1.A Study on the Activities of Daily Living, Self-efficacy and the Health Promoting Behavior in Stroke Patients.

PURPOSE: The purpose of this study to analyze the relationship the activities of daily living(ADL), self-efficacy, and health promoting behaviors. METHOD: The research was a descriptive survey study. The subjects of this study were 115 hospitalized stroke patients to two General hospitals and one Oriental treatment hospital in G. Metropolitan. Data was collected through questionnaires from December 1st, 2003 to January 30th, 2004. The survey instruments used in the study Shah, Vanclay and Cooper's MBI(11 items), and Sherer and Maddux's self-efficacy(10 items), Walker, Sechrist, and Pender's HPLP(23 items) was developed by the researcher. The obtain data were analyzed with an SPSS 10.0 program using frequency, percentage, Cronbach's alpha, t-test, ANOVA, Duncan test, and Pearson's correlation coefficient. RESULT: The results were as follows; The level of ADL the score was 50.5, self-efficacy 40.4, and health promoting behavior 54.6. There were significant differences of ADL by general and disease related characteristics as follows : for monthly income(F=2.72, p= .048), duration of stroke(F=7.98, p= .001), number of attack(F=5.21, p= .007), operation (F=17.88, p= .000). There were significant differences of self-efficacy by general and disease related characteristics as follows : for monthly income(F=4.83, p= .003), number of attack (F= 6.20, p=. 003), operation(F=4.03, p= .047). There were significant differences of health promoti ng behaviors by general and disease-related characteristics as follows : for aphasia(F=4.24, p=.042). There were significant correlated between ADL and self-efficacy(r= .698, p=.000), self- efficacy and health promoting behavior(r=.398, p= .000), ADL and health promoting behavior(r= .235, p= .011). CONCLUSION: As a result, There was a significant correlation between ADL, self-efficacy and health promoting behaviors. There will be considered useful nursing intervention effect to progress, support health promoting behavior of stroke patients.

2.Ultrastructural Changes of the Bile Canaliculi after Common Bile Duct Ligation.

The purpose of this study was to investigate the morphologic changes of the bile canaliculi and its associated structures of the liver induced by common bile duct ligation(CBDL) in the rat. The canalicular surface and lateral surface of the dry-fractured hepatocytes was studied with scanning electron microscopy at 1~6 weeks post ligation. The first week after CBDL, the bile canaliculi were dilated. The microvilli were increased in number and the lumens contained granular materials After 2 weeks or more, the bile canaliculi were dilated to a variable degree, and with irregularity, measuring from 1.5 to 5 micrometer in diameter, and in the advanced stage, the canaliculi showed blunting and the disappearance of microvilli. Some canaliculi had sprouting side branches. At 4~6 weeks post-ligation, the lateral surface of the hepatocytes also showed some irregularity and a tortuous appearance, and numerous small sized microvillous projections were formed. The tubular structures of the proliferated SER distributed adjacent to the lateral surface of the hepatocytes, and the direct connection of a tubular structure and the cytoplasmic membrane was observed. These results suggest that the deformity and loss of microvilli of bile canaliculi reflect the disturbance of bile secretion from the hepatocytes. And prolonged obstruction of bile flow may result in bile excretion via the lateral surface of hepatocytes.

3.A case of Behcets Esophageal Ulcer.

A 38-year-old man, with recurrent oral ulcers for 10 years, was admitted because of recent aggravation of odynophagia and sore throat. About 4 years earlier, he had been performed abdominal surgery for intestinal perforation. Gastrofiberscopic examination showed small round ulcers at hypiopharynx and 6cm sized longitudinal linear ulcer at mid esophagus. Biopsy specimens at mid-esophagus showed chronic inflammation. Besides oral ulcer, he had perianal ulcers and skin rashes. He was managed with steroid, colchicine and sulfasalazine under the diagnosis of esophageal involvement in Behcet's disease. After 3 months from discharge, esophagogram and gastrofiberscopic examination showed some improved appearance, but symptoms recurred for steroid tapering. He has been followed in much improved status for 8 months after discharge.

4.A Case of Low-Grade Endometrial Stromal Sarcoma of the Uterus (So-Called "Endolymphatic Stromal Myosis").

Uterine sarcomas are relatively rare tumors of mesodermal origin that constitute 2-6% of uterine malignancies. Uterine sarcomas include endometrial stromal sarcoma (ESS), leiomyosarcoma (LMS), malignant mixed m llerian tumor (MMMT) of both homologous and heterologous type, pure heterologous sarcomas, blood vessel sarcomas, and lymphoma. Endometrial stromal tumors are divided into three types: 1) endometrial stromal nodule, 2) low-grade stromal sarcoma or endolymphatic stromal myosis, and 3) endometrial stromal sarcoma. The low grade endometrial stromal sarcoma is very rare. It shows occasional local recurrence, which might arise from endometrial stroma, from adenomyosis, and rarely from endometriosis. We have experienced a case of low grade endometrial stromal sarcoma of the uterus in a 54-year-old woman, which is presented with a review of literatures.

5.Hydatidiform Mole with a Coexisting Fetus: Report of three cases with nuclear DNA ploidy analysis by flow cytometry or chromosomal analysis.

Three cases of hydatidiform mole with a coexisting fetus are described. In two cases, flow cytometric analysis of nuclear DNA content from paraffin-embedded, molar tissues and normal-appearing placental tissues showed diploid pattern. One case had karyotypes of 46, XX both in fetal skin fibroblasts and in molar tissues. Microscopically, the villi showed diffuse hydropic swelling with circumferential trophoblastic proliferations and consistent with the diagnosis of complete mole. Two patient had persistent gestational trophoblastic neoplasia on the follow-up. Therefore, in cases of hydatidiform mole with a coexisting fetus, dizygotic twin pregnancies composed of a normal pregnancy from one ovum and a complete mole pregnancy from the other ovum must be considered and the patients should be followed with serum beta-hCG measurements.

6.Dynamic MR Imaging of Hepatic Hemangioma and Hepatocellular: Findings and Differential Diagnosis.

PURPOSE: We performed dynamic MR imaging using GdDTPA to find characteristic enhancement pattern of hepatic hemangioma distinguishing from hepatocellular carcinoma. METHODS AND MATERIALS: 28 hepatic hemangiomas and 10 hepatocellular carcinomas were evaluated. Serial dynamic scans after Gd-DTPA(0.1mmol/kg) intravenous injection were obtained by using 0.5T machine and analyzed contrast-to-noise ratio(CNR) of the lesion and enhancement pattern on each scan. RESULTS: Hepatic hemangiomas had positive CNR from 1-2 minute images, and revealed typical "fill-in phenomenon" on early phase with prolonged enhancement in 26 cases(92.8%), and early homogeneous enhancement with isointensity on delayed phase in 2 cases(7.2%) of small hemangiome~ Hepatocellular carcinomas revealed inhomogeneous enhancement with hypointensity on delayed phase in 10 cases(100%) and 3 cases (30%) of capsular enhancement. CONCLUSION: Hepatic hemangioma can be easily distinguished from hepatocellular carcinoma by using Gd DTPA enhanced dynamic MR imaging according to its typical enhancement pattern of "fill-in phenomenon" and prolonged enhancement.

7.The Significance of Amniotic Fluid Index at Admission as a Predictor of Latency Period in the Patients with Preterm Premature Rupture of the Membranes.

OBJECTIVES: The purpose of this study was to determine the clinical significance of residual amniotic fluid volume, as measured by the amniotic fluid index(AFI), on the prediction of latency period and perinatal outcomes in patients with preterm premature rupture of the membranes(PPROM). METHODS: Study population consisted of 103 singleton pregnancies with PPROM between 24 and 34 weeks' gestation. Amniotic fluid index was determined using transabdominal ultrasound at admission. Latency period was defined as time interval in hours between admission and delivery. All medical records of mothers and neonates were reviewed. Spearman's rho rank correlation, receiver-operator characteristic(ROC) curve analysis, survival analysis and Cox's proportional hazard model were used for statistical analysis. RESULTS: There was a statistically significant correlation between latency period and AFI at admission. ROC curve analysis showed that AFI at admission had a significant predictive value in the prediction of latency period < 48 hours. Survival analysis demonstrated that AFI < 5.0 was strongly associated with the likelihood of shorter latency period, Cox's proportional hazard model indicated that AFI < 5.0 was a significant independent predictor for the occurrence of latency period < 48 hours even after adjustment of other independent variables. The patients with AFI <5.0 had a higher rate of cesarean section due to fetal distress than those with AFI > 5.0 but the incidence of chorioamnionitis was not significantly different between two groups. Comparing the perinatal mortality and neonatal morbidity including infectious morbidity between the neonates born to mothers with AFI <5.0 and those with AFI > 5.0, there was no significance after adjustment of gestational age at birth. CONCLUSION: AFI at admission has a significant correlation with latency period and predictive value in prediction of latency period in the patients with PPROM. AFI <5.0 is a independent predictor for the shorter latency period.

8.CT and MR Findings of Cavernous Sinus Lesionst.

PURPOSE: To classify the cavernous sinus lesions, to describe their radiological findings, and to assess the usefulness of MR compared to CT. METHODS AND MATERIALS: Fourty-five patients with lesions involving the cavernous sinus proved by histological and/or clinical and imaging methods were studied retrospectively and classified into neoplastic, vascular, and inflammatory lesions. CT and MR findings were compared in 21 patients evaluated by both modalities simultaneously according to these 4 categories. RESULTS: Pitiutary macroadenoma was the most common cavernous sinus lesion(42%). Diffuse convex bulging of the lateral wall of cavernous sinus was the most frequent radiological finding(84%), and the others were encasement of the cavernous carotid artery(49%), remodelling of the surrounding bones(44%), and complete obliteration of Meckel's cave(38%), in descending order of frequency. Bulging of the lateral wall of cavernous sinus was equally well demonstrated on both modalities, but encasement or displacement of the cavernous carotid artery and complete or partial obliteration of Meckel's cave were much better delineated on MR than on CT with the ratio of 3.8:1 and 4.6: 1, respectively. Only bone changes were much better demonstrated on CTthan on MR with the ratio of 3.8: 1. CONCLUSION: MR issuperior to CTin demonstrating thecavernouscarotid artery encasement and obliteration of Meckel's cave, but CT is much better than MR in demonstrating bone changes.

9.Depression and Anxiety in the Epileptic Patients: the Association With Demographic and Seizure-Related Variables.

BACKGROUND: Depression and anxiety are common psychiatric symptoms in patients with epilepsy, exerting a profound negative effect on health-related quality of life. Several issues, however, pertaining to their association with psychosocial, seizure-related and medication factors, remain controversial. In the present study we have investigated the association of depression and anxiety with various demographic and seizure-related variables in patients with newly-diagnosed and chronic epilepsy. METHODS: We investigated 80 patients with epilepsy (46.3% males, mean age 36.1+/-11.4 years, range 18~64; mean disease duration, 13.4+/-12.2 years). Depression and anxiety were assessed in the interictal state with the Beck Depression Inventory, 21-item version (BDI-21) and the state and trait subscales of the State-Trait Anxiety Inventory (STAI-S and STAI-T), respectively. The association of depression and anxiety with various variables was investigated using univariate analyses and multiple linear regression analyses. RESULTS: Duration of epilepsy, the number of side effects and the number of GABAergic anti-epileptic drugs were correlated with depression, together accounting for 25.1% of the variation of the BDI-21. The STAI-S index was significantly associated with occupation status and the number of GABAergic anti-epileptic drugs accounting for 27.4% of the variation of the STAI-S. Finally, the number of side effects was correlated with trait anxiety. CONCLUSIONS: Our study shows that the duration of epilepsy, occupation status, GABAergic anti-epileptic drugs and the number of side effects are risk factors for the development of depression and/or anxiety.

10.Intestinal Spargnanosis Presenting as an Inflammatory Mass: A case report.

A 39-year-old male patient presented with chronic abdominal pain and intermittent diarrhea for 8 months. Colon study showed an annular stricture at the ileocecal value. He underwent ileocecectomy with clinica impression of intestinal tuberculosis. The resected intestinal wall along the lieocecal junction demonstrated a localized, annular constriction and intramural nodular inflammatory growth in which were clusters of multiple microabscesses as well as acute and chronic inflammatory cell infiltration including eosinophils and fibrosis. Encountered were a few resolving phase of parasitic granulomatous tunnels in which fragments of degenerated sparganum with foreign body reaction were found in one focus. He had history of ingesting uncooked frogs 2 years ago. The above case suggests that differential diagnosis of inflammatory tumorous lesions in the intestine should include sparganosis in Korea.
