1.Treatment of Witiligo with Oral Methoxsalen and UVA.

Among ]20 vitiligo patients who visited our clinic during the period of January 1984 to February 1985, 39 patients were treated with oral methoxsalen (8-MOP) and UVA for more than 6 months, The following conclusions were obtained by analysis of the clinical histories and efficacy of PUVA treatment: 1. The mean age of 39 patients was 31 and that of onset was 21. An average of 1() years elapsed from the onset of the disease prior to visiting our clinic. The initial involvement was observed, in decreasing order of frequency, on the trunk, face and/or neck and extremities. 3, The repigmentation started most frequently from the trunk followed by the extremities, face and/or neck after treatment of 2 months and 14 irradiations on the average. 4, There was a direct relationship between the frequency and duration of treatment and its effectiveness. 5. The trunk showed the best response to the treatment, followed by the extremities; face and/or neck. The most resistant sites were the the hand and/or foot. 6. No specific relationship was found between the patient's age, duration of illness and responsiveness to the treatment. 7. The side effects were minimal and transient.

3.Ultrasonographic Findings of Breast Diseases During Pregnancy and Lactating Period.

PURPOSE: To evaluate ultrasonographic findings and usefulness in the diagnosis of breast diseases during pregnancy and lactati ng period. METHODS AND MATERIALS: The authors evaluated the ultrasonographic findings of 18 breast diseases during pregnancy and lactation retrospectively. The ultrasonographic examinations were performed with linear-array 5 MHz transducer (ATL). Final diagnoses were obtained by the excisional biopsy, fine needle aspiration and clinical follow-up. RESULTS: Total 18 cases of breast diseases were consisted of 8 cases of galactocele, 4 cases of fibroadenoma, 3 cases of axillary accessory breast, 2 cases of lactating adenoma, and I case of phylloides tumor. The ultrasonographic findings of the above breast diseases were valuable in the diagnosis and therapeutic planning. CONCLUSION: Ultrasonography is the initial and useful method of diagnosing breast diseases during pregnancy and lactating period.

4.A Case of Malignant Atrophic Papulosis (Degos' Disease).

A 31-year-old male patient had malignant atrophic papulosis that afected the skin only. He had multiple typical porcelain-white skin lesion on the face and trunk without abdominal symptoms. Histopathologic studies showed moderate inflammatory infiltration in addition to the typical cone-shaped region of necrobiosis, There are no satisfactory treatment modes, and in the present case, the patient's condition improved transiently on oral medication of aspirin, but we lost the contact with him afterwards.

5.Antenatal Diagnosis of Chorioangioma of the Placenta.

Since the placenta is an organ composed of blood vessels, it is not surprising that its primary neoplasm would be a vascular tumor. Placental tumors, primary or secondary, have been known to interfere with placental function. Chorioangioma(primary tumor of the placenta), which is the most common of them, occurs with an incidence for clinically significant ranges from 1~2.8:10000 births. These tumors are benign and are not usually associated with clinical sequelae unless they are larger than 5cm in long diameter. About one third of the large chorioangiomas may be associated with the maternal and fetal complications. For diagnosis of these lesions, the ultrasonography was used. If the chorioangioma is suspected, color doppler study is informative to confirm the presence of the vascular channels. We reviewed ultrasonograms and clinical records of seven patients who had been diagnosed as placental chrioangioma. The appropriate diagnostic tests and treatment can then be initiated in order to prolong gestation and decrease fetal mortality and morbidity.

The lesion showed typical findings of atypical fibroxanthoma and was located on the lesion of solar keratosis, So sunlight may be the most important one among the various insults to atypical fibroxanthoma of the skin.

10.Giatn Lymph Node Hyperplasia : Analysis of 17 Cases with Special Reference to 5 Cases of Plasma Cell Type.

This report describes the pathologic features of 17 cases of Castleman's disease, examined at the Department of Pathology, Seoul National University Hospital during a period from 1973 to 1989. The lesions in 12 cases were hyaline-vascular type and the remainders plasma cell type. The pathologic features favoring the plasma cell type over the hyaline vascular type included a sufficient number to large-sized follicles. However, a histologic overlapping between two types was present. In the hyaline vascular type the age of the patients ranged from 7 to 76 years and they appeared to be no particular sex predominence. The majority of the lesions occurred in the neck and within the chest. Almost all cases presented with a solitary mass except three cases. Neither conventional symptoms nor systemic manifestations were associated. The plasma cell type was characterized by presentation of constitutional symptoms, involvement of intra abdominal and inguinal lymphnodes, in association with unusual clinicopathologic features including IgA nephropathy, diabetes mellitus, systemic progressive sclerosis, peripheral neuropathy, and anemia. Immunohistochemical study was performed in three cases of the plasma cell type. Two cases revealed poly-clonal plasma cell infiltration. In a patient with IgA nephropathy, however, serum IgA was increase and a strong immunoreactivity to IgA heavy chain was found. Another case, associated with systemic progressive sclerosis and neuropathy, revealed monoclonal plasma cell infiltration (IgG and lambda light chain). The above results support a possibility that in some of the plasma cell type an altered immune mechanism is involved in its pathogenesis.
