1.A Study on Analysis of Mental Fitness, Perspective Taking, Attitude toward the Elderly, and Quantitative Encephalogram among Caregivers

Purpose:
This study aimed to identify the mental fitness, perspective taking, attitude toward the elderly, and quantitative encephalogram among caregivers.
Methods:
In this mixed method research, 14 female caregivers at an elderly care facility in Y city were participated from December 1st to December 31st, 2019. Structured questionnaires were applied to measure general characteristics, mental fitness, perspective taking, and attitude toward the elderly. Brain waves were measured by quantitative encephalogram in the prefrontal lobe. High frequency and high level of care tasks were analyzed using content analysis.
Results:
According to analysis of brain waves, caregivers experienced high level of physical strain & stress and psychological distraction & stress.Mental fitness (p<.05) and perspective taking (p<.001) were associated with attitude toward the elderly.Moreover, physical strain & stress was associated with psychological distraction & stress (p<.05, p<.001). High frequency and high level tasks were excretion and position change & movement care related tasks, respectively.
Conclusion
These findings should be consider as an evidence for supporting care tasks based on concrete and objective understanding of caregivers using self-reported index as well as neurophysiological indicators including brain waves.
2.Correlation Analysis for COVID-19 Stress, QEEG Stress Quotient, and Coping Style of Face-to-Face Service Industry Employees

Purpose:
This study aimed to measure COVID-19 stress and the quantitative electroencephalography (QEEG) stress quotient and identify the coping styles of face-to-face service industry employees during the COVID-19 pandemic.
Methods:
This cross-sectional study administered structured questionnaires consisting of sections on general characteristics, COVID-19 stress, and coping style for stress to 21 face-to-face service industry employees between April 1 and April 18, 2021. The physical tension & stress quotient and psychological distraction & stress quotient were measured in the prefrontal lobe with QEEG.
Results:
Emotional easiness (r=.62, p=.002) and escape-avoidance (r=.55, p=.009) as a passive coping style were associated with COVID-19 stress, and seeking social support as an active coping style was associated with the left physical tension & stress quotient (r=.47, p=.031).
Conclusion
These findings provide evidence regarding the objective status of the mental health of face-to-face service industry employees using both a self-reported scale and neuroscientific indicators, including brain quotients.
3.The Effect of Integrated Nursing using Cognicise and Neurofeedback

Purpose:
This study aimed to identify the effect of integrated nursing using cognicise and neurofeedback on cognition and ⍺ waves among elderly individuals.
Methods:
This quasi-experimental single group pretest-posttest study included 29 Korean adults aged 65 years or older who participated at a senior welfare center in Y city from February to May 2019. Each integrated nursing session consisted of cognicise (10 minutes) and neurofeedback (20 minutes). Subjects participated twice a week for 10 weeks. Structured questionnaires to assess participants' general characteristics and the Korean version of the Mini-Mental Status Examination for Dementia Screening (K-MMSE-DS) were applied. Alpha waves were measured using BrainMaster.
Results:
Following the integrated nursing program, alpha waves improved in F3 (t=2.41, p=.023), C3 (t=3.00, p=.006), C4 (t=2.60, p=.015), P3 (t=2.43, p=.022), O1 (t=2.30, p=.029), T3 (t=3.05, p=.005), T4 (t=2.28, p=.030), T5 (t=2.91, p=.007), Fz (t=2.30, p=.029), Cz (t=2.73, p=.011), and Pz (t=2.23, p=.034). Most subjects experienced improvement in concentration after participating in the intervention.
Conclusion
This study provides evidence for the use of a neuroscientific approach including brain wave measurement to improve cognitive health among community-living elderly people.
4.A case of epignathus tumor causing obstruction of upper airway immediately after birth.

Teratoma originate from one or more germ cell layer and commonly arise from sacrococcygeal area in neonate. Teratoma arising from the oropharyngeal cavity is called "epignathus tumor" and is extremely rare in neonate. Clinical manifestation of epignathus tumor vary from asymptomatic to severe respiratory distress symptom. It is reported that most of the tumor are benign in nature. Large teratoma can be diagnosed by prenatal ultrasonography, but most cases were diagnosed with computed tomography or magnetic resonance image after birth. Prognosis is determined by the need for neonatal resuscitation for respiratory distress at the time of birth and the extent of tumor, involving large vessles, skull base or communication with the brain. We experienced a case of epignathus tumor in a neonate with severe respiratory distress immediately after birth, so that reported with review of the literature.

Mature cystic teratoma of the ovary is the most common ovarian germ cell tumor and benign, but malignant transformation occurs in less than 2% of benign mature teratoma. Of the malignancies arising in teratomas, squamous cell carcinoma is the most common, however, carcinoid tumors or adenocarcinomas arising in mature cystic teratoma are uncommon, especially simultaneously. We present an unusual case of premenopausal woman having carcinoid tumor and mucinous adenocarcinoma simultaneously arising in ovarian muture cystic teratoma.

6.The Effect of Marital Satisfaction and Dysfunctional Attitudes on Married Women's Depression.

OBJECTIVES: This study was designed to examine the effect of marital satisfaction and dysfunctional attitudes on depression in married women. METHODS: Forty depressed married female patients and 34 non-depressed married women as normal control group completed three self-report questionnaires, Beck Depression Inventory(BDI) Dysfunctional Attitude Scale(DAS) and Dyadic Adjustment Scale(DYAS) Marital satisfaction and dysfunctional attitudes were compared by student t-test between the two groups. Also, relative importances of each variables to depression were examined with stepwise multiple regression analysis. RESULTS: Depressed patients reported significantly lower level of marital satisfaction and showed higher level of dysfunctional attitudes than normal controls. Dyadic satisfaction, approval need, and perfectionism subfactors were more important in predicting depressive symptomatology than other variables in patients. CONCLUSION: We conclude that marital satisfaction has significant influence on development and maintenance of depression in married women, and in underlying thinking process, dysfunctional attitudes serve as a cognitive vulnerability factor. This conclusion involves the expectation that if dysfunctional attitudes of depressed married women could be changed positively, their marital satisfaction will be increased to higher level and depression will be decreased. Overall, our findings stress that we need to consider the degrees of marital satisfaction and dysfunctional attitudes of depressed maried women, when treating them, in order to individualize treatments and optimize our ability to predict responsiveness to therapy.

7.Postprandial Hypertriglyceridemia Following a Single High-Fat Meal in Patients with Coronary Artery Disease and Normal Subjects: The Significance of the Postprandial Hypertriglyceridemia and the Effects of Fibrate on the Postprandial Hypertriglyceridemia.

BACKGROUND AND OBJECTIVES: It has been recently reported that coronary artery disease (CAD) is more correlated with postprandial triglyceride (TG) levels than fasting TG levels. We performed this study to compare the patients with CAD to age- and sex-matched controls in regard to postprandial TG levels and to know the effects of fenofibrate on postprandial TG levels. MATERIALS AND METHOD: Serum TG, total cholesterol (C), HDL-C and LDL-C were measured before, and 2, 4, 6, 8, and 24 hours after a high-fat meal in 22 patients (mean: 60 yr) with CAD and 12 normal subjects (mean: 54 yr). The same parameters were also serially measured after the high-fat meal plus fibrate in 10 patients with CAD (mean: 59 yr). RESULTS: he patients group without fibrate showed that more prolonged and exaggerated hypertriglyceridemia following the meal than normal subjects, especially 4 to 8 hours after the meal and that lower HDL-C throughout the test duration. These changes were also persisted when hyperlipidemic patients were excluded out of the patients group. The patients with fibrate did not show such a significant elevation of TG levels 4 to 8hours after the meal compared when normal subjects. The time to reach the peak TG levels after the meal was 4, 6, and 4 hours after the meal in normal subjects, patients with CAD, and fibrate group, respectively. CONCLUSION: Coronary artery disease is clearly related with postprandial hypertriglyceridemia than fasting TG levels and postprandial hypertriglyceridemia can be somewhat prevented by fibrate.

8.A Study of Susceptibility between Allergic Rhinitis and Glutathione S-Transferase Gene Polymorphism Study in Korean.

BACKGROUND AND OBJECTIVES: Glutathione S-transferase (GST) enzymes have an important role in preventing the build-up of reactive oxygen species. Polymorphisms in genes involved in response to oxidative stress may play a role in the susceptibility to allergic diseases in human. A common homozygous deletion(null type) polymorphism of the GST gene abolishes the antioxidative enzyme activity. We investigated whether the profile of GSTM1 and GSTT1 genotypes might be associated with the risk of allergic rhinitis. SUBJECTS AND METHOD: Blood samples for genetic analysis were obtained from 287 individuals with allergic rhinitis and from 262 healthy subjects without atopic diseases. Multiplex polymerase chain reaction-based assay for GSTM1and GSTT1 was used for genotyping. RESULTS: The null genotype was more frequent in controls and the frequencies of the genotypes of GSTM1 were statistically different between controls and patients (p<0.05). The null genotype was more frequent in controls, but there were no differences in the frequencies of the genotypes of GSTT1 in both groups (p>0.05). CONCLUSION: Our result suggests that the GSTM1 and GSTT1 polymorphism is not associated with the susceptibility to allergic rhinitis in Koreans.

9.Clarithromycin resistance and female gender affect Helicobacter pylori eradication failure in chronic gastritis

BACKGROUND/AIMS:
The eradication rate of the first-line triple therapy (a proton pump inhibitor, clarithromycin, and amoxicillin) for Helicobacter pylori infection has gradually decreased in Korea. We evaluated whether clinical parameters, clarithromycin resistance, and CYP2C19 genotype can affect the eradication failure.
METHODS:
A total of 203 patients with H. pylori-positive chronic gastritis were consecutively enrolled. They received clarithromycin-based triple therapy for 7 days. A clarithromycin resistance test was performed by detection of A2142G and A2143G point mutations in H. pylori 23S rRNA. The CYP2C19 genotype was examined for polymorphism G681A of exon 5 and G636A of exon 4 by polymerase chain reaction with restriction fragment length polymorphism. Eradication was assessed by a ¹³C-urea breath test 4 weeks after treatment.
RESULTS:
Of 203 patients, 190 completed the study. The eradication rate was 64.0% according to intention-to-treat analysis and 68.4% by per-protocol analysis. CYP2C19 genotypes were identified as follows: 75 poor metabolizers, 75 intermediate metabolizers, and 40 rapid metabolizers. Nonetheless, this polymorphism was not significantly associated with eradication failure (p = 0.682). Clarithromycin resistance was detected in 33/190 patients (17.4%), and their eradication rate was zero. Clarithromycin resistance (odds ratio [OR], 19.13; 95% confidence interval [CI], 9.35 to 35.09) and female gender (OR, 1.73; 95% CI, 1.15 to 4.25) were significantly associated with eradication failure. The other clinical parameters such as age, cigarette smoking, alcohol intake, the body mass index, hypertension, and diabetes were not significantly associated with eradication.
CONCLUSIONS
Clarithromycin resistance and female gender are factors affecting H. pylori eradication failure in patients with chronic gastritis.
10.The Effect of Green Tea on Endothelial Function and the Circulating Endothelial Progenitor Cell in Chronic Smokers.

BACKGROUND AND OBJECTIVES: Circulating endothelial progenitor cells (EPC) with an endothelial phenotype contribute to the regeneration and repair of arteries. The number of circulating EPCs has an inverse correlation with chronic smoking and endothelial dysfunction. Green tea cathechin many improve endothelial dysfunction. The effect of green tea cathechin on the number of circulating EPCs and the endothelial dysfunction in chronic smokers is not known. Subjects and METHODS: In 20 young healthy smokers (27.6+/-3.6 years, all male), the endothelial functions that were defined by flow-mediated endothelium dependent vasodilation (FMD) of the brachial artery, as well as the number of EPC isolated from peripheral blood, were determined at baseline and also at 2 weeks after taking green tea (8 g/day). The circulating EPCs were quantified by flow cytometry as CD45low CD34+ KDR2+ cells and as acyl-LDL and FITC-lectin double positive cells after culture for 7 days. RESULTS: The changes of the clinical characteristics and the laboratory findings were not different between baseline and at 2 weeks after green tea intake. The EPC levels were inversely correlated with the number of smoked cigarettes. Circulating EPCs, as determined-by flow cytometry, and the cultured EPCs increased rapidly at 2 weeks after green tea consumption (78.6+/-72.6/mL vs. 156.1+/-135.8/mL, respectively, p<0.001; 118.2+/-35.7/10 field vs. 169.31+/-58.3/10 field, respectively, p<0.001). The FMD was significantly improved after 2 weeks (7.2+/-2.8 vs. 9.3+/-2.4, respectively, p<0.001). The FMD was correlated with the EPC count before treatment (r=0.67, p=0.003) and after 2 weeks (r=0.60, p=0.013). CONCLUSION: The number of circulating EPCs and the FMD are reduced in chronic smokers. Green tea induces rapid improvements of the EPC levels and the FMD. A short-term of consumption of green tea may be effective for reducing the cardiovascular risk in chronic smokers.
