1.Chordoma in the Femoral Head and Gluteus Maximus Muscle: A Case Report

A case of chordoma occurred in the right femoral head and gluteus maximus muscle is reported. The case was a 61 year old female who complained of severe painful limitation of motion of the hip and radiating pain to the posterior aspect of right thigh for ten months. Roentgenological study revealed pathological fracture of femoral neck with osteoporotic change. For the treatment of femoral neck fracture, Moore type prosthetic femoral head replacement was performed. The cut surface of the resected femoral head showed multiple necrotic foci here and there, whitishgrey in color and gellatinous in consistency. The diagnosis was confirmed as a chordoma by microscopic examination. Femoral head and gluteus maximus muscle is one of very rare site of chordoma in the previous literature.

2.Socioeconomic factors related to tooth brushing behavior among adults in Korea: analysis with 2019 Community Health Survey data

Objectives:
This study aimed to identify the socioeconomic factors related to tooth brushing behavior among adults in Korea.
Methods:
This cross-sectional study used data from the Korea Community Health Survey (KCHS) in 2019. Respondents aged 19 years and older (n=299,099) comprised the study sample. The dependent variable was tooth brushing behavior, whereas the independent variables were demographic and socioeconomic factors. The data were processed using chi-squared test, independent t-test, ANOVA, and adjusted multiple logistic regression analysis were performed. Statistical analysis was performed using the STATA 17.0 program, with significance set at the 5% level.
Results:
About half (55%) of the respondents brushed their teeth at least twice a day. The findings also revealed significant differences according to socioeconomic status (P<.01). Those who brushed their teeth less than twice a day had the following characteristics: men, aged 45 years or older, low income, working outside, less educated, without a spouse, and living in rural areas. Adjusted multiple logistic regression analysis showed that the area of residence contributed to tooth brushing behavior.
Conclusions
The area of residence, as well as socioeconomic status, should be considered in oral health education and intervention strategies.
3.Modulation of lipid metabolism by mixtures of protamine and chitooligosaccharide through pancreatic lipase inhibitory activity in a rat model.

Overweight and obesity are usually related with high fat and calorie intake, and seriously causative of lifestyle-related diseases such as cardiovascular disorders, arteriosclerosis, and colon cancer. In this study, we propose a novel dietary therapy against overweight and obesity using mixtures of protamine and chitooligosaccharide (COS), which are known to interrupt the lipid metabolism in the body. Protamine is a dietary protein originated from salmon reproductive organ, and COS is an oligosaccharide made from chitin or chitosan by chemical or enzymatic hydrolysis. In the enzyme activity analysis in vitro, protamine and COS strongly suppressed the activity of pancreatic lipase, which is the primary enzyme for the digestion and absorption of lipids in the intestine. In in vivo animal test, the mixtures of protamine and COS significantly reduced the serum levels of triglyceride (TG), total cholesterol (T-CHO), and low density lipoprotein-cholesterol (LDLC) and inhibited the accumulation of lipids in liver tissue of Sprague Dawley (SD) rats fed high fat diets. On the other hand, they increased fecal TG and T-CHO contents. From these alterations in lipid metabolism, we verified that protamine and COS mixtures could effectively interrupt the digestion and absorption of dietary lipids in the body by inhibiting pancreatic lipase activity. In addition, protamine and COS mixtures increased the serum level of high density lipoprotein-cholesterol (HDLC), responsible for removing cholesterol from cells and protecting atherosclerosis, and therefore decreased the potential risks of cardiovascular diseases by lowering values of the atherogenic index (AI) and cardiac risk factor (CRF). Taken together, we suggest protamine and COS mixtures as a prominent dietary therapy for the prevention of overweight, obesity, and further cardiovascular diseases related with hyperlipidemia.

4.Primary non-Hodgkin's malignant lymphoma of the vulva: a case report.

A case of primary non-Hodgkin's malignant lymphoma of the vulva which occurred in a 68-year-old woman is presented. Non-Hodgkin's malignant lymphoma is infrequently involved in the female genital tract. Moreover, primary vulvar involvement of this tumor is very rare. To date only 6 cases have been reported in the literature. To our knowledge this is the first reported case of a non-Hodgkin's malignant lymphoma of the vulva in Korea.

5.Epidemiological Findings of Hepatitis B Infection Based on 1998 National Health and Nutrition Survey in Korea.

The purpose of this study was to investigate the epidemiological characteristics of hepatitis B virus (HBV) infection in Korea based on the 1998 National Health and Nutrition Survey. Study subjects consisted of 9,771 aged 10 yr or over, who were selected from across Korea using a stratified multistage probability sampling design. The prevalence of hepatitis B surface antigen (HBsAg) was compared by age, sex, residency, household income, education, family history, family size, and frequency of eating out. The prevalence of HBsAg was 5.1% (95% confidence interval (CI): 4.5-5.7) in males and 4.1% (95% CI: 3.6-4.6) in females with a low prevalence in those under 20 yr old. Generally, HBsAg seropositivity by administrative area was similar with the exception of Jeju province. HBsAg seropositivity of Jeju island was approximately three times higher in both men and women, as compared with the national average. HBsAg seropositivity by socioeconomic status unexpectedly showed a very consistent positive association in both gender. Comparing HBsAg seropositivity by the frequency of eating out, in both gender, the more frequent they ate out, the higher it was. Our study suggested that there might be another transmission route of HBV, which is possibly related to diet.

6.Pathologic Fracture in a Subchondral Cystic Lesion at the Superior Acetabulum.

Subchondral cyst is a benign cystic lesion, and it is often found in patients with rheumatoid arthritis or osteoarthritis. However, its occurrence in those joints without preexisted disease is rare. Furthermore, there has been no report in the medical literature regarding pathologic fracture in the bony plate of a subchondral cyst at the superior acetabulum. We report here on a case of the 44-year-old woman for whom a pathologic fracture was found in the bony plate of a subchondral cyst at the superior acetabulum.

7.Study on the External Causes of Mortality In Korea.

PURPOSE: This study was conducted to analysis the trends and states of external auses of mortality in Korea from 1985 to 1998. METHODS: This study was based on data published in 1985-1998 from the annual report of cause of death statistics of National Statistical Office. RESULTS: In the proportion of total death cases during 1985-1998, External causes of mortality was ranged from 11 percent to 15 percent. Death rate(per 100,000) of external causes of mortality(ECM) decreased recently from 85.7 in 1991 to 68.9 in 1998. Death rate of ECM in male was 2.6 times higher than that of female. Among total cases of ECM, the first leading cause of death was transport accidents. intentional self-harm was the second leading cause of death(26.8 percent in 1998). In trends of death rate(per 100,000) for ECM during 1885-1998, there were decreased for following causes of death; transport accidents, falls, accidental drowning and submersion, exposure to smoke, fire and flames, accidential poisoning by and exposure to noxious substances, Death rates of ententional self-harm and assault were increased during the periods. The age specific death rate increased with age for all ECM except for assault. The risk of death caused by ECM was higher in aged 65 and over than in ages 0-19 years among all ECM. The risk of death for male aged 65 and over were 6.9 times in transport accidents, 26 times in fall, 37 times in accidential poisoning by and exposure to noxious substances, and 18 times in intentional self-harm compared with 0-19 years. The risk of death for female aged 65 and over were 27 times in fall, 22 times in accidential poisoning by and exposure to noxious substances compared with 0-19 years. In seasonal variation of ECM cases for 1998. There was highest proportion in August(10.4 percent). The highest proportion by death seasion were winter(32.7 percent) in exposure to smoke, fire and flames, summer(27,7 percent) in fall, summer(52.4 percent) in accidental drowning and submersion, summer in 32.0 percent) in accidential poisoning by and exposure to noxious substances, spring(28.2 percent) in intentional self-harm, assault. and autumn(28.2 percent) in transport accidents. CONCLUSION: Above results suggest that a preventive education program for safety accidents should be developed by considering gender and age of the objecties and seasons of the year.

8.Osteonecrosis of the Jaw in a Patient with Osteoporosis Treated with Oral Bisphosphonates.

Bisphosphonates are widely used in the management of metastatic bone disease and in the prevention of osteomalacia and osteoporosis. In particular, oral preparations are more commonly used for the prevension and treatment of osteoporosis. Bisphosphonate-related osteonecrosis of the jaws (BRONJ) has been well documented recently in relation to intravenous preparations of the drug. But, a few cases have been reported of oral bisphosphonate-associated osteonecrosis. We could not find any risedronate cases in the Korean medical literature. Here we report a case of BRONJ in a 91-year-old woman patient receiving an oral bisphosphonate (risedronate) for the treatment of osteoporosis.

9.Evaluation of a Community-based participatory professional periodontal care program for hypertension and diabetes patients.

OBJECTIVES: To evaluate the Community-Based Participatory Research (CBPR) professional periodontal care program model for patients with hypertension and diabetes. METHODS: This descriptive case study included 151 participants of the professional periodontal care program. The CBPR-based professional periodontal care program consists of 5-steps: ‘Issue identification and prioritization’ (Step 1), ‘Strategy development’ (Step 2), ‘Entry into community’ (Step 3), ‘Implementation’ (Step 4), and ‘Transition’ (Step 5). Quantitative data were analyzed using frequency analysis, and descriptive data with PASW 23.0 (SPSS Inc., Chicago, IL, USA). The results of the Focus group interview (FGI) were classified as ‘general opinions regarding the program planning and operation receptiveness’, ‘sustainability’, ‘potential spread of the program’, and ‘improvement of program’. The interviews were qualitative research involving seven people. RESULTS: 1. Participants increased their interest in health and oral health by managing their hypertension, diabetes, and periodontal disease using community resources. Through this, healthy practices and improved awareness helped to prevent complications and manage periodontal diseases. 2. Community organizations actively cooperated, resulting in positive changes in oral health practices (increased registration of patients in education centers for hypertension and diabetes, and increased number of patients visiting the local dental clinic). In the future, it was positive to participate in the program continuously. CONCLUSIONS: The most important step is ‘Entry into community’, which has led to active participation and cooperation of community organizations and participants. Therefore, community organizations and strategy development should be discussed, and the role of community leaders should be emphasized to build cooperative relationships. In addition, participation in and collaboration with health-based projects should be achieved through a search of various community organizations.

10.The Significance of VEGF Expression in Stage II Carcinoma of Uterine Cervix Treated with Definitive Radiotherapy.

PURPOSE: We wanted to determine the clinical characteristics and prognosis according to the VEGF expression in stage II cervical carcinoma patients treated with definitive radiotherapy. MATERIALS AND METHODS: We enrolled 31 patients who were diagnosed with cervical cancer from 1995 to 2003 at Samsung Medical Center and their paraffin block tissue samples were available for study. The median age of the patients was 65 years. The mean tumor size was 4.1 cm (range: 1.2~8.2 cm). Seven patients (22.6%) were suspected of having pelvic lymph node metastasis. An external beam irradiation dose of 45-56.4 Gy was administered to the whole pelvis with a 15 MV linear accelerator, and an additional 24 Gy was given to point A by HDR intracavitary brachytherapy. VEGF staining was defined as positive when more than 10% of the tumor cells were stained. The median follow-up duration was 58 months. RESULTS: A positive VEGF expression was observed in 21 patients (67.7%). There was no significant correlation between the VEGF expression and pelvic lymph node metastasis, tumor size and the response of radiotherapy. During follow-up, 7 patients had recurrence. The complete response rate was not significant between the VEGF (-) and VEGF (+) tumors. However, the VEGF (+) tumors showed a significantly higher recurrence rate in comparison with the VEGF (-) tumors (p=0.040). The three year disease-free survival rates were 100% and 66.7%, respectively, for patients with VEGF (-) or VEGF (+) tumor (p=0.047). CONCLUSION: The VEGF expression was a significant factor for recurrence and disease-free survival. However, the significance of the VEGF expression is still controversial because of the various definitions of VEGF expression and the mismatches of the clinical data in the previous studies.
