1.In vivo31P MR Spectroscopy of Breast Tumors: Preliminary Results.

PURPOSE: To evaluate the various phosphorus metabolism' of breast tumors with use of in vivo phosphorus-31 (31P) M R spectroscopy (MRS) MATERIALS AND METHODS: Five patients with breast tumor (benign in two, malignant in three) and three normal healthy volunteers participated in this study. All in vivo31P MRS examinations were performed on 1.5 Twhole-body MRI/MRS system by using a Free Induction Decay (FID) pulse sequence. Tl-weighted MR images were used for localization of tumors. Peak areas for each phosphorus metabolite were measured using a Marquart algorithm. RESULTS: Breast carcinoma had a substantially larger phosphomonoester (PME) and a smaller phosphocreatine (PCr) peak intensity than normal breast tissue. This was reflected in the relatively higher PME/PCr ratio of breast carcinomas as well as phosphodiester (PDE)/PCr, inorganic phosphate (Pi)/PCr, and adenosine triphosphate (ATP)/PCr ratios, compared with normal controls. The mean pH value of breast tumor demonstrating the alkaline nature was higher than that of normal controls. Spectral patterns between benign breast disease and normal breast tissue were quite similar, and differentiation was not established. CONCLUSION: Our preliminary study suggests that in vivo 31P MRS is a noninvasive examination which may be useful in the early differentiation of malignant breast tumors from normal and benign conditions. However, normal control and benign conditions could not be characterized on the basis of the phosphorus metabolite ratios.

2.Analysis of salpingeal patency in young-aged women underwent diagnostic laparoscopy with Fitz-High-Curtis Syndrome.

OBJECTIVE: To analyze the tubal patency in the young-aged women underwent diagnostic laparoscopy with Fitz-High-Curtis Syndrome (FHCS). METHODS: Clinicopathologic results of young-aged women who underwent diagnostic laparoscopy with pelvic inflammatory disease from March 2005 through April 2007 were reviewed. Twenty six patients aged 19-29 years old and preserved their both tubes after diagnostic laparoscopy were included in this study. All were underwent chromopertubation test (CPT) during laparoscopy and postoperative hysterosalpingography (HSG) 3 months later. RESULTS: Of 26 patients, eight patients were diagnosed with FHCS and the others were not. There was no significant difference in mean age, mean hospitalization days, and frequency in past-history of parturition, abortion, chlamydia and gonococci infection between patients with FHCS and with non-FHCS (P>0.05). There was no significant difference in frequency of tubal obstruction in laparoscopic CPT (P>0.05) but not in postoperative outpatient HSG (P<0.05). CONCLUSION: The tubal obstruction may be more frequent in FHCS than non-FHCS and the reliability of laparoscopic CPT for diagnosing the tubal obstruction is supposed to be low. Therefore, as for FHCS patients, HSG should be executed to investigate tubal factor which is helpful to forecast the fertility.

3.Clinical Evaluation of Oculokinetic Perimetry.

Visual field examination plays an important role in the diagnosis of ocular and neurological disorders. Visual field screening devices have become popular because they require less time than quantitative perimetry, are easier for both the patient and examiner to perform, and can be administered to a larger number of patients. However, these instruments are expensive and in need of specific equipment. To eliminate these disadvantages, Oculokinetic perimeter was designed by Bertil E. Damato. Oculokinetic perimetry can be performed with rapid and simple method and we examined the visual field by Oculokinetic perimeter and compared the results with those using the Goldmann perimeter and Autofield-D static perimeter.

4.Comparison of Diagnostic Accuracy in Uterine Pathology among HSG, Hysteroscopy, and Sono-Hysterography.

Hysterosalpingography and Hysteroscopy have been used for the detection of intraute-rine pathology such as polyps, submucous myomas, intrauterine adhesion and endometrial hyperplasia or cancer. Recently the ultrasound has also been utilized for the detection of uterine pathology. Therefore the purpose of this study was to evaluate the diagnostic efficacy of SonoHysterography in detection of intrauterine pathology compared with HSG and Hysteroscopy. 32 patients underwent Sono-Hysterography and Hysteroscopy for the evaluation of the uterine pathology from september 1995 to January 1996. Nine of 32 patients had infertility problem and HSG performed prior to Sono-Hysterography and Hysteroscopy. The results are as follows : 1. The patients' ages ranged from 20 to 50 years(median 37.9). 2. All 9 patients with infertility who had positive HSG findings in uterine cavity showed the intrauterine pathology in Sono-Hysterography as well as Hysteroscopy. The detail findings are as follows :septated uterus(n=2), intrauterine adhesion :IUA(n=3), endometrial polyp(n=3), and IUA combined endmetrial hyperplasia(n=1). 3. Twenty two of 23 patients with abnormal uterine bleeding showed the intrauterne pathology and one patient had negative finding in Sono-Hysterography. However, Hysteroscopy revealed positive intrauterine pathology in 22 patients who had abnormal uterine bleeding. One patients who had positive finding in Sono-Hysterograply showed negative by Hysterography. In contrast, one patient who had negative finding in Sono-Hysterography had positive uterine pathology with polyp in Hysteroscopy. The histologic pathology in all 23 patients reported endometrial polyp(n=12), placental polyp(n=2), submucous myoma(n=1), endometrial hyperplasia(n=5), endometrial cancer(n=1), normal endometrial finding(n=2). 4. Sono-Hysterography, therefore, has a sensitivity and positive predictive value of 96.6%, 93.5% respectively. Our study showed a positive Sono-Hysterography is very predictive of the intrauterine pathology. Sono-Hysterography is safe, quick and minimal invasive procedure. So it is an invaluable technique in the evaluation of uterine cavity.

5.Carcinoid Tumors of the Gastrointestinal Tract: Analysis of 36 Cases.

We reviewed a total of 36 cases of gastrointestinal carcinoid tumors during a period of ten and a half years at Seoul National University Hospital, looking at their histologic and ultrastructural features, as well as immunohistochemical results, then we correlated these with location. The primary sites of carcinoid tumors were rectum(39%), stomach(25%), appendix(22%), small intestine(8%) and colon(6%), in order of frequency. The predominant histologic types by organ were mixed and solid(A) types in all location, the solid type(type A) in the stomach, and tubular type(type C) in the rectum. Six out of 8 cases of appendiceal carcinoid were type A and the other two cases were mucinous carcinoid. Tubular adenoma or adenocarcinoma was associated with 36% of the rectal carcinoids, with one case of carcinoid in the small intestine, and one(11%) of the gastric carcinoids was found near an adenocarcinoma. Metastasis was found in one case of ileal carcinoid(100%), one case of cecal carcinoid, 3 of gastric carcinoid(33%) and 2 of rectal carcinoid(14%). Primary tumor size ranged from 1 to 7 cm. Tumor necrosis and endolymphatic tumor emboli were found in 57% and 43% of the metastatic carcinoids, respectively. PCNA index showed a borderline significant difference between metastatic and nonmetastatic groups(P value=0.063). The low prevalence of appendiceal carcinoid and the relatively high prevalence of gastric-rectal carcinoid may reflect a low chance of incidental appendectomy and frequent detection of gastrointestinal endoscopy.

6.Primary Cardiac Angiosarcoma: A case report and rewiew of literture.

We report a case of primary cardiac angiosarcoma in a 35 year-old woman. She presented with dyspnea, facial edema and neck vein distension during 1 month. The oval round large mass(8x4 cm) filling the right atrium infiltrated into the entire thickness of the lateral wall and extended to the vena cava and ventricle. The interatrial septum and pericardium seemed to be intact. Despite the surgical excision and adjuvant chemo-and radiotherapy, the patient died 8 months later due to tumor recurrence. The histologic findings of tumor varied from benign looking capillary proliferation mimicking granulation tissue to sarcoma composed of spindle cells. But anastomosing vascular channels lined by anaplastic polygonal cells and intracytoplasmic lumen containing red blood cells were characteristic findings. These tumor cells showed positive reaction to endothelial cell antigen and factor VIII-related antigen. The intercellular junctional complex and vascular channels formed by cytoplasmic process of tumor cells were identified by the ultrastructural study.

7.Neurenteric Cyst of the Posterior Cranial Fossa: Case Report.

Neurenteric cyst(enterogenous cyst) is a rare congenital lesion, usually affecting the lower cervical or upper thoracic areas, and its occurrence in cranial vault is even rarer. We present a patient with intracranial neurenteric cyst anterior to the pons and medulla oblongata. A 40-year old man presented with two-month history of occipital headache and facial spasm. The cyst was decompressed via far lateral suboccipital approach, and the patient recovered without permanent neurological deficits. Features on imaging studies and histological findings are discussed with the review of the literatures.

8.A Case of Immunoglobulin A Nephropathy with Nephrotic Syndrome in a Patient with Behcet's Disease.

Behcet's disease (BD) is a rare multisystemic vasculitis characterized by recurrent oral and genital ulcers, uveitis, and skin lesions. Renal involvement in BD is more frequent than has been recognized. However, there have been a few reports of glomerulonephritis associated with BD in Korea. We report here a case of IgA nephropathy with nephrotic syndrome in a patient with BD. A 44-year-old woman with BD was admitted for generalized edema which had developed 1 month ago. Routine renal workup revealed heavy proteinuria and gross hematuria. Twenty-four hour urinary protein excretion was 9.8 g/day. Renal biopsy demonstrated IgA nephropathy. She was treated with oral prednisolone, colchicine and azathioprine. The edema and proteinuria gradually improved. Three months later, urine protein to creatinine ratio decreased to 1.8 and her renal function was well preserved.

9.Effect of TiN coating of abutment screw on detorque force.

PURPOSE: The aim of this study is to evaluate the effect of TiN coating of abutment screw on the unscrewing torque. MATERIAL AND METHODS: Titanium and Gold-Tite abutment screws were classified into two groups, Group A and C respectively, as control groups. Titanium abutment screws with TiN coatings were also classified into two groups, Group B and D, as experimental ones. Group A and B were tightened to 20 Ncm input torque, and Group C and D were tightened to 32 Ncm torque. Detorque values were measured with digital torque gauge during repeated closing and opening experiment. RESULTS: Abutment screws with TiN coating (Group B and D) showed statistically significant higher mean detorque values than those of Group A and C. DISSCUSION: Physical properties of TiN coating, such as low friction coefficient, high hardness and wear resistance, might contribute to higher detorque values. CONCLUSION: It is suggested that TiN coating of abutment screw help to reduce the risk of screw loosening and improve the stability of screw joint.

10.Clinical Observations on Hyper II(R) Soft Contact Lens.

To estimate the clinical usefulness of a newly developed soft contact lens, Hyper II(R) soft contact lens which is a product of 2-hydroxyethylmethacrylate and special cross linking agents by International Contact Lens Lab., Seoul Korea, clinical observations were carried out in 89 patients(154 eyes) during the follow up period of 3 months from April 1985 to February 1986. Excellent corrected visual acuity(over 1.0) was obtained in 134 eyes(87%). The most common subjective symptom after wearing contact lens was foreign body sensation, but this symptom had diminished during the follow up period. Twentyfour eyes discontinued wearing the lens because of lens related problems including lens deposit(11 eyes), lens lost(3 eyes), foreign body sensation(3 eyes), handling difficulty(2 eyes), lens torn (1 eyes) et al. It can be suggested that Hyper II(R) soft contact lens is clinically available with excellent improvement of visual acuity and few complications, either subjective or objective, which might influence wearing contact lens. Long-term follow up, however, might be necessary to understand and reduce the late complications of this soft contact lens.
