3.Three Cases of Bee-sting Granuloma.

Most insects including the honeybee cause erythema and edema at the site of a sting in non-allergic individuals. This reaction usually resolves within a few days. However, in certain cases, the persistence of inciting materials in the cutaneous tissue may promote a local inflammatory foreign body reaction. Localized nodular lesions were seen in our two patients and erythemstous patches were present in one patient due to rarely encountered complications induced by retained sting materials. In these cases histopathology showed multiple foci of foreign body granuloma containing yellow refraetile material in the center surrounded by histiocytes and foreign body giant cells. The granulomatous skin lesions in these patients were treated with intralesional injections of triamcinolone acetonide and partial excisions of the nodular masses were performed.

4.Nationwide Telephone Survey on Leisure Time Physical Activity among Koreans.

Epidemiologic research has demonstrated that varying grade of physical activity has protective effects on several chronic diseases, including coronary heart disease, hypertension, diabetes mellitus, osteporosis, anxiety and depression. Despite the importance of physical activity as an health promotion factor, there have been only a few research data available in Korea. This telephone interview survey on randomly sampled 1,060 adultsaged 15-69 years was carried out to find out the prevalence of self-reported leisure-time physical activity among Korean adults in terms of health promotion practice. The nationwide telephone interview survey was conducted from April through May 1997. Multistage stratified random sampling method was used for sample selection, the respondent was randomly selected from an entire group of adults who lived at the residence connected with the telephone number. A total of 1,060 adults were interviewed and the interviewees' leisure-time physical activity was categorized into four patterns: 'physically inactive', 'irregularly active', 'regularly active, not intensive', and 'regularly active, intensive'. About 53% of the respondents were physically inactive, 15% irregularly active, 14% regularly active, and only 18% were regularly active, intensive. Women were less active than men anc persons of lower socioeconomic status(SES) were less active than higher SES. Physical activity was not significantly associated with smoking status, drinking pattern and overweight, but negatively associated with daily smoking amount and total duration of smoking in smokers. Individuals with higher lever of physical activity were more likely to check blood pressure regularly and have had screening examination for stomach cancer. Conclusively, the proportion of Korean people who perform appropriate level of physical activity in terms of health promotion is still very low, thus an enforced intervention program becomes indispensable to achieve the national health goal by the year 2000.

5.A case of infected adrenal pseudocyst.

Cystic lesions of the adrenal are extremely rare. Pseudocysts (39% of all adrenal cysts) are the most common clinically detected cysts and result from hemorrhage into normal adrenal gland due to severe infection and trauma or adrenal neoplasm. Histologically, the lining of adrenal pseudocyst is not covered with epithelium and is composed of connective tissue. We are presenting a case of infected adrenal pseudocyst suffering from right lower chest and upper abdominal pain in a 35-year-old woman with brief review of the literatures.

Heterotopic pregnancy occurs when there are coexisiting intrauterine and ectopic pregnances. It occure, more often, due to assisted reproductive technology such as IVF-ET, GIFT and increased pelvic inflammatory diseases. We present a case of heterotopic pregnancy confirmed by surgical removal of ruptured left ovarian pregnancy and sonographic finding of the intrauterine pregnancy following natural conception.

8.Short Term Result of Pubovaginal Sling Procedure Using Polypropylene Mesh for Female Stress Urinary Incontinence: Success Rate, Satisfaction, Risk Factors.

PURPOSE: Sling procedures have been used successfully for the treatment of all type of female stress urinary incontinence. Polypropylene mesh has more biocompatibility with less erosion rate over other synthetic sling materials. We investigated the objective, subjective success rate, satisfaction on pubovaginal sling operation using polypropylene mesh in stress urinary incontinence women. MATERIALS AND METHODS: Between November 2001 and July 2002, thirty three women with stress urinary incontinence underwent polypropylene mesh sling procedure were analyzed. Preoperative evaluations included the patient's history, a physical examination, urinalysis, a urodynamic test, incontinence staging with Stamey grade, Balivas type, and so forth. A 1.5x20cm polypropylene mesh was placed under the bladder neck to proximal urethra. Postoperatively, the patients were evaluated with a symptom questionnaire, physical examination, uroflowmetry, and postvoid residual volume at 3 day, 3 months. RESULTS: The average follow-up period was 164.6 days (minimum; 2 months). The average operation time was 83.6 minutes (including the anesthesia). No major intra-operative, post-operative complication occurred. No patient has permanent retention, erosion, or repeated surgery. The 22 patients (66.7%) were completely continent, 11 (33.3%) had an improvement, subjectively. The 27 patients (81.8%) were completely continent, 6 (18.2%) had an improvement, objectively. The treatment result was showed satisfactory by all patients (very satisfaction; 20 (60.6%), satisfaction; 13 (30.4%)). The follow-up period, valsalva leak point pressure, Pdetmax, opened bladder neck at rest, preop. urgency, preop. and postop. urge incontinence, total score of preop. and postop. Urge syndrome were related to subjective success rate, satisfaction statistically (p<0.05). CONCLUSION: Placement of a polypropylene mesh under the bladder neck to proximal urethra provides a simple, safe, inexpensive and effective method to correct stress urinary incontinence.

9.Further understanding of fat biology: Lessons from a fat fly.

Obesity is a leading risk factor for insulin resistance, hypertension, hyperlipidemia, and cardiovascular complications, collectively referred to as metabolic diseases. Given the prevalence of obesity and its associated medical problems, new strategies are required to prevent or treat obesity and obesity-related metabolic effects. Here we summarize contributors of obesity, and molecular mechanisms controlling adipogenesis from studies in mammalian systems. We also discuss the possibilities of using Drosophila as a genetic model system to advance our understanding of players in fat biology.

10.The Effects of Intravesical BCG Instillation on Bladder Cancer Induced by N-butyl-N-(4-hydroxybutyl) Nitrosamine.

PURPOSE: We investigate the effects of intravesical BCG therapy on the occurance of superficial bladder cancer induced by N-butyl-N-(4-hydroxybutyl) nitrosamine(BBN) in Fisher 344 rats in vivo. We also examine whether NO mediated the antitumor activity of BCG against superficial bladder cancer in Fisher 344 rats. MATERIALS AND METHODS: BBN(0.1%) is orally administered for 20 weeks and it is combined with BCG(0.27mg) or NG-monomethyl-L-arginine (NG MMA, 10mg). once every week from 8th week to 19th week. The rats are sacrified at 20th week. NO secretion in urine for 24 ours is significantly increased in the BCG treated rats compared to the animals treated with saline or NGMMA. RESULTS: Pathologic findings demonstrate that the incidence of carcinoma is not statistically different in saline, BCG, NGMMA(p>0.05). However the size and number of tumor is decreased in the BCG treated rats compared with saline or NGMMA treated rats bearing bladder cancer induced by BBN(p<0.05). Inducible NO synthase(iNOS) is strongly induced in bladder tissue of rats treated with BCG and NGMMA but not in saline. CONCLUSIONS: Intravesical instillation of BCG on bladder cancer induced by BBN does not decrease the cancer occurrence but reduces the number and size of bladder cancer. Our results suggest that nitric oxide induced by intravesical instillation of BCG may mediate antitumor activity against the occupance of superficial bladder cancer.
