1.A Study about Dietetic Knowledge and Educational Needs in Patients with Coronary Artery Disease and their Families.

Diet compliance is very important issue in patients with coronary artery disease(CAD). Diet compliance needs dietetic knowledge of patients and their families basically and families support. Therefore we studied about the dietetic knowledge level and educational needs of CAD patients and their families. For this study, we developed the study tool from the literature review. This tool consists of two main categories, one is about general dietary guidelines and specific food information, and the other is about educational needs of CAD patients & their families. The data were collected from 47 CAD patients and their families from June, 30 to to Semptember, 30, 1998. And they were analyzed using frequency, percentile, paired t-test, and ANOVA by SAS/ WIN. The results of this study were as follows : The results of the analysis of dietetic knowledge were as follows. In the general dietary guidelines, CAD patients knowledge level was lower than families in the importance of diet. And both patients and families had low knowledge level in the nutrition balance, protein intake, and relationship between fiber and cholesterol. In the specific food information, CAD patients knowledge level was low in protein-contained food, cholesterol -contained food, cooking method. The results of the relationship between the knowledge level and the demographic characteristics were as follows. In patients, there were significant statistical differences in the knowledge level of general dietary guidelines according to age and educational level, and the knowledge level of food information according to sex. In families, there were significant statistical differences in the knowledge level of general dietary guidelines according to age and educational level, and the knowledge level of food information according to sex and age. The results of the analysis of educational needs were as follows. Television was most common dietetic knowledge source. The highest educational needs were about recommended/restricted food and daily menu in both CAD patients and their families In th diet compliance, low-cholesterol diet was most difficult in both CAD patients and their farnilies. In conclusion, CAD patients and their families had high educational needs but insufficient concrete knowledge in such as recommended/restricted food, daily menu and cooking method. Nurses must be aware that it is necessary to establish an concrete dietetic educational program for CAD patients and their families.

3.Clinical and Histopathological Observation of Porokeratosis.

We observed clinical and histopathological findings of porokeratosis in 16 cases which comprised 2 linear types, 4 superficial disseminat:ed forms, 4 disseminated superficial actinic porokeratosis and 6 plaque types. From the observation, the following results were obtained. l. Clinical results 1) Eleven cases were men, 5 cases were women, and the sex ratio was 2. 2 to 1, age distribution was from 6 to 61 years (mean age: 34. 2years), and the distribution of age of onsets was from 2 to 52 years(mean age of onset' 23. lyears). 2) Four cases had family history of porokeratosis, but linear forms did not. 3) Nine cases had pruritus especially, all cases of DSAP and DSP except 1 case complained pruritus. 4) Tmo cases of DSAP had tendency of summer exacerbations. 2. Histopathological results 1) Cornoid lamellae were prominent in all cases of plaque types and linear types, and minimal in all cases of DSAP and DSP except 1 case. 2) Epidermis between the cornoid lamellae were normal or acanthotic in all cases except 1 case of plaque types and linear types, and were atrophic in 5 cases of 8 cases in DSAP and DSP.

4.Histopathological Observation of Cafe au Lait Spots.

We observed 17 cases of cafe au lait spots histopathplogi ally. Out of 17 cases, 9 cases were neurofibromatosis with neurofibromas, 6 cases were neurofibromatosis without neurofibroma, and 2 cases were normal healthy persons. The results were as follows. l. In all 17 cases, melanin pigment was increased in melanocytes and keratinocytes. 2. Griant pigment granules were identified only in 3 cases of neurofibromatosis with neurofibromas.

6.Experience of High Risk Women Who Have Congenital Heart Disease: Transition to Parenthood.

PURPOSE: High risk women with congenital heart disease decide to get pregnancy is determined by not individual autonomous intention but complex interaction with their physical status and socio-psychological environments. This study tried to the answer to the question. : "What is experience high risk women who have congenital heart disease during transition to parenthood?". METHOD: A micro-ethnographic research method and oral historic research approach were done at the Grown-Up Congenital Heart Disease Clinic in one Korean metropolitan city from July 2002 to September 2003. RESULT: It was discovered that high risk women's experience of transitional parenthood is accounted as the process of lonely and fearful self-accomplishment. Their need for self- accomplishment creates them seek more opportunities to increase enduring abilities for their parenthood. CONCLUSION: We suggest that from the time of beginning of patient's making decisions about becoming pregnant, collaborative efforts must be considered that priority level of patient's needs be reviewed and find appropriate advices for their situation. Special counseling program should be provided to all the prospective parents with understanding their meaning of parenthood.

7.Rapid Preparation and Quality Control of 99mTc-ECD, MAG3 and MIBI using Microwave Heating and Sep-Pak Cartridges.

PURPOSE: We evaluated a rapid preparation procedures for the labeling and quality control of 99mTc-ECD, MAG3, and MIBI using microwave heating and Sep-Pak cartridges. MATERIALS AND METHODS: 99mTc labeling of ECD, MAG3, and MIBI kit preparation was performed according to the package inserts with microwave heating modification. Heating time was 10-15 sec, and heating was performed with 3 mm plastic bottle with screw cap to prevent radiation contamination. Labeling efficiency was obtained with C18 or Alumina N Sep-Pak cartridges. RESULTS: The radiochemical purity of 93~96% for 99mTc-ECD and 95~99% for 99mTc-MIBI was obtained using Alumina N Sep-Pak cartridge. The optimum irradiation time of microwave method for 3 ml 99mTc-labeled radiopharmaceutical solution was 10 sec for 99mTc-ECD and 99mTc-MIBI, and 15 sec for 99mTc-MAG3. The RESULTS of quality control data with Sep-Pak cartridges were well correlated with TLC method. The total preparation time of these radiopharmcaeuticals was 5~6 min including quality control procedure. CONCLUSION: This study demonstrates that radiopharmaceuticals preparation by microwave heating and quality control by Sep-Pak cartridges can be efficiently utilized as an alternative to the recommended method by manufacturer's manual.

8.A Case Combined Germ Cell Tumor in Testis.

Primary germ cell tumor of the testis is rare, which occupies 1 to 2% of all reported malignant male neoplasms. Combined primary germ cell tumor of the testis composed of embryonal carcinoma and seminoma is more rare tumor. The authors experienced a case of 50-year old male who presented with painless enlargement of right testicle. He has had radical orchiectomy and diagnosed by pathologically as combined germ cell tumor of testis. The testicle is measuring 180 gm in weight and 9×6×5 cm in dimension, and almostly replaced by tumor mass. Grossly the tumor is rubbery solid smooth tumor mass, with variegated cut surface with geographically outlined diffuse necrotic area. Histologically the tumor is composed of two components of tumor, which are solid growth pattern of large round to polyhedral cells with clear or granular cytoplasm and distinct cell border, and anastomosing glandular and papillary arrangement of anaplastic epithelial cells. The former corresponds to seminoma, and the latter to embryonal carcinoma. Each tumor lobule is separated by abundant fibrous stroma.

9.A Case of Multiple Cutaneous Focal Mucinosis.

We reported a case of cutaneous focal mucinosis in a 19 year-old male, which occured by multiple nodules on the both dorsa of hands, extensor surface of elbows, intergluteal folds and shins, and responded well to intralesional injection of triamcinolon acetonide. Histopathologically, most of the collagen in the dermis is replaced to homogeneous mucinous material which was confirmed as hyaluronic acid.

10.Waardenburg's Syndrome in Son and Mother.

We report a case of Waardenburgs syndrome in 18-year-old male patient who has total deafness of the right ear, heterochromia irides, hypopigmented patches on the face and trunk, and disseminated lentigines on the entire body surface since birth. His 51-year-old mother also has total deafness, heterochromia irides, white foreloek, and disseminated lentigines on the entire body surface. The patient with this disorder may complain of pigmentary abnormalities, and we believe dermatologists should pay more attention to systemic signs of the congenital pigmentary disorders.
