1.Angiofollicular Lymph Node Hyperplasia(Castleman's disease): 3 cases report.

Angiofollicular lymph node hyperplasia(AFLNH) was first described in 1956 by Castleman et al. It was initially reported as a solitary mediastinal mass but multicentric and extranodal disease is now well known. Histologically two distinct variants, e.g. the hyaline vascular type and the plasma cell type, of AFLNH are recognized. And the plasma cell type is typically associated with clinical syndrome consisting of fever, anemia, elevated erythrocyte sedimentation rate and polyclonal hypergammaglobulinemia. Recently, we experineced three cases of AFLNH. Histologically, two cases were hyaline vascular type, that were presented as a right supraclavicular mass of 49-year-old female, and as an anterior mediastinal mass of 53-year-old female. The remaining one case was plasma cell type that was presented as a left axillary mass of 63-year-old male. The former two cases showed typical features of hyaline vascular type but in case 1, exuberant proliferation of hyalinized vessels of capillary size was characteristic feature. The latter case of plasma cell type characteristically showed clinical syndrome consisting of fever, hypoalbuminemia, polyclonal hypergammaglobulinemia. All cases were presented as a single mass and they were well after surgical excision.

2.A case of emphysematous pyelonephritis with emphysematous cystitis

Emphysematous pyelonephritis is rare but frequently fatal disease that usually occures in diabetic patients, 6cases having been reported in Korea. We have experienced a case of simultaneously occuring emphysematouspyelonephritis and emphysematous cystitis in 54 years old female patient, which have never been reported in Korea,and got satisfactory improvement with medical treatment.

Malignant mixed miillerian tumor (MMMT) was one of the rare uterine sarcoma. This tumor is composed of sarcoma and carcinoma. Primary MMMT in the peritoneum is among the rarest sites for MMMT. MMMT is highly malignant and the prognosis is grave due to frequent recurrence and metastasis. In the world literature, only 22 cases with primary MMMT of the peritoneum have been reported and most of these were single report. We report one case of primary MMMT in the peritoneum with a brief review.

Aspergillosis is a rare disease of fungal infection produced by the genus Aspergillus in immunocompromised patients. It mainly involves the lung and may disseminate by invasion of biood vessels into the gastrointestinal tract. Brain and kidney. 5.Combined Clear Corneal Phacoemulsification and Pars Plana Vitrectomy.

PURPOSE: To report the technical effectiveness, outcome, and incidence of complications of combined clear cornea phacoemulsification with intraocular lens implantation and pars plana vitrectomy. METHODS: Single-center prospective, non-comparative, consecutive case series. Twenty-three eyes of 23 patients underwent combined clear cornea phacoemulsification and pars plana vitrectomy for various vitreoretinal disorders due to cataracts by a single surgeon between July 2003 and August 2004. Surgical feasibility, intraoperative and postoperative complications and outcome of clear cornea incisions in all combined procedure were evaluated. RESULTS: No significant adverse effect of clear corneal wound on pars plana vitrectomy procedures was noted in any of the patients. Operatively and postoperatively, no significant complications related to clear corneal wound were noted, even with the variations of pars plana vitrectomy procedures for cases with and without gas-fluid exchange and intravitreal tamponade. CONCLUSIONS: Clear corneal incisions is a safe and more reasonable alternative in combined phacoemulsification with intraocular lens implantation and pars plana vitrectomy. For a more comprehensive conclusion, a comparative study with scleral tunnel incisions may be necessary.

6.CT Findings of Spontaneous Subarachnoid Hemorrhage of Unknown Cause.

PURPOSE: Spontaneous subarachnoid hemorrhage(S-SAH) is a relatively rare disease with good clinical course and prognosis. The purpose of this study is to evaluate the cisternal blood distribution on CT in patient with S-SAH. MATERIALS AND METHODS: Out of 406 patients with subarachnoid hemorrhage on brain CT scan, 11 patients confirmed to be S-SAH by angiography and clinical follow-up were examined. We analysed the CT findings of these 11 cases retrospectively. RESULTS: Five patients had only perimesencephalic distribution of hemorrhage:the prepontine, the interped- uncular, and the suprasellar cisterns were involved in 4 cases each, the basal sylvian cistern in 3 cases, and the ambient cistern in 2 cases. The quadrigeminal and a portion of the lateral sylvians were also involved in 1 case each. Six patients revealed cisternal hemorrhage extending beyond the perimesencephalic cistern into the ventricles, cortical sulci, and lateral sylvian fissure. Intraventricular hemorrhage was noted in 3 cases of these 6 patients. CONCLUSION: Our study suggests that uncomplicated clinical course and normal anglographic finding are more important than CT distribution of cisternal hemorrhage in diagnosing S-SAH.

7.CT and MRI Findings of 4th Ventricular Tumors.

PURPOSE: The purpose of our study is to evaluate characteristic features of 4th ventricular tumors in CT and MRI. MATERIALS AND METHODS: Pathologically proved 9 patients with 4th ventricular tumor were examined by CT and/or MRI. 4th ventricular tumors were epedymoma(4 cases), medulloblastoma(2 cases), choroid plexus papilIoma(2 cases), and oligodendroglioma(1 case). Included in our study were only those mass lesions that were located at surgery predominently within 4th ventricle with or without ventricular expansion. RESULTS: The origin of 4th ventricular tumor was the roof (epedymoma 3 cases, medulloblastoma 2 cases), the floor (epedymoma 1 case), and the undetermined(remaining 3 cases). On MRI, all tumors were hypointense exept ependymoma(3 cases) showing isointensity on T1WI. All tumors were hyperintense on PDWI and T2WI. On Gd-DTPA enhanced T1WI, strong enhancement was seen in all but epedymoma(1 case) which showed mild enhancement. On CT, as compared with MR images, various density on precontrast and postcontrast images were seen. Calcification was seen in choroid plexus papilloma(1 case) and oligodendroglioma(1 case). Hydrocephalus is seen in all cases exept ependymoma(2 cases) and oligodendroglioma(1 case). Hemorrhage within tumor was present only in epedymoma(2 cases). Cystic change or necrosis of tumor was seen in ependymomma(3 cases), choroid plexus papilloma(1 case), and oligodendroglioma(1 case). Peritumoral edema was seen in medulloblastoma(1 case). Extension through the foramen Luschka and the Magendie was seen in ependymoma(2 cases), choroid plexus papilloma(2 cases), and medulloblastoma(1 case). Seeding along the CSF pathway was seen only in ependymoma(2 cases). CONCLUSION: The results of our study may suggest that specific diagnosis of 4th ventricular tumor can be suggested preoperatively by analysing the origin in 4th ventricle, difference of CT density or MRI signal inten. sity, presence of extension or seeding through cerebrospinal fluid of the lesion.

8.Chemical composition of nuts and seeds sold in Korea.

Eleven types of nuts and seeds were analyzed to determine their energy (326-733 mg), moisture (1.6-18.3 mg), carbohydrate (8.8-70.9 mg), protein (4.9-30.5 mg), lipid (2.5-69.8 mg), and ash (1.2-5.5 mg) contents per 100 g of sample. Energy content was highest in pine nuts (733 mg/100 g), carbohydrate level was highest in dried figs (70.9 mg/100 g) and protein was highest in peanuts (30.5 mg/100 g). The amino acid compositions of nuts and seeds were characterized by the dominance of hydrophobic (range = 1,348.6-10,284.6 mg), hydrophilic (range = 341.1-3,244.3 mg), acidic (range = 956.1-8,426.5 mg), and basic (range = 408.6-4,738.5 mg) amino acids. Monounsaturated fatty acids (MUFA) were highest in macadamia nuts (81.3%), whereas polyunsaturated fatty acids (PUFA) were highest in the walnuts (76.7%). Macadamia nuts did not contain any vitamin E, whereas sunflower seeds contained the highest level (60.3 mg/kg). Iron (Fe) content was highest in pumpkin seeds (95.85 +/- 33.01 ppm), zinc (Zn) content was highest in pistachios (67.24 +/- 30.25 ppm), copper (Cu) content was greatest in walnuts (25.45 +/- 21.51 ppm), and lead (Pb) content was greatest in wheat nuts (25.49 +/- 4.64 ppm), significantly (P < 0.05). In conclusion, current commercial nuts and seeds have no safety concerns, although further analysis of Pb contents is necessary to ensure safety.

9.Fermented soybeans by Rhizopus oligosporus reduce femoral bone loss in ovariectomized rats.

BACKGROUND/OBJECTIVES: Soy isoflavones are structurally similar to estrogen and bind to estrogen receptors, suggesting that they exhibit estrogenic activities; therefore, they are referred to as phytoestrogens. Fermentation may affect the bioavailability of isoflavones altering soy isoflavone glycosides in the form of aglycones. Thus, this study investigated the effects of fermented soybeans by Rhizopus oligosporus on bone metabolism in both young rats as a pilot test and in ovariectomized (ovx) old rats as a model of menopause. MATERIALS/METHODS: In the pilot test, a total of 24 seven-week-old female Sprague-Dawley (SD) rats were fed one of three diets for a period of four weeks: casein, unfermented soybean product, or fermented soybean product by R. oligosporus. In the ovx rat model, 20-week-old SD rats weighing 260-290 g underwent either sham-operation (n = 10) or bilateral ovariectomy (n = 30) and were then fed the AIN-93M diet for one week. Thereafter, rats were fed sham-casein, ovx-casein, ovx-soybean, or ovx-fermented soybean diet for five weeks. After decapitation, femoral bones were isolated and preserved in 9% formalin for assessment of bone mineral density (BMD), bone mineral content (BMC), and bone-breaking strength (BBS). RESULTS: Ovx rats showed significantly increased weight gain and decreased uterine wet weight. Of particular interest, ovx rats fed fermented soybeans showed increased uterine wet weights compared to control rats. Fermented soybean diet caused a significant increase in plasma 17-beta estradiol concentrations in young rats, and 17-beta estradiol levels were enhanced in ovx rats to match those of sham-operated ones. Significantly lower femoral BMD and BMC were observed in ovx rats compared to sham-operated controls, whereas bone areas did not differ statistically among the groups. In addition, BBS tended to be increased in ovx rats fed soybeans and fermented soybeans. CONCLUSIONS: Supplementation of fermented soybeans could have preventive and therapeutic effects against osteoporosis in postmenopausal women.
