2.CLINICO-ANATOMICAL STUDY OF SEPTUM IN THE MAXILLARY SINUS.

Several reports have suggested a risk of problems due to sinus septum during the sinus lift procedure. However, there has been no wide-ranging clinico-anatomical study of sinus septum. This study was performed to examine the occurrence and anatomical characteristics of sinus septum of the maxillary sinus. The computed tomographs of the maxillary sinus in 488 patients were analyzed. THe incidence, location, and orientation of the septum were examined. The septa were observed in 177 of 488 patients (24.0%). The septa could be observed at any age (from 7 to 74 years). The incidence of sinus septum was higher in female (30.8%) than in male (20.8%). The average number of present septa in one patient was 1.56+/-0.84. One hundred and fifty-seven septa (86.3%) were oriented in a buccopalatal plane, 16 septa (8.8%) in a sagittal plane, and 9 septa (4.9%) in a transverse plane. There were no significant differences in location of septa within the maxillary sinus. These results were suggested that the incidence of sinus septum was 24.0%, and its shape and location were irregular. The identification of sinus septum should occur prior to the sinus lift procedure so that the modification of the basic sinus lift procedure can be planned according to the shape and the location of septa. Then successful results may be achieved through surgical removal of septa or several modifications to the sinus lift procedure.

5.Neurosonographic diagnosis of periventricular-intraventricular hemorrhage in low birth weight infants.

Periventricular-intraventricular hemorrhage (PV-IVH)is one of the most important neurologic lesion of the low birth weight infants. Serial neurosonographic exeaminations were performed in 113 low birth weight infants who were admitted to the neonatal intensive care unit of Presbyterian Medical Center from November 1, 1990to July 31, 1991. The results were summarized as follows: 1) The incidence of PV-IVH in the study was 54% 2) According to Papile's grading system of PV-IVH, grade I was 32.8%, grade II was 45.9%, grade IIIwas 11.5% and grade IV was 9.8%. 3) The onset of PV-IVH was within the first 7 days of life in 82%. 4) Poor activity, apnea, bradycardia and hypotension were statistically significant clinical findings associated with PV-IVH(P<0.05). 5) The risk factors associated with PV-IVH were gestational age, birth weight, hyaling membrane disease, patent ductus arteriosus and artifical ventilation. 6) The mortality of PV-IVH was 0% for grade I, 10.7% for grade II,42.9% for grade III and 83.3% for gradeIV.

6.THE EFFECTS OF VASCULAR FREEZING TIME ON ARTERIAL VASOSPASM IN RATS.

The outcomes of microvascular surgery may be jeopardized by factors that reduce the blood flow across the freshly the performed anastomosis Arterial vasospasm is one of such factors causing the failure of an anastomosis. Vascular spasm has treated by systemic or topical administration of pharmacologic agents. However, despite the works done to prevent and elucidate the cause of vasospasm, a apasmolytic agent Capable of consistently protecting against it or reversing it has not been found. Vascular freezing as an powerful spasmolytic maneuver has recently been reported. The purpose of the present study was to determine optimal vascular freezing time. The effect of vascular freezing time was examined on the relief of arterial vasospasm. The vascular was freezing was performed on the artery of rats whit hand-held liquid nitrogen oxide spray. the artery was frozen for 5, 15,or 30 seconds. Vascular spasm and patency were evaluated immediately after and 1, 3, 7, 14, and 56 days after the freezing. Femaral arteries were havested at 1, 3, 7, 14, and 56 days after freezing for histologc examination The results obtained were as follows : 1. immediately after the freezing-thawing, the vasospasm was relieved and not induced by spasmogenic induction manipulation in all groups. 2. Immediately after the freezing-thawing, the artery become dialated, the diameter of which was slightly more than the intial, ranging between 0-17% and the diameter of 5-second group was smaller than those of 15-and 30-second group(P<0.05). 3. One to 56 days after freezing, arteral diameter was still 1-22% larger than the initial and there was no significant difference among groups. 4. One to 56 days after the freezing, vasospasm was not induced by spasmogenic induction manipulation in all groups. 5. One day after the freezing, almost all the vessel wall died and the normal architeture of the layer was deranged in all group. The shorter was the duration of freezing, the more early was the healing of artery occurred, 6. Fifty-six days after the freezing, cellularity was increased and the architecture was regenerated, There was no significant differance among groups. It was suggested that vascular freezing for 5 seconds is useful to relieve the vasospasm and to lessen the injury of artery.

8.Plasma Glucose and Insulin Changes during IV Theophyline Therepy in Preterm Infants with Apnea.

PURPOSE: The purpose of this study was to evaluate the effects of theophylline in preterm infants with apnea on glucose homeostasis and insulin values. METHOD: In this prospective study, level of glucose and insulin were measured from peripheral blood of 8 neonates(1,450+/-114gm, 31+/-2.1week), who were admitted from April 1, 1997 to July 30, 1997 in Neonatal Intensive Care Unit of Wonkwang University Hospital, for apnea of prematurity(> 20 sec with bradycardia and/or cyanosis) were given aminophylline intravenously. Blood samples were collected at pretreatment, posttreatment 2hours, 1-2days, 3-4days, 5-7days and posttreatment 48hours, and compare to those of the 8 control neonates(1,711+/-232gm, 32+/-1.7week). RESULTS: The results were as follows: 1) Plasma glucose values were significantly higher in the treatment group than those of the control group at 1-2days(104.67+/-20.39mg/dL vs 83.43+/-15.86mg/dL) and 3-4days(111.0+/-32.39mg/dL vs 79.25+/-14.03mg/dL)(p<0.05). 2) Plasma glucose values which were increased at 1-2days(104.67+/-20.39mg/dL)and 3-4days(111.0+-32.39mg/dL), were significantly higher than pretreatment values(66.33+/-31.19mg/ dL)(p<0.02), but were not increased to the level of hyperglycemia(> 125mg/dL). 3) The mean posttreatment glucose levels drawn at 48hours after discontinuation of theophylline was significantly decreased to the values of pretreatment values compared to those of the 1-2days and 3-4days(p<0.01). 4) Insulin concentrations were also increased insignificantly when blood glucose were increased in the treatment group compared with control group. CONCLUSION: In conclusion, intravenous administration of theophylline produces hyperglycemia significantly, including an increase in a serum insulin. But, clinically significant hyperglycemia(> 125mg/dL) was not noted. So, plasma glucose may not need to be monitored in preterm apneic infants receiving theophylline. But, further studies are need to elucidate the effect of theophylline considering the serum toxic level of theophylline.

9.Effects of topical and intravenous heparin on thrombosis of microvascular anastomoses.

This study was performed to evaluate the effect of topical and intravenous heparin on thrombosis and patency in the microvascular anastomosis of the traumatized veins. Nine white rabbits weighing about 2kg were used. After exposure of both femoral veins, the veins were crushed by the jaws of smooth needle holder in order to create a thrombosis model. Transectional incision was made in the vein. The animals were then divided into 3 groups based on the administration method of heparin: 1) Experimental Group I, topical irrigation of lumen with heparin saline solution (n=6); 2) Experimental Group 2, topical irrigation of lumen with heparin saline solution and intravenous injection of heparin (0.75 mg/kg) via the marginal ear vein for 3 days; 3) Control Group, topical irrigation of lumen with saline solution (n=6). The patency was evaluated with empty-and-refill test and thrombus formation was judged by surgical microscope. The results were as follows: 1. Thirty minutes after microvascular anastomosis, the patency fall Experimental Groups was better than Control group. However, there was no significant difference among groups. 2. Three days after anastomosis, the patency of all Experimental Groups was much more improved than that of Control Group (P<0.05). There was no significant difference between Experimental Group 1 and 2. 3. Three days after anastomosis, the amount of thrombus in all Experimental Groups was much less than that of Control Group (P<0.05). 4. In histologic findings a lot of luminal thrombus were observed around sutured area in Control Groups. Few luminal thrombus was observed in all Experimental Groups. Mild necrosis in the vessel wall was observed around sutured area in all specimens. These results indicate that topical irrigation of heparin may improve the patency and inhibit the formation thrombus in the microvascular anastomosis of the traumatized veins.

10.Application of acidic primer for orthodontic adhesive system.

Acidic primer is the bonding agent which combines the conditioning and priming agent into the single solution and was originally developed for the dentin bonding system. It is less harmful to the tooth structure and more convenient to manipulate than the traditional etching procedure. The purpose of this study is to evaluate the shear bond strength of various bonding materials when the enamel is treated with acidic primer for the bracket bonding procedure. Fifty recently extracted human premolars were randomly separated into five groups : Group I using Clearfil Liner Bond 2 adhesive system to the enamel treated with acidic primer, Group II using Transbond XT adhesive system to the enamel treated with acidic primer, Group III using panavia 21 adhesive system to the enamel treated with acidic primer, Group IV using Fuji-Ortho LC adhesive system to the enamel treated with acidic primer, Group V using Transbond XT adhesive system to the enamel treated with 37% phosphoric acid. The shear bond strength was measured with Instron universal testing machine after storing in 37oC water bath for 48 hours. After debonding, the teeth and brackets were examined under scanning electron microscope (SEM) and assessed with the adhesive remnant index (ARI). The results were as follows : 1. There were no significant differences in shear bond strength between group III (8.69 +/- 2.72 MPa), group IV (9.7 +/- 3.16 MPa), and group V (10.48 +/- 2.60 Mpa) (p>0.05). 2. The shear bond strength of group III and group IV was significantly higher than that of group I (1.09 +/- 0.53 Mpa), and Group II (2.70 +/- 1.46Mpa) (p<0.05). 3. The ARI of group IV (2.1 +/- 1.1) and group V (2.9 +/- 0.3) was significantly higher than that of group I (0.2 +/- 0.4), group II (0.3 +/- 0.9) and group III (0.2 +/- 0.4) (p<0.05). 4. There were no significant difference between the ARI of group IV and group V (p>0.05). This result suggests that the combination of acidic primer and some bonding adhesive can provide sufficient shear bond strength for clinical orthodontics.
