2.Quantitative analysis on economic contribution of community health service in China.

In China, the implementation of community health service shows that the prevention is an essential and important part of our national health system and is helpful to decrease the medical expenditure gradually. According to the data from Health Statistic Information Center of Ministry of Health in China, we calculated that the total health expenditure of China would be decreased 8000.0 million yuan only in 2001, among which, 1188.3 million, 1953.9 million and 4833.0 million yuan were respectively saved for the government budget, the society and resident if implementing the policy of community health service powerfully. And every outpatient can save 15.46 yuan per time. By the quantitative analysis on the economic contribution of community health service, it can be proved that a great economic benefit could be gotten from the implementation of community health service.

The conclusion is drawed from industrial organization theory and successful experience of foreign countries, high market attention met with the demand of complicated variety and specification of drugs in pharmaceutical circulation.

4.Development of health service in Hunan Province.

OBJECTIVE:
To study the health service development in Hunan Province, and to find out the advantages and disadvantages, and to make tentative specific suggestions.
METHODS:
Comparison was made in Yunnan, Hunan, and Zhejiang Provinces in 4 major areas: health institutions, personnels, facilities, and funding.
RESULTS:
The overall health service of Hunan Province was good: The health resources increased, the allocations were reasonable, and the health expenditure decreased. Some problems appeared, such as backward primary health, shortage of medical talents, inefficient use of hospital beds, deducted government investment, increased business income, and large urban-rural gap.
CONCLUSION
There is urgent need to further develop health institutions and increase personnels, particularly improving the primary health care and making use of traditional Chinese medicine; improving the management of hospital beds to increase efficiency, increasing financial support to reduce the patients' burden, and ensuring the need of rural health care.

5.Estimating the Cost of Visiting Nursing Service by Visiting Nursing Model for Urban Public Health Center in Korea.

PURPOSE: This study focused on analysing costs per visiting nursing care based on nursing activities in a public health center. METHOD: The Easley-Storfjell Instrument(1997) was used for a prospective descriptive analysis of self-records for workload data from 10 visiting nurses during 4 weeks on all nursing activities. In addition, analysis of the 478 visiting nursing records and cost data from 5 home visiting departments in public health centers during one year of 2003 was done. RESULT: The workload of visiting nurses by the type of model was identified as follows: Type I showed that caseloads made up 32.9 % of all nurse activities, and type II showed that the caseloads made up 45.8 %. Second, The cost per visit in type I was 33,088 won and 31,323 won in type II. Third, the estimated budgets were 1,902,436 won to 12,057,696 won for the type I model. and 4,151,316 won to 17,432,712 won for the type II model for one year. CONCLUSION: This study's results will contribute to baseline data used to establish on infrastructure for visiting nursing program and visiting nursing agencies based on the budget of visiting nursing services.

6.Analysis of the cost of public health service items in four centers for disease control and prevention in county level in China.

OBJECTIVETo analyze the unreasonable part of full cost of the public health service items in county level.
METHODSOn basis of typical survey in 2005, 18 questionnaires were released and the response rate was 100%. The whole cost and ladder apportionment of expense methods were employed to account the full cost of items including practice items, items required by government and nationwide items required by government provided by 4 centers for disease control and prevention in county level.
RESULTSIt was found that 28.4% - 54.9% nationwide items required by government had not been provided, but 2.8% - 10.2% items being not required by government had been provided. Furthermore the frequency of the items required by government was not up to par from 8 topmost to 2 bottommost on average every year. The efficiency was not high because of lacking in the vehicle for work, and 33.3% - 43.6% shortage of equipments for laboratory, and 18.1% - 45.8% logistic staff and technicians only 2/3 time of the whole year had a full workload. All the above resulted in the bias from the standard cost of items required by government.
CONCLUSIONFor compensating the cost of items reasonable and accurate by government, a kind standard cost of the items according to the government request should be established.

7.Economic analysis of health promotion conducted in an enterprise.

OBJECTIVETo take intervention measures for health promotion after investigation of occupational health needs among employees, to analyze the economic input and output of the intervention measures, and to analyze the feasibility of health promotion through cost-effectiveness analysis and cost-benefit analysis.
METHODSA survey was conducted in an enterprise using a self-designed questionnaire to investigate the general information on enterprise, occupational history of each employee, awareness of occupational health knowledge, awareness of general health knowledge, awareness of hypertension, acquired immune deficiency syndrome, etc., lifestyle, and needs for health knowledge. Intervention measures were taken in the enterprise according to the investigation results, and then investigation and economic analysis of investment in health promotion, economic benefit, and absence of employees were performed using the questionnaire.
RESULTSAfter intervention, the awareness rate of the Code of Occupational Disease Prevention increased from 4.5% to 15.3%, the awareness rate of the definition of occupational diseases increased from 4.5% to 73.5%, and the awareness rate of the prevention and control measures for occupational diseases increased from 38.4% to 85.8%. Before intervention, 25.4%of all employees thought salt intake needed to be reduced, and this proportion increased to 92.5% after intervention. After the control strategy for health promotion, the benefit of health promotion that results from avoiding absence of employees and preventing occupational diseases was more than ten times the investment in health promotion, suggesting a significant benefit of health promotion conducted in the enterprise.
CONCLUSIONThe return on health promotion's investment for enterprise is worth. Health promotion really not just contribute to improve hygienic knowledge but increase the economic benefit.

8.Dynamic analysis of the new rural cooperative medical system in Hunan Province from 2003 to 2009.

OBJECTIVE:
To analyze the new rural cooperative medical system in Hunan from 2003 to 2009 and to provide reference for scientific decision making.
METHODS:
We dynamically analyzed the participation rate, fund raising and fund operation efficiency of the new rural cooperative medical system in the past 7 years.
RESULTS:
From 2003 to 2009, the participation rate increased from 60.7% to 91.22%, funding increased from 30 to 100 yuan/person, degree of hospitalization income from 26.75% to 41.63%, hospitalization rate from 4.57% to 8.26%, and cost of hospitalization times from 2389.46 to 2518.00 yuan.
CONCLUSION
Implementation of the new rural cooperative medical system is indeed a boon to farmers, and the enthusiasm of farmers' participation is significantly improved. We must strengthen the supervision of the designated medical institutions, to effectively reduce health care cost and the burden of medical expense of farmers.

9.Influential factors of treatment cost of antihypertensive drugs for hypertensive patient in community.

OBJECTIVE:
To explore the influential factors of treatment cost of antihypertensive drugs for hypertensive patient in community.
METHODS:
A total of 220 community health centers (CHCs) from 15 provinces were selected across China in view of geographical location, economic level and previous cooperative experience to implement standardized blood pressure management for hypertensive patients for 1 year, based on guidelines for prevention and control for hypertension in China (2009 Community-based revision). Baseline and follow-up information for each hypertensive patient under the care of these CHCs was collected. A total of 22 683 hypertensive patients in hypertension community standardization management were enrolled in this study. We used multivariate linear regression model to analyze the influential factors of treatment cost of antihypertensive drugs.
RESULTS:
Cultural degree, regional distribution, medical security system, the blood pressure classification, complications, and treatment options were statistically significant independent variables.
CONCLUSION
In hypertension community standardization management, the blood pressure of hypertensive patients should be controlled in advance to reduce the economic burden, , the occurrence of complications should be reduced, and economic factors should also be considered when selecting a treatment option.
