1.A Clinical Evaluation of Endoscopic Retrograde Cholangiopancreatography (ERCP).

To investigate the diagnostic accuracy of ERCP in pancreatobiliary disease, the authors analysed the 1,244 cases of ERCP. The success rate of ERCP was 91.5%. The diagnostic accuracy of ERCP was 93.6% and complications occurred in 2.1%. Complications included pancreatitis,cholangitis and drug reactions. Among the biliary tract disease diagnosed, by ERCP, biliary tract stone was the most common finding. Compared with operative diagnosis of biliary tract stones,the sensitivity of ERCP diagnosis was 92.7% while that of sonographic diagnosis was 81.4%. The specificity of ERCP diagnosis was 94.4% while that of sonagraphic diagnosis was 80.9%. ln the diagnosis of common bile duct stones, ERCP was more accurrate than sonographic diagnosis. The results show that ERCP was a safe and effective diagnostic tool in the assessment of pancreatobiliary tract disease.

3.Moderating effects of Professional Self-concept in Relationship between Workplace Bullying and Nursing Service Quality among Hospital Nurses

PURPOSE: To identify the effects of workplace bullying of hospital nurses on the quality of nursing service and to investigate the moderating effects of professional self-concept in the relationship between workplace bullying and quality of nursing service. METHODS: The participants in this study were 280 nurses working in hospitals located in J Province. Data were collected from December 20, 2016, to January 20, 2017. A self-administered structured questionnaire was used. Data were analyzed using the SPSS/WIN 22.0 statistical program. RESULTS: Workplace bullying had significant negative correlations with professional self-concept and nursing service quality. There was a significant positive correlation between professional self-concept and nursing service quality. Professional self-concept was the only variable to show a moderating effect in the relationship between the outcome of workplace bullying and quality of nursing service (β=−.12, p=.023). CONCLUSION: The findings from this study suggest building a systematic strategy to prevent workplace bullying at the organizational-level, where it threatens quality of nursing service and developing programs to promote professional self-concept, which can control the negative effect of workplace bullying.

4.Arthroscopy in Traumatic Hemarthrosis of the Knee

We have investigated a series of 45 acute traumatic hemarthrosis of the knee between Jan. 1989 and Jan. 1993 at National Medical Center. All the patients who had normal findings on plain X-ray with hemarthrosis underwent arthroscopic examination under anesthesia. The arthroscopic results were as follows. 1. There were 14 cases(31%) of anterior cruciate ligament(ACL) tear, a case(2%) of avulsion fracture of tibial spine, 13 cases(29%) of lateral meniscus tear, 8 cases(18%) of medial meniscus tear, a case(2%) of osteochondral fracture of patella, and 8 cases(18%) showed no specific injury of internal structure. 2. The positive predictive value of Lachman and anterior drawer tests under anesthesia was 93% and 92%, respectively. 3. The positive predictive value of McMurray tests for medial and lateral menisci was 33% and 64%, respectively. 4. We concluded that a traumatic hemarthrosis indicated a significant knee injury and arthroscopy allowed a more accurate diagnosis of injury to joint structure.

5.A Study on Gender Differences in Influencing Factors of Office Workers' Physical Activity.

PURPOSE: The purpose of this study was to determine gender differences in effects of self-efficacy, exercise benefits and barriers, and demographic factors on the physical activity. METHODS: Seventy sedentary office workers, 35 male and 35 female, from a major airline company, completed a questionnaire from March 28 to April 5, 2012. Steps and body mass indices were measured using a CW-700/701 (Yamax) pedometer and Inbody 720 (Biospace), respectively. Data were analyzed using t-test, chi2-test, multiple linear regression, and simultaneous quantile regression. RESULTS: For male workers, exercise self-efficacy had a significant effect on physical activity, but only when respondents were at 10%(3,431 steps/day, p=.018) and 25%(4,652 steps/day, p=.044) of the physical activity distribution. For female workers, marital status was significantly related to physical activity, but only when respondents were at 10% (3,537 steps/day, p=.013) and 25%(3,862 steps/day, p=.014) of the physical activity distribution. CONCLUSION: Quantile regression highlights the heterogeneous effect of physical activity determinants among office workers. Therefore intervention strategies for increasing physical activity should be tailed to genders as well as physical activity levels.

6.Comparison of Characteristics of Ganoderma lucidum According to Geographical Origins : Consideration of Growth Characteristics(I).

Nine species of genus Ganoderma collected in Korea and abroad including Ganoderma lucidum complex and G. lucidum were compared by investigating growth characteristics. In the bottle culture, the mycelial growth periods of G. lucidum from Taiwan and North America was 26 to 30 days compared to that of Korean G. lucidum, which was 30 to 32 days. Cultivation period of Taiwan and North American isolates was 30 to 32 days which were 11 to 17 days shorter than those of Korean isolates. Biological efficiency of Taiwan and North American isolates were ranged from 3.3 to 5.5%, which were apparently lower than that of Korean isolates which ranged from 6.2 to 9.4%. Korean isolates had longer stipes(15~40 mm) and more number of pileus(4~6/bottle) than those of Taiwan and North American isolates. The G. lucidum isolates collected from Korea will be regarded as the independent species from the G. lucidum collected from Taiwan and North America since, the G. lucidum from Korea showed much different growth characteristics in various aspects compared to the G. lucidum from Taiwan and North America.

7.March Fractures of Metatarsals in Infantry Soldiers

Stress fracture of the metatarsal was first described as March fracture by Breithaupt, a Prussian military surgeon in 1855, According to the classical references, the second metatarsal is the most frequently involved and radiological evidence of fracture appears as late as several weeks. The purpose of this study is to document the incidence and clinical feature of march fractures in Korean military soldiers. From may 1991 to April 1994, at one military hospital in Korea, 52 march fractures in 46 patients were detected among infantry soldiers. There are different findings of fracture site and its clinical features from the previous foreign reports. 1. By physical examination, direct point tenderness on the location of the metatarsal shaft is characteristic, with careful history taking. 2. On roentgenographic examination, cortical fissuring or break is seen one week after onset of symptoms and external callus is seen from two weeks or at the latest four weeks. 3. The third metatarsal is the most frequently involved site(34 cases, 65.4%), and the second metatarsal is less frequent(12 cases, 23.1%) 4. The discrepancy between previous reports and present findings should be emphasized and analyzed in further study.

8.The Paradigm Model of VIP Ward Nurses' Decision Making.

PURPOSE: The purpose of this research was to describe the decision making of nurses in a VIP ward. METHOD: The methodology of collecting and analyzing the data was based on the grounded theory of Strauss and Corbin (1998). The data were collected through an in-depth interview, which were audio-taped and transcribed. The data were collected from 10 nurses from July to November 2007. RESULTS: The core category on VIP ward nurses' decision making was named as "adjusting with flexibility and deepened insight." The causal condition was established by 'the patients who wanted to be treated specially.' The contextual conditions included 'caring patients from various departments,' 'differences depending on the nurses' clinical experience,' and 'client-centered atmosphere in the VIP ward.' The intervening conditions included 'problem solving styles of nurses', 'attitudes of patients and family members', 'nurse-doctor relationships', and 'accessibility to information.' It was confirmed that nurses changed their action-interaction strategies depending on the intervening conditions, thus resulted in the nurses' role conflict and the need to expand their consciousness. CONCLUSION: The result of this study indicates that nurse's decision making depends on their experiences and the nature of social context in which nursing occurs.

9.Histopathologic Change and Clinical Use of Glycerin-stored Corneosclera.

Corneoscleras were stored in 4 degrees C, 98% glycerin and were examined histologically at various storage periods. There were no remarkable pathologic changes in the stainability or arrangement of collagen fibers in stored corneoscleras from one month to 33 months. 12 eyes of 12 patients underwent tectonic or therapeutic keratoplasty with them. The integrity of the globe was maintained in all cases and vision improved in three cases without the second operation. One case achieved visual improvement by optical keratoplasty. In four cases, the vision may be improved by the second operation, such as optical keratoplasty and keratoprosthesis.

10.A Case of Hemolytic Disease of the Newborm due to Anti - Dib Antibody.

The Diego blood group system consists of two pairs of antigens, Dia and Dib The incidence of Dia is low among pure Caucasian, Blacks, Polynesian and Eskimo, however, the rnongolians and American indians have both Dia and Dib. We report a case of two days old male who was admitted on first day of life for jaundice and subsquently exchange transfusion was performed on second day of life for bilirubin of 20 mg/dl. The blood groups of patient and his mother were both Rh D positive 0 type. Direct and indirect Coombstest were strong positive in the patient and indirect Coombstest was positive in his mother. We found anti-Dib antibody in his rnother's serum. The phenotype of Diego blood group system of the patient and his mother were Di (a+b+) and Di (a+b- ), respectively and hemolytic anemia in this case was due to anti-Dib antibody.
