5.The Clinical Effects of Amitriptyline in Patients with Urinary Frequency and Chronic Pelvic Pain Syndrome.

PURPOSE: We evaluated the effects of amitriptyline in patients with urinary frequency or pelvic pain syndrome and compared with propiverine hydrochloride (BUP-4(R)) and diazepam (Valium(R)). MATERIALS AND METHODS: One-hundred and sixty-eight patients with urinary frequency or pain were included and 38 out of 168 patients were lost to follow up. The patients with interstitial cystitis, UTI and neurogenic bladder were excluded. Amitriptyline group were instructed to take 25mg to 75mg of amitriptyline before bed. Propiverine hy drochloride and diazepam group were instructed to take 20mg, 4-6mg before bed, respectively. The clinical effects and side effects were evaluated after 4 weeks of medication. RESULTS: In the amitriptyline group, 73.1% in 78 patients with frequency, 68.3% in 60 pain, 65.6% in 32 nocturia, 53.8% in 26 weak urinary stream, 53.8% in 13 dysuria, 72.7% in 11 urgency showed symptom improvement. Urinary frequency and pain score were significantly decreased in the amitriptyline group. In the propiverine hydrochloride group, 66.7% in 21 patients with frequency, 38.8% in 18 pain showed symptom improvement. In the diazepam group, 37.5% in 24 patients with frequency, 31.3% in 16 pain showed symptom improvement. Side effects of amitriptyline had appeared in 24 (27.6%) out of 87 patients; 17 cases of dry mouth, 10 drowsiness, 2 agitation, 2 nausea, 1 constipation and 1 skin rash. CONCLUSIONS: Amitriptyline is useful in vague voiding symptoms especially in urinary frequency and chronic pelvic pain syndrome. It is equally effective in both men and women. Side effects are minimal and tolerable. A double-blind placebo-controlled trial is necessary for general acceptance in the future.

7.Influence of Job Stress and State of Sleep on the Depression of Emergency Medical Technicians.

PURPOSE: The purpose of this study was to identify the relationship of job stress, state of sleep, and depression and to identify factors influencing depression of emergency medical technicians. METHODS: The research was a cross-sectional, descriptive study. Data collection were done using self-report questionnaires with 162 emergency medical technicians working in hospitals who agreed to participate in the study. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients and multiple regression with the SPSS/WIN 18.0 Program. RESULTS: Depression had significant negative correlations with state of sleep, (r=-482, p<.001) and positive correlations with job stress (r=.196,p=.013). State of sleep had significant negative correlations with job stress (r=-.168, p=.033). The variance for life satisfaction was 23.7% (F=25.964, p<.001). Variable of state of sleep significantly predicted depression of emergency medical technicians. CONCLUSION: Considering these results, for emergency medical technicians' job stress, state of sleep, and depression, further studies are needed. Furthermore, there is a need to develop and implement new strategies to improve depression among emergency medical technicians.

8.A Study on the Relation of BMI to Lipid Metabolism and Health Promoting Behavior in Middle-Aged Women.

PURPOSE: This study examined the relation of BMI to lipid metabolism and health promoting behavior in middle-aged women. METHODS: The subjects were 113 women between 40 to 60 years of age. The data was collected from April 2003 to July 2004. BMI was measured using body composition analyzer and lipid metabolism were measured using automatic biochemical analyzer. Health promoting behavior was collected using a structured questionnaire. The SPSSWIN(10.0 version) program was used to analyze the data and get the descriptive statistics and Pearson's correlation coefficients. RESULTS: BMI was negatively correlated with HDL-c and positively correlated with triglyceride, but there was no significance between BMI and total cholesterol. There was no significance of correlation between BMI and total health-promoting behavior. CONCLUSION: This study showed that BMI is meaningful in the identification of the high risk women to prevent HDL-c & triglyceride metabolism disturbance, but additional research is needed to study the correlation between BMI and health promoting behaviors in middle-aged women.

9.Treatment of alopecia areata with diphencyprone.

Alopecia areata is a relatively common disorded but the exact psthogenesis is still unknown and there is no satisfactory treatment. Recently contact immunotherapy has been studied with dinitrochlorogenzene(DNCB) which is known to have in vitro mutagenecity limiting its clinieal application. In contrast, diphencyprone (DPCP) is a potent contact sensitizing agent which is not known to be mutagenic. We treated 40 patients with alopecia areata who were refraetory to standard treatment.DPCP was dissolved in acetone in concentrations of 0.0001 2%. After sensitization, DPCP was topically applied to the right side of the scalp with left side sewing as control. After obvious hair growt,h, DPCP was applied on both sides of the scalp. A positive respanse was defined as growth of terminal hairs on the scalp. The response rate was estimated by determing the percentage of hair covered areas com- pared to the total area of the scalp. The results were as follows. 1) Of 20 patients who were followed over 3months, 9 patients(45% ) showed positive response, and 6 patients(30%) showed almost complete regrowth of terminal hairs (above 90% of scalp area). 2) The response time varied from 4 weeks to 33 weeks, with average duration of 10.6 weeks. 3) Nine out of forty patients developed side effects such as severe contact eczema, cervical lymph node swelling and vitiligo. Three out of 40 patients had to discontinue therapy due to side effects. 4) Patients with mild alopecia areata ganerslly showed a higher response rate(50%) than those with alopecis totalis(33%) and alopecia universalis(43%), but the differences were not statistically signifieant(p>0.05). 5) No correlation was found between reaponse rates and clinical factors such as age, sex and duration ot the disease. In conclusion, treatment with DPCP was thought to be effective in patients with alopecis areata who were refractory to standard treatment. Further long-term studies may be neees- sary to find out clinical course of the disease after trestment, and long-term safety of this treatment.

10.A 10 Year Period (1968 - 1977) of Clinical Observation of Cutaneous Malignant Tumors.

From january 1968 to Septomber 1977(approximately 10 years) the authors clinically observed 105 cases of malignant curaneous tumors that visited the Department of Dermatology at Chon-nam University Hospital. The results obtained can be summarized as follows: 1) The incidence of curaneous malignat tumors versus the total number of outpatients visitiong the Department of Dermatology was 0.33 2) The age of onset was quite varied but the majority of cases, 71 cases(67.62%), were over 50 years old. #) The ratio of men to women was 1.56:1. 4) Among the 105 cases of malignant cutaneous tumors the most frequently observed type was squamous cell carcinoma, 40 cases(40.0%) The next most frequent rypes were basal cell carcinoma, 19 cases (18.10%) and malgnant melanoma, 12cases(11.43%) 5) The most common predilection sites were the head and neck in which 55cases(52.38%) were observed. Among these cases the face was most commonly affected, 40 cases(38.10%). The predilection sites of the basal cell carcinoma cases were the nose(47.37%), cheek(21.04%) and eyelid(15.79%). Those of the squamous cell carcinoma cases were the lower lip(26.90%), cheek(24,43) and penis(14,29%). Finally that of the malignant melanoma was the plantar surface(58.34%) 6) We observed metastasis only in wquamous cell carcinoma, Among the 42 cases of squamous cell carcinoma seen, I1 cases showed metastasis, Among these I1 cases of metastasis, 9 cases were metastajzed into the regional lymphnodes, and in 3 cases methstsis to the lung was demonstracted. 7) We found no relationship between the presence of a particular kind of original skin lesion or injury and the development of a cutaneous malignant tumor. some original lesions related to the curaneous malignant tumors we mentosem, radiation, sporotrichosis, neurofibromatosis, etc. 8) One or more of the four following methods of treatment were used on 19 of the observed cases: 1) Surgical Operation 2) Irradiation Therapy. 3) Electrosugery, 4) Chemotherapy, In seven cases(Basal cell carcinoma 5 cases, Squamous cell carcinoma 2 cases) a complete cure was seen, while in five cases(Kaposi's sarcoma 1, case Squamous cell carcinoms 4 cases) the curaneout maligant tumor recurred No improvement was demonstrated in two (Squamous cell carcinoma 1 case, Malignant melanoma 1 case) of the cases and in four cases(Squamous cell carcinoma 3 cases, Malignant Melanoma 1 case) no prognosis could be made because the patients did nor retum to the clinic to complete treatment.
