3.The Experience of Concurrent Review of blood Transfusion Requests.

Blood utilization review has recently been used to minimize inappropriate transfusion, reduce post transfusion infections and improve the quality of transfusion practices. Ajou University Hospital is a 906 bed tertiary care teaching institution. Since March, 1995, the concurrent review of blood transfusion requests using institutional blood transfusion criteria has been initiated. To evaluate the effectiveness of the transfusion review, the units of blood components transfused per patient(total number of units of blood components transfused/total number of patients discharged) and inappropriate transfusion ratio of blood components(total number of units of blood components inappropriately transfused/total number of units of blood components transfused) were compared during the concurrent review for 9 months and pre-concurrent review for 9 months, respectively. The possible savings of blood components realized by the review were also extrapolated. The results were as follows : 1. The unit transfused per patient for pre-review and during review were 0.675 (9,080/ 13,422) and 0.654(12,123/18,546) for packed RBC(PRBC); 0.417 (5,602/ 13,422) and 0.296 (5,444/18,546) for fresh frozen plasma(FFP); 0.372(5,007/13,422) and 0.424(7,868/18,546) for platelet con-centrates(PC), respectively. 2. The inappropriate transfusion ratio of blood components for pre-review and during review were 0.039 and 0.009 for PRBC; 0.321 and 0.064 for FFP; 0.143 and 0.008 for PC, respectively. These differences were statistically significant(p=0.000). 3. The PRBC and FFP anticipated to have been saved by this review was 519 units and 2,992 units annually, respectively. 4. However, 2,621 units of PC were used additionaly during this review due to increased number of oncology patients and better hemostatic therapy practiced during the same period. In conclusion, the concurrent review of transfusion requests was very effective tool to educate residents in clinical departments the transfusion medicine and improve utilization of blood and its components.

4.A Study on Adolescents' Smoking Behavior and the Status and Need of Smoking Prevention Education for Adolescents.

PURPOSE: This study was to identify adolescents' smoking behavior and the status and need of smoking prevention education for adolescents. METHOD: The subjects were 375 students selected through convenient sampling from three middle schools and three high schools in Jeonbuk. Data were collected through a questionnaire survey and analyzed using SPSS 10.1. RESULT: The smoking rate was 20.8% and 83.7% the students experienced smoking prevention education. Students were educated by a special instructor (39.8%) or a health teacher (26.8%). Among the teaching methods, lecture was 69.4%, but some students (30.7%) wanted anti-smoking school. Among the students, 85.0% answered they want smoking prevention education and 46.7% said that it should be covered in regular curriculum. CONCLUSION: High interest and continuity are emphasized for the success of smoking prevention education. In addition, various teaching methods need to be developed including anti-smoking school, lectures, counselling and long term education program. Moreover, smoking prevention education programs need to be covered in regular curriculum.

7.Computed tomography of osteitis condensans ilii

The CT is a more accurate technique for detecting sclerotic bony change of osteritis condensans ilii thanplain radiograph. We analysed a comparison between CT and plain radiography of osteitis condensans ilii, acorrelation between osteitis condensans ilii and women of childbearing age. The result were as follow: 1. Theincidence of osteitis condensans ilii is 5.3% on KUB, 11.7% on CT when the width of iliac sclerosis is more than7.5mm as diagnsotic criteria. 2. We observed a osteitis condensans ilii between 19 years and 51 years of age, mostfrequently in fourth decade. 3. The width of iliac sclerosis is 10-13 mm in 3 cases of osteitis condensans ilii onboth CT &KUB, 7.5-9mm in 4 cases of osteitis condensans ilii on CT only. 4. The incidence of osteitis condensansilii is increased significantly when the width of iliac sclerosis is less than 7.5mm as diagnostic criteria. 5.Relatively high correlation between osteitis condensans ilii and delivary in our study (66.6%)

8.Value of the lumbar lordotic angle taken angle from CT scanogram as an index of back pain

Normal" spinal posture is generally accepted as moderate lordosis of the cervical and the lumbar sections ofspine and moderate kyphosis of the thoracic and sacrococcygeal sections. Among these normal range of the lumbarlordosis is less well defined. CT is very useful method to identified the bony structure and adjacent soft tissueof the spine, so it is very available method to detect accrate lumbar lordotic angle by using lateral Scanogram.Analysis and comparison of lumbar lordotic angles, crossing angle are drawn each parallel line to the superior endplate of body of 1st. lumbar vertebra and to the inferior end plate of the 5th. Lumbar vertebra, in 174 cases withbackache and 50 cases normal groups at Kangnam General Hospital Public Corporation, from Aug. 1985 to Jul 1986.Male and female were almostly same affected in backache group (1.1:1). And most lumbar lordotic angles were 10 degrees to 40degrees between all backache age group. On backache group, over all mean lumbar lordotic angleswere 24.7±8.9degrees but no significant difference at mean value of the each disease, such as HIVD, DegenerationSpondylosis or No Remarkable Finding group. On control group, over all mean ones were 29.2±8.0degrees. So,significant difference of lumbar lordotic angle between backache and control group, and most difference is at 5th.decade group(p<0.01).

9.The Effect of the Solution-focused group counseling program on problem-solving capacity of the nursing students.

PURPOSE: To examine the effect of the solution-focused group counseling program on the nursing students' problem-solving capacity. METHODS: The research design was one-group pretest-posttest design in quasi-experimental research. The subjects were 16 sophomore nursing students being in K university located in D city. Korea. The period of this research was from January 9 to February 20, 2002. from January 10 to February 21. 2003. The group counseling program that was developed by Kim, Hyeon Mi (2001) on the basis of the fundamental principles of solution-focused therapy of de Shazer. et al. (1986) and the solution-focused group counseling process proposed by Selekman (1991). LaFountain. et al. (1996) and Metcalf (1998) was used after being modified and complemented for the nursing students. The group counseling was carried out six times for 120 minutes per week. The instruments used for this study were the Problem Solving Inventory developed by Heppner and Petersen (1982) and Scaling question. Collected data were analyzed by paired t-test with SPSS program. RESULTS: 1) Problem solving capacity increased after solution-focused group counseling program, but a significant difference was not revealed(t=0.835, p=0.057). 2) The scores of self-confidence with the problem-solving were significantly increased when it was compared with the 1st and the 6th(t=3.587, p=0.001). CONCLUSION: It seems that the solution-focused group counseling program should be considered as an effective nursing intervention program for the nursing students by improving self-confidence with the problem-solving. As a results, this study suggests that the specific techniques used in the solution focused therapy should be applied actively in areas of counseling and communication training for the nursing students and nurses.
