1.Clinical Course of IgA Nephropathy in Children.

A statistical analysis of the diagnostic value for 244 aspiration biopsy cytology(ABC) among a total 1,043 cases from various sites was performed. ABC, using diagnostic terminology similar to that of a surgical pathology reports, was compared to the final tissue diagnosis. For the entire series, a sensitivity of 91.8%, a specificity of 99.3%, a positive predictive value of 98.9%, a negative predictive value of 94.8%, and an efficacy of the test of 96.3% were shown. There were 8 false negative and 1 false positive diagnosis. The diagnostic accuracy was 89.8%. Those results indicate that the ABC is a considerably highly accurate procedure that should be routinely employed.

2.Cyclosporine A ( Cipol-N R ) Therapy in Children with Idiopathle Nephrotic Syndrome.

A 30-year-old woman who was diagnosed as peripheral neuroblastoma by fine needle aspiration of a soft mass of the right upper arm is described. She presented a slowly growing, soft mass of the right upper arm for 1 month. The right humerus revealed no abnormal finding on X-ray. Ultrasonogram of the right upper arm revealed a well demarcated, smooth marginated solid mass without invasion of adjacent structures. Fine needle aspiration was done under the impression of soft tissue tumor with undetermined biologic behavior. The aspirates were highly cellular and the tumor cells were dispersed both singly and in clusters of varying size. The clusters occasionally showed a central capillary core and rosette-like structures. The tumor cells were small in size and had a small to medium amount of cytoplasm. Some of them revealed slender cytoplasmic processes. The nuclei showed distinct nuclear membranes, finely clumped chromatin and small conspicuous nucleoli. Cellular pleomorphism or mitotic figure was not definite. These cytologic findings were interpreted as a malignant, non-lymphomatous, small round cell tumor, most likely representing peripheral neuroblastoma or Ewing's sarcoma. Final diagnosis was confirmed by simple excision as peripheral neuroblastoma.

3.Colonic Diverticulosis: Evaluation wth Double Contrast Barium Enema.

PURPOSE: To evaluate the pattern of colonic diverticulosis according to age and sex, and recent trend. MATERIALS AND METHODS: The authors retrospectively reviewed 120 cases of colonic diverticulosis in 1,020 patients who had undergone a double contrast barium enema examination between January 1st, 1993, and December 31st, 1995, and analyzed the frequency, size, multiplicity and anatomical site, according to age and sex. Diverticulum size was classified into one of three groups : less than 5mm, 5-10mm, over 10mm in diameter. RESULTS: The overall incidence of colonic diverticulosis was 120 cases among 1,020 patients(11.8%) with an incidence 5.3 times higher in males than in females. Peak incidence was in the fifth decade, with 19 cases (15.8%) among males, and after the sixth decade, with four cases(3.3%) among females. Mean age was 57.7 years. Diverticulum size of 5-10mm in diameter was predominant 2% of cases) ; average diameter was 5-6mm. The incidence of colonic diverticulosis was 5.1 times more frequent in the right colon(101 cases) than in the left (20 cases). CONCLUSION: The overal overall incidence of colonic diverticulosis has continually increased; in addition it has also recently increased slightly in left-sided colon. This is thought to be due to various factors, both congenital and acquired, including longer life with good health care, constipation, irritable bowel syndrome, stress and the tendency of eating patterns to more closely resemble those of the west.

5.The Impact of Nursing Professionalism on the Nursing Performance and Retention Intention among Psychiatric Mental Health Nurses.

PURPOSE: This study aimed to investigate the impact of nursing professionalism on the nursing performance and retention intention among psychiatric mental health nurses. METHODS: As a descriptive correlational study, this study sampled 206 psychiatric mental health nurses in six hospitals in Seoul and Gyeonggi area through convenience sampling. Data were collected from March 2 to 31, 2009 using a self-report questionnaire. The collected data were analyzed using SPSS WIN 16.0. RESULTS: In the subscales of professionalism, the 'Sense of calling' had the highest mean score while the 'Professional organization' had the lowest mean score. A significant positive correlation was observed in nursing professionalism, nursing performance and retention intention. According to an analysis on the impact of each subscale of nursing professionalism on nursing performance and retention intention, the 'Sense of calling' and 'Autonomy' were the most significant predictor variable. CONCLUSION: The results confirmed that the improvement of psychiatric mental health nurses' professionalism increases their nursing performance and retention intention and the 'Sense of calling' and 'Autonomy' are critical prediction factors. It is necessary to come up with a strategy which strengthens nursing professionalism in order to improve psychiatric mental health nurses' performance and retention intention.

6.The Change of Serum Cholesterol level in Children with Fever.

PURPOSE: This study was undertaken to assess the change of the serum cholesterol level according to the duration of fever in children. METHODS: A retrospective study presents data on fasting serum cholesterol assessed 346 children aged from 3 to 14 years, admitted to National Medical Center from Jan. 1992 to Jun. 1994 due to febrile disease. These patients were divided into three groups according to the duration of fever at the time of cholesterol measurement. Group 1 comprised children in whom cholesterol was evaluated on the first 2day of fever, group 2 children in whom cholesterol was assessed on day 3 to 5 and group 3 subjects evaluated 6 to 20 days after the onset of fever. 200 healthy children without fever were also enrolled in this study. The fasting serum cholesterol levels were also measured in both febrile period and afebrile convalescent period in 34 children with febrile diseases during the 7months period of Apr. to Oct. 1994. RESULTS: As the retrospective study, the fasting serum cholesterol levels of group 1, 2 and 3 were lower than those of the controls. And the difference of the serum cholesterol levels between group 1 and group 3 was significant. The prospective study was significantly associated with a decrease of serum cholesterol in the febrile period compared with that in the convalescent period. CONCLUSIONS: Timing of blood collection for screening test and diagnostic measurement of serum cholesterol level should be carefully defined taking into account that febrile illnesses are biologic variables that may significantly affect serum cholesterol.

7.Effects of Edible and Medicinal Plants Intake on Blood Glucose, Glycogen and Protein Levels in Streptozotocin Induced Diabetic Rats.

The hypoglycemic effects of four edible plants (Angelicae tenuissimae (A. ten.), Pleurospermum kamtschaticum (P. kam.), Adenophora remotiflora (A. rem.) and Zanthoxylum schinifolium (Z. sch.)) in streptozotocin (STZ)-induced diabetic rats were investigated. Sprague-Dawley male rats weighing 190-230 g were induced diabetes mellitus by the STZ injection (45 mg/kg) into the tail vein and were divided into six groups ; normal, STZ-control and four edible plant groups (A. ten., P kam., A. rem. and Z. sch. groups). Normal and STZ-control groups were fed a AIN-93 diet and four groups of STZ-induced diabetic rats were fed one of each experimental diets containing 10% of the edible plant powder for 4 weeks. Diabetic rats showed the lower weight gain compared to the normal rats. In experimental groups except P. kam., AST activities were close to normal. A. ten. group were lowered ALT activities slightly. The plasma glucose levels of the diabetic experimental groups were significantly decreased at 4th week. The plasma insulin levels in diabetic experimental groups were not significantly different compared to the STZ-control group. The liver glycogen levels in STZ injected rats were significantly lower in compared to the normal rats. However no significant differences were found in response experimental plants intake in diabetic rats. The muscle glycogen were not significantly different among all the groups.

8.Glomerular Hypertrophy and Sclerosis in Rats with Chronic Puromycin Aminonucleoside Nephropathy.

PURPOSE: This experimental study was conducted to determine serial morphological changes of rat's kidney with chronic puromycin aminonucleoside (PAN) nephropathy. Special emphasis was given to the occurrence of glomerular hypertrophy and its relationship to the subsequent development of focal segmental glomerulosclerosis(FSGS). METHODS: Sprague-Dawley rats weighing 200-230g were used and divided into control(n=9) and experimental group(n=15). Rats were given subcutaneous injections of PAN at a dose of of 2mg/100g body weight, or an equivalent volume of normal saline and six injection were given over a period of 9 weeks, at weeks 0, 1, 3, 5, 7 and 9. At weeks 4, 8 and 11, rats were sacrificed and kidney weight, kidney weight/body weight(%) and various laboratory tests including serum protein and albumin were determined. Renal tissues were prepared with Histochoice(R) fixative and paraffin embedding for morphologic study. RESULTS: Kidney weight and kidney weight/body weight(%) were increased significantly in experimental group compared to controls at 4, 8 and 11 weeks. Heavy proteinuria along with lowering of serum protein and albumin and elevation of serum cholesterol was seen in experimental group at week 4 and this change became more marked on weeks 8 and 11. The frequency of FSGS in experimental animal, at week 4, 8 and 11 were 0.6%, 10.6% and 26.2% respectively(p<0.05) and the development of FSGS was more marked in juxtamedullary glomeruli compared to cortical glomeruli. Glomerular surface area showed significant increase in experimental animals compared to controls(p<0.01), the percentage of increase being 12.0, 14.7 and 12.3% at week 4, 8 and 11. And the surface areas of juxtamedullary glomeruli were larger than those of cortical glomeruli throughout the study period. CONCLUSION: In summary, present study indicates that glomerular hypertrophy occurs and precedes the development of FSGS in rats with chronic PAN nephropathy and juxtamedullary glomeruli are more susceptible to developing FSGS compared to cortical glomeuli.

9.The Efficacy of Autotransfusion and Induced Hypotension in Revision Total Hip Arthrophasty.

BACKGROUND: During total hip arthroplasty, replacement of a substantial amount of blood loss is necessary. The adverse effects of homologous blood transfusion have been documented consequently and the intraaperative autotransfusion has attracted considerable interest. The authors have attempted to reduce the amount of the homologous blood transfusion by inducing hypotension and autotransfusion. METHODS: Twenty patients who underwent revision total hip arthroplasty were divided into two groups. Group1 received homologous blood perioperatively. Group2 was transfused with predonated autologous blood and intraoperative salvage technique with Cell Saver apparatus, were used along with sodium nitroprusside for hypotensive anesthesia to decrease intraoperative bleeding. We observed the amount of blood loss and transfused blood, and hematologic laboratory data, and complications. RESULTS: The mean requirement for transfusion was 3080 ml in group1 and 1289 ml in group2. In the level of the hematocrit, there was a significant smaller hematocrit drop (8%) over the first two postoperative days in group2I. There were no complications such as disseminated intravascular coagulopathy, severe thrombocytopenia, microemboli, and nephrotoxicity. CONCLUSIONS: The autologous transfusion and induced hypotension in the total hip arthroplasty is thought to be an effective method to reduce the amount of homologous blood transfusion.

10.Changes of Plasma and Urine IL-8 and TNF-alpha in Children with Minimal Change Nephrotic Syndrome.

PURPOSE: Minimal Change Nephrotic Syndrome (MCNS) is one of the most common primary nephrotic syndromes in children. T-cell dysfunction has been thought to be involved in the pathogenesis of MCNS. However, the exact pathogenesis of MCNS has not been proven yet in spite of many studies which support T-cell dysfunction being involved in the pathogenesis of MCNS. A present study was done to determine the role of IL-8 and TNF-alpha in the pathogenesis of MCNS. METHODS: Study patients consisted of 19 biopsy-proven MCNS children aged 2-15 years old. Ten age-matched healthy children were used as controls. Both plasma and urinary IL-8 and TNF-alpha were measured during relapse and remission period using ELISA kit. Urinary cytokine values were corrected for urinary creatinine. RESULTS: Each value of urinary IL-8 measured during relapse and remission period in MCNS and controls was 13,996+/-2,811, 2,811+/-3,734 and 5,331+/-6,403ng/mg cr, respectively, and we noted that the value of urinary IL-8 measured during relapse period in MCNS significantly increased compared to those measured during remission period and in controls(p< 0.05). And each value of urinary TNF-alpha measured in the same group was 364.4+/-512.1, 55.3+/-208.0 and 36.0+/-45.0ng/mg cr, respectively, and we also noted that the value of urinary TNF-alpha measured during relapse period in MCNS was signficantly increased compared to those measured during remission period and in controls. The plasma cytokine values measured during relapse and remission period and in controls were 1.19+/-1.23, 0.51+/-0.84 and 0.77+/-0.62ng/mL, respectively, in the case of IL-8 and 2.42+/-3.86, 1.95+/-3.24 and 2.25+/-3.50ng/mL, respectively, in the case of TNF-alpha, and we noted the value of plasma IL-8 measured during relpase was also significantly increased compared to those of remission period and in control(p<0.05), but the change of plasma TNF-alpha values was not significant. CONCLUSION: It can be said that both IL-8 and TNF-alpha play an important role in the pathogenesis of MCNS in children. The fact that changes of urinary IL-8 and TNF-alpha were more prominent than those of plasma suggests that immune dysregulation may occur intrarenally rather than systemically in MCNS.
