1.Health status and health services utilization of children under 5 in 28 communes

The study was conducted in 28 communes of 7 diverse geographical area, on 1457 children under 5 year old, monitored 4 times each year for 2 years. Results showed that:
1. The common diseases were as follows: acute respiratory infectious of eyes, skin and gastrointestinal tract. It seems to be related with poor environmental condition. The frequency of illness is estimated 2 times per year.
2. Self-medication is common. Only 56% of cases use common health station, hospital…
3. The determinants for health service untilization are: convenient, habit, the skill and attitute of health worker, price…
4. The barriers preventing the use of public health facilities are: lacking of medicaments, bed and equiment, long distance from residence, the education background and occupation of the mothers.

2.Study on the health needs and utilization of curative care in Hanoi metropolitan

The household sampling survey was conducted on 2099 household in 3 urban district of Hanoi, using a questionaire. Results showed that: the frequency per person was 1.05 episode per year; there are differences between age groups, for children above 5 year old age group, the frequency was the highest (2-3 times higher than other age group). The most choice for health care was self medication (36.5%), outpatients care (27.6%), using commune health station (21%) and private clinic (11.3%). Only 2.6% using state hospital as inpatients

3.Equity of health service utilization of urban residents: data from a western Chinese city.

BACKGROUNDGetting medical treatment is still difficult and expensive in western China. Improving the equity of basic health services is one of the tasks of the new healthcare reform in China. This study aimed to analyze the parallel and vertical equity of health service utilization of urban residents and then find its influencing factors.
METHODSIn August 2011, a household survey was conducted at 18 communities of Baoji City by multi-stage stratified random sampling. Based on the survey data, we calculated a concentration index of health service utilization for different income residents and a difference index of different ages. We then investigated the influencing factors of health service utilization by employing the Logistic regression model and log-linear regression model.
RESULTSThe two-week morbidity rate of sampled residents was 19.43%, the morbidity rate of chronic diseases was 21.68%, and the required hospitalization rate after medical diagnosis was 11.36%. Among out-patient service utilization, the two-week out-patient rate, number of two-week out-patients, and out-patient expense had good parallel and vertical equity, while out-patient compensation expense had poor parallel and vertical equity. The inpatient service utilization, hospitalization rate, number of inpatients, days stayed in the hospital, and inpatient expense had good parallel equity, while inpatient compensation expense had poor parallel equity. While the hospitalization rate and number of inpatients had vertical equity, the days stayed in hospital, inpatient expense, and inpatient compensation expense had vertical inequity.
CONCLUSIONSUrban residents' health was at a low level and there was not good health service utilization. There existed rather poor equity of out-patient compensation expense. The equity of inpatient service utilization was quite poor. Income difference and the type of medical insurance had great effects on the equity of health service utilization.

4.Impact of Regional Health Insurance on the Utilization of Medical Care by the Rural Population of Korea.

This study was conducted to determine how the regional health insurance program, put into effect nation- wide, might affect paterns and extent of medical care utilization in rural areas. The study employed a "onegroup, before-after design" and the data were collected from two sampling surveys conducted in Kangwha County, based on "multi-stage, stratified cluster sampling. "Changes in ambulatory care utilization, as measured in terms of the number of visits per 100 persons during the two-week survey period, varied with the type of facility. Out-of-pocket expenses for medical care connected with all forms of facilities were found to have decreased during the time interval due to insurance coverage. Before insurance, when a person sought medical help at a drugstore, it was more often because it was conveniently close and he or she was old and believed himself or herself to not be seriously ill; when a person sought medical help at a clinic or hospital, it was because he or she believed himself or herself to be seriously ill(i.e. only morbidity condition was a significant factor). After insuranc, when a person sought help at a drugstore, it was mainly because he or she was old and the drugstore was conveniently located. when a person sought help at a clinic or hospital, it was because he or she believed himself or herself to be seriously ill and, in addition, because the facility was conveniently located (i.e. geographical accessibility became an added factor of significance). Furthermore, knowledge of benefit coverage increased as residents gained more experience with the program during the interval between surveys.

5.A path analysis on prisoners' health behavior and medical utilization.

In this thesis, Korean prisoners' health behavior and the characteristics of their medical utilization were surveyed and analysed. Because prisoners are inclined to be mediators of communicable diseases or unhealthy behaviors between prison institution and the outside world, health care for prisoners is directly related to the national population. Data were collected through a self-administered survey of 5 Korean prisons out of a total of 38 correctional facilities and analysed in accordance with a causal model based on a path frame, by serial multiple regressions on health behavior, health status, and medical utilization, etc. According to the survey analysis, while prisoners were generally concerned with their health much more than they were before imprisonment, they perceived that their health status had deteriorated after imprisonment, and that their need for health services was increasing gradually during their time in prison. In the path analysis on the causal relations among variables related to the prisoners' health status and medical utilization, the prisoners' characteristics affected their health concern and health behavior, and subsequently affected their health status and medical utilization, respectively. To sum up these exploratory studies on prisoners' health behavior and health service utilization, some efforts to organize a health care system embracing the correctional institution and health care administration should be made on the level of establishing a health care delivery system for special social groups like prisoners.

6.Factors Influencing Utilization of Medical Care Among Osteoarthritis Patients in Korea: Using 2005 Korean National Health and Nutrition Survey Data.

OBJECTIVES: The purpose of this study was to define the association between the medical utilization of osteoarthritis patient and its related factors. METHODS: We used the 2005 Korean National Health and Nutrition Survey data and we enrolled 2833 participants who were forty or older and who were diagnosed as having osteoarthritis by a doctor within 1 year and who had suffered from osteoarthritis for more than 3 months. The Andersen behavioral model was used as the analytic framework, and the variables were categorized into predisposing, enabling, and need factors. To determine the influence of each variable on the medical utilization of osteoarthritis patient, we applied hierarchical logistic regression analysis with two stages: the first stage included the predisposing and enabling factors and the second stage included the need factors. RESULTS: On the hierarchical logistic analysis, the variables of personal income, the type of medical security, the duration of arthritis related symptoms within 1 month, the subjective health status and the duration of osteoarthritis showed a statistically significant difference between whether the medical utilization in men patients. And the variables of age, limitation activity due to osteoarthritis, arthritis related symptoms within 1 month, and the subjective health status had a statistically significant difference between whether the medical utilization in women patients. CONCLUSIONS: The patients who tend to receive less care are those suffer less from symptoms of osteoarthritis, those who are within the initial phase, or those with a low-level severity of osteoarthritis. It is necessary to encourage patients to receive the treatment in the initial phase.

7.Home Health Resource Utilization Measures Using a Case-Mix Adjustor Model.

PURPOSE: The purpose of this study was to measure home health resource utilization using a Case-Mix Adjustor Model developed in the U.S. METHOD: The subjects of this study were 484 patients who had received home health care more than 4 visits during a 60-day episode at 31 home health care institutions. Data on the 484 patients had to be merged onto a 60-day payment segment. Based on the results, the researcher classified home health resource groups (HHRG). RESULT: The subjects were classified into 34 HHRGs in Korea. Home health resource utilization according to clinical severity was in order of Minimum (C0) < 'Low (C1) < 'Moderate (C2) < 'High (C3), according to dependency in daily activities was in order of Minimum (F0) < 'High (F3) < 'Medium (F2) < 'Low (F1) < 'Maximum (F4). Resource utilization by HHRGs was the highest 564,735 won in group C0F0S2 (clinical severity minimum, dependency in daily activity minimum, service utilization moderate), and the lowest 97,000 won in group C2F3S1, so the former was 5.82 times higher than the latter. CONCLUSION: Resource utilization in home health care has become an issue of concern due to rising costs for home health care. The results suggest the need for more analytical attention on the utilization and expenditures for home care using a Case-Mix Adjustor Model.

8.Where do people with mental disorders in Singapore go to for help?

INTRODUCTIONThis study aims to examine the pattern of services utilisation and the factors associated with help-seeking behaviour among those with mental disorders in the multi-ethnic Asian population of Singapore.
MATERIALS AND METHODSA household survey was carried out on a nationally representative sample of the adult (18 years and above) resident population. The main instrument used to establish the diagnosis of mental disorders and the services sought was the Composite International Diagnostic Interview version 3.0 (CIDI 3.0). The 'services' component of the instrument contains questions, which examine service utilisation for mental health problems.
RESULTSA total number of 6616 completed respondents constituted a representative sample of the adult resident population in Singapore. Only 31.7% of those with mental disorders had sought help: 15.7% from mental health providers, 8.4% from general practitioners, and 7.6% from religious/ spiritual advisors or other healers. Among respondents with severe disability across any disorder assessed in our survey, 50.1% had sought help from some service in the past 12 months. Individuals with moderate or mild levels had lower rates of consultation, i.e. 35.4% and 30.6% respectively. The rate of using the Internet as a source of help was low in this population.
CONCLUSIONThere is a need to engage and work collaboratively with healthcare providers (including religious and spiritual healers) in the community to detect, assess and treat those with mental illness. More general practitioners need to be involved, and the role of the Internet also requires further consideration as a source for help.

9.Survey on factors influencing the use of suitable techniques on Chinese medicine in the countryside in 10 provinces.

OBJECTIVETo study the effective factors and difficulties in the application of Suitable Technique of Chinese Medicine(STCM) in the countryside.
METHODSA survey on current situation was carried out on incumbency doctors in 12 cities with a self-designed questionnaire.
RESULTSIn 1387 available questionnaire, 51.8% of the doctors agreed with the active application of some STCM in the countryside while 42.8% thought that they could be promoted. Factors as age,titles of professional, jobs and holding a diploma had close relation with their attitude to the application of techniques in their cities (P < 0.01).
CONCLUSIONSuitable technique of CM could be adopted by the rural doctors working at the grassroot healthcare unit, suggesting that these techniques should be popularized step by step.

10.Current Status of Costs and Utilizations of Hospital Based Home Health Nursing Care in Korea.

PURPOSE: The purpose of this study was to describe the current status of utilization and costs of home health nursing care by the levels of medical institutes in Korea. METHOD: A secondary analysis of existing data was used from the national electronic data information(EDI) of 148 home health agencies for 6 months from May to Oct 2005 in total. RESULT: The 148 agencies had multiple services in cerebral infaction, essential hypertension, sequela of cerebrovascular disease, type 2 diabetes mellitus, etc.. The highest 10 rankings of 76 categories of home health nursing services were composed of 96.4% of the total services, such as simple treatment, inflammatory treatment, urethra & bladder irrigation, inserting indwelling catheter etc., in that order. The highest 20 rankings of 226 categories of home examination services were composed of 77.0% of the total home examination services. In addition, the average cost of home health care per visit was 46,088 Won (approximately equal to 48 Dollars, 1 Dollar=960 Won). The costs ranged from 74,523 Won (approximately equal to 78 Dollars, loss of chronic kidney function, N18) to 32,270 Won (approximately equal to 34 Dollars, other cerebrovascular diseases, I67). CONCLUSION: Results suggest that client characteristics of hospital based HHNC are not different from community based HHNC or visiting nursing services for elderly. The national results will contribute to baseline data used to establish a policy for the home health nursing care system and education.
