1.A Mycoparasitic Ascomycete Syspastospora parasitica on the Entomopathogenic Fungus Paecilomyces tenuipes Growing in Bombyx mori.

While studying Bombyx mori "Zipnuae" inoculated with an entomopathogenic fungus, Paecilomyces tenuipes, a mycoparasitic hymenoascomycete with a long neck was isolated from the silkworm rearing room. The fungus was identified as Syspastospora parasitica which has been recognized as a hyperparasite on various moniliaceous hyphomycetes. A descriptive illustration is presented in this study.

4.A Case of Unilateral Nevoid Telangiectasia.

The brief report of Zeisler (1922) of a pregnant woman with scattered telangiectasia in a unilateral distribution is perhaps the first recorded case of this syndrome. The syndrome has been described in association with pregnancy, puberty, alcoholisrn and cirrhosis, hormonal therapy and congenital status. The authors observed in a 17-year-old male unilateral telangiectatic lesions over the left C, C, C, Tl and T, dermatomal regions. The lesions developed at first at his puberty on the anterior part of the chest as speckled bright red discoloration. Thereafter, the lesion has been progressed to involve the left upper extremity, sholder, upper middle part of the chest, eventually. Family history was negative and physical, lahoratory and X-ray examination were all negative or within normal limit. No mucous membrane involvement was observed. On histopathologic examination of the skin lesion, thin walled ectatic vessels lined with single layer of endothelial cells were observed in the upper dermis. The lesions were partly dermabrased with favorable result.

5.The relationship between the amount of stress and mental health of the job holder.

BACKGROUND: Job holders are groups that have suffered from chronic stressful condition. Nowadays, occupational stress are increasing due to Korean economic crisis. Therefore, this study was designed to evaluate the stress scores of several kinds of jobs, the relationship between the amount of stress and mental health status, factors associated with stress. METHODS: Among workers who had visited a medical college hospital, Taejeon, Korea, for regular health evaluation from April 1 to June 30 1998, we selected 50 clerks, 50 industrial workers, 50 professionals, 50 service or salesmen and 50 public servants in the survey. A total of 159 workers were evaluated as subjects excluding those who had answered in adequately to the questionnaire. Stress scores were measured by Korean-translated BEPSI(Brief Encounter Psychosocial Instrument) and Bae Jong Myun's 30-items SRRQ(Social Readjustment Rating Questionnaire). Mental health status were checked by Korean-translated SCL-90-R(Symptom checklist-90-Revision). RESULTS: The average BEPSI score of workers was 2.07+/-0.72. The mean score of female workers was 2.27+/-.70 and it was obviously higher than male worker's score of 1.85+/-.66. 20-29 year age group and over 40 year age group's amount of stress were obviously higher than 30-39 year age group. The low education level group had higher stress score than those with high education level but the amount of stress showed no significant difference in the monthly income, job tenure, number of family members to support, hobby and religion. According to occupation, clerks, industrial workers, service or salesmen's stress scores were higher than those of professional or public servants. CONCLUSIONS: This investigation has showed that there is a correlation between the amount of job holder's stress and mental health including somatimtian, obsessive-compulsive, interpersonal sensitivity, depression, anxiety, hostility, phobic anxiety, paranoid ideation and psychotism Therefore, we conclude that the best way to stay in good menta

6.Comparison of mecA Gene Detection with Susceptibility Testing Methods in Coagulase Negative Staphylococcus According to the New NCCLS Guidelines(1999).

BACKGROUND: Coagulase negative staphylococcus (CNS) spp. is a major pathogenic organism of nosocomial and community-acquired urianry tract infections, and causes infrctions in the immunocompromised host, and in particular, bloodstream infetions in patent with indwelling devices. High prevalance of methicillin resistance has been noticed in CNS which also have been recongnized as an important multidrug resistant pathogen. The optimal phenotypic method for detecting methicillin resistance still remains controversial, and new guidelines for detecting methicillin resistance of CNS was proposed by NCCLS in January 1999. We evaluated the relationship between mecA gene by PCR method and antimicrobial susceptibility tests according to the new NCCLS guidelines. METHODS: A total of 82 CNS isolates were examined for MICs and penicillin MICs by disk diffusion and agar dilution method according to NCCLS guidelines, and detections, and detection of mecA gene by PCR. RESULT: In disk diffusion method, 66 strains (80.5%) and 63 strains (76.8%) showed resistance to penicillin and oxacillin, respectively, and in agar dilution method, 71 strains(86.6%) and 53 strains (64.6%), respectively. In PCR method, mecA genes were detected in 49 strains(59.8%). Comparing with mecA gene detection by PCR method, the sensitivity of disk diffusion and agar dilution method was 95.8% and 89.8%, repectively. However, the sensitivity of disk diffusion and agar dilution method was 65.3% and 75.5%, respectively using previous NCCLS criteria. CONCLUSION: The new criteria of NCCLS detects the methicillin resistance induced by mecA gene more sensitively than previous one.

7.Clinical and Structural Outcomes of Arthroscopic Intraarticular Knotless Fixation for Upper Subscapularis Tendon Tears: A Preliminary Report.

BACKGROUND: A novel technique for the repair of tears of the upper subscapularis tendon—intraarticular knotless fixation—has been introduced recently. The purpose of this study was to evaluate the clinical and structural outcomes of arthroscopic intraarticular knotless fixation for the treatment of upper subscapularis tendon tears. METHODS: We retrospectively analyzed the clinical and radiological outcomes of 27 patients who underwent arthroscopic intraarticular knotless fixation for upper subscapularis tendon tears. Finally, a total of 10 patients who could participate in at least a 6 month follow-up of magnetic resonance imaging evaluation and in a least 1-year follow-up on an outpatient basis were enrolled in our study. The mean age at the time of operation was 60.7 years, and the mean duration of follow-up was 14.7 months. Two patients had concomitant tears of the supraspinatus tendon and 8 patients had concomitant tears of the supraspinatus and the infraspinatus tendons. RESULTS: The clinical and radiological outcomes improved after the patients had undertaken arthroscopic intraarticular knotless fixation. The mean visual analogue scale score for pain during motion improved from 6.7 preoperatively to 1.4 at the final follow-up (p<0.001). The mean Constant score improved from 59.3 preoperatively to 79.6 at the final follow-up, and the mean the University of California at Los Angeles score, from 21.7 to 30.2, respectively (p<0.001 and p<0.001). The upper subscapularis tendon tear was healed in every patient except one (90%), for the patient had suffered from a postoperative trauma that resulted in a retear. CONCLUSIONS: We found that arthroscopic intraarticular knotless fixation gives good clinical and structural outcomes for the repair of upper subscapularis tendon tears. Arthroscopic intraarticular knotless fixation provided such a reliable and efficient restoration of the subscapularis tendon footprint that we anticipate it will become a widely-used procedure for upper subscapularis tendon tears.

8.Panclavicular Dislocation: A Case Report

Dislocation of both ends of a clavicle in a 49-yesr-old man during an auto-accident is presented. The medial end of the right clavicle was severely displaced antero-inferiorly while the 1ateral end displaced posteriorly. And the clavicle was rotated along its long axis. Sever al attempts of manual and a surgical reduction through separate incision on both ends of the clavicle failed to have anatomical reduction. It was presumed that the cause of failure in reduction were delayed reduction due to combined chest injury, blockade by first rib that was broken and was anteriorly displaced as well as the complexity of the dislocation. It is speculsted that anatomic reduction in this rare injury require wider surgical exposure. Though authors failed to have anatomic reduction in present case, stabilization of the medial end of the clavicle with a strip of fascia lata after medial clavicular resection and ligament repair at the lateral end resulted in satisfactory regain of shoulder motion and cosmetic improvement.
