1.Accessory Hepatic Vein: MR Imaging.

PURPOSE: To evaluate the MR appearance of the accessory hepatic veins. MATERIALS AND METHODS: The study included 87 consecutive patients for whom abdominal MR images were obtained. The subjects who had liver lesion or hepatic vascular abnormalities were excluded. Couinaud classified accessory hepatic veins into inferior and middle right hepatic veins. Our major interests were evaluation of the incidence, morphology, and location of the accessory hepatic vein. RESULTS: Inferior right hepatic vein was demonstrated in 43 out of 87 patients(49%). The morphology was linear in 35 patients(80.5%), and V-shaped in 8 patients(19.5%). In 40 patients(93%), the inferior right hepatic vein was located in the posteroinferior aspect of the right lobe. Middle right hepatic vein was demonstrated in 7 out of 87 patients(8%). All were single linear in morphology, combined with the inferior right hepatic vein, and located between the right hepatic vein and inferior right hepatic vein. CONCLUSION: The accessory hepatic vein was demonstrated in 49% among the Korean adult population, and was located in posteroinferior portion of the liver, in 93%.

4.Clinical Trials of Frason on Various Dermatosis.

Esperson(R), a new topical steroid ointment containing 0. 05% 17-Desoxymethasone,. was used in several kinds of dermatoses. Framycetin which was isolated and uientified by the research organisation of Roussel Laboratories (SOFRAPEN) in 1950 from culture broths of a strain of Streptomyces species is a wide spectrum antibiotics on various bacterial infectious diseases. Total 50 patients including 10 cases of 2ndary infected Atopic dermatitis, 10 of infected Contact dermatitis, 6 of infected Seborrheic dermatitis, 7 of infected Neurodermatitis, 3 of infected Chronic dermatitis, 5 of lmpetigo, 2 of infected Nummular eczema, 2 of infected Infantile ecze;na, 1 of infected Intertrigo, 1 of Eczematoid Otitis extema, 1 of 2nd degree Bum, 1 of infected Pruritis ani, 1 of infeeted 1nsect bite. were treated with Frason(R) (0.05% 17-desoxymethasone & 0.5% Framycetin combination). Among these 50 patiants, we observed excellent effects in 7 patients (14%), good effect in 29 patients (58%), fair effect in 10 patients (20%), Totally 36 patients (72%) revealed excellent to good therapeutic effects on their skin conditione after unoccluded application of Frason(R). 3 times daily for about 14 days. There was no sid ffect in all subjects after application of Esperson-F.

5.Diffuse Cavernous Hemangioma of the Rectosigmoid Colon: a Case Report.

Diffuse cavernous hemangioma of the rectosigmoid is a rare lesion usually presented in children and young adult, often with life threatening hemorrhage. The clinical diagnosis is difficult and often delayed because of lack of awareness of the clinical features. We report a case of diffuse cavernous hemangioma of the rectosigmoid in a 12-year-old boy who had undergone suture ligation under the impression of hemorrhoid at his age 3. Sphincter-saving coloanal pull through procedure were applied as the surgical treatment. Sclerotherapy was needed with recurrent rectal bleeding 6 months after the pull through operation. The patient is well at 12 months follow-up.

6.Osteotomy of the Spine to Correct the Spinal Deformity.

There are a number of reports on Smith-Petersen osteotomy (SPO), pedicle subtraction osteotomy (PSO) and vertebral column resection (VCR). However, there are few systematic reviews of all three kinds of osteotomies. Literature review and author's experience of SPO, PSO and VCR osteotomy will be described. Various surgical techniques can be applied according to the disease entity and magnitude of the deformity. The most appropriate methods for deformity correction should be chosen and the potential complications should be considered. Before attempting an osteotomy of the spine for a spinal deformity, sufficient surgical experience and a thorough understanding of the anatomy of the spine and adjacent structures are needed. In addition, a well-organized team with the other departments is essential.

7.Gouty Spondyloarthropathy in the Lumbar Spine Mimicking Pyogenic Discitis : A Case Report.

A 55-year-old-male patient had acute lower back pain and radiculopathy with high spiking fever, which indicated pyogenic discitis. Intravenous antibiotics were administered but a high spiking fever and elevated laboratory findings indicated an infection. Therefore, surgery was performed. However, histological diagnosis revealed tophaceous gout. A diagnosis of spinal gout should be considered when there are clinical presentations of acute back pain and fever, especially in patients with acute back pain and a prior history of hyperuricemia or gout.

8.Association of Estrogen Receptor 2(ESR 2) Gene Polymorphisms with Ossification of the Posterior Longitudinal Ligament of the Spine.

STUDY DESIGN: Genetic screening of the estrogen receptor 2 (ESR2) genes in patients with ossification of the posterior longitudinal ligament (OPLL). OBJECTIVE: We studied the relationships between ESR2 gene polymorphisms and OPLL to understand the pathophysiology of OPLL. SUMMARY OF LITERATURE REVIEW: The OPLL has a strong genetic component. Several familial surveys and human leukocyte antigen (HLA) haplotype studies reveal that genetic background is an important component in the occurrence of OPLL and a large number of gene analysis studies were utilized to clarify the susceptible gene for OPLL, including COL11A2, BMP-2, TNF-alpha, NPPS, leptin receptor, transforming growth factor (TGF)-beta, Retinoic X receptor, ER, IL-1, PTH, and VDR have been performed. MATERIALS AND METHOD: Genomic deoxyribonucleic acid (DNA) samples obtained from 164 patients (93 men and 71 women) with OPLL and 219 control subjects, without the disease (105 men and 114 women) were amplified by polymerase chain reaction, and polymorphism genotypes were determined by the restriction endonuclease digestion. The distribution of genotypes was compared between the patients with the disease and the control subjects. RESULTS: The polymorphism of ESR2 [rs1256049, exon6, Val328Val, p=0.018, odd ratio (OR)=2.41, 95 confidence interval (CI)=1.15-5.02 in the recessive model] only showed statistically significant association between the control and the OPLL groups. The rest SNPs of ESR2 did not show any significant differences between the control and the OPLL groups. CONCLUSIONS: Estrogen receptor 2 (ESR2) gene polymorphisms (rs 1256049) was associated with OPLL. In future studies, we will perform target SNP chip between OPLL and candidate gene.

9.The Clinical Investigation of Pediatric Caudal Anesthesia .

Many studies have cited occurrences of severe and long-lasting emotional disturbances of almost every kind after surgical procedures, particularly due to the sedatives or narcotics which were administered to control the post-operative pain. In order to control this post-operative pain, pediatric caudal analgesia was performed in 15 infants and children from the age 2 months to 6 years, who were to undergo lower abdomen, perineum and lower extremity surgery. They were induced with inhalation anesthoeia, using nitrous oxide, oxygen and halothane by mask before and during the block. Following the block, infants and children were turned to supine position and the initial concentration of halothane was reduced. All cases were given 10mg/kg of 1% solution of lidocaine. On examination of the level of the analgesia, 13 out of 15 cases had spread higher level than T10, with effective block for surgical procedures. The identification of the sacral cornu is easy and technical failureis lees in comparison than with the lumbar epidural block for infants and children. Thus we can perform this block successfully for infants and children without any complications or accidents. It is especially helpful in the management of postoperative cases due to the effect in control of the postoperative pain. Thus it gives satisfaction to their parents, nurses and doctors. Despite no follow-up study on postoperative emotional reactions, it was considered that there were minimal occurrences of emotional and behavioral sequelae following the hospital and surgical experiences.

10.Clinical Results after Corrective Osteotomy of the Kyphotic Deformity in Ankylosing Spondylitis.

STUDY DESIGN: A retrospective analysis of clinical results through questionnaire based instrument and radiologic changes after correction in ankylosing spondylitis. OBJECTIVES: To evaluate the radiographic changes and verify the clinical results of the extension osteotomy through pedicular subtraction and intracorporal decancellization in patients with ankylosing spondylitis who have severely fixed kyphotic deformity. MATERIALS AND METHODS: Total 45 patients were included in this study. In all cases, the deformity was corrected by a onestage pedicle subtraction extension osteotomy. Radiographic assessment for sagittal balance was performed on the criteria of thoracic kyphosis, lumbar lordosis, distance between the vertical line on anterosuperior point of T1 and that of S1 and sacrohorizontal angle. Finally clinical assessment for function, indoor activity, outdoor activity, psychosocial activity, pain and overall subjective satisfaction on the basis of patients' satisfaction(5 grade) were performed. RESULTS: The osteotomy was performed at single level in 43 cases, double levels in 2 cases. The mean follow-up was 36.2 months. The criteriae of radiographic assessment were changed on preoperative, postoperative 3 months and the last follow-up as follows; thoracic kyphosis: 50, 51 and 54 degrees, lumbar lordosis: 10, 46 and 44 degrees, distance between vertical lines of T1 and S1: 94, 15 and 8 mm, sacral inclination: 8, 28 and 24 degrees. At the last follow-up, clinical results were high as 3.2 in function, 2.8 in indoor activity, 3.3 in outdoor activity, 3.3 in psychosocial activity, 3.4 in pain and 3.2 in overall subjective satisfaction. There was no any serious complication, such as death or permanent neurologic deficit. CONCLUSIONS: Nearly all the patients have maintained good correction. Eighty five percent of patients have revealed self-satisfaction to corrective osteotomy. But in connection with the clinical results, the changes of parameters of radiographic assessment didn't revealed any significant correlation and so it should be traced much longer with care.
