2.Adenomyoepithelioma of the Breast.

Adenomyoepithelioma is a rare benign tumor which occurs mainly in the skin, salivary gland and very rarely in the breast. Histologically this tumor demonstrates biphasic differentiation of luminal epithelial cells and myoepithelial cells. We report a case of adenomyoepithelioma occuring in the outer lower quadrant of the right breast of a 56-year-old female, confirmed histologically with an aid of immunohistochemistry. This is the first documented report in Korean literature.

3.Self Care in Depression.

OBJECTIVES: Depression disturbs the individual life from the loss of productivity to suicide. Furthermore the whole society is also influenced by depression in social and economic areas. Many scientific treatment methods are known to be effective in depression. But in reality more than half of patients with depression prefer self help treatment rather than medically based treatment. For the patients with mild depression and primary physicians, it is reasonable to provide the evidence based self care including self help or alternative treatment. This study is supported by the Korean Health 21 R&D Project for the Depression Center to establish the Korean treatment guideline for depression. METHODS: The members of Depression Center including the authors attended the workshops where the methods of evidence based medicine such as literature search, quality evaluation of the articles, making the level of evidence were educated. The professional librarians helped the authors for get the searched articles. Among the total of 354 abstracts of systemic review and 300 abstracts of random controlled trials, 3 systemic reviews and 2 random controlled trials were the main structure of evaluation. RESULT: Among the 37 self care methods known to be effective for depression, 11 kinds of methods were worth of evaluation. The first step for the self care of mild depression is to use St. John's wort, exercise, bibliotherapy, and light therapy in depression during winter season. Acupuncture, negative ion therapy, massage, relaxation technique are known to be efective with evidence. Music therapy, hypnotherapy, and aromatherapy are known to be effective but until now the evidences is insufficient and further evaluations are needed. CONCLUSION: Among 11 listed self-care methods, several methods need more evidence especially in Korean situation. Exercise is the most recommendable evidence based self care method in Korea nowadays. St. John's wort is effective with evidence, but it is not the OTC drug in Korea, so it is not recommendable now. For the bibliotherapy, there is no book such as"Feeling Good"in Korea. Only the introductory book about depression is available. Publishing the self help book based upon cognitive behavioral approach or psychodynamic approach is needed. Light therapy is practiced in a few clinics, but not self-help style. Exposure to the sunlight is recommended in the level of commen sense. These self care methods are for the mild patient. Moderate to severe patients with depression should seek the professional treatment.

4.A Case of Immature Intrapericardial Teratoma.

This paper describes a case of intrapericardial teratoma in a 20-day-old female meonate, who suffered from cyanosis and respiratory difficulty. She was evaluated by echocardiography, chest CT and MRI, the suspected cyanosis and respiratory difficulty. She was evaluated by echocardiography, chest CT and MRI, the suspected intrapericardial mass was identified and surgically removed. The postopervative diagnosis was intrapericardial teratoma, grade II. This case is reported with brief review of some related literature.

From May lst to November 30th, 1971, a study was conducted by authors on the health status of industrial workers through applying the modified Cornell Medical Index forwards a total of 4,565 labourers working in Busan City. The obtained results were as follows : 1. The largest number of industries and labourers was in Busanjin Gu, 9 and 2,386 (52.2%), respectively. 2. Total numbers of labourers subjected to the survey were 2,355 (51.6% in male, 2,210(48.4%) in female. 3. By age and sex distribution, the largest was seen with the age group of 25-34, 1,260 in male, otherwise with under the age group of 24, 1,994 in female labourers. 4. The highest average number of complaints by section among all age groups and industries was as follows ; In case of male labourers, textile was the highest with numbering 8.7 (35-39 age group) in the section of physical complaints, in the section of mental and total complaints, food industry was the highest with numbering 8.2 and 16.4, respectively, both under 19 age group, otherwise in case of female labourers, food industry was the highest with numbering 12.3 (physical), 10.3(mental) and 22.7 (total complaints) with all in 20-24 age group. 5. The lowest average number of complaints by section among all age groups and industries was as follows; In case of male labourers, other industry was the lowest with numbering 0.4 (55and over) in the physical, 0.3 (19and under) in the mental and 0.7 (55and over) in the section of total complaints, otherwise incase of female labourers, basic metal was the lowest in the all sections with all in 50-54 age groups.

7.Isolation of Trichophyton rubrum from Diabetic Foot Ulcers of 4 Patients.

Trichophyton rubrum is a common cause of superficial dermatophytosis in humans, such as tinea pedis, tinea unguium, tinea corporis and tinea cruris. T. rubrum usually attaches to keratin of the epidermis and uses it as a source of nutrients. Therefore, isolation of T. rubrum from non-keratinous skin, such as ulcer, is very rare. We describe 4 patients with T. rubrum-infected diabetic foot ulcer.

9.Pediatric Orbital Medial Wall Trapdoor Fracture with Normal Computed Tomography Findings.

With advances in diagnostic technology, radiologic diagnostic methods have been used more frequently, and physical examination may be neglected. The authors report a case of pediatric medial orbital trapdoor fracture in which the surgery was delayed because computed tomography (CT) findings did not indicate bone displacement, incarceration of rectus muscle, or soft tissue herniation. A healthy 6-year-old boy was admitted to the emergency room for right eyebrow laceration. We could not check eyeball movement or diplopia, because the patient was irritable. Thus, we performed facial CT under sedation, but there was normal CT finding. Seven days later, the patient visited our hospital due to persistent nausea and dizziness. We were able to perform a physical examination this time. Lateral gaze of right eye was limited. CT still did not show any findings suggestive of fracture, but we decided to perform exploratory surgery. We performed exploration, and found no bone displacement, but discovered entrapped soft tissue. We returned the soft tissue to its original position. The patient fully recovered six weeks later. To enable early detection and treatment, thorough physical examination and CT reading are especially needed when the patient shows poor compliance, and frequent follow-up observations are also necessary.
