2.A Case of Prader-Willi Syndrome with Diabetes Mellitus.

Prader-Willi syndrome is characterized by infantile hypotonia, mental retardation, hyperhagia, hypogonadism and obesity. Approximately 60% of all PLW syndrome show an interstitial deletion of chromosome 15, 37% have apparently normal chromosome, and 3.6% have a variety of other abnormalities involving chromosome 15. Diabetes mellitus has been considered a component of PLW syndrome and the incidence is about 7%. We experienced a 17-year-old female who revealed mental retardation, hypogonadism, obesity, and non-insulin dependent type DM, compatible with Prader-Willi syndrome.

4.Apical membrane antigen-1 (AMA-1) gene sequences of re-emerging Plasmodium vivax in South Korea.

Plasmodium vivax malaria re-emerged in South Korea in 1993, and epidemics continue since then. We examined genetic variation in the region encompassing the apical membrane antigen-1 (PvAMA-1) of the parasites by DNA sequencing of the 22 re-emerging P. vivax isolates. The genotype of the PvAMA-1, which was based on sequence data previously reported for the polymorphic regions, showed that two haplotypes were present at one polymorphic site. Compared with reported data, the two types, SKOR type I and type II, were similar to Chinese CH-10A and CH-05A isolates, respectively. Thus, the present study showed that two genotypes of AMA-1 genes coexist in the re-emerging Korean P. vivax.

5.Nasal Carriage of Staphylococcus aureus from Healthy Children Attending Day Care Center.

PURPOSE: This study was performed to investigate the prevalence of Staphylococcus aureus (S. aureus) nasal carriage in Korean children attending day care centers. METHODS: During September and October 2009, a survey for nasal carriage of S. aureus and methicillin-resistant S. aureus (MRSA) was conducted among children attending day care centers located in Seoul with questionnaire survey for evaluation of risk factors of acquisition of MRSA was obtained from their guardians. A culture of the anterior nares swabs using enrichment broth was executed for isolating S. aureus and oxacillin susceptibility was assessed by the disk diffusion method. RESULTS: Out of the 428 children enrolled whose mean age was 55 months old, 163 (38.1%) were colonized with S. aureus. Of the 163 isolates, 40 (24.5%) were MRSA. The nasal carriage rate of S. aureus showed an increasing trend with increase of age. Based on the answer to the questionnaire, 9.2% and 3.6% of children had a recent history of hospitalization and surgery, respectively, and approximately 40% of children had a history of prescription of antibiotics within 1 year prior to enrollment. Of the 428 subjects, 40 (9.3%) were MRSA nasal carriers. CONCLUSION: S. aureus and MRSA carriage rate of children attending day care center in Korea was 38.1% and 9.3%, respectively. Continued surveillance for nasal carriage rate of S. aureus and MRSA (especially community-associated MRSA) is mandatory.

6.The Effects of Education According to Constitution & Character of Patients Who have Stroke.

This study was designed to explore the effectiveness of education to improve self-care of stroke patients with three types of character health locus of control and constitution. A total of 56 patients were selected by convenience sampling at one oriental hospital. The teaching of self-care was conducted by the researcher. The self-care performance was measured for 10 days. The data were analyzed using t-test, ANOVA and Scheffe method as post hoc test. The result of this study are as follows : 1. The self-care performance by patients characteristics : Age and education had impact on the self-care performance. The level of self-care performance of young and high educated patients was high, gender, religion, occupation, monthly income, and the paralyzed side, however, didn't have influence on the self-care performance. 2.. The self-care performance of patients with three types of constitution : There was no statistical significance on the self-care performance of patients group by constitution, However, average score of self-care performance of patients with So-Um was high. 3. The self-care performance of patients with three types of health locus of control : There was no statistical significance on self-care performance of patient group by health locus of control. 4. The correlation between the constitution and the health locus of control : There was no statistical significance on the introverted and chance health locus of control of patients with three types of constitution. However, there was a statistically significant relationship between the powerful others health locus of control and So-Yang. Even though there was no statistically significant self-care performance of patients with So-Um, there was a clinical significance on the high score of self-care performance of patients with So-Um as compared with the self-care performance of the patients. The health locus of control didn't have influence on the self-care performance of patients. There was a statistically significant relationship between the powerful others health locus of control and So-Yang. Therefore, this study suggest the need to develop the nursing intervention to improve the self-care of stroke patients with the powerful others health locus of control and So-Yang, Furthermore, the longitudinal research is needed to determine the effectiveness of training to imp.

7.Clinical Applications of Plasma B-type Natriuretic Peptide Assays in Preterm Infants with Patent Ductus Arteriosus.

Hemodynamically significant patent ductus arteriosus (hsPDA) in preterm infants is a common and important problem associated with significant morbidity and mortality including: intraventricular hemorrhage, necrotizing enterocolitis and bronchopulmonary dysplasia. The rapid and accurate determination of the indications for therapeutic closure of hsPDA in preterm infants is important but often difficult. Currently used tools, such as the clinical findings, including heart failure and the typical echocardiographic features of hsPDA, have unsatisfactory sensitivity and/or specificity. Over the last decade, measurement of plasma B-type natriuretic peptide (BNP) levels using a commercially available bedside testing kit has been proposed to aid in the diagnosis and treatment of hsPDA in preterm infants. The present review analyzes the clinical usefulness of a rapid plasma BNP assay for the management of hsPDA in preterm infants and speculates on potential future therapeutic directions, including predicting subsequent hsPDA for early targeted treatment and predicting of treatment response for decision-making regarding surgical intervention before and after medical treatments.

9.Clinical Application of Inhaled Nitric Oxide Therapy in Persistent Pulmonary Hypertension of the Newborn.

Inhaled nitric oxide (iNO) is recognized as a potent and selective pulmonary vasodilator that does not decrease systemic vascular tone. The therapeutic application of iNO in human was first described in 1991. Subsequent reports showed that iNO therapy was effective to improve oxygenation in infants with persistent pulmonary hypertension of the newborn (PPHN). Owing to its selective pulmonary vasodilator effects, iNO therapy is an important treatment for term newborns with hypoxemic respiratory failure due to PPHN. The Food and Drug Administration of the United States of America first approved iNO in 1999 for use as a medical gas to treat hypoxic respiratory failure associated with clinical or echocardiographic evidence of pulmonary hypertension in term and late preterm neonates. Thereafter, iNO therapy is clinically applied to treat PPHN in term and late preterm neonates without consensus. In this review, we focused on the clinical practice of iNO therapy in PPHN. Based on published studies, we discuss iNO initiation and withdrawal methods, respiratory support devices that complement iNO therapy, and the patient and gas monitoring during iNO therapy.

10.Alterations of the TSH Levels in the Breast Feeding Newborn Infants after the Mother's Eating Brown Seaweed Soup.

PURPOSE: After delivery, eating brown seaweed soup is a typical Korean tranditional habit. But, excessive intake of iodine transiently inhibits biosynthesis and secretion of thyroid hormones if a certain threshold of iodine is reached in the serum. The aim of our study was to demonstrate whether the observed the elevations of TSH level in the breast fed neonatal infants was caused by mother's eating brown seaweed soup or not. METHODS: We performed neonatal TSH test by enzyme linked immunosorbent assay(FRELISA Screenig TSH) and cheked TSH level for 178 newborns with a appropriate gestational age. We divided the study subject into 3 groups, the group used breast feeding, mother had taken brown seaweed soup and blood sampled at 6th day after birth was categorized Group A, formular feeding, blood sampled at the 4th days after birth was categorized Group B, breast feeding and had taken brown seaweed soup and blood sampled at the 19th day after birth was categorized Group C. RESULTS: 1) The mean TSH level was 1.5+/-1.3 uU/ml in Group A, 3.3+/-2.5uU/ml in Group B, 1.7+/-l.3uU/ml in Group C(Group A vs Group B: p<0.05, Group A vs Group C: p<0.05). 2) There was no statistical significances between the TSH levels and the amount of brown seaweed soup which mother had one day, the duration which mother had brown seaweed soup and duration of breast feeding. CONCLUSIONS: Mother had taken brown seaweed soup about 3 times a day in korea at present days, There was no significant changes of TSH levels in the beast fed newborn infants after the korean mother had taken brown seaweed soup.
