2.Antibacterial Activity of Ceftizoxime Against Gram Negative Enteric Bacteria in vitro and in vivo.

Ceftizoxime sodium is a new synthetic β-lactam antibiotic combining potent antibacterial activity with high stability to a wide range of bacterial β-lactamase. This experiment was achieved to evaluate the antibacterial activities of ceftizoxime sodium against. Gram negative enteric bacteria isolated from in outpatient visiting Yeungnam university hospital and to study the emergence of drug induced bacterial variants which resist to ceftizoxime in vitro. The antibacterial activity of the ceftizoxime was compared with that of antibiotics and its effect on population of normal intestinal flora in mice was observed. The results are summarized as follows: 1. Highly effective antibacterial activity of ceftizoxime against Gram negative enteric bacilli was demonstrated and this antibacterial activity was superior to that of ampicillin. 2. Several test strains shows multiple antibiotic resistance. Among 15 strains of Escherichia coli, 1 strain was resistant to ampicillin, cefadroxil, gentamicin, tetracycline, and 2 strains were resistant to ampicillin, cefadroxil, tetracycline, five strains of Escherichia coli and Enterobacter cloacae was resistant to ampicillin, tetracycline and Shigella dysenteriae was resistant to ampicillin, gentamicin, tetracycline. 3. The frequency of in vitro emergence of resistant variants among ceftizoxime sensitive bacteria in the presence of increasing concentrations of the compound was found to be low. 4. Plasmid was isolated in 6 of 9 strains (6 strains of Escherichia coli, Shigella dysenteriae, Enterobacter cloacae and Salmonella typhi). That showed different antibiotic resistance. They were 5 strains of Escherichia coli and 1 strain of Shigella dysenteriae. However, plasmid could not be considered as a hallmark for antibiotic resistance by this Further studies with curing experiment are to be accomplished for this purpose. 5. Changes in the bacterial count of normal intestinal flora following 25 mg/kg/day administration of ceftizoxime over 5 consecutive days were not significant. In conclusion, ceftizoxime appeared to be a drug of choice in the treatment of Gram negative enteric bacilli infection.

3.Chromosomal Vulnerability to gamma-irradiation of Peripheral Blood Lymphocytes in Patients with Various Cancers.

BACKGROUND: Human cancel can be induced by various environmental factors such as virus, chemicals, and radiations. However, susceptibility of host to these environmental factors is not well studied. The purpose of this study was to investigate the chromosomal vulnerability to gamma-1rradiation of peripheral blood lymphocytes in patients with luring, gastric, and liver cancer. METHODS: Micronuclei (MN) test was done in 15 patients with gastric cancer, 18 patients with lung cancer, and 20 normal controls. Sister chromatid exchange (SCE) test was done in 13 patients with hepatocellular cancer and 14 normal controls. RESULTS: The baseline frequency of MN before irradiation in patients with lung cancer and gastric cancel was significantly higher than in controls (47+/-8, 73+/-9, and 8+/-l, respectively; p<0.01). After gamma-irradiation, the frequency of MN was also significantly higher than in controls (223+/-19, 269+/-43, and 285+/-56, respectively; p<0.05). The frequency of SCE in Patients with hepatocellular cancer was significantly higher her than in controls (9.0+/-0.6, 4.3+/-0.7, respectively; p<0.001). CONCLUSIONS: From this preliminay data, we concluded that chromosomes of peripheyal blood lymphocytes in patients with various cancers were more vulnerable to gamma-irradiation as compared to normal controls.

Choriocarcinoma associated with a normal pregnancy is rare. Futhermore, choriocarcinoma coexistent with a viable pregnancy is even rarer and associated with a greater risk of hepatic and cerebral involvement. So timely diagnosis of the disease is important for successful treatment and aggressive diagnostic procedures may therefore warranted. The patient should be treated with primary intensive combination chemotherapy(EMA-CO) and the selective use of irradiation and surgical therapy. We experienced a case of metastatic choriocarcinoma of lung and kidney following preterm delivery at 34th week of pregnancy and so present it with brief review of literature.

6.Study on the Effects of the Family Support in the Very Low Birth Weight Infants Follow-Up: Focus on Dodam Dodam Bring-Up Center.

PURPOSE: After discharge from the hospital, very low birth weight infants require care consultation, information, and interaction and so forth, due to sequelae, different growth and developmental pattern; which increase the burdens and the worries of patients in nurturing them. With the counseling of experts in the related fields, the authors and the Food for the Hungry have run a department of childcare for a year, emphasizing on more family support than extreme early detection of disabilities. This study reports on relieving child care anxiety, promoting growth and development, educating parents on home treatment of sequelae, finding and coping with minor disabilities. METHODS: The subjects were chosen among infants under 1,500 g of birth weight and younger than a corrected age of 6 months. Meetings were held once in a month for a year. Infant care support classes were taught by experts in their fields including a music therapist (with play program), neonatologist, a children's nurse, a child-care director, a clinical psychologist, a rehabilitation therapist, a social worker and, a nutritionist. Within the self-dependent groups, the families were able to share their concerns and experiences on child-caring. Second home visits were carried out to monitor home oxygen therapy, tube feeding, cerebral palsy signs, and to find neglect or abuse in suspected cases. RESULTS: Fifty-one infants participated; the average weight was 1,060 g, the average gestational age was 27 weeks and 2 days. Eighteen were boys and 23 girls. The average age of the mothers was 33.4, and there were 26 appropriate for gestational age, 8 small for gestational age, and, 5 large for gestational age. Twenty three were first-born, 13 were second-born, 1 was third-born, and twins were 4 pairs. Home visits were done in 33 families, and secondary visits were done in 6 families. The depression score of mothers in the attendant group decreased from 10.47+/-5.18 to 8.18+/-5.87 (P=0.080). CONCLUSION: The depression score of mothers in the attendant group decreased after infant care support classes.

7.Radical abdominal trachelectomy with pelvic lymphadenectomy: Report of 2 cases.

Traditionally, radical hysterectomy is the main surgical method for the treatment of early cervical carcinoma and always results in the loss of fertility. But, large numbers of young women are recently being diagnosed with cervical carcinoma and fertility preservation has become a concern. So, there has been a move towards more conservative approaches for the treatment of cervical carcinoma in recent years. Radical trachelectomy, which allows preservation of uterus but removes the cervix, parametrium and upper one third of the vagina, is a conservative but locally radical procedure. We performed radical abdominal trachelectomy with pelvic lymphadenectomy for two cases of invasive cervical carcinomas, which may be the first report in korea. One patient was 37 years old single women who had stage Ib cervical cancer and was disease free for 17 months after treatment. The other patient was 19 years old student who was also single and had stage IIa cervical cancer and was disease free for 14 months after treatment. We report the first two cases and review the literature on radical trachelectomy.

8.MR imaging of internuclear ophthalmoplegia due to cerebrovascular diseases.

Internuclear ophthalmoplegia is a conjugated gaze disorder characterized by impaired adduction on the side of a lesion involving the medial longitudinal fasciculus with dissociated nystagmus of the other abducting eye. Six patients with INO (who had clinical cerebrovascular diseases) underwent MR imaging and the results were as follows: 1. The MLF lesions were identified by MR imaging in 5 cases 2. The ratio of unilateral INO to bilateral INO was 5:1 3. The nature of lesions was infarction in 4 cases and hemorrhage in 1 case 4. The sites of MLE lesion were in the midbrain in 4 cases and in the pons in 1 case 5. All 5 cases of INO identified by MR imaging had other lesion sites in addition to MLE lesion.

9.Task Analysis of Korean Transplantation Nurse Practitioner.

PURPOSE: This study was designed to create the job description of Korean transplantation nurse practitioner and examine performance frequencies, criticality, and difficulties of task elements. METHOD: The sample consisted of 63 nurses and coordinators who performed duties related to transplantation at medical center in Korea. A survey method was used, and the questionnaire included frequencies, criticality, and difficulties of task elements in job description by the DACUM method. Using SPSS WIN 10.0, descriptive statistics such as frequency distribution, means, and standard deviations were conducted to examine the subject's general characteristics, the frequencies, criticality, and difficulties of task performance. RESULT: The job description of transplantation nurse practitioners revealed 5 duties, 22 tasks, and 85 task elements. On the all five duties, the averages of the performance frequency, criticality, and difficulty were 2.41, 3.38, and 2.78, meaning that the respondents rarely perform the 5 duties, but consider them critical and easy to perform. CONCLUSION: The job description of the transplantation nurse practitioner included duty, task, and task element and definition of job completed. Thus we recommended a data based trial to confirm and validate the information gathered.
