal of the mass for 10 months. As far as we know, this is the first report of hemarthrosis associated with lipoma arborescens in korea.

Spontaneous hemarthrosis of the knee after a total knee arthroplasty is a rare complication, and thereis no report is available in Korea. We report a case that was cured with an open synovectomy in the spontaneous hemarthrosis, which developed 10 months after the total knee arthroplasty for degenerative osteoarthritis in a 60-year-old woman.

We studied 16 cases intracapsular pressure in displaced fracture of femoral neck(Garden stage III, IV), the pressures were measured range between 12-62 mmHg(mean, 23.4 mmHg). Aspirated blood amount from hip joint did not exceed 3cc, this amount not correlate with intracaspsular pressure. Of 10 cases examined scintimetry, 2 had incressed uptake after aspiraton, it is suggested that early decompression of hemarthrosis in femur neck fracture regaredless of displacement may be considered.

OBJECTIVE: To assess the clinical outcome of chemical synovectomy with rifampicin in hemophilic arthropathy by using the World Federation of Hemophilia (WFH) scoring system and plain radiograph. METHODS: We performed rifampicin synovectomy (RS) on 30 joints of 28 hemophilic patients diagnosed as hemophilic arthropathy stage I–III (based on Fernandez-Palazzi clinical classification). Clinical status (bleeding frequency, pain, joint physical status) and radiological staging were evaluated as parts of the WFH scoring system before and 1 year after RS. The patients were divided into two groups by the Arnold-Hilgartner scale of the initial X-ray as stage 3 or less for the low-stage group (n=17) and over 3 for the high-stage group (n=13). RESULTS: Total WFH joint physical scores were reduced after injection, and the number of bleeding episodes and pain showed especially significant improvement. For other subscores of the WFH joint physical score, only swelling, range of motion, and crepitus showed statistically significant improvement. According to the severity of the radiologic finding, the WFH joint physical score of both the low-stage and high-stage groups showed significant improvement. In the radiological aspect, the low-stage group, without joint space narrowing at the initial plain radiograph, showed no further aggravation after injection. However, in the high-stage group, radiology found aggravation regardless of the procedure. CONCLUSION: It is suggested that chemical synovectomy with rifampicin may prevent hemarthrosis and improve clinical symptoms. Especially in the early stage of arthropathy without joint-space narrowing, it seems to have an additional benefit that delays radiological aggravation and preserves joint status.

PURPOSE: To evaluate the relationship between flexion-extension gap discrepancy and clinical results in total knee arthroplasty (TKA). MATERIALS AND METHODS: We evaluated the clinical results of 38 knees, which were performed TKA between March 2003 and June 2003, and followed up for at least 2 years. We divided all cases by the difference of medial and lateral joint gap in flexion and extension. Group A was no flexion-extension gap difference(17 cases) and group B was different gap(21 cases). We evaluated Knee Score, Function Score, range of motion and radiologic results RESULTS: The difference of group B was less than 2mm. In Knee Score and function score, Group A improved from preoperative 50.1 and 45.0 to the last follow up to 95.6 and 95.9, and those of Group B improved from 48.2 and 45.2 to 96.9 and 91.4. There was no statistical difference between two groups(p>0.05). Flexion angle of Group A increased from preoperative 108.2 degree to the last follow up 115.3 degree. That of Group B increased from preoperative 101.9 degree to the last follow up 113.6degree. But, there was no statistical difference between two groups(p>0.05). 2 cases of recurrent hemarthrosis were in Group B. CONCLUSION: We got good clinical results by flexion-extension gap difference within 2 mm. But long term followed up was needed.

Pseudoaneurysm is extremely rare complication after ankle arthroscopy with standard anteromedial and anterolateral portals. We report a case of a pseudoaneurysm of the anterior tibial artery detected at 3 months after ankle arthroscopy in a 16-year-old male. He had sustained painful swelling of his right ankle after the arthroscopic surgery, and referred to our hospital with an MRI checked postoperatively. We failed to make the diagnosis of pseudoaneurysm with the postoperative MRI, thus the patient underwent another arthroscopy which revealed massive hemarthrosis within the joint. The diagnosis was confirmed with an angiography, and the vascular lesion was ligated.
