Emphysematous pyelonephritis is rare but frequently fatal disease that usually occures in diabetic patients, 6cases having been reported in Korea. We have experienced a case of simultaneously occuring emphysematouspyelonephritis and emphysematous cystitis in 54 years old female patient, which have never been reported in Korea, and got satisfactory improvement with medical treatment.

We report a case of polycythemia vera combined with coagulation disorder. The patient was 54 years old man who complained of continuous bleeding after incision of skin abscess 20days ago. Laboratory tests were revealed prolonged aPTT and slightly prolonged PT. Coagulation factor, I, VIII, IX, XI and fibrinogen decreased, however FDP did not increased. It appears that patient with polycythemia vera have chronic activation of coagulation system, probably initiated by activation of factor XII. Platelet aggregation test to ADP, collagen, epinephrine was also revealed poor response.

The Journal of Korean Academy of Prosthodontics 2020;58(4):321-327
Purpose:
The purpose of this study was to measure and compare the strain value exerted on the cervical area using different screw tightening protocols in implant-supported, screw-retained 3-unit prostheses.
Materials and methods:
Strain gauges were attached to four implants: two external and two internal. Thereafter, two study model were designed each type using acrylic resin. CAD-CAM was used to design hex and nonhex abutments for each group (EH, ENH, IH, and INH group) and Screw-cement-retained prostheses were also designed using a nonprecious base metal. Abutment was fixed with 10 Ncm torque, and the prosthesis was cemented. Screws were fixed with 30 Ncm torque using different three protocols. After 5 min, the strain gauge level was measured, and group analysis was performed (α=.05).
Results:
External group showed significantly lower strain values than internal group and the EH group showed significantly lower strain values than the ENH group (P<.05). There was no difference in strain value based on the types of screw tightening protocols in same group (P>.05). The IH group exhibited significantly higher strain values than the INH group and the IH group showed a significant difference in strain values based on the types of screw tightening protocols used (P<.05).
Conclusion
There was no significant effect on the external type in the implant-supported, screw-retained prostheses. However, strain values were high in the internal type, and the types of screw tightening protocol significantly affected these implants.

With the recent development of digital dentistry, fully digitalized methods for fabricating dentures, using intraoral scans and computer-aided design/computer-aided manufacturing (CAD-CAM), are getting popular. Digital methods have the advantage of simplifying the fabrication process in the clinic and laboratory, supplementing digital data. This case report shows a fully digital fabrication method for interim removable dentures in a patient with anterior tooth loss in which implant placement is impossible or delayed. Interim removable dentures were fabricated using two methods. One method is printing tooth and base parts separately and combining, and the other method is printing the whole denture at one time and coloring on the base part. Afterward, dentures were delivered and adaptation was evaluated using the triple scan technique. The extracted site was scanned intraorally (first scan) and the interim removable denture was digitally scanned both intraorally (second scan) and, after removal extraorally (Third scan). In both method, denture adaptation was shown favorable. We report this case report as both the patient and the operator were satisfied with a simplified process using a fully digital method in the clinic.

Colonoscopy is a frequently performed procedure used for colorectal cancer screening. Most frequent complication is colonic perforation and bleeding. Other visceral injuries are very rare, but injuries to the spleen and liver have been described. We report on 66-yr-old female who presented with abdominal pain after uneventful colonoscopy. Abdominal CT scan revealed a band at previous hysterectomy site and dilated small bowel suggesting strangulated loop. A broad based knowledge of the different complication is necessary to decrease the risk of associated morbidity and mortality.

9.Rehabilitation using milled-bar with attachment maxilla overdenture in a patient with peri-implantitis fixed prosthesis: A case report

There are various methods for restoring the dentition of completely edentulous patients. Removable complete dentures have the advantage of being relatively economical, but they can be uncomfortable to wear. With the introduction of implant prosthodontics, various options such as implant-supported overdentures and hybrid prostheses have become available. If there is inadequate remaining ridge or limited financial resources, an overdenture supported by a few implants with additional attachments may be more suitable. In this case, due to severe peri-implantitis and other complications, the implants were removed. Subsequently, four implants (two on each side) were placed in the maxilla and a milledbar with attachment was fabricated for each side of the maxilla.

Goltz syndrome is known as a rare mesoectodermal hereditary disease, characterized by focal dermal atrophies with hernias of adipose tissue and also associated with a multitude of possible skeletal, dental, ophthalmological and other abnormalities. We experienced a case of Goltz syndrome. An one day old female newborn had focal atrophic and telangiectatic skin lesions, microphthalmia, syndactyly and urinary tract abnormality. The finding of skin biopsy was consistent with focal dermal hypoplasia. We report the case with a brief review and related literatures.
