3.The Experience of Concurrent Review of blood Transfusion Requests.

Blood utilization review has recently been used to minimize inappropriate transfusion, reduce post transfusion infections and improve the quality of transfusion practices. Ajou University Hospital is a 906 bed tertiary care teaching institution. Since March, 1995, the concurrent review of blood transfusion requests using institutional blood transfusion criteria has been initiated. To evaluate the effectiveness of the transfusion review, the units of blood components transfused per patient(total number of units of blood components transfused/total number of patients discharged) and inappropriate transfusion ratio of blood components(total number of units of blood components inappropriately transfused/total number of units of blood components transfused) were compared during the concurrent review for 9 months and pre-concurrent review for 9 months, respectively. The possible savings of blood components realized by the review were also extrapolated. The results were as follows : 1. The unit transfused per patient for pre-review and during review were 0.675 (9,080/ 13,422) and 0.654(12,123/18,546) for packed RBC(PRBC); 0.417 (5,602/ 13,422) and 0.296 (5,444/18,546) for fresh frozen plasma(FFP); 0.372(5,007/13,422) and 0.424(7,868/18,546) for platelet con-centrates(PC), respectively. 2. The inappropriate transfusion ratio of blood components for pre-review and during review were 0.039 and 0.009 for PRBC; 0.321 and 0.064 for FFP; 0.143 and 0.008 for PC, respectively. These differences were statistically significant(p=0.000). 3. The PRBC and FFP anticipated to have been saved by this review was 519 units and 2,992 units annually, respectively. 4. However, 2,621 units of PC were used additionaly during this review due to increased number of oncology patients and better hemostatic therapy practiced during the same period. In conclusion, the concurrent review of transfusion requests was very effective tool to educate residents in clinical departments the transfusion medicine and improve utilization of blood and its components.

4.Postirradiation Fibrosarcoma of Bone: Report of 2 Cases

Ionizing radiation has brought powerful aids to medical practitioners, but it has also brought new and bewildering problems. Radiation in the form of systemically administered radionuclides or external radiation may produce such abnormal reactions in the musculoskeletal system as growth disturbances, myelofibrosis, radiation osteitis, necrosis of bone, exostosis, pathologic fracture, and on rare occasion malignant tumors. The incidence of postirradiation sarcoma was quite rare and there were some histologic hypes of postirradition sarcoma. Of them, incidence of osteosarcoma and fibrosarcoma is most common, although there is some difference between each reports. The authors have experienced recently two cases of postirradiation fibrosarcoma developed in sacrum. In one case, the patient was 55 years old female and the latent period was about 12 years. In another case, the patient was 67 years old female and the latent period was about 15 years. In these cases, they received radiation therapy due to cervical cancer of uterus. They had nerve roots compression symptoms and recieved operation of nerve roots decompression. The diagnosis was confirmed by core bone biopsy. A clinical analysis of these cases is to be reported with review of references.

5.A roentgenologic study of diverticula throughout the entire gastrointestinal tract

Diverticulum is considered as common lesion involving any gastrointestinal tract from the pharynx to therectum. We reviewed 5806 cases of upper GI series and 801 cases of Barium enema during the period from Jan. 1978to Dec. 1981 in the Departement of Radiology, school of medicine, JeonBug National University Hospital to analizediverticula of the entire GI tract. The results are follows : 1. Roetgen examination of 5806 esophagus, stomachs,duodenums and small bowels, and 801 colons during the past four years: diverticular of esophagus, 60 cases(1.0%);diverticula of stomach, 42 cases (0.7%); diverticula of duodenum, 358 cases (6.2%); diverticula of small bowel,20( 0.3%); diverticula of the colon, 26 cases (3.2%). The location of diverticula inorder of frequency wasduodenum, colon, esophagus, stomach, and small bowel. 2. the most common site of diverticula of each GI tract wasfollows : diverticula of the esophagus, middle portion (84.7%); diverticula of stomach, the cardia(59.5%);diverticula of the duodenum, second portion(81.3%); diverticula of small bowel, the jejunum(96.4%) especially thelarger percentage were observed at the upper jejunum near the ligament of Treitz; diverticula of the colon, rightsided colon(80.8%), the cecum and ascending colon(57.1%) 3. Diverticula may occur at any age. The majority ofdiverticula of the entire GI tract were observed over 40 years of age. Especialy in diverticula of duodenum andcolon, thir frequency increase with age. 4. Duodenal diverticula were observed more frequently in womanthan in manbut in diverticula of the esophagus, stomach, small bowel, and colon, male was more frequently affected. 5. Thesize of diverticula of entire GI tract was variable. The majoprity of diverticula of the esophagus, stomach,duodenum and small bowel were intermediate size (10-49 mm). Diverticula of the colon were usually smaller than 10mm. 6. Multiplicity of diverticula of entire GI tract was 16.2% of 506 cases. In diverticula of esophagus,stomach, duodenum and small bowel, single lesion was more frequently found. on the other hand, the majority ofdiverticula of the colon were found as multiple lesion(69.2%).

6.The Aphyllophorales of Mungyong Saejae.

To evaluate the establishment of Mungyong Saejae Natural Ecology Park located in the northwestern Gyongbuk Province, a scientific survey for the mushroom flora of the park was carried out from May to December of 1999. A checklist of the Aphyllophorales collected from the park was prepared. The list included 67 species of 44 genera belonging to nine families in the Aphyllophorales. Among them, seven species, Antrodia malicola, Ceriporia purpurea, Oligoporus leucospongia, Perenniporia tephropora, Phanerochaete xerophila, Sistotrema diademiferum and Vuilleminia comedens, were confirmed as new to Korea and are registered here as unrecorded species along with descriptions and microscopic drawings.

7.Treatment of the Sequelae of Septic Hip in children: Report of 2 Cases

Septic arthritis of the children's hip causes acute or chronic inflammatory change in structure about the joint, and results in serious destruction. Due to recent advance of the diagnostic technique and of the antibiotics, the incidence of it's sequelae is decreased. The management of acute stage with antibiotics and early surgical drainage has been well established in the literature, but the management of residual anatomic deformity is less documented, and little comprehensive classification or treatment program has been outlined. We experienced two patients who had sequelae induced by delayed diagnosis and improper treatment. One had a complete loss of femoral neck with femoral head remaining in the acetabulum. Femoral neck reconstruction was performed using proximal femur : i.e., Open reduction of femoral head to femur shaft with distal transfer of greater trochanter (1st stage) followed by varus osteotomy(2nd stage) and follow-up period was 3.5 years. Another had loss of femoral head and neck, dislocation of the hip, and Larsen's trochanteric arthroplasty was performed with 6 Years follow-up. Satisfactory results were obtained in both patients who had reconstructive surgery, so we report these cases with related literatures.

8.Clinical Nurses' HPV-related Knowledge and Perception of Cancer Causes: HPV Vaccinated vs. Not Vaccinated.

PURPOSE: The purpose of this study was to compare knowledge level of those clinical nurses who received HPV vaccine and those who did not and their perception of the relatedness of HPV vaccine to causes of cervical cancer. METHODS: A total of 249 clinical nurses were surveyed from June to July, 2009. The questionnaire originally developed by Kim & Ahn (2007) examined HPV-related knowledge originally and the tool for perception of the causes of cervical cancer was originally developed by Kim (1993). The total number of subjects equaled to: vaccination group of 52 (20.9%) and non-vaccination group of 197 (79.1%). RESULTS: Vaccination group showed significantly higher score of both knowledge of HPV vaccination and the perception of the cause of cervical cancer in comparison to the nonvaccination group at (p<.05). Among 4 subscales of the perception of causes of cervical cancer, destiny and constitution subscale scores were significantly different between the two groups at (p<.05). CONCLUSION: Clinical nurses need to constantly update with current knowledge of HPV and be prepared with currently changing cancer prevention strategies, especially in cervical cancer.

10.Blood Folate Level Determined by a Microplate Reader and Folate Intake Measured by a Weighted Food Record.

Microbiological method using a 96-well microplate reader for folate assay was established, and folate intake and blood folate concentrations of 23 female college students were assessed. To evaluate folate intake, dietary data were collected by a 3-day weight food record, and serum and RBC folate concentrations were measured by the new method. The coefficient of variation for the new method was less than 10%. Mean daily folate intake of the subjects was 126.7ug which is only 50.7% of the RDA. The mean concentrations of serum and RBC folate were 7.46ng/ml and 294.4ng/ml, respectively, which were within the normal range. These results indicate that folate intake seems to be underestimated due to incomplete food composition database. Therefore, folate database should be appropriately in order to asses folate intake accurately.
