1.A Study of Complementary and Alternative Medicine used by Cancer Patients in Korea.

PURPOSE: The purpose of this study was to study complementary and alternative medicines (CAM) used among cancer patients in Korea. METHOD: This study was a descriptive survey. Study participants were 1,150 cancer patients selected from 28 representative hospitals in Korea. Data were collected between December 1, 2004 and March 30, 2005. RESULTS: Their diagnosis were as follows : 20.0% for breast cancer, 15.6% for stomach cancer, 14.4% for liver cancer, 10.8% for lung cancer, 10.0% for colon cancer, and 10.0% for uterine cancer. 75.0% of cancer patients had used CAM. The most popular categories of CAM use were diet with 153.2%, non-prescription of oriental medical doctor 47.9% and mind-body control 20.3%. CAM use gender, age, cancer type, cancer treatment method, and current treatment type was significantly different in. CONCLUSION: CAM use among cancer patients in Korea was very popular (75.0%) and so systemic guidelines of CAM use are needed.

2.Influence of Clonidine on the Postoperative Analgesic Effect of Epidural Bupivacaine and Fentanyl.

BACKGROUND: Epidural clonidine reduces pain after surgery. The aim of this study was to evaluate the effect of adding low-dose clonidine to continuous epidural local anesthetics and fentanyl on pam relief. METHODS: 100 patients scheduled for gynecologic low abdominal surgery were investigated. All patients were given 10cc of 0.25% bupivacaine with fentanyl 100 ug through epidural catheter. Group I was infused with combined 0.15% bupivacaine and fentanyl 5 ug/ml at a rate of 2cc/hr. Group II was infused with combined 0.15% bupivacaine and fentanyl 5 ug/ml and 150 ug of clonidine at a rate of 2cc/hr. Pain was assessed on a visual analogue pain scale for 2 postoperative days. Changes in blood pressure, heart rate, and incidence of side effects were observed. RESULTS: VAS observed 20min, 1hr, 1day, 2days after operation were significantly lower in Group II than Group I . Blood pressure and heart rates were significantly changed in Group II but not in Group I for 2 hours after epidural injection. The incidence of side effects was similar between Group I and Group II. CONCLUSIONS: Continuous low-dose epidural clonidine infusion reduces blood pressure and heatt rates significantly but enhances postoperative analgesic effect of combined epidural bupivacaine and fentanyl without increased side effects.

3.Influence of Lifestyle-Related Diseases on the Oral Health of the Rural Elderly in Korea.

OBJECTIVES: The purpose of this study was to analyze the influence of four lifestyle-related diseases, diabetes, hypertension, obesity and hypercholesterolemia, on oral health in the rural elderly in Korea. METHODS: We enrolled 399 subjects over 60 years of age over a six year period, year 2000-2006/year 2001-2007. All subjects received a routine health examination as part of a program conducted by the National Health Insurance Corporation at Seongju-gun Public Health Center in Gyeongsangbuk-do Province, South Korea. All subjects were surveyed and examined to determine their general and oral health statuses. RESULTS: Our results suggest that the duration of lifestyle-related diseases has a significant influence on oral health. The following factors were all significant in the results of analyses: duration of diabetes, tooth decay, which teeth had decayed, missing and filled teeth(DMFT), duration of hypertension, and duration of obesity and hypercholesterolemia(p<0.05). CONCLUSIONS: In summary, lifestyle-related diseases such as diabetes, hypertension, obesity and hypercholesterolemia have significant influences on oral health in the elderly. In this context, the prevention and management of lifestyle-related disease is critical for the maintenance and promotion of oral health.

4.Protective Field Efficacy Study of Influenza Vaccines for Korean Children and Adolescent in 2010-2011 Season.

PURPOSE: We conducted a prospective comparative clinical study to determine the field efficacy of the 2010-2011 influenza vaccines [Influenza virus strains; A/California/7/2009 (H1N1), A/Perth/16/2009 (H3N2), B/Brisbane/60/2008] in healthy Korean children under 18 years of age. METHODS: In this study, we enrolled subjects aged between 6 months and 18 years and divided them into 2 study groups: a group who received the influenza vaccines (407 subjects), and a control group who did not receive the influenza vaccines (230 subjects). Ours was a multicenter study that involved 7 hospitals, including the Korea Cancer Center Hospital. The study was conducted between September 2010 and February 2011. We collected nasal wash or throat swab samples from subjects who presented with acute febrile respiratory or influenza-like illnesses at the hospital. We used PCR to confirm the presence of the influenza virus in the respiratory samples and characterize the virus type. RESULTS: In this study, we collected 22 respiratory samples from the influenza-vaccinated group and found 3 cases of influenza virus infection. Similarly, we collected 21 samples from the control group and found 12 cases of influenza virus infection among 10 subjects during the study period. We determined the field efficacy of the 2010-2011 seasonal influenza vaccines to be 83.2% in healthy Korean children and adolescents. CONCLUSION: In this study, we determined the field efficacy of the 2010-2011 seasonal influenza vaccines in healthy Korean children and adolescents. We found that the field efficacy of 2010-2011 seasonal influenza vaccines was adequate.

5.Muscle Fiber Conduction Velocity and Histopathologic Findings after Intramuscular Injection with 7% Phenol Solution in Rat.

OBJECTIVE: To investigate the muscle fiber conduction velocity and histopathologic changes in skeletal muscle after 7% phenol solutions intramuscular injection. METHOD: Forty-five Sprague Dawley rats were divided into two groups: Experimental group, 30; Control group, 15. Experimental and control groups were injected with 0.1 cc of 7% phenol solution and 0.9% saline in right gastrocnemius, respectively. The histopathologic findings and muscle fiber conduction velocity were evaluated at 0, 1, 5, 14, and 28 days after each injection. RESULTS: In the light microscopic examination of experimental groups, vacuolar change, atrophy of myofibers, and intermyofiber cell proliferation were shown. The expression of synaptophysin began to be seen at 5 days and S-100 protein was increased 14 days after injection. In the electron microscopic examination of experimental groups, vacuolar change and denuded postsynaptic membrane were shown. In muscle fiber conduction study of experimental groups, the mean velocity were significantly slowed at 1, 5, 14, and 28 days after injection than those of control groups (p<0.05). CONCLUSION: On the basis of histopathologic findings, it is presumed that the mechanisms of paralysis following intramuscular injection of 7% phenol solutions were both myogenic and neurogenic. The slowing of muscle fiber conduction velocity tended to be related with the histopathologic changes of skeletal muscles after intramuscular injection with 7% phenol solution.

6.Histopathologic Findings and Muscle Fiber Conduction Studies after Intra-muscular Injection of Botulinum Toxin in Rat.

Recently, botulinum toxin has been widely used for the management of spasticity. However it's mechanism of action in the skeletal muscle has not been well clarified. This study was performed to investigate the histopathologic changes in the skeletal muscle after botulinum toxin injection, and to determine the clinical standards of muscle fiber conduction study as an objective indicator for the changes of muscle fiber. As a study group, 35 Sprague Dawley rats were injected intra-muscularly with the botulinum toxin type A around two heads of right gastrocnemius muscle. After the injection of botulinum toxin, histopathologic studies and muscle fiber conduction studies were performed in 5 rats of the study group at 0, 1, 3, 5, 7, 14, and 28th day respectively. Based on the morphologic studies, the mechanisms of paralysis following the botulinum toxin injection were found to be both myogenic and neurogenic, and the motor function recovered through the formation of new motor end-plate and proliferation of Schwann cells. The muscle fiber conduction study revealed that the mean latencies of study group at 1, 3, 5, 7, and 14th day after the injection of botulinum toxin were significantly prolonged than those of the control group(p<0.05). The prolongation and slow recovery of latencies in a muscle fiber conduction study after the injection of botulinum toxin significantly reflect the morphologic changes of paralized skeletal muscles.

7.Progressive Familial Heart Block Type I in a Korean Patient.

A 29-year-old man was referred to the emergency department with a complaint of abdominal pain and dizziness. He had experienced two previous syncopal episodes. His family history revealed that his mother and his two uncles had received permanent pacemaker implantation. His initial heart rate was 49 beats per minute. The electrocardiography (ECG) showed atrial flutter and right bundle branch block (RBBB) with left anterior fascicular block (LAFB). On admission, 24-hour Holter showed ventricular pause up to 16 seconds during syncope. Radio frequency catheter ablation (RFCA) of atrial flutter was performed. The ECG revealed bifascicular block (RBBB and LAFB) and first-degree atrioventricular block. He received a permanent pacemaker implantation. His brother's and his sister's ECGs also showed trifascicular block and the pedigree showed autosomal dominant inheritance. This patient was diagnosed with a progressive familial heart block (PFHB) type I. This would be the first report of a PFHB type I case documented in Korea.

8.Left Atrial Mass with Stalk: Thrombus or Myxoma?.

A 63-year-old female was presented to emergency room with an abdominal pain. The patient had moderate mitral valve stenosis and atrial fibrillation. Abdominal computed tomography revealed right renal infarction. Transthoracic echocardiography showed a large mobile mass in the left atrium. Transesophageal two-and three-dimensional echocardiography showed a large mobile ovoid mass with a narrow stalk attached to the left atrial septum. It was thought to be a myxoma rather than thrombus. Anticoagulation with heparin was continued. When the operation was performed, there was no mass in the left atrium. It must be a thrombus and melt away.

9.Overview of the Annual Frequency and Clinical Manifestations of Colonic Diverticulosis.

BACKGROUND/AIMS: The prevalence of colonic diverticulosis in Korea is lower than in Western contries and the involved site is different. But, the recently increasing prevalence in Korea may be due to changing life style and advances in diagnostic approachs. We analyzed the annual incidence and clinical manifestations of colonic diverticulosis of the patients who underwent colonofibroscopy. METHODS: Medical records of 25,808 patients who underwent colonofibroscopy at Yeungnam University Medical Center from January 2000 to December 2007 were retrospectively reviewed. RESULTS: Overall prevalence was 5.5%. The mean age of the patients was 57.02+/-12.41 years (range 19-97 years) and the most frequent occurrence (28.8%) occurred in the 7th decade of life. Male to female ratio was 977:159. The annual incidence rate increased from 4.6% to 7.2% since 2000. Diverticulum occurred more frequently in the right side colon (997 cases) than the left side colon (159 cases), with involvement of both sides in 62 cases. The rate of left sided colon increased with age. In 516 patients, only one diverticulum was detected and the other 682 patients had multiple diverticuli. Complications were evident in 51 cases, consisting of diverticulititis in 35 cases and bleeding in 16 cases. CONCLUSIONS: According to this single center analysis, the annual incidence rate of colonic diverticulosis is increasing with right sided colon as the predominant site. But, the frequency in left-sided colon is increasing with age.

10.Isolated Splenic Vein Thrombosis Associated with Acute Pancreatitis.

Pancreatic disease is the most frequent cause of isolated splenic vein thrombosis. Splenic vein thrombosis causes a localized form of portal hypertension known as sinistral or left-sided portal hypertension. Splenic vein thrombosis may be complicated by the formation of gastric varices, with the potential of massive upper gastrointestinal bleeding. Whereas splenectomy is considered to be the treatment of choice for symptomatic splenic vein thrombosis, the role of splenectomy in the patient with asymptomatic splenic vein thrombosis remains controversial. We report a rare case of acute pancreatitis complicated by isolated asymptomatic splenic vein thrombosis. Recognition of this disease entity is important because the risk of secondary variceal bleeding, while uncommon, can be life-threatening.
