2.Analysis of Effectiveness of the Maximum Surgical Blood Order Schedule.

BACKGROUND: The maximum surgical blood order schedule (MSBOS) is a viable option for reducing unnecessary crossmatching and achieving significant cost savings in the blood bank. In this study, we showed the process establishing MSBOS and through a prospective study, we evaluated the efficacy of MSBOS. METHODS: We organized task force team for transfusion management improvement composed of a director of the blood bank, surgeons and anesthesiologists in the Committee for Quality Improvement (CQI) of Asan Medical Center. In this team, we established MSBOS for most elective surgeries through the review of the previous transfusion and crossmatching data. We introduced MSBOS in April 1998 and prospectively analyzed surgeon's acceptance rate of MSBOS, crossmatch-to-transfusion ratio (C/T ratio), blood wastage rate, and cost savings. RESULTS: During the first 19 months after introducing MSBOS at our hospital, there was gradual increase in the surgeon's compliance rate of MSBOS from 30% to 94.0% through continuous education. The C/T ratio was changed from 3.5 to 1.6 and blood wastage rate was decreased from 4.0% to 1.9%. And also we could save more than 38,400,000 won through not performing the unnecessary crossmatches of 7,680 cases per year. CONCLUSION: Introduction of MSBOS can have a significant impact in reducing C/T ratio, blood wastage rate, and unnecessary crossmatches for the unused blood units. For successful application of MSBOS, cooperation with surgeons and anesthesiologists and continuous education is essential.

Gray scale ultasonography of the kidneys was performed in 30 patients with KHF., significant morphologicchanges were noted representing the pathologic involvement of the kidneys, and aids in the clinical assessment ofseverity of the diseases. 1. Peak increased renal volume is seen in oliguric stage. 2. Renal cortical echogenicityis increased in oliguric stage, and returend to normal in convalescency. 3. Medullary pyramid enlargement is seenmainly in diuretic stage. 4. Retroperitoneal edema and thickening of renal capsule are noted in oliguric stage. 5.Abnormal fluid accumulation was seen in two-third of cases in the peritoneal and perirenal space only in oliguricstage. Renal sonography is simple, non hazardous modality, providing satisfactory information about the renal andretroperitoneal anatomy.

4.The Relationship between 5-year Overall Survival Rate, Socioeconomic Status and SEER Stage for Four Target Cancers of the National Cancer Screening Program in Korea:Results from the Gwangju-Jeonnam Cancer Registry

Purpose:
The aim of this study was to investigate the relationship between the 5-year survival rate, socioeconomic status, and SEER (Surveillance Epidemiology and End Results) stage of stomach, colorectal, breast and cervical cancer patients.
Methods:
A total of 11,770 cases of four target cancers, which were diagnosed during 2005-2007, were extracted from the database of Gwangju-Jeonnam Regional Cancer Registry. The subjects of the study were 11,770 including stomach (n=5,479), colorectal (n=3,565), breast (n=1,516) and cervical cancers (n=710). Cox’s proportional hazards model was used to obtain the hazards ratio (HR) according to the SEER stage and socioeconomic status.
Results:
Stomach cancer had a significantly higher HR in the medical aid recipients (HR=1.39), and the group below 20% (HR=1.20) compared to the group with the highest income level. Colorectal cancer had a significantly higher HR in the medical aid recipients (HR=1.26) than in the group with the highest income level. In addition, stomach, colorectal, breast and cervical cancers had a significantly higher HR according to the SEER stage in regional direct (stomach=4.10, colorectal=1.76, breast=12.90, cervical=3.10), regional lymph only(stomach=2.58, colorectal=2.33, breast=4.32, cervical= 4.43), regional both (stomach=6.74 colorectal=3.04, breast=15.57 cervical=6.50), and regional NOS (Not Otherwise Specified)/distant (stomach=17.53, colorectal=11.53, breast=25.34, cervical=26.51) than in situ and localized only.
Conclusion
In order to increase the cancer survival rate, a support system for early detection and early treatment of cancer should be established for groups with low individual income levels, and regular health checkups and management measures should be actively implemented through the National Cancer Screening Program.

Tracheobronchial disruption is a rare injury associated with blunt trauma, and for proper management, accurate diagnosis is essential. We describe a case of isolated tracheal rupture following blunt trauma, and the related CT findings, including details of the injury site. Preoperative bronchoscopy was not performed, but the patient immediately underwent surgery, which was successful. This case demonstrates that CT scanning is very helpful both for diagnosis of tracheobronchial injury and for delineation of the exact injury site.

Chronic constrictive pericarditis is an uncommon disease, but an importanat one because of its potential curability. It usually begins with an initial episode of acute pericarditis often with a pericardial effusion which may not be detected clinically. This then progresses to resorption of the effusion followed by obliteration of pericardial abity with formation of fibrotic tissue, which results in symmetrical scarring that produce uniform restriction. In general, there are no specific problems due to remained pericardial effusion in the clinically manifestated case of constrictive pericarditis. We report a case of chronic constrictive pericarditis with localized pericardial effusion, which caused to hemodynamic compromise due to local compression of the right ventricle.

Primary chondrosarcoma of the heart and its metastasis to the brain is extremely rare. We describe the case ofa patient who underwent resection of cardiac chon-drosarcoma only to relapse with brain metastasis two yearslater. MR images showed that these metastatic tumors were slightly hypointense relative to the brain onT1-weighted images and hyperintense on T2-weighted images. The masses were highly enhanced. Peritumoraledema appeared less extens ive and the focal cystic portion was seen within the mass, corresponding to myxoid degeneration.

Over the last 20 years, the frequency of multiple pregnancy has increased mainly because of the introduction of exogenous pituitary gonadotropins in the treatment of infertility. Since the advent of assisted reproductive technology, the concern about ectopic implantation of embryos has increased dramatically and it continues to be a major complication of in vitro fertilization and embryo transfer (IVF-ET). Bilateral tubal pregnancy is the least common type of ectopic implantation of two embryos. Of all extrauterine pregnancies, 1:725~1:1580 are bilateral1,2. Simultaneous tubal pregnancies have been reported in natural cycles, recently, after ovulation induction, in-vitro fertilization2,3. The high incidence of ectopic pregnancy associated with IVF-ET continues to be a problem and frequencies of between 2.4 and 12.4% have been only a few reports of simultaneous bilateral tubal pregnancies following IVF-ET4. This paper describes a case of a simultaneous bilateral tubal pregnancy after IVF-ET in a 33 year old patient. The diagnosis was confirmed by diagnostic laparoscopy performed 25 days after embryo transfer(sixth week of gestation), which revealed bilateral tubal pregnancy. Bilateral salpingectomy was performed. With a review of the literature on this topic, diagnostic aspects and treatment options are discussed.
