5.A Comparison of Peripheral Doses Scattered from a Physical Wedge and an Enhanced Dynamic Wedge.

In order to evaluate the radio-protective advantage of an enhanced dynamic wedge (EDW) over a physical wedge (PW), we measured peripheral doses scattered from both types of wedges using a 2D array of ion-chambers. A 2D array of ion-chambers was used for this purpose. In order to confirm the accuracy of the device, we first compared measured profiles of open fields with the profiles calculated by our commissioned treatment planning system. Then, we measured peripheral doses for the wedge angles of 15 degrees, 30 degrees, 45 degrees, and 60 degrees at source to surface distances (SSD) of 80 cm and 90 cm. The measured points were located at 0.5 cm depth from 1 cm to 5 cm outside of the field edge. In addition, the measurements were repeated by using thermoluminescence dosimeters (TLD). The peripheral doses of EDW were (1.4% to 11.9%) lower than those of PW (2.5% to 12.4%). At 15 MV energy, the average peripheral doses of both wedges were 2.9% higher than those at 6MV energy. At a small SSD (80 cm vs. 90 cm), peripheral dose differences were more recognizable. The average peripheral doses to the heel direction were 0.9% lower than those to the toe direction. The results from the TLD measurements confirmed these findings with similar tendency. Dynamic wedges can reduce unnecessary scattered doses to normal tissues outside of the field edge in many clinical situations. Such an advantage is more profound in the treatment of steeper wedge angles, and shorter SSD.

7.A Case of Painful Piezogenic Pedal Papules.

Piezogenic pedal papules are small, occasionally painful, fat herniation, which become apparent when weight is placed on the heel. We present a case of painful piezogenic pedal papules occurring on both heels of 30-year-old female patient without family history. Clinical and histopathological features confirmed diagnosis. Foot-ankle stocking was effective therapy for painful piezogenic pedal papules.

8.Supramalleolar Osteotomy in Patients with Varus Ankle Osteoarthritis.

This article has reviewed the alignment of the ankle and hindfoot in varus ankle osteoarthritis. The indication and surgical technique of the supramalleolar osteotomy has been reviewed. Alignment of tibial plafond and hindfoot is variable in early stages of ankle osteoarthritis. Supramalleolar osteotomy is indicated for the treatment of ankle osteoarthritis in patients with minimal talar tilt and neutral or varus heel alignment.

9.A Case of a Solitary Type of Congenital Self-healing Reticulohistiocytosis.

Congenital self-healing reticulohistiocytosis (CSHRH) typically presents at birth or in the first few weeks of life as a widespread eruption of cutaneous red-brown papulonodules that resolve spontaneously without involvement of other organs. While multiple lesions are common, a solitary lesion is rare. We describe a solitary type of CSHRH in a full-term, male neonate. He had an erythematous papule with a yellowish crust on the left heel without any systemic symptoms. Four weeks later, the skin lesion had disappeared spontaneously.

10.The Height and Volume of Medial Longitudinal Arch in Normal and Painful Feet.

PURPOSE: The purpose of this study was to investigate the differences in simple radiographic parameters and results of 3-D scan among normal and patient groups. MATERIALS AND METHODS: Seventy subjects in each group were studied. Control group consisted of subjects without plantar foot pain (normal group), and two patient groups were one with plantar forefoot pain (metatarsalgia group), the other with plantar heel pain (heel pain group). Simple radiographic parameters were obtained and 3-D scan was done with foot scanner (Nexscan, K&I, Korea) and The height and volumn of the space under the medial longitudinal arch was analyzed (Enfoot, K&I, Korea). These parameters were compared and correlation between radiological parameters and results of the 3-D scan were studied. RESULTS: The results of all parameters distributed normally. There was no signigicant differences among the groups in radiological parameters (talo-first metatarsal angle, calcaneal pitch angle and height of the talar head in standing lateral radiograph) and arch height and arch volumn on 3-D scan. There were statistically significant correlations between radiological and 3-D scan results. CONCLUSION: This study revealed that there is no significant differences in medial longitudinal arch height and volumn among normal and different patient groups and there are variety of arch height in patients with similar symptoms.
