1.Correlation among plasma fibrinogen, serum uric acid level and premature coronary heart disease

Objective: To explore the correlation among plasma fibrinogen (Fg), serum uric acid (SUA) level, severity of premature coronary heart disease (PCHD) and major adverse cardiovascular events (MACE).Methods:A total of 195 PCHD patients diagnosed by coronary angiography (CAG) were selected, including 72 cases with stable angina pectoris (SAP), 67 cases with unstable angina pectoris (UAP) and 56 cases with acute myocardial infarction (AMI).Another 75 healthy subjects were enrolled as normal control group.Plasma Fg and serum urea acid(SUA)levels were compared among four groups, and their relationship with coronary disease severity and incidence of MACE during six-month follow-up were analyzed.Then ROC curve was used to assess predictive value of plasma Fg and SUA levels for PCHD.Results: Compared with normal control group and SAP group, there were significant rise in levels of plasma Fg[(2.80±0.26) g/L, (2.85±0.32) g/L vs.(3.44±0.32) g/L, (4.25±0.35) g/L]and SUA[(306.56±15.55) μmol/L, (308.81±16.77) μmol/L vs.(349.48±15.59) μmol/L, (368.86±20.72) μmol/L]in UAP group and AMI group (P<0.05 or <0.01), and those of AMI group were significantly higher than those of UAP group (P<0.05 both);Spearman correlation analysis indicated that plasma Fg and SUA levels were significant positively correlated with Gensini score (r=0.750, 0.743, P<0.01 both);levels of plasma Fg and SUA in those suffering from MACE during follow-up were significantly higher than those of patients without MACE, P<0.05 or <0.01.Meanwhile, ROC curve suggested that plasma Fg and SUA possessed certain diagnostic value for PCHD[area under the curve (AUC) =0.867, 0.736].Conclusion: Plasma fibrinogen and serum uric acid detection can provide certain evidence for diagnosing PCHD and determining disease severity.

Establishment of an ideal partner is one of the most important subject of human monoclonalantibody researches.At Present,the human-mouse hybrid myeloma cell line has more advan-tages over plasmacytoma and lymphocyte cell lines.In this subject,heteromyeloma cell line HM—KMC1 was derived from fusion by using murine myeloma SP_(2/0)—AG_(14) with healthy adult pe-ripheral blood lymphocytes that was induced to be a 8—Ag resistant and HAT sensitive cell lineafter a serious cultivation in RPMI1640 complete medium containing 8—Ag,then fused with Blymphocytes from human spleen,tonsil,lymph nodes and peripheral blood,respectively,Six fu-sions have been done successfully and the fusion frequency was 41—129/10~7 cells.

A simple rapid and sensitive Dot-ELISA method was establishhed for identifying the human seminal stains by using anti-human semen monoclonal antibody (A10C6)labeled with horseradish Peroxidase of grayish color revealed a positive reaction. The results showed that the extract of human semen was positive. No Cross-reaction was observed when human tissues body fluids and animal seminal stains were tested.
4.Surgical treatment of the old thoracolumbar vertebral fractures

Objective To report the preliminary experiences in the treatment of old unstable thoracolumbar vertebral fractures by using anterior decompression, bone graft or titanium mesh and internal fixation with Z-plate system. Methods There were 23 cases (18 males and 5 females) with age range of 25-66 years (mean 41.7 years) as well as thoracolumbar vertebral fracture history for 1.2-28 years (mean 5.6 years). All cases complained of severe lower back pain, 17 of whom had slight neurological deficits (Frankel Grade D). Preoperative radiographic evaluation showed that all 23 cases had various degrees of kyphosis (25?-40?, averaged 34?), spinal cord compression and vertebral instability. Of 23 cases, 15 were treated with anterior decompression, full-thickness iliac crest autograft and internal fixation with Z-plate and the other 8 with titanium mesh instead of full-thickness iliac crest autograft. Results No severe complications happened during and after operation. The lower back pain and neurological deficits were improved or disappeared 2-3 months postoperatively. The average kyphotic correction degrees ranged from preoperative 34? to postoperative 12?. No patients' symptoms worsened during follow-up. In three cases with preoperative Frankel Grade D, the myodynamia recovered slightly but the Frankel Grade remained unchanged. The postoperative MRI examinations showed that vertebral canals were well decompressed. Conclusions Anterior decompression, autograft or titanium mesh and internal fixation with Z-plate are proved to be safe and effective techniques for clinical management of old thoracolumbar fractures, for they have advantages in effective decompression of neural structures, high healing rate of autograft and correction of kyphotic deformity.
5.Correlation between peri-procedural changes of serum inflammatory cytokines and early prognosis in patients with acute myocardial infarction undergoing percutaneous coronary intervention

Objective To investigate the variation and clinical significance of serum levels of inflammatory cyto-kines after percutaneous coronary intervention (PCI)treatment in acute myocardial infarction (AMI)patients. Methods 118 incipient AMI patients with successful underwent PCI (Defined as treatment group,blood samples were collected from pre-operation,12 h after operation,24 h after operation,48 h after operation and 90 d postop-erative follow-up period)and 52 AMI patients with diagnostic coronary angiography (CAG)(Defined as control group,blood samples were collected prior to CAG,12 h after CAG,24 h after CAG,48 h after CAG and 90 d fol-low-up period)were enrolled in this study.Serum levels of IL-6,IL-18,hs-CRP,TNF-αand MMP-9 were detec-ted in all the subjects by enzyme-linked immune sorbent assay(ELISA)and major adverse cardiac events(MACE) occurrence rate was analyzed in 90 days followed-up cases.Results No significant differences in baseline levels of IL-6,IL-18,hs-CRP,TNF-αand MMP-9 were found in the two study groups(P >0.05).No significant differences of levels of IL-6,IL-18,hs-CRP,TNF-αand MMP-9 were found after CAG in control group (P >0.05).The serum levels of IL-6,IL-18,hs-CRP and TNF-αafter PCI were significantly increased (P <0.01 )while no significant differences were found in level of MMP-9 (P >0.05)in PCI group.There were significant differences of levels of IL-6,IL-18,hs-CRP and TNF-αbetween MACE group and without MACE group after PCI.The multivariable lo-gistic analysis showed that IL-6,IL-18,hs-CRP and TNF-αwere risk factors of MACE after 90 days follow-up. Conclusion The concentrations of serum IL-6,IL-18,hs-CRP and TNF-αare significantly increased in AMI pa-tients treated with PCI.PCI operation may induce inflammatory reaction.High serum levels of peripheral inflamma-tory cytokines IL-6,IL-18,hs-CRP and TNF-αhave an important role in major adverse cardiac events(MACE)and short-term prognosis in the first AMI patients treated with successful primary PCI.
6.Effect of Huahongjiangxian decoction on hemorheology in patients with hypertension

Objective To observe the effect of Huahongjiangxiantang on hemorheology and plasma fibrinogen (FIB).Methods 100 patients with hypertension were randomly divided into two groups:the treatment group ( n=50) which were treated with Huahongjiangxian-tang,and the control group( n=50)were treated with Aspirin.Hemorheology and plasma fibrin were detected before and after treatment for 8 weeks.Results After 8 weeks treatment,there were significant depressions in whole blood viscosity(low shear rate),RBC deformability index and RBC aggregation index( P<0.01 ),and better than the control group( P <0.05 ).There were significant changes in whole blood viscosity ( high shear rate),plasma viscosity,RBC rigidity index and plasma fibrin ( P < 0.05 ).A number of indicators declined significantly in control group ( P < 0.05 );The rate of platelet aggregation to improve the situation in the control group was better than that of the treatment group,but there was no statistical significance.Conclusion Huahongjiangxiantang can effectively reduce blood viscosity in patients with hypertension and improve the body's red blood cell function and fibrinolysis,benefit to the prevention and treatment of atherosclerosis and reduce the occurrence of thrombotic events.
7.Pathological Changes in Gastric Polyp Tissues and Its Background Mucosa under Gastroscopy

Objective To investigate the relationship between the pathology of gastric polyp tissues and the precancerous and malignant transformations of its background mucosa by endoscopic biopsy. Methods From 2003 to 2008,1825 patients were diagnosed with gastric polyps in our hospital by gastroscopy and pathological examination. We classified the lesions and then analyzed the relationship between pathological changes of gastric polyp tissues and the precancerous and malignant transformations of its background mucosa by endoscopic biopsy. Results In this study,the detection rate of gastric polyps was 4.3% (1825/42 003). The lesions included fundic gland polyps (FGP,62.5%,1141/1825),hyperplasia polyps (HPP,32.2%,587/1825),adenoma (0.8%,14/1825),inflammatory polyps (IP,1.7%,31/1825),and coincident polyps (two types of polyps were found in one case,2.8%,52/1825). The rates of intestinal metaplasia (IM,5.3%,31/587) and dysplasia (6.8%,40/587) in HPP tissue were higher than those in FGP tissues (0.2%,2/1141 and 0.4%,4/1141,P
8.Arthroscopic acromioplasty for the treatment of impingement syndrome of shoulder

Objective To explore the methods and clinical results of the arthroscopic acromioplasty for the treatment of impingement syndrome of shoulder. Methods 15 cases with impingement syndrome of shoulder were studied. There were 9 males and 6 females. 10 right shoulders and 5 left ones were involved. The average age of patients was 47 years ranging from 32 to 54 years. 3 injuries were classified as degree I, 4 as Ⅱ, and 8 as Ⅲ according to Neer classification. All the patients had a chronic injury and painful history around shoulder. The positive arch pain test was presented in 12 cases and impingement test in 13 cases. 13 patients had been received MR examinations, and 5 arthrographies of the shoulder. The AP and the supraspinatus outlet projection of the X-rays were obtained before surgery. The procedure of arthroscopic acromioplasty consisted of 5 following steps: the first was arthroscopic subacromial inspection with posterior portal; the second was subacromial bursectomy with electro-shaver or burr; the third was release or resection of the coracoacromial ligament with electrocautery; the fourth was subacromial decompression with full-radius resector or burr; and the last was debridement of the soft tissue around the acromio-clavicular joint and resection of the the spurs. 4 patients underwent reparation of the rotator cuff simultaneously. Results All cases were available for follow-up. The average time was 15 months. 9.Oxaliplatin combined with vinorelbine in drug resistant non-small-cell lung cancer

Purpose: To observe the efficacy and the side effects of oxaliplatin( OXA) in combination with vinorelbine (VRB) in the treatment of drug resistant non-small-cell lung cancer( NSCLC) . Methodii: Thirty-five patients with drug resistant lung cancer were treated by OXA + VRB. OXA 100 mg/m2, VRB 25 mg/m , on d 1, which were given by bronchial arteries injection, and VRB 25mg/m2 was given by venous injection on d 8. One cycle was given every 3 weeks and total six cycles was given. Results: There was 1 patient who showed completed remission( CR), 14 patients showed partial remission( PR), 13 patients showed stable disease( SD) and 7 patients showed progress disease( PD) after the sixth cycle. The response rate was 42. 9%. The major toxicities were peripheral sensory neuropathy and leukopenia. Conclusions: The regimen of OXA plus VRB is effective on drug resistant and advanced NSCLC and the side effects are tolerable. There is higher response rate and lower neurotoxicity when the medicines are given by arterial injection than by venous injection.
10.The relationship of ACS patients complicated with diabetes with carotid artery IMT, serum IL-18,MMP-9 and plasma Fib

Objective To analyze the characteristic of some important clinical indicators, including carotid artery intima-middle thickness ( IMT) , peripheral blood Fib levels, serum IL-18 concentrations and serum MMP-9 con-centrations of acute coronary syndrome ( ACS) patients. On the other hand, to investigate the correlation among those indicators and major adverse cardiac events ( MACE) , and further to evaluate the value of those indicators in early prediction and prognosis judgement of ACS patients with diabetes( DM) in clinic. Methods This study en-rolled 120 ACS patients who had received coronary arteriography( CAG) examination. Among them, 60 cases had DM. Meanwhile, 60 cases who had DM were selected as DM group,60 cases of healthy people who received physi-cal health examination were selected as the control group. Carotid artery IMT was determined by color doppler ultra-sonography. The levels of Fib in peripheral blood were detected by solidification method. Immune turbidimetric method and ELISA were used to measure the concentrations of serum IL-18 and MMP-9 respectively. The CAG re-sults, complications and in-hospital mortality of ACS patients were also collected and analyzed. Results Carotid artery IMT, peripheral blood Fib levels, serum IL-18 concentrations and serum MMP-9 concentrations of ACS pa-tients were significantly increased compared with healthy control(P<0. 01). A comparison of those clinical indica-tors of ACS patients between with DM group and without 2 diabetes group showed a significant increase in ACS pa-tients with DM group. Carotid artery IMT, plasma Fib, serum IL-18, MMP-9 and Gensini score in ACS patients were positively correlated. The incidence of multi-vessel diseases of coronary artery was more frequent in ACS pa-tients with DM versus without DM ( 61. 7%) , while one or two vessel diseases of coronary artery was much more common in ACS patients without DM (61. 7%) . The Gensini score of ACS patients with DM group was much high-er than that of ACS patients without DM group ( P<0. 01 );severe and extremely severe lesions occurred more fre-quently in ACS patients with DM group (73. 3%) ,while mild-to-moderate lesions occurred more frequently in ACS patients without DM group (56. 7%) . Peripheral blood Fib levels, serum IL-18 and MMP-9 concentrations were significantly higher in ACS patients with MACEs compared to ACS patients without MACEs. Peripheral blood Fib levels, serum IL-18 concentrations and serum MMP-9 concentrations of ACS diabetes mellitus extended the down-ward trend with the treatment time after standardized treatment show time-effect relationship. Conclusion Carotid artery IMT, peripheral blood Fib levels, serum IL-18 concentration and serum MMP-9 concentrations have great po-tential to become effective clinical indicators, which could be used to quantitatively predict coronary atherosclero-sis,and further to determine the risk degree and short-term prognosis of ACS patients with DM.
