3.The Effect of the Experience of Diabetes Education on Knowledge, Self-Care Behavior and Glycosylated Hemoglobin in Type 2 Diabetic Patients.

PURPOSE: This study was to identify the effect of the experience of diabetes education on knowledge, self-care behavior and glycosylated hemoglobin (HbA1C). Further, this study was held to examine about patient's preferred methods of education and re-education frequency. METHODS: 166 type 2 diabetes patients from two hospitals in Incheon participated in this study. Data were analyzed by using descriptive analysis, t-test, ANOVA, Scheffe's test and multiple regression analysis. RESULTS: 72.3% patients needed re-education and the average interval of re-education was 8.53 months. Patients preferred education methods were lectures, practical training, and studying from pamphlet. Depending on the frequency of diabetes education, there were significant differences in the level of diabetes knowledge (F=10.88, p<.001) and self-care behaviors (F=4.59, p=.012), but there was not significant difference with HbA1C (F=1.53, p=.220). As to how much the diabetes education helped managing diabetes, there was a significant difference in the level of self-care behaviors (t=2.01, p=.049), but there were not significant differences in level of knowledge (t=1.10, p=.275) and HbA1C (t=-.33, p=.746). The experience of diabetes education was a significant factor which influenced patient's knowledge (t=3.93, p<.001) and self-care behaviors (t=2.21, p<.001). But HbA1C was not influenced by the experience of diabetes education (t=-1.68, p=.096). CONCLUSION: It is necessary to provide diabetes education with appropriate interval and methods and subjects that reflect the needs of patient through the study results.

4.A Case of Atypical Spitz Nevus.

A 6-year-old boy presented with a fast-growing, 0.8x0.7 cm-sized, dome-shaped, erythematous nodule on the back. Histopathologically, the lesion showed a spitzoid melanocytic proliferation from the dermo-epidermal junction to the reticular dermis. Melanocytes arranged in nests were epithelioid and spindle-shaped with abundant eosinophilic cytoplasm. The histopathological and clinical features were consistent with atypical Spitz nevus in that they showed no Kamino body, prominent cellular density, more than three mitoses per high power field, a high Ki-67 level, large size, and the lesion was located at an unusual location. Herein, we report a case of an atypical Spitz nevus on the back of a 6-year-old boy.

5.Pleomorphic Lipoma on the Scalp of a Middle-aged Woman.

Pleomorphic lipoma is an asymptomatic, slowly growing, solitary, well-circumscribed dermal or subcutaneous benign tumor. It is commonly found on the back or the neck of elderly male patients. Clinically, the lesion looks like a lipoma. Histopathologically, mature and immature fat cells are situated singly or in groups in a mucinous stroma. There can also be floret giant cells with overlapping nuclei. Histologically, pleomorphic lipoma may resemble liposarcoma but behaves in a perfectly benign manner. Our case is unique in terms of the sex and location of the tumor. In our case, complete excision was performed, and the patient shows no sign of recurrence 1 year after surgery. We herein report a case of pleomorphic lipoma which developed on the scalp of a 49-year-old female.

6.Analysis of the Effect of Oral Antihistamines in Patients with Chronic Urticaria in Terms of the Disease Outcome and Quality of Life.

BACKGROUND: Chronic urticaria (CU) has a considerable influence on the well-being of patients and it has a negative impact on the quality of life (QOL). Oral antihistamines are commonly used as the first-line treatment for CU. Although the efficacy and safety of antihistamines are well established for treating CU, a responder-based analysis to identify the improvement of symptoms and QOL has not yet been performed for Korean patients. OBJECTIVE: The purpose of this study was to compare the baseline urticaria severity score and the QOL with those after antihistamines treatment, and to assess the clinical outcome of CU after long-term antihistamine treatment. METHODS: A total of 103 CU patients were asked to complete a questionnaire about the effect of CU on their QOL at their first visit. Both the urticaria severity score and QOL were determined before and after a 6-month period, during which time the patients were administered a regular antihistamine prescription. QOL was assessed using a modified version of the Dermatology Life Quality Index (DLQI). The clinical efficacy was measured at baseline and at all visits. RESULTS: Oral antihistamines significantly improved the total QOL scores after 6-months treatment (p<0.05). The QOL dimensions that showed a statistically significant difference before and after antihistamine treatment were the mental status, daily living activities, leisure activities, self perceptions and social functions. The urticaria severity score values were positively correlated with QOL impairment for the CU patients. Also, the changes in the QOL following antihistamine treatment were reflected by the changes observed in the urticaria severity score. After taking oral antihistamines for 6 months, 65 patients (63.1%) stated that the urticaria had improved, and 43.1% of those patients had > or =75% reduction of symptoms from baseline. CONCLUSION: Our data highlights the importance of continuous antihistamine treatment for the management of CU. Oral antihistamines, when taken regularly, can improve the QOL and urticaria severity of CU patients.

7.A Case of Squamous Cell Carcinoma of the Liver in a Patient with Arsenic Keratosis Associated with Multiple Bowen's Disease.

Arsenic keratosis can be a precursor of both Bowen's disease and squamous cell carcinoma. In fact, there is a direct dose-response relationship between the amount of arsenic exposure and the development of skin cancer. Therefore, meticulous dermatologic surveillance and follow-up are advised to detect malignancy at an early and curable stage. We herein report on a case of a 62-year-old man who presented with multiple Bowen's disease and arsenic keratosis. His past history revealed chronic exposure to arsenic through consumption of Chinese traditional herbal medicine. After a follow-up loss of 2 years, he returned with multiple extracutaneous metastases of squamous cell carcinoma, including the liver. Our case emphasizes the importance of regular follow-up and survey for skin cancer and other internal malignancies in patients with history of chronic arsenic exposure.

8.Chemical Burn by a Herbicide, Methylchlorophenoxypropionic Acid (MCPP).

MCPP (2-(4-chloro-2-methyl phenoxy) propionic acid) is a phenoxy herbicide found in many household weed killers and lawn fertilizers. Some of the phenoxy acids, salts, and esters are moderately irritating to skin, eyes, and respiratory and gastrointestinal linings. In a few individuals, local depigmentation has apparently resulted from protracted dermal contact with phenoxy compounds. Although acute irritant contact dermatitis induced by herbicides is common, chemical burns induced by herbicides have been reported infrequently in the literature. Here we describe a chemical burn in a 25 year old female after sitting on a lawn that had been sprayed with MCPP 3 days earlier. This case illustrates potential hazards of the herbicide on the lawn of a public park.

9.Idiopathic Calcinosis Cutis: Clinical and Histopathologic Review in a Series of 30 Patients (2000-2009).

BACKGROUND: Idiopathic calcinosis cutis (CC) is currently sub-classified as tumoral CC, sub-epidermal calcified nodules, idiopathic CC of the scrotum, and milia-like CC. This sub-classification system is obscure and can be confusing at times. OBJECTIVE: This study was designed to characterize the clinical and histologic features of idiopathic CC, to evaluate the adequacy of the current sub-classification system, and to propose a new sub-classification system. METHODS: Thirty patients with idiopathic CC were sub-classified according to the Lever classification and the Walsh and Fairley classification and were also categorized based on the type of the primary lesion. RESULTS: Idiopathic CC lesions were most common on the buttock (n=7), followed by the scrotum (n=5), arms and legs (n=5), hands and feet (n=4), inguinal area (n=3), ears (n=2), neck (n=2), trunk (n=1), and face (n=1). With the Lever classification, twenty-three of 30 cases were categorized as "unclassified". With the Walsh and Fairley classification, twenty-three cases were categorized as "unclassified", whereas with our new classification system, we managed to classify all cases as either nodular or papular calcification according to the type of the primary lesion. CONCLUSION: In this study, we identified a common location and the primary morphological features of idiopathic CC. Our findings suggest that our new sub-classification system based on the type of the primary lesion would be useful.

10.Non-familial Acrokeratosis Verruciformis of Hopf.

Acrokeratosis verruciformis of Hopf is a rare genodermatosis with an autosomal dominant mode of inheritance. It is a disorder of keratinization, characterized by multiple, flat-topped, skin-colored keratotic lesions resembling plane warts typically observed on the dorsum of the hands and feet. Histopathologically, the lesion shows considerable hyperkeratosis, acanthosis, and papillomatosis, mimicking a "church spire", and a thickened granular layer. It arises in early life, often at birth or infancy. Herein, we report on a rare sporadic case of acrokeratosis verruciformis of Hopf. A 44-year-old Caucasian man presented with multiple, grouped, hyperkeratotic, whitish, flat papules on his shins and feet, which had been present for more than one year. Histopathological examination showed typical findings of acrokeratosis verruciformis of Hopf. Our case is unique in that the patient had no familial history of similar skin lesions.
