1.Spinal Cord Compression Caused by a Metastatic Bone Tumor in Buerger's Disease Patient.

Buerger's disease (thromboangiitis obliterans) is an inflammatory, nonatherosclerotic, occlusive disease of small- and medium-sized arteries and veins that involves distal vessels of the extremities. Patients show symptoms related to ischemia of distal lower extremities and upper extremities (claudication, gangrene, rest pain, distal cyanosis). We report a case of progressive ascending motor weakness of the lower extremities in a Buerger's disease patient due to a metastatic bone tumor on the T3 level after lumbar epidural block and continuous cervical epidural block.

2.FDG-PET/CT Complements Bone Scan with Respect to the Detection of Skip Metastasis of Osteosarcoma: A Case Report.

Skip lesion is not uncommon feature in osteosarcoma and considered to be importantly associated with poor prognosis factor, and thus, should be excised with the main mass. The accurate pre-operative evaluation of the intramedullary extent of osteosarcoma is essential, because it determines the level of bone resection. Among the reliable detection methods, bone scan has a drawback of high rate of false negative results and regional MRI has a difficulty to cover the whole involved lesions without clinical suspicion. The authors report a case of osteosarcoma of the distal femur with a proximal skip lesion that was not detected by either regional MR imaging or by bone scan, but which was visualized by FDG-PET/CT.

3.An Approach for Endotracheal Intubation in Acromegaly Patient.

Acromegaly is a chronic,debilitating disease by excessive growth hormone production due to pituitary adenoma. Anesthesiologists are frequently confronted with problems such as excessive mandible protrusion, macroglossia and tracheal stenosis, which make intubation very difficult and risky in acromegalic patients.So, the authors tried to predict difficult intubation because acromegalic patients have anatomical change of airway.With X-ray film of acromegalic patient, measuring atlanto-occipital gap(2.5 mm), mandible ramus angle(8.4 cm), mandibulo-hyoid distance (35 mm), level of hyoid bone (between 4th and 5th cervical bone) provide clues to perceive difficult intubation. By abnormal data of these measurements, we can anticipate the difficult intubation with moderate accuracy compared with normal patient. It is prudent to consider using other methods such as fiberoptic bronchoscopic intubation, blind endotracheal intubation and transtracheal jet ventilation when difficult endotracheal intubation is predicted in those patients. 4.A clinical study on early diagnosis in neonatal sepsis.

Neonatal sepsis is a major contributing factor for increased mortality and morbidity in the newbron and its successful outcome depends on early detection and prompt institution of an effective antimic robial therapy. Accurate diagnosis of the neonatal septicemia, however, is quite difficult because its early signs and symptoms are rather subtle. This study was conducted in an attempt to search for useful early markers for the clinical entity A total of 224 newborns, over a 17 month pcriod (1/90 5/91), who were evaluated for probable neonatal sepsis were analysed. The laboratory parameters that showed highest correlation with bacteriologically proves sopsis were as follows: Total WBC counts (<5,000/mm), Band/Total ncutrophils(>0.2), elevated ESR(>15mm/hr), CRP(>6ng/ml). Of the 224 study subjects, 41 had two or more parameters of which 12 subjects had positive blood cultures(positive predictive value:29%), only 4 of the 183 newborns who had less than 2 parameters had positive blood cultures(false negative:2%). It is concluded that the presence of two or more of the above parameters can be used as a strong indicator for neonatal sepsis and its sensitivity is 75% while specificity is 86%.

5.Diathermic Capsulotomy for Phacoemulsification in Hypermature Cataract.

To evaluate the result of diathermic capsulotomy for phacoemulsification in hypermature intumescent cataracts, we retrospectively studied 34 patients(35 eyes) who were operated from January 1995 to February 1997. Central corneal endothelia cell counts were taken preoperatively and 2 months postoperatively by non-contact specular microscopy and endothelial cell loss(%) was calculated. The anterior capsular radial tear occurred in 24 eyes(68%), posterior capsular rupture occurred in 1 eye(2.9%) and the percentage of postoperative endothelial cell loss was 6.45(+/-7.27)%. The high rate of anterior capsular radialtear was due to altered mechanical integrity of the diathermic capsulotomy edge. But the thermal effect on the corneal endothelium was minute. Based on these findings, the diathermic capsulotomy for phacoemulsification in hypermature intumescent cataracts is very useful technique.

6.Implantable Contact Lens to Correct High Myopia(Clinical Study with 24 Months Follow-up).

A ICL TM(STAAR Collamer Posterior chamber Implantable Contact Lens) lents ranging from -11D to -21.3D. And then we investigated the visualwas implanted in 12 eyes of 7 patients with preoperative spherical equivalents ranging from -11 D to -21.3 D. And then we investigated the visual acuity, mean endothelial cell loss, change of refractive error and postoperative complications.Average follow-up was 28.1 months. In the immediate postoperative days, all eyes had a significant increase in uncorrected visual acuity and the best corrected visual acuity was improved in 7 eyes(58.3%). Mean percent difference between preoperative and postoperative endothelial cell count was 3.22+/-0.05% . The mean preoperative spherical equivalent refraction was -15.65+/-2.78 D. At a mean follow-up of 1 month, mean spherical equivalent refraction was -1.67+/-0.72D, at a 6months, 12 months, 24 months, mean spherical equivalent refraction was each -1.56+/-0.72D, -1.63+/-0.68D, -1.73+/-0.76D. During follow-up days one eye developed an faint anterior subcapsular cataract but the patient remained symptom free. Other reported complication was transient increased intraocular pressure. Although the follow-up in our study was limited cases(12 eyes), from our experience, the posterior chamber implantable contact lens was efficacious and safe in the correction of high myopic refraction errors.
