1.Study on the Practice of Regulating Individualized Infusion Sequence in PIVAS of Our Hospital

ABSTRACT OBJECTIVE:To establish the infusion sequence regulation procedure of PIVAS,and implement individual regulation. METHODS:By combining factors such as liquid volume of each batch,incompatibility between groups and specific solvents of specific drugs,intelligent prompt system and the artificial intelligent regulation system of infusion sequence were successively established to sequence the therapeutic drugs and adjuvant drugs used for the patients with different diagnoses and different purposes in different inpatient areas in a day,so as to realize intelligent control of individual infusion sequence. From Jan. to Sept. in 2017 and from Jan. to Sept. in 2019 as research stages before and after the intelligent control of individualized infusion sequence,the control effect was evaluated with the proportion of reasonable number of infusion priority cases,the proportion of appropriate number of elderly patients’batch liquid volume cases,the proportion of incompatibility cases between separate groups, the number and cost of flushing tubes as index. RESULTS:After regulating infusion sequence individually,the proportion of reasonable infusion priority rose from 5.56% to 98.72% ;the proportion of batch liquid quantity appropriate cases in elderly patients rose from 9.58% to 98.10%;the proportion which separated the incompatibility between groups rose from 41.03% to 99.12%;the number of washing tube dropped to(0.95±0.43)times/ward/d from(12.95±0.57)times/ward/d;the cost of washing tube could be saved 85 800 yuan/year. CONCLUSIONS:The implementation of individualized infusion sequence regulation promote safe and effective infusion,and provide reference for pharmacists of PIVAS in China to carry out precise pharmaceutical care.

4. Exploration of regulating infusion administration sequence by an intelligent prompt system in pharmacy intravenous admixture services

Eight sub-databases of pharmacological information on rational use of medicines were designed and established. These sub-databases cover dosing frequency, therapeutic drug priority to auxiliary medication, infusion shelf-life after admixture, incompatibility of two continuous infusion in the vein infusion tube, chronopharmacology, drug interaction involved in administration sequence , venous irritative drugs and preventive medicine. Once the standing order delivery to PIVAS involves any of these sub-databases, the system will prompt in turn " Auxiliary" , " Time limit" , " Taboo" , " Chronopharmacology" , " Interaction" , " Irritative" and " Preventive (therapeutic)" , starting from therapeutic drug priority to auxiliary medication. Pharmacists would make a reasonable sequence of the corresponding infusion and label the administration sequence on the infusion tag according to the prompt.Compared to the circumstance prior to this system, the wards with the infusion sequence labeling and equipped with intelligent prompt increased from 2 to 43; the percentage of such inpatients increased from 0.50% to 66.33%; the percentage of medical orders increased from 0.72% to 78.94%. The time spent by pharmacists for the same workload reduced from 73.44 h per day to 1.94 h per day. The times of flushing infusion tube reduced from 34.42 to 1.49 per ward and per day. The cost of flushing infusion tubes reduced by a big margin. PIVAS has established a method of regulating infusion administration sequence by an intelligent prompt system, based on improved safety and effectiveness of medication.
5. Prediction of disease prognosis by four blood biochemical indicators in patients with perianal necrotizing fasciitis

ObjectiveNecrotizing fasciitis (NF) has a high rate of deterioration and rapid progress, and the mortality rate is still high even after receiving treatment. The study was to explore evaluation value of serum procalcitonin (PCT), C-reactive protein (CRP) and plasma albumin (ALB) and prealbumin (PA) in the prognosis of patients with perianal necrotizing fasciitis (NF).MethodsAll data and detection data of serum PCT, CRP and plasma ALB, PA at admission from 41 perianal NF patients admitted to Hefei first people's Hospital Group from May 2009 to August 2019 were retrospectively collected. The death status was statistically analyzed. The patients in this group were divided into survival group and non-survival group, and the predictive value of various indicators on the prognosis of NF was evaluated. ResultsThe ratio of male to female was 9.25∶1. There were multiple complications, and diabetes mellitus was most common (41.46%). The patients with mixed infection and negative bacterial culture accounted for 12.20% and 17.07%, respectively. There were many kinds of bacteria detected out. Klebsiella pneumoniae (20.45%), Escherichia coli (15.91%) and streptococcus (15.91%) were common. All underwent one or more surgical treatments, with mortality of 17.07%. There was no significant difference between survival group and death group in gender, complications, microbiological test results, operation status within 6h or and nutrition support (P>0.05), while there were significant differences in time from onset to admission, proportion of patients in ICU, serum PCT and CRP, plasma ALB and PA and proportion of patients undergoing vacuum sealing drainage (VSD) (P<0.05). Logistic multivariate regression analysis showed that long duration from onset to admission, staying in ICU, high serum PCT and CRP, low plasma ALB and PA, and no conducting VSD were risk factors of poor prognosis (P<0.05). Serum PCT, CRP and plasma ALB, PA predict the prognosis receiver operating characteristic(ROC) curve, the area under the curve is 0.859, 0.811, 0.802, 0.747; the area under the curve of the four indicators combined prediction is 0.926.ConclusionThere are certain features in terms of gender, complications and microorganisms in perianal NF patients. Death risk is high. In addition, serum PCT and CRP, plasma ALB and PA are also related with prognosis, which can be applied as biochemical indexes for prognosis evaluation.
6.Correlation analysis of epidemiological characteristics and changes of influenza subtypes in Hefei City in 2015-2021

Objective To analyze the correlation between detection of influenza pathogenic subtypes and epidemic situation in Hefei City during the surveillance years of 2015-2021, and to provide references for developing influenza prevention and control strategies. Methods The relevant data of influenza-like illness (ILI) surveillance, influenza etiology, and outbreak/cluster outbreaks reported from influenza surveillance sentinel hospitals in Hefei City from 2015 to 2021 in the China Influenza Surveillance Information System were analyzed. The statistical analysis was carried out. Results A total of 3 332 553 outpatient and emergency visits in 2 sentinel hospitals were reported in Hefei City from 2015 to 2021, of which 139,082 were ILI cases, accounting for 4.17%. The monthly proportion ranged from 1.60% to 7.15%. A total of 14 663 ILI specimens were submitted for detection, the submission rate was 10.54%, and the positive rate of influenza virus detection was 11.30%. There was no correlation between the proportion of monthly influenza like cases and the detection rate (rs=0.176, P=0.107). The composition ratio of each subtype of influenza virus varied greatly, showing diversified seasonality, and the epidemic strains changed constantly. The Victoria subtype (BV subtype) was dominant (40.21%). There was a moderate correlation between the positive rate of ILI in sentinel hospitals and the number of outbreaks per month (rs=0.696, P=0.000). There was a strong correlation between the monthly number of outbreaks and the detection rate (rss=0.696, P=0.000). There was a strong correlation between influenza A virus H3 subtype and BV subtype (rs -H3=0.686, P=0.030; rs -BV=0.632, P=0.000). There was a moderate correlation between the new A1 subtype and B Yamagata subtype (rs -new A H1=0.481, P=0.000 0; rs -BY=0.515, P=0.000). Conclusion There are two epidemic peaks in spring and winter in Hefei. Influenza subtypes are diverse. The results of ILI etiology can predict the outbreaks of different subtypes of influenza. Prediction and surveillance should be used for influenza control and outbreak management in a timely manner.
7.Comparative study of angle Kappa and angle Alpha measured with three instruments before age-related cataract surgery

AIM: To compare the consistency of angle Kappa and angle Alpha in age-related cataract eyes with three different instruments.
METHODS: Prospective study. Totally 60 eyes of 30 patients with age-related cataract from December 1 to December 30, 2020 were collected. After adapting to 10min in the darkroom before operation, the angle Kappa and angle Alpha were measured with iTrace, Lenstar 900 and IOL Master 700 biometric instruments, respectively. The difference and consistency of the three instruments were analyzed.
RESULTS: The results of angle Kappa measurement of iTrace, Lenstar 900 and IOL Master 700 were 0.25±0.13, 0.19±0.11, 0.21±0.11mm(F=1.903, P=0.155). The results of angle Alpha measurement of the three instruments were 0.31±0.11, 0.36±0.16, 0.36±0.14mm(F=2.999, P=0.052). The proportion of angle Kappa of the three instruments greater than 0.5mm is 3%, 2% and 2%, and the proportion of angle Alpha greater than 0.5mm is 5%, 15% and 22%. The distribution of angle Kappa and angle Alpha is mainly in the temporal side of both eyes. Compared with the angle Kappa distribution of both eyes, the temporal distribution ratio of angle Alpha is higher, and the distribution of angle Alpha measured by iTrace is closer than that of Lenstar 900 and IOL Master 700 along the horizontal line. There was a positive correlation between angle Alpha and angle Kappa measured by each instrument(r=0.2941, 0.4082, 0.5137; P=0.0226, P=0.0012, P<0.0001). The consistency of angle Kappa measurement results of three kinds of instruments was good, while that of angle Alpha measurement results was poor.
CONCLUSION: When making the decision of multifocal intraocular lens implantation before cataract surgery, the consistency of angle Alpha measured by Lenstar 900, IOL Master 700 and iTrace is poor, so it is necessary to compare and refer to the results of various instruments in clinical application. When the data of angle Alpha could not be obtained before cataract surgery, the angle Kappa measured with the three instruments could provide useful information for multifocal IOL implantation decision.
8.Study on Preparation Technology and in vitro Drug Release Behavior of Baicalin Monolithic Osmotic Pump Tablets

OBJECTIVE:To prepare Baicalin monolithic osmotic pump tablets and investigate its in vitro drug release behavior. METHODS:Using accumulative release rate as evaluation index,baicalin solid dispersion was prepared to improve solubility,sin-gle factor test and orthogonal test were used to optimize preparation technology(the amount of penetrating agent and pore-forming agent,weight gaining of coating film) of monolithic osmotic pump tablets using baicalin solid dispersion as intermediate. Release rate and mechanism of samples prepared by optimized technology were investigated in 3 kinds of release medium (water,0.1 mol/L HCl,simulated gastric fluid). RESULTS:The optimal preparation technology was that penetrating agent sodium chloride was 30 mg;pore-forming agent polyethylene glycol 400 was 20% amount of excipient cellulose acetate;weight gaining of coating film was 2%. RSD of 12 h accumulative release rate was 1.06%(n=3)for 3 batches of Baicalin monolithic osmotic pump tablets pre-pared by optimized technology. 12 h accumulative release rate of them in 3 kinds of medium were similar to each other,being all more than 80%. Release equation was in line with zero-order drug release model (r=0.9985). CONCLUSIONS:Prepared Ba-icalin monolithic osmotic pump tablets after optimization can release drug at controlled rate.
