The effects of gypenosides (Gps) on the levels of fasting plasma glucose (FPG),fasting in- sulin (INS),total cholesterol(TC),triglyceride (TG),serum lipid peroxide (LPO) value and serum super- oxide dismutase (SOD) activity in STZ-induced diabetic rats were studied.The levels of FPG、INS、TC、TG and serum LPO value were decreased while serum SOD activity was increased significantly in diabetic rats after four weeks of Gps treatment in comparison with those in control rats.These results suggest that Gps have the effects of decreasing the levels of blood sugar,lipid and possibly scavenging free radicals.
2.Pharmacokinetic interaction of pioglitazone hydrochloride and atorvastatin calcium in Beagle dogs.

The object of this study is to investigate the pharmacokinetic interaction of pioglitazone hydrochloride and atorvastatin calcium in healthy adult Beagle dogs following single and multiple oral dose administration. A randomized, cross-over study was conducted with nine healthy adult Beagle dogs assigned to three groups. Each group was arranged to take atorvastatin calcium (A), pioglitazone hydrochloride (B), atorvastatin calcium and pioglitazone hydrochloride (C) orally in the first period, to take B, C, A in the second period, and to take C, A, B in the third period for 6 days respectively. The blood samples were collected at the first and the sixth day after the administration, plasma drug concentrations were determined by LC-MS/MS, a one-week wash-out period was needed between each period. The pharmacokinetic parameters of drug combination group and the drug alone group were calculated by statistical moment method, calculation of C(max) and AUC(0-t) was done by using 90% confidence interval method of the bioequivalence and bioavailability degree module DAS 3.2.1 software statistics. Compared with the separate administration, the main pharmacokinetic parameters (C(max) and AUC(0-t)) of joint use of pioglitazone hydrochloride and atorvastatin calcium within 90% confidence intervals for bioequivalence statistics were unqualified, the mean t(max) with standard deviation used paired Wilcoxon test resulted P > 0.05. There was no significant difference within t1/2, CL(int), MRT, V/F. Pioglitazone hydrochloride and atorvastatin calcium had pharmacokinetic interaction in healthy adult Beagle dogs.
3.Pharmacokinetics and tissue distribution of irinotecan hydrochloride nanoparticles.

To investigate the pharmacokinetics of irinotecan hydrochloride (CPT-11) in rats and the tissue distribution of CPT-11 in mice after injection of irinotecan hydrochloride nanoparticles (CPT-11 NPs) via tail veins, separately, a LC-MS/MS method was established to determine the concentration of CPT-11 in whole blood of rats and in different tissues of mice. The pharmacokinetics and tissue distribution of CPT-11 were compared after the intravenous injection of CPT-11 NPs and CPT-11 solution. Compared with CPT-11 solution, the elimination half-life of CPT-11 was prolonged from 2.28 h to 3.95 h after the intravenous injection of CPT-11 NPs, and its AUC was 1.47 times than that of CPT-11 solution. After the injection of CPT-11 NPs in mice, the concentrations of CPT-11 loaded in CPT-11 NPs were significantly higher in the whole blood, colon and lungs than those in CPT-11 solution, but lower in the spleen, liver, kidney and heart, but the least in brain. CPT-11 NPs could improve CPT-11 's AUC, and help CPT-11 to reach long circulation activity.
4.Effect of Orem self-care theory on the perioperative stress reaction and allomeric function recovery in elderly patients with fracture of proximal femur

Objective To investigate the impact of Orem self-care mode on perioperative stress response and overall functional recovery of elderly patients with proximal femoral fractures nursing. Methods The elderly patients with proximal femoral fractures of the Second Hospital of Jilin University were divided into experimental group and control group, and the control group patients were given conventional nursing, when the experimental group patients were given Orem self-care nursing on the basis of conventional nursing. Contrastively analysis were implemented in the perioperative anxiety, depression and other negative stress reaction change, functional exercise compliance, hip joint function and daily life function change of the two group patients.Results When the patients were out of hospital, the anxiety (14.9±6.2) and depression (13.3±4.8)scores of the experimental group patients were significantly lower than that in the control group (t=2.303, 3.347;P<0.05). 3 months after discharge, the good rates of hip joint were 66.7% in experimental group and 46.7% in control group, but the difference of two groups had no statistical significance (χ2=2.443, P>0.05). After 3 months discharge, the daily life function total score was (68.4±9.2)in experimental group and (62.2±8.4) in control group (t=2.726,P<0.01).Conclusions Orem self-care nursing mode can effectively improve the perioperative negative psychological stress reaction of the old patients with proximal femoral fractures, improve their behavior compliance and self-nursing ability, and it also has great significance on promoting the overall patients postoperative function recovery.
5.Fatal deterioration of delayed acute subdural hematoma after mild traumatic brain injury: two cases with brief review.

Both delayed posttraumatic intracerebral hemorrhage and epidural hematoma have been well described in the neurosurgical literatures. However, delayed posttraumatic acute subdural hematoma which happens more than a week with a rapid progress after mild traumatic brain injury and causes death of patient is rarely reported. We show two such cases and briefly review the literature and discuss the probable pathogenesis of their rapid progress.

6.A qualitative study on the suggestion of key nursing techniques and procedure optimization by medical staffs for patients with nuclear exposed in hospital

Objective:To understand the views and suggestions of medical staff on the key nursing techniques and norms of in-hospital treatment of patients with nuclear exposure, so as to provide reference for the continuous optimization of follow-up processes.Methods:From September to October 2021,purpose sampling was adopted to select doctors and nurses who engaged in medical emergency rescue of nuclear exposure-related work in two divisions of the Nuclear Accident Medical Emergency Center of the National Health Commission as the research subjects, using a combination of online and offline methods to carry out semi-structured interviews with 6 research subjects,using content analysis methods in descriptive research for data analysis.Results:A total of four themes were refined including the need for the establishment of nursing technology and process specification for nuclear accident emergency rescue specialty; the need for specialized training of nursing technology in nuclear accident emergency rescue; the need for the construction of specialized nursing team for nuclear accident emergency rescue; the need to strengthen the closeness of multi-team cooperation.Conclusions:The current in-hospital care technology and processes for nuclear exposed patients need to be further refined and standardized, and in the future, we need to establish a perfect in-hospital care technology and processes for nuclear exposed patients, and according to the corresponding technology and processes, strengthen nursing staff professional training and simulation training in nuclear accident emergency rescue, and establish a nuclear emergency rescue professional nursing team to promote the development of nuclear accident emergency rescue nursing specialists.
7.Application of painless management based on clinical pathway in elderly patients with femoral fractures

Objective To explore the influences of painless management based on clinical pathway on pain during perioperative period, postoperative rehabilitation and the nursing satisfaction in elderly patients with femoral fractures.Methods A total of 112 elderly patients with femoral fractures who underwent surgeries in the Second Hospital of Jilin University were recruited from February 2014 to December 2015. They were divided into observation group (56 cases, nursing care with the painless management mode based on clinical pathway) and control group ( 56 cases, routine nursing care) according to the method of random number table. We compared the situation of pain during perioperative period, postoperative rehabilitation and the nursing satisfaction of patients between two groups.Results The results of analysis of variance for repeated measurement revealed that the scores of visual analogue scale ( VAS) of each postoperative time point in observation group were lower than those in control group, scores of VAS between two groups decreased with time increasing, the scores of hip joint function of each postoperative time point in observation group were higher than those in control group, scores of hip joint function between two groups increased with time increasing with significant differences ( P<0.01) . The satisfaction with nursing care of patients in observation group ( 92. 86%) was higher than that in control group (78.57%) at discharge with a significant difference (χ2 =4. 667, P=0. 031). Conclusions The painless management based on clinical pathway can relieve the pain of during perioperative period in elderly patients with femoral fractures, promote the postoperative rehabilitation and improve patients′ satisfaction. It is worthy of popularization in clinical work.
8.Analysis of 19 cases undergoing reoperation for complications following esophagectomy.

OBJECTIVETo investigate the cause and the management of treatment and prevention of reoperation following esophagectomy.
METHODSClinical data of 946 cases with esophageal cancer undergoing esophagectomy from January 2000 to December 2012 by the same surgical team in the Beijing Cancer Hospital were retrospectively analyzed. Among them, 19 patients underwent reoperation after esophagectomy because of serious complications. Clinical features and treatment course of these 19 cases were summarized.
RESULTSThe indications and procedures of reoperation included thoracotomy for hemorrhage (n=4), diaphragmatic hernia repair (n=4), thoracic duct ligation for chylothorax (n= 4), re-suturing for incision dehiscence (n=4), re-laparotomy and re-thoracotomy for drainage of traumatic pancreatitis (n=1), re-laparotomy for intestinal obstruction (n=1), and tracheotomy for bilateral recurrent laryngeal nerve paralysis (n=1). All the 19 patients were successfully cured without perioperative deaths and further complications.
CONCLUSIONSThe indications of reoperation following esophagectomy include postoperative bleeding, diaphragmatic hernia, chylothorax and abdominal incision dehiscence.

Objective:
To explore the effects of human erythropoietin (hEPO) on healing related transforming growth factor β1 (TGF-β1)/Smad3 signal transduction pathway in acute wounds of rats.
Methods:
Seventy-two healthy Sprague Dawley rats were divided into normal saline control group, low dose group, middle dose group, and high dose group according to the random number table, with 18 rats in each group, after round acute wounds with diameter of 2.5 cm were inflicted on the back of rats. Rats in the 4 groups had debridement routinely. Wounds of rats in normal saline control group were covered by gauzes infiltrated with 1 mL normal saline, while wounds of rats in low dose group, middle dose group, and high dose group were respectively covered by gauze infiltrated with 1 mL hEPO in doses of 50, 100, and 150 U every day, and then the wounds were bandaged with 6 layers of dry gauze. Dressing change was performed once every day. On treatment day (TD) 3, 7, and 14, 6 rats from each group were taken for general observation and calculation of wound healing rate. Then the wound tissue samples were harvested after the rats were sacrificed for observation of expressions of CD31 and transforming growth factor β1 (TGF-β1) with immunohistochemical method. Protein expression of phosphorylated Smad3 of the wound tissue of 3 rats were detected by Western blotting. Data were processed with analysis of variance of factorial design, one-way analysis of variance, least-significant difference test, and Bonferroni correction.
Results:
(1) On TD 3, obvious exudation and scab were observed in the wounds of rats in the 4 groups. On TD 7, the wounds of rats in low dose group, middle dose group, and high dose group were reduced compared with those in normal saline control group. On TD 14, all wounds of rats in the 4 groups were healed. On TD 7, the wound healing rates of rats in middle dose group and high dose group were significantly higher than the rate in normal saline control group (
