1.Echocardiographic Findings in Children with Heart Murmur Detected at Mass Health Screening in Kyeong-Nam Province.

PURPOSE: This study was designed to analyze the results of echocardiographic findings in children with heart murmur detected at mass health screening in Kyeong-Nam province. METHODS: Between 1996 and 2001, children with heart murmur underwent the echocardiographic examination at Gyoeongsang National University Hospital. RESULTS: The number of children examined was 129 in 1996, 26 in 1997, 44 in 1998, 61 in 1999, 60 in 2000, and 49 in 2001. By the echocardiographic examination, innocent murmur was the most commonly diagnosed(31-65%) and ventricular septal defect(VSD) was the most common cardiac defect. Between 1996 and 1998, complex cyanotic heart diseases were more frequent. The number of patients undergoing surgical treatment was 9 in 1996, 10 in 1997, 7 in 1998, 5 in 1999, 2 in 2000, and 2 in 2001. CONCLUSION: By the echocardiographic examination, functional murmur was the most commonly diagnosed and VSD was the most common cardiac defect. Currently, heart defects detected tend to be more simple and the surgical treatment needed seems to decrease in number.

2.Cytochrome P-450, Drug Metabolizing Enzymes and Arachidonic Acid Metabolism in Pig Ocular Tissues.

The eye is a relatively isolated organ and tends to retain or accumulate compounds within itself. Unless the eye possesses a mechanism for detoxifying drugs and chemicals that are carried to ocular tissues via the circulating blood, the compounds may cause visual impairment. The purposes of this experiment was judged by drug metabolizing action by cytochrome P-450 system, distribution and effect of metabolism of arachidonic acid and distribution of lipid peroxide formation and phospholipids in the lens, cornea and choroid-retina of pig ocular tissues. The results were as follows: 1. The activities of mixed-function oxidase, cytochrome P-450 and NADPH-cytochrome C reductase were the highest in the choroid-retina compared to the lens and cornea. 2. The activities of drug metabolizing enzymes, aryl hydrocarbon hydroxylase, 7-ethoxycoumarin-o-deethy lase and benzphetamine-N-demethy lase, heme oxygenase activity, and the activity of arachidonic acid metabolizing enzyme, cyclooxygenase were the highest in the choroid-retina compared to the lens and cornea. 3. Lipid peroxide formation was the highest in the cornea compared to the lens and choroid retina. 4. In distribution of phospholipids, sphingomyelin and phosphatidylcholine were the highest in the choroid-retina compared to the lens and cornea and phosphatidylethanolamine was the highest in the cornea campa red to the lens and choroid-retina. 5. Cytochrome P-450 isoenzyme of pig ocular tissues was confirmed in the same form of MAb(Monoclonal Antibody) PCN(pregenolone-16alpha-carbonitrile) 213-1.

3.Multidimensional Factors Influencing Burnout in Intensive Care Unit Nurses

PURPOSE: This study aimed to identify multidimensional factors influencing burnout in intensive care unit(ICU) nurses. METHODS: A descriptive cross-sectional design was used. Data were collected during February 2016 from a convenience sample of 222 tertiary hospital ICU nurses and analyzed using t-test, ANOVA, correlation analysis, and multiple regression analysis. RESULTS: Burnout correlated positively with compassion fatigue (CF)(r=.37, p < .001), and negatively with compassion satisfaction (CS)(r=-.66, p < .001). The regression model explained 57% of the variance in burnout. For individual characteristics in the model, perceived health status (β=-.27, p < .001) and gender (β=.14, p=.028) were the most influential factors for ICU nurses' burnout. In the model with added work-related characteristics, nursing environment (β=-.22, p=.001), perceived health status (β=-.20, p=.001), and satisfaction with department (β=-.19, p=.007) were the most influential factors. Finally, for the model with psychological characteristics added, CS (β=-.56, p < .001) and CF (β=.35, p < .001) were the most influential factors. CONCLUSION: Results reveal that most ICU nurses have a moderate level of CF and a moderate to high level of CS and burnout, and that individual, work-related and psychological factors are relevant in ICU nurses' burnout. Programs or interventions to reduce burnout should be developed taking into account these multidimensional factors.

4.Serum cholesterol changes by duration of GnRH-agonist therapy in premenopausal women with breast cancer

Objectives:
Ovarian suppression using Gonadotropin releasing hormone (GnRH)-agonist in premenopausal women with breast cancer has been known to improve disease-free survival and overall survival. However, long-term effect of ovarian suppression on lipid metabolism has not been studied yet. In this retrospective cohort study, we aimed to investigate cholesterol changes during medical ovarian suppression.
Methods:
and Materials: We reviewed medical records and blood test results of 152 women who have been diagnosed as breast cancer and started GnRH-agonist every 12 weeks therapy for more than 24 weeks in Asan Medical Center between 2018.1.1 and 2020.12.31. Patients who had previously diagnosed dyslipidemia or diabetes, or newly received lipid-lowering agents during study period were excluded from the cohort. Age at diagnosis and preoperative Body Mass Index (BMI) were investigated as baseline demographics. Generalized additive mixed model was applied to analyze the relationship between duration of GnRH-agonist and cholesterol changes.
Results:
The age was distributed as 42.5±5.2 years old (mean±SD), and preoperative BMI was 23.0±3.6 kg/m2 (mean±SD). Duration of GnRH agonist therapy ranged from 5.6 to 37.7 months, with mean of 19.3. Total cholesterol was 171 mg/dL before starting GnRH-agonist, whereas 181 mg/dL on last check, which was significantly higher than initial value (p=0.03). Duration of GnRH-agonist did not affect total cholesterol level until 19.3 months, while significantly increased by 1.8 mg/dL for each month thereafter (p=0.011). There was no significant effect of age, preoperative BMI and GFR on total cholesterol.
Conclusion
While long-term use of GnRH-agonist is applied, patients should be monitored for dyslipidemia after 19 months of treatment and lipid-lowering agents may be considered especially when indicated.
5.Full mouth rehabilitation of the patient with crossed occlusion using removable partial denture restoration: A case report.

In removable partial denture with crossed occlusion, it is difficult to meet the satisfaction of patient due to variable movement of denture and unfavorable stability under loading. In a case with few unilaterally teeth remaining, additional treatments such as implantation or residual root are required to assure bilaterally stable support. However, due to the medical condition of the patient with hypertension, angina and old age etc., removable partial denture was planned as treatment in this case. Proper diagnosis, accurate analysis and full understanding of the adaptation for neuromuscular system are necessary to recover the vertical dimension of patient from a decreased vertical dimension due to crossed occlusion.

6.The Association of Hypertension and Dyslipidemia in Postmenopausal Women.

BACKGROUND AND OBJECTIVES: The purpose of this study was to figure out the correlation of hypertension and atherogenic dyslipidemia in postmenopausal women. MATERIALS AND METHODS: The study population consisted of consecutive 380 postmenopausal women (age> or =50, FSH>40IU/L and triglyceride[TG]<400 mg/dl) visiting health promotion center at Samsung Cheil Hospital from March 1997 to July 1998. We measured blood pressure by automatic sphygmomanometer (A & Dco.LTD.TM-2654) and divided them into four groups by blood pressure (Group I:systolic blood pressure[SBP]< 120 mmHg and diastolic blood pressure[DBP]<80 mmHg, n=87. Group II:SBP 120-139 mmHg or DBP 80-89 mmHg, n=109. Group III: SBP 140-159 mmHg or DBP 90-99 mmHg, n=127. Group IV: SBP> or =160 mmHg or DBP> or =100 mmHg, n=57). We measured thier body mass index (BMI) and waist-hip ratio (WHR). We also measured their levels of glucose, total cholesterol and TG by automatic biochemical analyzer (Hitachi 7150) and HDL cholesterol by direct method using AutoDAIICHI reagent. We calculated LDL cholesterol levels by Friedewald formula. RESULTS: MI, WHR and glucose level of group IV were significantly higher than those of group I. But there were no significant differences among the groups in the levels of total cholesterol, LDL cholesterol and apolipoprotein B. HDL cholesterol level of group I was statistically higher than that of group IV. TG level and LDL cholesterol/apo B ratio of group IV were significantly higher and lower than those of other groups respectively. CONCLUSION: Postmenopausal women who had moderate or severe hypertension show the trend toward abdominal obesity and atherogenic dyslipidemia. We might have to pay attention to these metabolic abnormalities in postmenopausal women with hypertension.

7.A Case of Multiple Endocrine Neoplasia Type 1 with Primary Liver Gastrinoma.

Gastrinoma is the most frequent functional pancreaticoduodenal endocrine tumor in patients with multiple endocrine neoplasia type 1 (MEN 1). Primary hepatic gastrinomas in MEN 1 are very rare, with no previous reports published in the literature. We reported the case of a 39 yr old female patient with a history of repeated peptic ulcers and a hypoglycemia episode. Abdominal CT indicated a well-defined liver mass and a pancreatic head mass. Somatostatin-receptor scintigraphy with [111In] DTPA octeotride demonstrated a strong uptake of the radiotracer in the left lateral segment at the site of the hepatic mass. After operation, immunohistochemical staining was consistent with pancreatic insulinoma and primary hepatic gastrinoma. As the liver is a common site of metastases from gastrinoma, primary liver gastrinoma has not yet been reported with MEN 1. We diagnosed this patient using immunohistochemical studies and treated this patient by hepatic segmentectomy.
8.The bonding durability of total etching adhesives on dentin.

The purpose of this study was to evaluate the effect of different etching times on microtensile bond strength (microTBS) to dentin both initial and after thermocycling with 3 different types of total-etching adhesives. Fifty four teeth were divided into 18 groups by etching times (5, 15, 25 sec), adhesives types (Scotchbond Multipurpose (SM), Single Bond (SB), One-Step (OS)), and number of thermocycling (0, 2,000 cycles). Flat dentin surfaces were prepared on mid-coronal dentin of extracted third molars. After exposed fresh dentin surfaces were polished with 600-grit SiC papers, each specimen was acid-etched with 35% phosphoric acid (5, 15, 25 sec) and bonded with 3 different types of total etching adhesives respectively. Then, hybrid composite Z-250 was built up. Half of them were not thermocycled (control group) and the others were subjected to 2,000 thermocycle (experimental group). They were sectioned occluso-gingivally into 1.0 x 1.0 mm2 composite-dentin beams and tested with universal testing machine at a crosshead speed of 1.0 mm/min. Within limited data of this study, the results were as follows 1. There was no statistically significant difference in microTBS between the thermocycled and non-thermocycled groups, except for both SM and SB etched for 25 sec. 2. In thermocycled SM and SB groups, bond strength decreased by extended etching time. In total etching systems, adhesive durability for dentin could be affected by type of solvents in adhesive and etching time. Especially, extended etching time may cause deteriorate effects on bond strength when ethanol-based adhesive was used.

9.Study on the Development of Critical Pathway for the Cesarean Section using Data Mining Technique.

The objectives of this study was to discover knowledge in predicting lengths of stay of Cesarean Section by patients characteristics and treatment method using data mining technique and to suggest the approach to the development of critical pathway. The findings suggest that data mining technique from the large pool of accumulated patients data can be utilized to systematize newly observed correlations, patterns and trends and to develop critical pathway for the treatment and management. The results of the study can be contributed to aid developing the clinical pathway for cesarean section suitable to Korean patients. And the application of the developed critical pathway in clinical practice will produce the actual effect and value.

10.Effect of Korean Native Cimicifuga Species on Proliferation of Vascular Smooth Muscle Cells.

OBJECTIVES: To examine the effect of extracts of Korean native Cimicifuge species on cell proliferation in vascular smooth muscle cells (VSMC). METHODS: VSMC were isolated from rat aorta. Cell proliferation was assessed by measure of bromodeoxyuridine incorporation into the cells. Differences in Reactive oxygen species (ROS) levels were examined after exposure to the extracts of Korean native Cimicifuge species using the detection reagents dichlorofluorecin diacetate. The rhizomes/roots were air-dried and milled with a commercial food mixer. Milled rhizomes/roots of each Cimicifuga species were separately extracted by 80% ethanol, absolute methanol, and 40% 2-propanol using homogenizer and evaporated under reduced pressure at low temperatures. Effects of extracts dissolved in phosphate-buffered saline (0.3 mg/mL) were examined. RESULTS: Ethanolic, methanolic or propanolic extracts of 4 Korean native Cimicifuge species (Cimicifuga [C] davurica, C. japonica, C. heracleifolia var. bifida Nakai, C. simplex) were screened. The addition of extracts of each Korean native Cimicifuge species to cells in the presence of 10% fetal bovine serum (FBS) potently inhibited cell proliferation. Significant decrease of 23%-30% was observed. Vitamin E, a potent antioxidant, inhibited 10% FBS-stimulated cell proliferation of VSMC. We also demonstrated that extracts of each Korean native Cimicifuge species decreased intracellular ROS generation induced with 10% FBS. The effect of Korean native Cimicifuge species was not species-specific and solvent-specific. CONCLUSION: TExtracts of Korean native Cimicifuge species inhibit VSMC proliferation via inhibition of intracellular ROS. These findings suggest that Cimicifuge species used for reducing menopause symptoms might be cardioprotective in women.
