Patellofemoral complications are the most frequent problems in total knee arthroplasty. In the literature, the subject of patella resurfacing and nonresurfacing is controversial. Twenty-seven osteoarthritic patients who received bilateral total knee prostheses were studied to evaluate the advantages and disadvantages of patella resurfacing prospectively. Only patients with mild patellofemoral disease were included in the study. The Ortholoc modular implant was used in all cases. Cementless femoral and tibial component were used. But, if we resurfaced the patella, cement was used. In all patients, patella resurfacing had been done in the left knee but not in the right knee. For the clinical analysis, We used Knee Rating Score of the Hospital for Special Surgery(HSS), For the roentgenographic analysis, We used Laurin's lateral patellofemoral angle & Merchant's congruence angle. In the follow up, blind test for patellar pain was performed to compare between right and left knee joint clinically. The average age was 61 years(range 28-72 years) and the average follow up was 21 months(range 12-27 months). Preoperative ROM was 104.4 degrees in right side and 106.7 degrees in left side. Postoperative ROM was 112.7 degrees in right side and 109.6 degrees in left side. Preoperative HSS Knee Rating Score was 53.1 point in right side and 54.8 point in left side. Postoperative HSS Knee Rating Score was 81.5 point in right side and 81.2 point in left side. In the plain knee AP view, preoperative tibiofemoral angle was varus 7.0 degrees in right side and varus 5.6 degrees in left side. Postoperative tibiofemoral angle was valgus 5.1 degrees in right side and valgus 4.9 degrees in left side. We evaluated the degree of ptosis, the function of levator muscle, and postoperative interval to recovery of ptosis in 20 patients who underwent scleral buckling for retinal detachment. The MRD 1(Margin reflex distance 1), function of levator muscle by Berke method and interpalpebral fissure height were evaluated at preoperative day and 1, 3 days, 1, 2, 4, 6, 8, weeks, and 6 months after surgery. The results were compared according to age, gender, extent of buckling, location of buckle. All patients were reported to develop ptosis at postoperative day one, and 20% of them showed mild ptosis persistent until 8 weeks after surgery. The decrement of MRD 1 was statistically significant until postoperative 2 weeks, but impaired function of levator muscle was statistically significant until postoperative 6 weeks(P<0.05). The postoperative interval to recovery of MRD 1 was delayed in group of old age(order than 40 years), in male and when the buckle was located under the superior rectus muscle(P<0.05). The postoperative interval to recovery of function of levator muscle was delayed in group of female, when the buckle extent was more than 180 degrees and when the buckle was located under the superior rectus muscle(P<0.05).

This study was undertaken for the clinical analysis and evaluation on 121 patients with incompetent internal os of the cervix, who were admitted and treated with McDonald operation or Shirodkar operation at the Soonchounhyang Medical Center from January 1991 to December 1995. The results of this study were as follows : 1. The incidence of this IIOC was 1.1% of 11,116 cases of total delivery. 2. The mean age of IIOC was 31.7 years old. 3. The average number of gravida was 3.2. 4. The most common contributary factor was previous history of artificial abortion (51.2 %), and midtrimester abortion (17.4 %), cervical laceration due to previous vaginal delivery (8.3 %) etc. was followed. 5. The success rate of operation was 76 %, and the highest success rate (85.7 %) was reveald with period from 15th weeks to 16th weeks of gestation. 6. When cervical dilatation was abscent or small, the success rate of operation was high. 7. The factors of failed operation were preterm labor (58.7 %), PROM (34.5 %), and PIH, bleeding. 8. The delivery method after operation was vaginal delivery in 83 cases (68.6%) and cesarean section in 38 cases (31.4 %).

PURPOSE: The morphological changes of the eyelids according to gender among different age groups in Korea were analyzed. METHODS: Six-hundred adults without any ocular disease were selected, and sorted by age (ranging from 20 to 79 years), and gender. Each group consisted of 50 adults. Interpalpebral fissure (IPF), marginal reflex distance 1 (MRD1), amount of the upper lid, degree of browptosis, lateral hood width of the eyelid, and protrusion of the eyelid fat were measured. The measured values were analyzed to determine changes related to the aging process. Other age groups were analyzed and compared with the subjects between 20 and 30 years old, using the Student's t-test with SPSS. RESULTS: MRD 1 and IPF slightly decreased with age, but there was no statistical significance. The upper lid amount showed a statistically significant increase at the 7th and 8th decade. The degree of browptosis showed a statistically significant increase from the 7th decade of age in men, and from the 6th decade in women. In particular, the lateral browptosis was more drooped than the center browptosis. The lateral hood width of the eyelid showed a statistically significant increase from the 7th decade of age in men, and from the 6th decade in women. Eyelid fat was most protruded at the central lower part, but there was no statistical significance. CONCLUSIONS: It is important to understand the change related to aging in the study of eyelid morphology and eyelid operation. The results from present study may be used to determine standard for the safe amount of skin resection in Korean eyelid operations.

PURPOSE: In this study we analyzed and compared the orbital anatomy in Korean and Caucasian subjects using computed tomography (CT) measurements. METHODS: Two observers performed a cross-sectional, retrospective analysis of 44 CT scans of subjects (22 Koreans, 22 Caucasians) with no appreciable orbital or globe disease. Ten length measurements and 3 angle measurements of various orbital aspects were obtained. Analysis was performed to determine if changes in these parameters were associated with race. RESULTS: Anterior medial interorbital length was 24.05 +/- 2.00 mm in Korean and 21.96 +/- 1.96 mm in Caucasian subjects. Anterior vertical orbital length was 34.19 +/- 1.67 mm in Korean and 35.03 +/- 1.18 mm in Caucasian subjects. The anterior medial interorbital length and anterior vertical orbital length (p < 0.05) were significantly different. Interorbital angle was 47.69degrees +/- 1.49degrees in Korean and 46.15degrees +/- 2.19degrees in Caucasian subjects; the difference was statistically significant (p < 0.05). CONCLUSIONS: Compared with Caucasians, the orbit of Korean subjects has a narrower orbital opening and longer interorbital distance. The present study regarding the anatomy of Korean and Caucasian orbits will assist physicians with the evaluation and treatment of orbital diseases.

BACKGROUND AND OBJECTIVES: The aim of this study was to determine the effect of ethanol on the regulation of vascular tone. MATERIAL AND METHODS: Using rat aorta ring, isometric contraction and 45Ca uptake were measured. Phorbol 12,13-dibutyrate (PDBu), phenylephrine, KCl were used for the regulation of smooth muscle tone. RESULTS: Ethanol induced transient contraction in rat aorta ring by dose-dependent manner. Ethanol suppressed the dose dependent contractile responses of vascular strip by phenylephrine, KCl and PDBu. Endothelium-dependent relaxation by acetylcholine was inhibited by ethanol. Ethanol depressed 45Ca uptake by high KCl but not by phenylephrine or PDBu in rat aorta. n-butanol selectively suppressed tonic contraction by high KCl, but t-butanol did not at the same concentration of butanol in rat aorta. PDBu-induced contraction was selectively suppressed by n-butanol but not by t-butanol. CONCLUSIONS: These findings suggest that the action of ethanol on phospholipase D is involved in the decreased response of rat aorta strip by vasoconstrictors.
