4.Detection and Subtyping of Epstein-Barr Virus in Gastrointestinal Adenocarcinomas and Malignant Lymphomas.

Epstein-Barr virus (EBV) has been linked to a spectrum of neoplastic conditions, including Burkitt's lymphoma, nasopharyngeal carcinoma, Hodgkin's disease, lymphoepithelioma-like carcinomas and malignant lymphomas in immunocompromised state. To determine the prevalence and the subtype of EBV in gatrointestinal malignancies, fifty cases of adenocarcinomas and seventeen cases of malignant lymphomas were analyzed by EBERs in situ hybridization and polymerase chain reaction using primers for EBNA-1, EBNA-2A and EBNA-2B, on the paraffin sections. In addition, immunohistochemical stain for p53 protein was performed to investigate the potential role of EBV infection on tumor suppressor gene, p53, during tumorigenesis. EBER was detected in 6 of 26 gastric adenocarcinomas, 2 of 24 colon adenocarcinomas, and 8 of 17 malignant lymphomas. EBER was more prevalent in malignant lymphoma arising in the intestine (6/6) than in the stomach (2/11), and was detected in both B and T cell phenotypes. EBNA-1 was positive in 11 of 16 EBER positive cases and the subtyping was possible in 8; both type 1 and 2 were detected in gastric cancers, whereas only type 2 was found in intestinal neoplasms. In adenocarcinomas the high rate of p53 protein overexpression was found in both EBER positive (8/8) and negative cases (32/42), whereas the positive rate was higher in EBER positive cases (7/8) than in EBER negative cases (4/9) of malignant lymphomas. From the results, it can be concluded that EBV infection and the p53 tumor suppressor gene are independently associated in a significant portion of the gastrointestinal malignancies, but the mechanism of action remains to be elucidated.

5.The Experience of Concurrent Review of blood Transfusion Requests.

Blood utilization review has recently been used to minimize inappropriate transfusion, reduce post transfusion infections and improve the quality of transfusion practices. Ajou University Hospital is a 906 bed tertiary care teaching institution. Since March, 1995, the concurrent review of blood transfusion requests using institutional blood transfusion criteria has been initiated. To evaluate the effectiveness of the transfusion review, the units of blood components transfused per patient(total number of units of blood components transfused/total number of patients discharged) and inappropriate transfusion ratio of blood components(total number of units of blood components inappropriately transfused/total number of units of blood components transfused) were compared during the concurrent review for 9 months and pre-concurrent review for 9 months, respectively. The possible savings of blood components realized by the review were also extrapolated. The results were as follows : 1. The unit transfused per patient for pre-review and during review were 0.675 (9,080/ 13,422) and 0.654(12,123/18,546) for packed RBC(PRBC); 0.417 (5,602/ 13,422) and 0.296 (5,444/18,546) for fresh frozen plasma(FFP); 0.372(5,007/13,422) and 0.424(7,868/18,546) for platelet con-centrates(PC), respectively. 2. The inappropriate transfusion ratio of blood components for pre-review and during review were 0.039 and 0.009 for PRBC; 0.321 and 0.064 for FFP; 0.143 and 0.008 for PC, respectively. These differences were statistically significant(p=0.000). 3. The PRBC and FFP anticipated to have been saved by this review was 519 units and 2,992 units annually, respectively. 4. However, 2,621 units of PC were used additionaly during this review due to increased number of oncology patients and better hemostatic therapy practiced during the same period. In conclusion, the concurrent review of transfusion requests was very effective tool to educate residents in clinical departments the transfusion medicine and improve utilization of blood and its components.

6.Radiologic study of silicosis in Korean

These radiologic studies were carried out on 265 cases of silicosis which were diagnosed clinically atindustrial Accident Hospital affiliated with Catholic Medical Collage, suring the period of 3 years from 1974 to1976. 265 cases of silicosis consisted of 96 cases (35%) of coal miner, 91 cases (34%) of coal choicer and 82cases (31%) of others. The average age was 42.2 years and average working period was 9.2years. Qualitative andquantitative features in the analysis of roentgen findings were based on UICC/Cincinnati and KLO classification.The qualitative features showed 26% of "p" opacity, 46% of "q" opacity 4% of "r" opacity as rounded profusion and5% of "s" opacity, 11% of "t" opacity, 2% of "u" opacity as irregular profusion of small opacities.Large opacitiesshowed 3% of group A, 2% of group B and 1% of group C. Quantitative features revealed 25% of Type 1, 54% of Type2, 16% of Type 3 and 5% of Type 4. The qualitative and quantitative features showed significant differences as ageand working period increased with age and working period. Major complications were pulmonary emphysema,tuberculosis and pleural abnormalities.

7.A Case of Advanced Gastric Cancer, Developed from Endoscopic Diagnosis of Early Gastric Cancer.

Early gastric cancer is defined carcinoma that is limited to the gastric mucosa and submucosa and that does not extend into the muscularis propria. The development and widespread use of upper gnstrointestinal endoscopy with biopsy and cytology have enhanced our ability to make early diagnosis of gastrie carcinoma. Gastric cancinoma is being diagnosed at an ealry stage aad the survival rates for these patients are significantly greater than those with advanced gastric carcinoma. These ealry lesion can be cured in over 90% of the cases. In fact, it is almost impossible to follow up cases of early gastric cancer without surgical treatment. We report a case developed advanced gastric carcinoma 34 months after initial endoseopic diagnosis of early gastric cancer. A, 48-year-old woman was diagnosed early gaatrie cancer on June 21, 1985 by endoscopy. Proper surgical intetvention was not carried out due to her liver cirrhosis. The follow-up endoscopic examination was done on March 16, 1988. The gastric cancer looked like an advanced stomach and extended to other site.

8.The Successful Dissolution of 2 Cases of Fat Bezoars by Pancreatic Lipase.

Gastric bezoars are large masses or concretions, composed with plant fiber, hair and miscellaneous foreign bodies, found in the stomach and occur usually as sequele of gastric surgery. We experienced 2 cases of fat bezoars which developed after drinking of melted beef fst. They had postprandial epigastric fullness and pain, those were relieved on supine and left recumbent position. Upper gastrointestinal series showed a large movable mass in the stomach in hoth cases. On gastrofibercopic examination, a large egg sized white bezoar was cordirmed in both cases and a gastrie ulcer at antrum associated in one case. In attempt to removal of the bezoars, we tried to break them, but failed because of its character. The patients were treated with pancreatic lipase containg digestives, Pancreon-F and Azintal. From one day after this treatment, symptoms were completely relieved. Five days later, we confirmed disappearance of bezoars by gastrofiberscopy. and upper gastrointestinal barium

9.The Effect of Cimetidine in Treatment of Erosive Gastritis.

In an attempt to evaluate the efficacy ef cimetidine in treatment of erosive gastritis, a praspective study wras done in 9 cases of heworrhagie gastritia, 4 cases yunctiform gastritis, 9 caees verrueous gastritia, and 9 caseis mixed type, Four hundred mg of cimetidine was given in the morning and at bedtime for 3 weeks. Evaluation ef the efficacy was assessed by clinical syinptoms acoring and by endoscopic findings of erosion before treatment, on the 8th day, on the 15th day, and on the 22th day of treatment in all patients. The results were as follows; 1) Clinical sytnptoms in patients with erosive gastritis were epigastric discamfortness(83. 9%), heart burn(80. 0%), indigestion(77, 6%), hunger pain(74.2%), belching(71.0%), anotexla (48.4%) and nausea(45.5%). There was no differenees of symptoms among the type of erosion. 2) Percent decrease in clinical eyxeptoms were; 45% on the 8th day, 68% on the 15th day, 81% on the 22th day of treatment. Ciinical symptoms in patients with punetiform gastritis lasted longer than other typea. 3) Disappearance of the gastric mucossl erosion was seen in 8 cases of hemorrhagic gastritis, 6 case of mixed type, 5 cases of verruceous gastritis, 2 cases of punctiform gastritis, and overall 21 Cases(67.7%) on the 22th day of treatment, endoscopieally. Faur eases af punctiform gastritis were transformed into verrueous gastritis during the treatment. 4) Disappearance of the erosion was observed more frequently in the mueosa of oxyntic area than that of antrum. This study suggests that cimetidine appears to be effective for relief of elinical symptoms in patients with erosive gastritis, and for treatment of hemorrhagic gastritis and erosions in the mucosa of oxyntic area,
