1.CT findings of thoracic involvement of lymphoma.

Chest CT scans of 70 patents with malignant lymphoma were reviewed to evaluate the thoacic manifestation of malignant lymphoma. Sixteen patients had Hodgkin's disease and 54 patients had non-Hodgkin's lymphoma. The thoracic involvement of malignant lymphoma was observed i 47 patients (67.1%) and 11 of these patients had Hodgkin's disease, and 36 had non-Hodgkin's lympoma. The most common finding was mediastinal lymphadenopathy and the most frequently involved sites were anterior mediastinal and paratracheal lymph nodes. Pulmonary parenchymal involvement was seen in 11 patients (15.7%), and CT scan showed thickening of bronchovascular bundle, parenchymal consolidation and nodules. Pleural effusion was seen in 18 patients (25.7%), however, without any evidence of parietal pleural thickening in all cases. Involvement of chest wall and breast was seen in two patient(2.9%). The data obtained through the current study showed no differences from those of previous reports, except the fact that there was no CT evidence of pleural thickening in patients who had pleural effusion.

3.A Case of Eccrine Spiradenoma.

We present a case of eccrine spiradenoma occurring on the trunk in a 33 year old woman. She developed a pea sized, pinkish tender nodule on the anterior chest 8 years ago. On histologic examination, there were several lobules surrounded by thin connective tissue septa in dermis. The lobules themselves consisted of small dark staining cells and large pale-staining cells, forming solid cords or ducts.

5.The p53 Mutation and DNA Ploidy in Human Metastatic Breast Cancer.

The p53 gene, one of the tumor suppressor genes, is believed to play an important role through mutation and overexpression in the progression of various human malignant tumors. To compare the p53 mutation status between the primary and metastatic lesions of breast cancers and to investigate the mutational pattern of p53, immunohistochemistry (IHC) and polymerase chain reaction and single strand conformational polymorphism (PCR-SSCP) were performed in 25 cases of breast cancers with paraffin embedded tissue. Mutant protein products or point mutation were detected through IHC or PCR-SSCP method. And flow cytometrical (FCM) analysis were performed in the same paraffin blocks to correlate the DNA ploidy and p53 mutation. The following results are summarized. 1. The detection of the p53 gene mutation and overexpression of the p53 protein were measured in 40% and 48%, respectively, in 25 primary tumors, either or both methods was detected in 64%. 2. A concordance rate of the p53 protein expression between the primary and metastatic lesions of 25 breast cancers was 100%, but the concordance rate of the p53 gene mutation was 72%. 3. The correlation between the p53 mutation and the DNA aneuploidy was not statistically significant (p=0.38) 4. A p53 mutation by IHC or PCR-SSCP was more frequently detected in grade III breast cancers than in grade I or II. 5. Among 5 to 9 exons of the p53 gene, exon 7 was the most frequent mutation spot in this study. 6. Additional mutation of the p53 gene was developed in the three metastatic lesions. With the above results it is suggested that the p53 protein overexpression by immunohistochemistry is not correlated with the p53 mutation by PCR-SSCP. The p53 mutation pattern between the primary and metastatic lesions are not idenitical and an additional point mutation can occur in the metastatic lesion. The DNA aneuploidy is more frequently detected in the cases with the p53 protein overexpression than in the p53 protein negative, but it is not statistically significant.

7.A Case of Wells' Syndrome Associated with Pancreatic Carcinoma.

Wells' syndrome or eosinophilic cellulitis is an uncommon cutaneous disorder characterized by recurrent erythematous and edematous plaques that often resemble acute cellulitis. The characteristic microscopic findings are diffuse dermal eosinophilia with marked edema, and flame figures consisting of amorphous or granular eosinophilic material around collagen bundles of the dermis. Etiology and pathogenesis are unknown, but the disease has been found to be associated with infections, arthropod bites, drug administration, surgery and hematologic disorders. We report the case of a 55-year-old man with pancreatic carcinoma who was presented with an asymptomatic erythematous to brownish subcutaneous nodule on the left buttock of several days' duration. A biopsy specimen taken from the erythematous to brownish nodule showed many flame figures and diffuse infiltration of eosinophils in the dermis and subcutaneous fat tissue. The rapid resolution of the skin lesion with chemotherapy of the pancreatic carcinoma supports a relationship between the two conditions.

8.A Study on the Skin Thickness in Koreans by Computed Tomography.

BACKGROUND: Several methods have been developed to measure skin thickness, including the use of the radiography, micrometer screw gauge, Harpenden's caliper, and ultrasound, but there are no reports to measure skin thickness using computed tomography. OBJECTIVE: We measured skin thickness on the CT films retrospectively. This study was to investigaste variations of skin thickness by age, gender, and different sites. METHODS: Skin thickness was measured on the CT films of 562 patients, including 357 males and 205 females who had undergone brain, chest, and abdominal CT in our hospital. Patients with endocrinologic, malignant, or collagen disease and those on antimetabolite or steroid therapy were excluded. Comparisons of histometric and computed tomographic methods weremade in 5 skin tumors and 1 localized lipodystrophy. Results: 1. The measurements of the skin thickness(mean) in examined sites varied from 0.84 to 3.07 milimeters : the posterior part at the level of the chest(aortic arch level) was the thickest and the anterior part at the level of the forehead(aortic arch level) was the thickest and the anterior part at the level of the forehead(frontal sinus level) was the thinnest. 2. Skin thickness in males wal generally thicker than in females : the anterior and lateral part at the level of the forehead(P<0.0001), the posterior part at the level of the bladder(P<0.2) were thicker in males than in females. 3. In each gender, the thickness of the skin increased with age of 40 or 60, and then decreased with age at every level. 4. Skin on posterior parts of the body was thicker than on other parts. 5. Unlike other levels, skin was thicker on the lateral parts than on the anterior parts at the levels of the forehead and bladder. 6. The measured skin thickness in about 7% greater in formalin fixed tissues than on CT films. CONCLUSION; Computed tomographic measurement of the skin thickness in a reliable method in the aquisition of normal values and their individual variations during cutaneous aging. Moreover, this method is useful in evaluating skin tumors and monitoring the response to therapy of inflammatory conditions.

9.A Case of Morphea Profunda.

We report a case of morphea profunda in a 21-year-old male who had diffuse brown sclerotic plaques on his extremities. Laboratory findings showed peripheral eosinophilia and an increased titers for anti-DNA and anti-nuclear antibodies. Histopathologic findings showed diffuse fibrosis and a thickening of the lower dermis and subcutaneous tissue. He has been treated with hydroxychloroquine 400mg per day and the sclerosis of the skin improved.
