1.The Long-Term Results of Transcanalicular Dacryocystorhinostomy with a Diode Laser.

PURPOSE: This study evaluated the clinical outcomes of transcanalicular laser-assisted dacryocystorhinostomy (TCL-DCR) using a diode laser in patients with nasolacrimal duct obstruction (NLDO). METHODS: A total of 71 patients (76 eyes) who underwent TCL-DCR between May 2004 and April 2010 were analyzed. The functional and anatomic success rates were evaluated and the causes of failure were analyzed. RESULTS: The anatomic and functional success rates in primary TCL-DCR were 73.9% (51 of 69 eyes) and 62.3% (43 of 69), respectively. The causes of failure were membranous obstruction in 50.0% of the cases, granuloma formation in 38.9%, synechia formation in 5.6%, and canalicular stenosis in 5.6%. The anatomic and functional success rates were both 42.9% (3 of 7eyes) after TCL-DCR revision. CONCLUSIONS: The success rate of TCL-DCR is relatively comparable to that of conventional surgery. Additionally, the advantages of the procedure are its minimal invasiveness and convenience in an outpatient setting, suggesting that TCL-DCR may be an effective procedure for primary and secondary NLDO.

2.Efficacy of Intravitreal Anti-vascular Endothelial Growth Factor or Steroid Injection in Diabetic Macular Edema According to Fluid Turbidity in Optical Coherence Tomography.

PURPOSE: To determine if short term effects of intravitreal anti-vascular endothelial growth factor or steroid injection are correlated with fluid turbidity, as detected by spectral domain optical coherence tomography (SD-OCT) in diabetic macular edema (DME) patients. METHODS: A total of 583 medical records were reviewed and 104 cases were enrolled. Sixty eyes received a single intravitreal bevacizumab injection (IVB) on the first attack of DME and 44 eyes received triamcinolone acetonide treatment (IVTA). Intraretinal fluid turbidity in DME patients was estimated with initialintravitreal SD-OCT and analyzed with color histograms from a Photoshop program. Central macular thickness and visual acuity using a logarithm from the minimum angle of resolution chart, were assessed at the initial period and 2 months after injections. RESULTS: Visual acuity and central macular thickness improved after injections in both groups. In the IVB group, visual acuity and central macular thickness changed less as the intraretinal fluid became more turbid. In the IVTA group, visual acuity underwent less change while central macular thickness had a greater reduction (r = -0.675, p = 0.001) as the intraretinal fluid was more turbid. CONCLUSIONS: IVB and IVTA injections were effective in reducing central macular thickness and improving visual acuity in DME patients. Further, fluid turbidity, which was detected by SD-OCT may be one of the indexes that highlight the influence of the steroid-dependent pathogenetic mechanism.

3.Nursing Students' Hearing Levels and Blood Pressure Measurement Accuracy.

PURPOSE: This study was done to examine the relationship between nursing students' hearing levels and accuracy of blood pressure (BP) measurements. METHODS: Participants were 107 students who had finished their fundamental nursing practice and clinical practicum and who used earphones. Data were collected from October 13 to November 30, 2014 and from April 30 to May 19, 2015. Students' hearing thresholds were examined using an audiometer. Students were assigned to take two BP measurements on BP measurement training simulators, but only the second measurement was used for analysis. RESULTS: All nursing students' hearing levels were within normal range, and there was no significant difference found among the settings for diastolic blood pressure. However, there were significant differences between low systolic blood pressure (SBP) (below 120mmHg) and high SBP (over 140mmHg) (z=9.02, p=.011). Measurement error in SBP showed a positive correlation with hearing threshold in the right ear at frequencies of 1000Hz and 500Hz. CONCLUSION: Findings indicate that BP measurement error is correlated with hearing threshold at some frequencies. To reduce measurement error, nursing students should be provided with health education about hearing and to improve training for students, further studies need to examine other factors influencing BP measurement error.

4.Multiple System Atrophy Manifested by Bilateral Vocal Cord Palsy as an Initial Sign.

A 71-year-old male initially presented with vocal cord palsy and underwent tracheostomy. After thorough examination, urogenital dysfunction, orthostatic hypotension, and Parkinsonism were found, which led to the diagnosis of multiple system atrophy (MSA). After the tracheostomy, bi-level positive airway pressure ventilation was required during the night due to nocturnal hypoxemia. Night-time hypoxemia is related to central sleep apnea, which is one of the manifestations of MSA. This is the first case of MSA manifested by bilateral vocal cord palsy as an initial sign in Korea. This case supports the notion that MSA should be taken into consideration when vocal cord paralysis is observed.

5.Ab Interno Trabeculotomy with Trabectome(R) for Refractory Primary Open-Angle Glaucoma: A Case Report.

PURPOSE: To report a case of ab interno trabeculotomy with Trabectome(R) (NeoMedix Corp., CA, USA) conducted on a refractory primary open angle glaucoma (POAG) patient. CASE SUMMARY: Trabectome(R) has microelectrocautery with simultaneous infusion and aspiration of debris and ablates a segment of trabecular meshwork and the inner wall of Schlemm's canal. The patient, a 54-year-old man had uncontrolled intraocular pressure (IOP) with topical anti-glaucoma medications after trabeculectomy and Ahmed valve implantation for POAG. For the patient, ab interno trabeculotomy with Trabectome(R) was performed. There were no other postoperative complications except for microhyphema immediately after surgery. The IOP was controlled between 14 to 24 mm Hg up to 3 months postoperatively with topical anti-glaucoma medications (Cosopt(R), Alphagan-P(R), Lumigan(R)). CONCLUSIONS: Ab interno trabeculotomy with Trabectome(R) appears to offer a newer method of lowering IOP in POAG than conventional trabeculectomy and glaucoma drainage device surgery.

6.Objective Ultrasonographic Findings for Diagnosis of Liver Cirrhosis: A Comparison between Images Obtained with Low Frequency and High Frequency Probes on a PACS.

PURPOSE: We aimed to evaluate objective ultrasonography (US) findings for diagnosis of liver cirrhosis and to correlate the utility between the use of the low frequency and high frequency probes for images analyzed on the picture archiving and communications system (PACS). MATERIALS AND METHODS: A total of 87 patients participated in the study; 19 patients that had a clinically proven normal liver and 68 patients that had biopsy-proven chronic liver disease were evaluated with the use of US for the status of the liver. Multiple variables such as a 'smooth surface,' 'irregular nodular surface,' 'homogeneous echotexture,' 'heterogeneous appearance mixed with hypoechoic and hyperechoic echotexture' and 'hypoechoic honeycomb like echotexture' were evaluated based on images obtained with the use of both low frequency and high frequency probes by two observers. The diagnoses obtained after US were correlated with the histological results using assess agreement as statistical method. The concordance rate was calculated to correlate the utility between the use of the low frequency and high frequency probes. RESULTS: An 'irregular nodular surface' showed high interobserver agreement for the diagnosis of liver cirrhosis on images obtained with the low frequency probe (kappa= 0.61). In addition, 'hypoechoic honeycomb' showed high interobserver agreement on images obtained with the high frequency probe (kappa= 0.60). The use of the low frequency probe was associated with more accuracy for the diagnosis of liver cirrhosis (73%). CONCLUSION: Objective US findings for the diagnosis of liver cirrhosis are an 'irregular nodular surface' on images obtained with use of a low frequency probe and a 'hypoechoic honeycomb' on images with use of a high frequency probe.

8.Comparing Ruminative and Distracting Responses and Emotion Regulation Difficulties in Early Community Adolescents With and Without Self-Harm

Objectives:
This study aimed to compare the demographic characteristics, responses to negative emotions, and difficulties in emotion regulation between self-harming adolescents and control individuals aged 12–14 years from the community.
Methods:
Data were collected from adolescents in Chungcheong Province, South Korea, between September 2021 and November 2022.Demographic characteristics and responses to the Depressed Mood Questionnaire and Difficulties in Emotion Regulation Scale-16 (DERS-16) were compared between the self-harm and control groups.
Results:
The self-harm group exhibited a higher prevalence of child abuse (odds ratio [OR]=4.787, 95% confidence interval [CI]=1.591– 14.409, p=0.005) and school bullying victimization (OR=4.495, 95% CI=2.353–8.588, p<0.001) than those in the control group. The selfharm group displayed higher levels of rumination (t=7.88, p<0.001) and reduced distraction responses (reverse score t=2.25, p=0.025) than those of the control group. Additionally, the self-harm group scored higher on all subscales and the total DERS-16 score (t=7.61, p<0.001).
Conclusion
Interventions for self-harming adolescents should address child abuse and bullying victimization. Prevention programs for self-harming adolescents should focus on reducing rumination responses, increasing distractive responses, and addressing difficulties in emotion regulation using dialectical behavior therapy-skill training.

The quality of life can decreases significantly during exacerbations of the disease. The incidence and prevalence of UC in Korea are still lower than those of Western countries, but have been rapidly increasing during the past decades. Various medical and surgical therapies are currently used for the management of UC. However, many challenging issues exist and sometimes these lead to differences in practice between clinicians. Therefore, Inflammatory Bowel Diseases (IBD) Study Group of Korean Association for the Study of Intestinal Diseases (KASID) set out the Korean guidelines for the management of UC. These guidelines are made by the adaptation using several foreign guidelines and encompass treatment of active colitis, maintenance of remission and indication for surgery in UC. The specific recommendations are presented with the quality of evidence. These are the first Korean treatment guidelines for UC and will be revised with new evidences on treatment of UC.
