2.The Classification of Standard Nursing Activities in Korea.

A nursing activity classification for hospitalized patients was performed based on an article review regarding nursing definition and nursing activity classification system. The study was conducted as follows: 1) Taxonomy was developed by the research team through the Delphi process and review article. The taxonomy consists of four nursing processes, (assessment, diagnosis, intervention and evaluation) and twelve nursing activity domains space (resperation, nutrition, elimination, exercise/alignment maintenance, comfort, hygiene, safety, spiritual support, counseling/ education, medication, communication, patient and information management). 2) First, nursing activities of the intervention process were listed and then classified by the nursing process of assessment, diagnosis, intervention and evaluation. The list consists of twelve nursing activity domains and 136 nursing activities. 3) A pilot study was conducted in two hospitals to verify validity and appropriateness of nursing activities. 4) The content validity index, which was calculated by 6 clinical practice experts, was 0.95. Also, a nursing activity classification system should also be developed in the department of community nursing and home health care nursing.

3.Exposure to toluene diisocyanate ( TDI) induces IL - 8 and RANTES production from bronchial epithelial cell.

BACKGROUND: There have been a few reports suggesting involvement of neutrophil as well as eosinophil in inducing bronchoconstriction aft,er inhalation of TDI. OBJECTIVE: In order to observe the source of chemokines in TDI-induced asthma, this investigation was designed to determine whether IL-8 and RANTES could be produced by human bronchial epithelial cells and whether dexamethasone had any effects on their production. Materials and METHODS: We cultured Beas-2B, a bronchial epithelial cell line, with five concentrations of TDI-human serum albumin (HSA) conjugate and compared them with those having no conjugate. The levels of IL-8 and RANTES in the supernatant were measured by ELISA. To evaluate the effect of pro-inflammatory cytokines, cells were incubated with peripheral blood mononuclear cell (PBMC) culture supernatant, which was derived from PBMC culture of a TDI -induced asthmatic subject under exposure to TDI-HSA conjugate, and then compared to those without PBMC supernatant addition. To evaluate the effect of dexamethasone, four concentrations of dexamethasone were pre-incubated and the same steps were repeated. RESULTS: There was significant production of IL-8 from bronchial epithelial cells with addition of TDI-HSA conjugate in a dose-dependent manner (p<0.05, respectively), which was significantly augmented with additions of PBMC supernatant (p<0.05, respectively) at each concentration. RANTES production was negligible, however, it increased significantly with addition of PBMC supernatant and TDI-HSA conjugate in a dose response manner(p<0.05, respectively). Compared to the untreated sample, pre-treatment of dexamethasone induced remarkable inhibitions of IL-8 and RANTES production. CONCLUSION: These results suggest that IL-8 and RANTES released from bronchial epithelial cells may contribute to neutrophil and eosinophil recruitment occurring in TDI-induced airway.

4.Changes of serum neutrophil chemotactic activity(NCA) and myeloperoxidase(MPO) level following lysine-aspirin(L-ASA) bronchoprovocation test in aspirin-sensitive asthmatic patients.

BACKGROUND: There have been few reports suggesting involvement of mast cell and neutrophil to induce bronchoconstriction in aspirin-sensitive asthrna. OBJECTIVE: To evaluate mast cell and neutrophil activation in pathogenesis of aspirin-sensitive asthma. MATERIAL AND METHOD: We observed changes of serum NCA and MPO levels during L-ASA bronchoprovocation test in 14 subjects with aspirin-sensitive asthma. RESULTS: Serum NCA was significantly increased at 30 min(p=0.01) after the inhalation of L-ASA and then, no significant changes were noted at 240 min (p=0.14). NCA was significantly higher in subjects with late asthmatic responses than in those without it (p=0.04). Serum MPO level tended to increase at 30 min with no statistical significance (p=0.08), and then it significantly decreased at 240 min (p=0.05). There was no significant correlation between serum NCA and MPO level (r=0.22, p=0.58). CONCLUSION: These results support the view that NCA derived from mast cell may contribute to neutrophil recruitment into the airway in aspirin-sensitive asthmatic patients.

5.An experimental study on the changes of the doppler patterns in the common carotid artery after clamping of the internal carotid artery.

Recently, duplex sonography has been used as a screening test for the evaluation of carotid arterial disease. If an occlusion of atherosclerosis is located at the bifurcation of the common carotid artery or the lower portion of the internal carotid artery, the luminal change may be directly visualized on high-resolution B-mode sonography or color-Doppler images. But when the lesion is located at the high cervical, petrous or cavernous protion of the internal carotid artery, it is difficult to directly visualize the lesion with the sonography. The purpose of this paper is to evaluate the indirect changes on the Doppler patterns of both common carotid arteries with clamping of an internal carotid artery. Thirty common carotid arteries in fifteen normal rabbits were examined with duplex ultrasonography using high-resolution real-time imaging and 7.5MHz pulsed-wave Doppler flow measurements with velocity waveform analysis. Systolic velocity (SV) was 45.4±8.4cm/sec, end-diastolic velocity (EDV) 15.3±4.9cm/sec and resistative index(RI) 0.66±0.08 in the common carotid artery before clamping of the internal carotid artery. SV was 26.3±7.8cm/sec, EDV 6.0±5.2cm/sec and RI 0.78±0.18 in the ipsilateral common carotid artery and SV was 56.6±13.0cm/sec, EDV 22.3±8.2cm. sec and RI 0.61±0.10 in the contralateral common carotid artery after clamping of the internal carotid artery. During clamping of the internal carotid artery, the difference between SV of bilateral common carotid arteries was 30.3±13.8cm/sec and EDV 16.3±9.2cm/sec. There was no difference of the velocity patterns of the common carotid artery between preclamping and declamping of the internal carotid artery. In conclusion, lower SV and EDV in a common carotid artery than that in contralateral side on Doppler patterns strongly suggests an occlusion of ipsilateral internal carotid artery.

6.Angioedema-like facial swelling as a first manifestation of systemic lupus erythematosus.

Angioedma is a group of disorders with multifactorial etiology but a similar clinical expression, is characterized by swelling of subcutaneous or submucosal tissue. Systemic lupus erythematosus(SLE) is a chronic multi-systemtic disease characterized by inflammation and tissue damage resulting from deposition of auto-antibodies and immune complex. We experienced a case of angioedema which was the first manifestation of SLE in 24-year-old female patient. She had suffered from severe facial edema and multiple lymphadenopathy for six months and she also had pleural effusion, positive anti-nuclear and anti-DNA antibody test. Marked decrease of C3 and C4 levels was noted with normal antigenic level, and activity of Cl esterase inhibitor. The angioedema was not improved with anti-hitamine agents and instead disappeared with use of corticosteroid and non-steroidal anti-inflammatory drugs. Complement levels normalized after corticosteroid treament. We report a case of SLE which initial manifestation was angioedema.

7.A Case of Tuberous Sclerosis Diagnosed by Computed Tomography.

Tuberous sclerosis is a rare heredofamilial disease involving multiple organs, classically associated with the clinical triad of adenoma sebaceum, epilepsy, and mental retardation. We have recently experienced a case of tuberous sclerosis, 18-year-old male patient, who had typical adenoma sebaceum and generalized epileptic seizure. Plain skull film showed multiple calcified paraventricular subependymal nodules and brain CT scan disclosed characteristic calcified tubers adjacent to the frontal horn and body of the lateral ventricle.

8.Computed Tomographic Findings in Juxtasellar Tumors.

Twenty three consecutive patients admitted to our department with the suspicion of juxtasellar tumor were subjected to computed tomographic examinations. The diagnostic accuracy of CT scan was compared to that of plain skull x-ray, carotid angiography, and pneumoencephalography. The characteristic CT findings of pituitary tumors, carniopharyngiomas and parasellar meningiomas were observed, including their contrast enhancement patterns. In pituitary adenomas, the author evaluated the enlargement of the sella turcica by calculating the sella turcica size index with the use of measured interclinoid distance on CT Polaroid pictures. The results were as following: 1) CT gave the highest accuracy of various diagnostic tools, indicating that CT has become the screening method of choice in the detection of juxtasellar masses along with its another advantage of noninvasiveness. 2) Pituitary adenomas showed significant enlargement of the sella turcica according to the author's sella turcica size index. Size index above 2.5 was thought to have to be interpreted as an abnormally enlaged sella turcica. 3) Suprasellar cistern was visualized in 87% of 23 normal CT scans as a pentagone(48%) or a hexagone(43%) or a tetragone(9%), but it was identified only in 51% of supratentorial mass lesions. 4) Pituitary adenomas invariably showed positive contrast enhancement(100%), but rarely contained calcification(10%) or dilatation of ventricles(8%). 5) Craniopharyngiomas generally contained large dense calcifications or partly calcified peripheral ring(83%) and cyst formation(83%). Contrast enhancement(67%) and hydrocephalus(50%) were often found, but enlargement of the sella turcica was unusual(3%). 6) As compared to the tendency of anterolateral growth in pituitary adenomas, craniopharyngiomas were often found to extend posterosuperiorly. 7) It was advantageous to perform the operation at the side of predominant location of the tumor in suprasellar cistern or at the side of the cystic portion of the tumor. 8) A case of hypothalamic glioma was operatively verified that appeared as an isodense suprasellar mass, showing marked contrast enhancement.

9.Effect of Multifaceted Interventions for Ward Nurses on the Storage, Conditions, and Transportation of Specimens for Microbial Culture

Purpose:
The purpose of this study was to provide multifaceted interventions for nurses and to confirm changes in their knowledge, perception, and actual practice on the adequate storage, conditions, and transportation of culture specimens.
Methods:
A onegroup pretest-posttest experimental design was conducted with 41 nurses in two general wards of a tertiary acute care hospital in Seoul. Multifaceted interventions including education, feedback, posting guidelines and reminders, and improvement in specimen management accessibility were provided from May 2019 to January 2020. Outcomes were measured before and after the interventions. Knowledge and perception of the nurses were evaluated using self-reported questionnaires and actual practice by observation.
Results:
After the interventions, the average knowledge score on transportation time was significantly increased (Z = -4.89, p < .001). However, the knowledge score on storage methods was not significantly increased. The perception score was significantly increased (t = -3.19, p = .003). The proportion of specimen storage times, places, and conditions managed properly was significantly increased from 43.0% (46/107) to 77.1% (84/109) (p < .001). The average transportation time of blood samples to the laboratory significantly decreased from 3 hours 36 minutes ( ± 1 hour 52 minutes) to 3 hours 1 minute ( ± 1 hour 41 minutes) (t = 2.51, p = .013). The percentage of blood culture specimens arriving within 2 hours was increased significantly from 22.9% to 39.2% ( χ 2 = 6.90, p = .009).
Conclusion
The interventions were effective. However, some specimens remained in the ward longer than expected after the interventions. This requires further interventions.
