1.Tanshinone II combined with bone marrow mesenchymal stem cell transplantation promotes nerve regeneration following cerebral infarction

BACKGROUND:Studies have shown that tanshinone II can improve microcirculation, dilate cerebral blood vessels, increase cerebral blood flow, reduce infarct size, and improve brain function after cerebral metabolic disorder.
OBJECTIVE:To investigate the effect of tanshinone II combined with bone marrow mesenchymal stem cel transplantation on nerve regeneration folowing cerebral infarction in rats.
METHODS:Rat models of acute cerebral infarction were prepared using the thread occlusion method and then given tanshinone II combined with bone marrow mesenchymal stem cel transplantation (combined group), bone marrow mesenchymal stem cel transplantation (cel transplantation group), and no treatment (model group), respectively. Neuromotor function was assessed using Longa scores. Expression of brain-derived neurotrophic factor gene around the infarction region was detected using RT-PCR. Cel apoptosis was detected using TUNEL. Immunohistochemistry staining was used to determine peri-cortex Nogo-A and NgR protein expression at the infarction region.
RESULTS AND CONCLUSION:Longa scores, apoptotic index, and expression of Nogo-A and NgR proteins exhibited significant differences among three groups (P< 0.05) as folows: combined group < cel transplantation group < model group. Conversely, the expression of brain-derived neurotrophic factor gene was highest in the combined group successively folowed by the cel transplantation group and model group (P< 0.05). These data show that tanshinone II combined with bone marrow mesenchymal stem cel transplantation accelerate the recovery of neurologic function and promote nerve regeneration after cerebral infarction by incresing mRNA expression of brain-derived neurotrophic factor.
2.Effects of Ulinastatin on Pathological State of Lung Tissue and Aquaporin 1 and Matrix Metalloproteinase Expressions in Silicosis Rats

OBJECTIVE:To study the effects of ulinastatin on pathological state of lung tissue and aquaporin 1(AQP-1)and matrix metalloproteinases (MMP-2),MMP-9 expressions in silicosis rats. METHODS:Wistar rats were divided into normal con-trol group,model group,ulinastatin high-dose,medium-dose,low-dose groups(50×104,10×104,2×104 u/kg),20 in each group. Except for normal control group,rats in other groups were given 40 mg/mL SiO2 suspension 1 mL by non-exposed endotracheal per-fusion to induce silicosis model;1 h before modeling administration,corresponding doses of ulinastatin were intraperitoneally in-jected,for 3 d. Hematoxylin-eosin staining was used to observe silicon nodules in lung tissue of rats in each group;immunofluores-cence method was used to detect AQP-1 expression in lung tissue;real-time fluorescence quantitative polymerase chain reaction method was used to detect MMP-2,MMP-9 mRNA expressions in lung tissue;Western blot method was used to detect MMP-2, MMP-9 protein expressions in lung tissue. RESULTS:There were obvious silicon nodules and serious structural damage in lung tis-sue in model group,ulinastatin groups showed macrophage foci visible dust,but pulmonary fibrosis was obviously reduced. Com-pared with normal control group,AQP-1 expression in lung tissue in other groups were reduced,MMP-2,MMP-9 mRNA and pro-tein expressions were enhanced (P<0.05). Compared with model group,AQP-1 expression in lung tissue in ulinastatin groups were increased,MMP-2,MMP-9 mRNA and protein expressions were decreased (P<0.05),in which improvement effects in high-dose,medium-dose groups were superior to low-dose group(P<0.05). CONCLUSIONS:Ulinastatin can reduce the patholog-ical state of silicosis rats,increase AQP-1 expression and decrease MMP-2,MMP-9 expressions.

4.Fleabane combined with bone marrow mesenchymal stem cell transplantation for cerebral infarction

BACKGROUND:Cel transplantation becomes a new approach for treatment of cerebral infarction. In recent years, bone marrow mesenchymal stem cels (BMSCs) have become an important kind of seed cels in cel transplantation.
OBJECTIVE: To investigate the effect of fleabane injection combined with BMSC transplantation on S100B protein and superoxide dismutase expression in acute cerebral infarction rats.
METHODS:Animal models of acute cerebral infarction were made in Sprague-Dawley rats using suture method. After successful modeling, 80 model rats were randomly divided into control group, fleabane group, BMSC group and combined group (fleabane combined with BMSC transplantation). Changes of serum S100B protein and serum superoxide dismutase levels were detected using enzyme-linked immunosorbent assay and xanthine oxidase method, respectively, before and after treatment. NIHSS neurological function scores were measured to observe neurological behavior changes in model rats. The infarct volume was measured by TTC staining.
RESULTS AND CONCLUSION:At 36, 7, 14 days after treatment, S100B protein levels in the fleabane group and BMSC group were significantly lower than that in the control group, but higher than that in the combined group (P <0.05); serum superoxide dismutase levels in the fleabane group and BMSC group were significantly higher than that in the control group, but lower than that in the combined group (P < 0.05). At 1, 2, 3 weeks after treatment, NIHSS neurological function scores were ranked as folows: combined group < fleabane group and BMSC group < control group, and there was a significant difference between groups (P < 0.05). At 2 weeks after treatment, the infarct volume in the fleabane group and BMSC group was higher than that in the combined group but lower than that in the control group (P < 0.05). These findings indicate that fleabane combined with BMSC transplantation can inhibit the expression of S100B protein in rats with acute cerebral infarction, and promote the activity of superoxide dismutase, thereby playing a neuroprotective role.

Objective To explore the role of the medical engineering department in integration of the health care,nursing and medical technology departments.Methods The medical engineering department and integration of health care,nursing and medical technology departments were introduced from the aspects of concept,role,characteristic,working mode and etc.Results The medical engineering department contributed to enhancing medical service,medical safety and patient satisfaction,while the role needed strengthening.Conclusion Some measures are put forward to the medical engineering department during integration of health care,nursing and medical technology departments from the aspects of department positioning,examination and assessment,management mode and etc.

Objective To investigate early reduction of the dose of immunosuppressive drug after allogeneic peripheral blood stem cell transplantation(allo-PBSCT)for patients with refractory or relapsed leukemia.Methods Between Janaury 2004 and December 2006,15 patients with relapsed or refractory leukemia in Department of Hematology,the First Affiliated Hospital,Sun Yat-Sen University and Institute of Hematology & Blood Diseases Hospital,CAMS & PUMC,received allo-PBSCT from their relatives,12 from HLA-identical siblings.The preparative regimens included BuCy and TBICy with or without cytarabine.Cyclosporine A(CsA)or tacrolimus was used for graft-versus-host disease(GVHD)prophylaxis,with rapid decreasing starting on day 30 of post transplant if no GVHD appeared in receipts of matched sibling tranplantation.Results(1)Faster engraftment was achieved in all patients.Grade Ⅰ~Ⅱ acute GVHD appeared in 5 patients.Chronic GVHD occured in 7 of 11 evaluable patients.(2)Of 9 patients with an lower CsA or tacrolimus dosage,only 1 developed grade Ⅰ acute GVHD,4 chronic GVHD,2 extramedullary relapse.(3)After a median follow-up of 328 days,8 patients has leukemia-free-survival(LFS),4 relapsed,and only 1 had transplantation-related mortality(TRM)in the first 3 months post-transplant.The estimated LFS at 1 year and 2 years was 51% and 25%,respectively.Conclusion Patients with advanced leukemia might benefit from allo-PBSCT with significant lower treatment failure incidence.Dose reductions of CsA and tacrolimus in early transplant might enhance graft-versus-leukemia effect,and improve long-term LFS.
8.Gender dependence of metabolomic phenotypes for human saliva using ultra-high performance liquid chromatography with quadrupole time-of-flight mass spectrometry

Objective · To investigate the gender-related metabolomic differences in human saliva. Methods · The saliva metabolomic profiles of 5 male and 5 female healthy volunteers with matched age, body mass index (BMI), living and tooth-brushing condition were acquired using ultra-high performance liquid chromatography with quadrupole time-of-flight mass spectrometry. Metabolites were identified using publicly accessible databases and further confirmed individually with standard compounds. Both multivariant and univariate statistics were conducted to find inter-gender differences.Results · Forty-eight metabolites in human saliva were identified including 13 amino acids, 6 choline metabolites, 15 carnitines, 4 sphinganine metabolites,7 lysophosphatidylcholine and 3 organic acids. Levels of phenylalanine, acetylcarnitine, propionylcarnitine, butyrylcarnitine, isobutyrylcarnitine,isovalerylcarnitine and sphinganine were higher in the saliva of females than that of males. Conclusion · Rich metabolic information present in human saliva with significant gender diffference which ought to be taken into consideration in study on the metabonomics of saliva.

Objective To explore the establishment and application of a centralized purchase and distribution mode for medical instruments.Methods The centralized purchase and distribution mode was constructed based on the professional distributor and logistics team,and the application of medical instruments intra-hospital supply,processing and distribution (SPD) system was analyzed in the centralized purchase.Results Centralized purchase and distribution enhanced management efficiency and decreased inventory cost,and SPD system simplified the distribution mode and increased transport efficacy.Conclusion It's pointed out that professional logistics services be introduced to assist hospitals in centralized purchase and distribution.
10.Effect of metformin on serum vitamin B12 level in patients with type 2 diabetes mellitus:a meta analysis

Objective To systematically evaluate the effects of metformin on serum vitamin B12,folic acid and homocysteine (Hcy) levels in the patients with type 2 diabetes mellitus (T2DM).Methods The databases of Cochrane Library,EMbase,PubMed,CBM,CNKI,VIP and Wanfang were retrieved by computer to collect the randomized controlled trials (RCTs) on the association between metformin and serum vitamin B12 level in T2DM patients.Two researchers independently screened the literatures,assessed the risk of bias of included RCTs and extracted the data.The RevMan5.3 software was used for conducting the meta analysis.Results A total of 5 RCTs were included.The Meta-analysis results showed that serum vitamin B12 level in the metformin group was significantly decreased compared with the placebo group (MD=-55.86,95 % CI(-86.89,-24.84),P =0.000 4);the subgroup analysis showed that serum vitamin B12 level was negatively correlated with the duration of diabetes,metformin use time and dose.The serum Hcy level in the metformin group was increased compared with the placebo group (MD=2.44,95 % CI(1.41,3.46),P<0.01);the serum folic acid level had no statistical differences between the two groups [MD=-2.39,95 %CI (-4.93,0.15),P=0.06];the incidence rate of adverse reactions in the metformin group was higher than that in the placebo group[RR=2.0,95 % CI(1.32,3.03),P =0.001].Conclusion Metformin treatment may lead to decrease of serum vitamin B12 level and increase of Hcy level while has no obvious influence on the folic acid level in T2DM patients,but has higher incidence rate of adverse reactions.
