4.Clinical Use of Whole Body Scanning

The skeleton is a frequent site of bone tumor metastasis. Radiographic examination Is not sufficiently reliable in early detection since an abnormality is unlikely to be observed until more than 50% of the bone material has been lost. Therefore there was much effort to discover radiographic materials for use in scanning. At the present tirne, 99m Tc-labeled diphosphonate is the best available material for bone scanning. We applied whole body scanning with 99m Tc-to 40 patients who had bone tumors (primary or secondary), infection and other cases. The results obtained are as follow: 1. The scan can detect the extensiveness of the tumor better than plain X-ray in the early stage. 2. In metastatic disease, 60% of all patients were positive in the scan and all of there were negative in X-ray. 3. In cases of cancer, definitive treatment planning can be facilitated by the information obtained from whole body scanning. 4. In inflammatory disease, we can differentiate osteomyelltis from cellulitis or pyogenic arthritis in the early stage. 5. In fibrous dysplasia, it was possible to differentiate monostatic from polyostotic disease. 6. In nonunion of bone, scan was positive but X-ray was eouivocal.

It has been suspected that various infections, including cytomegalovirus(CMV) infection, are associated with IgA nephropathy. In case of CMV infection, ganciclovir is known to be a treatment of choice for severe CMV infection in general. But ganciclovir has a lot of severe toxicity, so children with normal immunity are seldom treated by ganciclovir when CMV infection is suspected. On the other hand, intravenous immunoglobulin can also be used to treat CMV infection.

7.Clinical observation of aseptic meningitis associated with mucocutaneous lymph node syndrome.

Clinical observation was carried out for 147 patients with mucocutaneous lymph node syndrome (MCLS) who were admitted to the Department of Pediatrics, Wonju Christian Hospital during the period from June, 1983 to June, 1992. A spinal tap was performed on 114 of these patients and the following results were obtained. 1) The incidence of aseptic meningitis in patients with MCLS was 52.6%. (60 cases of the 114 cases who had spinal taps (52.6%).) 2) The predominent age group for aseptic meningitis was 6 months to 1 year of age (35%). 3) For the majority, the value of glucose and protein in the CSF were within normal limit or only mildly elevated. 4) The neurologic manifestations associated with aseptic meningitis in patients with MCLS were irritability (78.3%), vomiting (25.0%), nuchal rigidity (11.7%), convulsion (5.0%) and facial nerve palsy (1.7%) in that order.

8.Effect of microthreads on removal torque and bone-to-implant contact: an experimental study in miniature pigs.

PURPOSE: The objective of this study was to evaluate the effect of microthreads on removal torque and bone-to-implant contact (BIC). METHODS: Twelve miniature pigs for each experiment, a total of 24 animals, were used. In the removal torque analysis, each animal received 2 types of implants in each tibia, which were treated with sandblasting and acid etching but with or without microthreads at the marginal portion. The animals were sacrificed after 4, 8, or 12 weeks of healing. Each subgroup consisted of 4 animals, and the tibias were extracted and removal torque was measured. In the BIC analysis, each animal received 3 types of implants. Two types of implants were used for the removal torque test and another type of implant served as the control. The BIC experiment was conducted in the mandible of the animals. The P1-M1 teeth were extracted, and after a 4-month healing period, 3 each of the 2 types of implants were placed, with one type on each side of the mandible, for a total of 6 implants per animal. The animals were sacrificed after a 2-, 4-, or 8-week healing period. Each subgroup consisted of 4 animals. The mandibles were extracted, specimens were processed, and BIC was analyzed. RESULTS: No significant difference in removal torque value or BIC was found between implants with and without microthreads. The removal torque value increased between 4 and 8 weeks of healing for both types of implants, but there was no significant difference between 8 and 12 weeks. The percentage of BIC increased between 2 and 4 weeks for all types of implants, but there was no significant difference between 4 and 8 weeks. CONCLUSIONS: The existence of microthreads was not a significant factor in mechanical and histological stability.

10.A Case of Lymphocytic Interstitial Pneumonia.

Lymphoid interstitial pneumonia (LIP) is an uncommon process in childhood, and is characterized by interstitial accumulation of mature lymphocytes, plasma cells and reticuloendothelial cells. LIP is believed to have autoimmune or immunologic pathogenesis, because it is frequently associated with acquired immune deficiency syndrome, Epstein-Barr virus infection, common variable immunodeficiency. The patient described in this report had idiopathic thrombocytopenic purpura, Evans syndrome and LIP. The diminished levels of IgG1 & IgG2 were the only immunologic abnormality, which suggest that this patient may be in the early phase of common variable immunodeficiency or selective IgG2 & IgG4 deficiency due to immunoglobulin heavy chain deletion. We can rule out the possibility of lymphoma by clonality study, which showed polyclonal lymphocyte infiltration. On our knowledge, this is the first report of LIP in Korean children.
