1.A Case of Kaposi's Varicelliform Eruption Occuring in Pemphigus Foliaceus.

This report presents a case of a 17-year-old-girl who developed a, Kaposis varicelliforrn eruption(eczema herpeticum) during oral prednisolone therapy in order to control pemphigus foliaceus. Diagnosis was made by the history, clinical feature and histologic examination. Within 24 hours after the initiation of therapy with ribavirin(Viramid), new lesions had ceased to develop.

2.Detection of Human Papillomavirus in Lesions of Uterine Cervix Immunohistochemistry and in situ Hybridization.

To evaluate the detection of HPV DNA according to subtype of lesions of uterine cervix and its clinical applicability, in situ hybridization (ISH) and immunohistochemistry for HPV were performed in 189 cases of uterine cervical lesion, including 23 cases of low grade squamous intraepithelial lesion (SIL), 115 cases of high grade SIL and 51 cases of invasive carcinoma. Positive immunostaining, brown precipitate, was mainly noted in the nucleus of koilocytes in the superficial and intermediate layer. Positivity of immunostaining was 21.7% in low grade SIL, 13.0% in high grade SIL and 9.8% in invasive carcinoma. Positive reaction in ISH, red precipitate, was noted in the nucleus of not only koilocytes but also non-koilocytes in the superficial and intermediate layer, and dot precipitate was rarely identified in the nest of squamous cell carcinoma. Based on HPV subtype, 6/11 was 21.7% in low grade SIL, 16/18 was 32.2% and 39.2% in high grade SIL and invasive carcinoma, respectively. With regard to their associated HPV types, low grade SILs were heterogeneous and high grade SILs and invasive carcinomas were related with the high oncogenic risk group only. The correlation of HPV subtypes with panHPV was 91.3% in low grade SIL, 91.3% in high grade SIL and 98.0% in invasive carcinoma. These results suggest that detection of HPV infection by ISH may be a more useful method than immunohistochemistry and application of the HPV subtype probe with the panHPV probe could improve the sensitivity of ISH.

5.Effects of gamma-irradiation on intracellular proliferation of Toxoplasma gondii RH tachyzoites.

A quantitative assay was performed on the effects of gamma-irradiation (30-300 Gy) on intracellular proliferation of Toxoplasma gondii RH tachyzoites in human leukemic HL-60 cells and murine peritoneal macrophages by means of 3H-uracil uptake assay. Infected non- irradiation group (NI) and uninfected group (incubating only host cells) were prepared. The 3H-uracil uptake by tachyzoites of NI group 12-24 hrs after infection was 2,190-4,787 counts per minute for macrophages and 2,967-8,254 for HL-60 cells, whereas the irradiated tachyzoites revealed only 381-703 (100 Gy) and 218-408 (300 Gy) for macrophages, and 1,911-2,618 (30 Gy), 1,253-1,384 (70 Gy), 1,013-1,090 (100 Gy), and 483-588 (300 Gy) for HL-60 cells. The proliferation inhibition rate was similar in macrophages and HL-60 cells, for example, 89-94% and 80-94% respectively by 300 Gy, 12-24 hrs after infection. It is concluded that RH tachyzoites of T. gondii are severely affected by gamma-irradiation in their capability of intracellular proliferation.

6.Radiology findings of abdominal cystic lymphangioma.

Abdominal cystic lymphangioma is a rare congenital malformation of lymphatics. Prognosis is excellent with exact diagnosis and complete surgical excision. The aughors analysed 10 US scans and 9 CT scans of surgically proven cystic lymphangiom for the last 5 years. US scan showed it as a septated cystic mass and 2 cases showed fluid-fluid level. CT scan showed a huge unilocular or multilocular density mass with uniformly thickened septae. A huge unilocular or multilocular cystic mass with uniformly thickened septae could suggest cystic lymphangioma would be differentiated from the other cystic masse.

7.Leiomyosarcoma of the Pancreas: A case report.

Most of the malignant tumors of the pancreas are adenocarcinomas arising from the ductal epithelium. Primary leiomyosarcoma of the pancreas, even though it is the most common sarcoma of the pancreas, is very rare. We present a case of leiomyosarcoma of the pancreas, probably primary, with metastases to the stomach, lymph nodes, and abdominal wall. A 52-year-old woman visited the hospital with vague right upper abdominal pain and weigh loss of 6 kg for 2 months. The radiological and endoscopic examination revealed that she had a large heterogeneous mass, 11 cm in size, in the pancreatic body and tail, a 4 cm-sized mass in the paraaortic area, and a 3 cm-sized polypoid mass in the stomach. Histologically, they were all similar to one another and composed of markedly pleomorphic cells. Immunohistochemical and electron microscopic studies showed definite smooth muscle differentiation of the tumor cells. Two months later, the patient underwent an excision of a new 3 cm mass in the right lower abdominal wall, showing features of well differentiated leiomyosarcoma.

9.Clustered Microcalcifications on Mammography: Histopathologic Correlation in Benign and Malignant Lesions.

PURPOSE: To evaluate differential points of clustered microcalcification in malignant and benign lesions on histopathology and correlate with mammographic findings. MATERIALS AND METHODS: Authors evaluated differential points of malignant and benign calcifications in histopathologically confirmed 24 cases. In two patients, mapping studies and paraffin block mammographic examinations were done to correlate calcification on mammography with histopathologic morphology. RESULT: On histopathology, malignant calcifications were located in ductal lumen, while benign ones were usually located in ductal epithelium(p<0.05). However, associated three benign lesions of the 10 malignant lesions showed still benign calcifications adjacent to proven cancer. The typical malignant calcifications were the shape of lamination, while benign calcifications showed granular and salty on light microscope. In 2 mapping studies, malignant-looking calcifications within the area of predominant benign calcifications yielded atypical ductal hyperplasia. Mammographic pepper, granular, punctate, salty and S-shaped calcifications were correlated with granular and salty calcification on light microscope. Mammographic linear, branching, comma, tadpole and wormiform calcifications were correlated with histopathologic laminated calcification. CONCLUSION: Authors have recognized about characteristics of malignant Vs. benign clustered microcalcifications on histopathology. Authors concluded that if malignant-looking calcification such as linear, branching, comma, tadpole and wormiform are present in an are a of predominant benign calcifications, specimen radiographic correlation and mapping study will be necessary for better demonstration of the atypical ductal hyperplasia, hidden or associated breast cancer.

10.A clinical study of Henich-Schonlein purpura in childhood.

BACKGROUND AND OBJECTIVE: The clinical manifestation of Henoch-Schonlein purpura and existance of renal involvement may influence on its course and prognosis. To verify prevention with early administration of steroid, we studied the efficacy of corticosteroid treatment. MATERIAL AND METHOD: We analysed 65 children under 15 years of age with Henoch-Scho nlein purpura according to their age, sex, and seasonal incidence. Forty children showed typical skin lesions, arthralgia and abdominal pain. We have divided them two groups. Group A consisted of 20 children who received 1 mg/kg of prednisolone/perday for 2 weeks and group B did not. We carried out their physical examination and urinalysis monthly for a year. RESULT: The main clinical manifestations were skin rash(100% ), abdominal symptoms(41.5 %), joint symptoms (49.2%), and renal involvement(34%). As for gastrointestinal symptoms, abdominal pain(66.7% ) was most commonly observed one and others were nausea or vomiting (44.7%) and melena(25.9%). The joint involvement was observed mostly in knee(56.3%) and ankle joint(31.3% ), Hematuria was observed in all cases with renal involvement and proteinuria, in 28% of them. The improvement of renal manifestations were noted in 84.2% of them within 4 months. Mild elevation of IgA was more frequently observed in renal involvement group than non-involved group (p< 0.01). There were no significant differences in immonologic parameters such as IgG, IgM, IgE, duration of the acute phase and severity of cutaneous symptoms between two growps. None of steroid treated growp showed progression of nephropathy. Of the 20 non-steroid treated growp, 2(10%) developed nephropathy. Conclusion We may suggest that existance of renal involvement in Henoch-Schonlein purpura influences its course and prognosis.
