4.Antihypertensive Effect of Debrisoquin Sulfate in the Treatment of Hypertension.

Authors observed the antihypertensive effect of debrisoquin sulfate in 35 cases of essential hypertension and following results were obtained. 1) Results of short term control of blood pressure with debrisquin in 35 hypertensive patients revealed good control in 74.3%, fair in 14.3%, poor in 5.7% and failure in 5.7% of cases. Significant reduction of blood pressure was achieved in 7 of 9 cases of hypertension with addition of hydrochlorthiazide to debrisoquin. 2) During long term therapy with debrisoquin (duration of average 3.9 months), good control cases were slightly reduced(good in 58.6%, fair in 24.1%, poor in 6.9%, failure in 10.3%). But average effective dose of debriioquin was not increased significantly. 3) Antihypertensive effect of debrisoquin was slightly reduced in the cases of severe hypertension, hypertension of over 10 years duration and complication of cerebrovascular accident. 4) The side effects during treatment with debrisoquin were dizziness in 31.4%, headache in 11.4%, dry mouth, blurred vision, general weakness, insomnia in 8.6% respectively. There were no side effects in 48.6% of cases.

6.Clinical Studies on Idiopathic Myocardiopathy.

Twenty-two cases of idiopathic myocardiopathy were observed during the period of 1962 to 1971 and follow up clinical study was made in 5 cases. The criteria of diagnosis was based mainly on exclusive diagnosis in etiology unknown marked cardiomegaly. 1) Male and female ratio of idiopathic myocardiopathy was 1.1:1 and age of onset were distributed from first decade to fifth decade with similar number of cases. 2) Cardinal symptoms were dyspnea, palpitation, chest pain and cough. Common physical findings were protodiastolic gallop (in 2 cases), moist rales (in 3 cases) and hepatomegaly(in 3 cases). 3) Laboratory findings were normal except elevated T.T.T. in one case. 4) Electrocardiogram were abnormal in all cases. There were 3 cases of left ventricular hypertrophy, 3 cases of non-specific ST-T changes and one case of first degree A-V block, intraventricular conduction defect, abnormal Q wave and low valtage, respectively. 5) After medical treatment, symptoms were improved in 4 cases but heart size was reduced in only one case and E.C.G. abnormalities were unchanged in all cases.

7.The Treatment of Scaphoid Nonunion

Fracture of the scaphoid constitute 60% to 70% of all diagnosed carpal injury. The acute fracture will heal approximately 90% of the time if recognized early and properly immobilized. But nonunion are common since the symtoms do not alert patients to seek early medical treatment and the diagnosis is easily missed. Authors have experienced 19 cases of scaphoid nonunion and accomplished good result in all case by Russe procedure. The results were summerized as follow: 1 The cause of fratures was mainly due to fall down dinjury (36.8%). 2. The most common mechanism of the fracture was fall on the outstretched hand (52.7%). 3. Fractures were shown on the anterior-posterior, lateral and billiards view in all cases. 4. In ten cases, the fractures were found on the waist of scaphoid. 5. The cases of nonunion were probably inadequate intial treatment and delayed diagnosis. 6. The good results were obtained by bone graft according to Russe procedure.

8.The Significance of the Myelography in the Herniation of Intervertebral Disc

The herniation of intervertebral disc is possibly diagnosed through the myelography, electromyography, discography or computerized tomography. Among them, the myelography is regarded as the popularized method that helps accurate analysis on the location and pattern of the lesion. The authors have studied 363 cases of herniated intervertebral disc with respect to the diagnostic accuracy of myelographic examination from January, 1982 to December, 1985 at the Department of Orthopedic Surgery and Radiology, National Police Hospital. The results were as follows; 1. The most common age group was 21 to 30 years old and ratio of male and female was 9.7: l. 2. The most common site of the lesion was between L4 and L5 vertebra in myelography. 3. Of the 363 cases which underwent the myelography, 305(84.0%) represented positive findings. 4. Myelographic accuracy was coincided with the clinical diagnosis regarding the location of herniation of intervertebebral disc in 262 cases(72.1%).

9.Percutaneous Epiphysiodesis

Leg length discrepancy in the growing children was developed by multiple causes; such as congenital, infection, traumatic, and mechanical facotrs etc. The discrepancy have been treated by several methods including bony lengthening which is preferable method, but epiphysiodesis also has been one of the valuable methods in which was applied to the treatment of mild to moderate leg length discrepancy in the growing children. We report on patients(15 cases) with leg length discrepancy treated by percutaneous epiphysiodesis since July 1986. There were 8 males and 7 females and the average age of them was 12.6 years old. And we also performed an experimental study which was to find out the effectiveness of the pereutaneous physeal destruction comparing the techningues between curette and drill in the bony models. The causes of leg length discrepancy were trauma in 8 cases and osteomyelitis in 2 cases, congenital dislocation of the hip in 2 cases, poliomyelitis, Legg-Galve-Perthes disease, congenital hemihypertropy in each 1 case. The average preoperative leg length discrepancy was 2.6cm(range:1.9-3.7cm), and average anticipated discrepancy without operation have been 3.7cm(range:2.2-6.3cm) at maturity. The final discrepancy was 0.9cm after 43 months follow-up. According to the result by experimental study, percutaneous epiphysiodesis with curette technique would be more effective and safe than drill thchnique. Percutaneous epiphysiodesis might be one of the useful methods for the treatment of mild to moderate degree of leg length discrepancy in the growing children, and also could be applied combining with bony lengthening for treatment of severe degree of leg length discrepancy.
