2.A Case of Septic Arthritis due to Haemophilus influenzae associated with Atypical Kawasaki Disease.

We report a case of septic arthritis due to H. influenzae associated with atypical Kawasaki disease. A 2-year-old boy was admitted to our orthopedic department due to limping gait of the right leg. CT finding of right hip joint revealed fluid collection. He was diagnosed as transient synovitis and septic arthritis. He was treated with antibiotics, but no incision nor drainage was performed. During the antibiotics therapy, limping gait subsided, but high fever, skin rash and lip fissure developed. Echocardiographic finding showed both coronary artery aneurysms. We diagnosed the boy's condition as atypical Kawasaki disease, and treated it with intravenous immunoglobulin(IVIG) and aspirin. Even, with the use of IVIG, fever was persistent. A second IVIG was tried, but fever did not subside. An MRI of right hip joint was followed up. The finding was consistent with septic arthritis associated with osteomyelitis of the right femur head. An incision and drainage was performed on the right hip joint, and the pus culture revealed beta-lactamase negative H. influenzae.

Fetal alcohol syndrome is characterized by distinctive facial appearance, prenatal onset growth deficiency, an increased frequency of development and mental retardation, cardiac anomaly and genitourinary anomaly. Complete abstinence during pregnancy is recommended, since alcohol consumption in each trimester has been associated with abnormalities. We experienced a case of fetal alcohol syndrome in a 11-month-old female infant. Her mother had drunk as much as 500cc of alcohol every day from 1st trimester to 3rd trimester of pregnancy. The child was small for gestational age, other distinctive features were microcephaly, thin upper lip, short palpebral fissure, hypertelorism, low nasal bridge, hypoplasia of philtrum, dysplastic ear, developmental and mental retardation, mild pulmonary stenosis and ovarian cysts. On the basis of maternal history and clinical features mentioned above, we diagnosed the case as fetal alcohol syndrome.

5.Effects of Aromatherapy on Depression, Anxiety and the Autonomic Nervous System in Breast Cancer Patients Undergoing Adjuvant Radiotherapy.

PURPOSE: The study was designed to verify effects of aromatherapy on depression, anxiety and the autonomic nervous system in breast cancer patients who are undergoing adjuvant radiotherapy. METHODS: Data were collected from November 2006 through March 2007 at the C university hospital in Seoul. The study included 33 patients and they were assigned to three groups. Group I had aroma oil inhalation for 2 minutes per time, three times a day for six weeks whereas Group II and a control group inhaled aroma oil for 2 minutes per time, once a day for six weeks. For Groups I and II, a mixture of lemon, lavender, rosewood and rose essential oils were used while control group inhaled tea tree oil. RESULTS: Depression was significantly decreased in patients in the experimental groups only. In Groups I and II patients, anxiety level was lower than that in control patients. Patients in Groups I and II also showed stronger physical resistance to stress than control group patients. CONCLUSION: Aromatherapy should be considered as a method that can significantly decrease depression in breast cancer patients who are undergoing adjuvant radiotherapy.

6.Two Neonatal Cases of Pacemaker Implantation in Congenital Complete Atrioventricular Block.

Congenital complete atrioventricular heart block(CCAVB) is a rare disease of the newborn that carries significant mortality and has a heterogenous etiology. It may occur as a result of the presence of maternal autoantibodies that are transferred to the fetus and affect the fetal heart or be associated with a congenital structural abnormality of the heart. Infants with CCAVB are at risk of diminished cardiac output and the subsequent development of congestive heart failure. We report two cases of CCAVB in newborns treated with pacemaker implantation after birth. The first case revealed CCAVB with patent ductus arteriosus and anti- Ro(SS-A) antibody in both of the mother and the newborn. The second case was accompanied with mitral regurgitation and tricuspid regurgitation, but anti-Ro(SS-A) antibody was absent in both of the mother and the newborn. Pacemaker implantation was performed for both cases and the result was favorable. We concluded that our experience supports that the pacemaker implantation in the newborn is feasible and beneficial in the treatment of CCAVB.

7.Midgut Volvulus of Infant Simulating Intussusception Diagnosis with Color Doppler Ultrasonography.

Although midgut volvulus is clinically characterized by bilous vomiting and abdominal distention, plain abdominal X-ray is usually non-specific and therefore it can be misdiagnosed to other diseases. Upper gastrointestinal contrast study and computed tomography have been used as a routine diagnostic tool but it takes cost and time. Abdominal ultrasonography is a relatively good alternatives in diagnosing midgut volvulus and it relatively saves cost and time. But case presentation of midgut volvulus diagnosed with abdominal ultrasonography are rarely found in literature. We experienced a 6 month old girl who had come to our hospital with bilous vomiting and was diagnosed as midgut volvulus with ultrasonography. Thus we report this case with the presentation of typical ultrasonographic findings of midgut volvulus.

8.The Effect of Long Distance Air-travel on Serum Viscosity and Prothrombin Time.

BACKGROUND: Economy class syndrome is medically described as deep vein thrombosis, it is worldwide issued that Long-Distance Air-Travel is a risk factor for venous thromboemborism. Some study provides evidence and theoretical explanations for the hypothesis that long-distance is a risk factor for venous thromboembolism. The purpose of this study is to provide the useful prevention of venous thromboembolism for long-distance travelers in comparison to the effect of long distance air-travel on serum viscosity and prothrombin time between flight level and ground level. METHODS: Subjects selected healthy volunteers that had no abnormal findings in evaluation (alpha2-plasmin inhibitor, plasminogen, anti-thrombin III, protein C, protein S) for hypercoagulability. 5subjects (1 male, 4 females) had a seat on economy class of ground training facility designed for flight attendant and their activities limited as possible during about 8 hours at Sep. 16, 2001. Also, 5 subjects had a seat on the economy class of Asiana Airlines (B-777) from Sydney in Australia to Seoul in Korea at Mar. 3, 2002. In the flight, Actual flight altitude was 31,000 feet, cabin altitude was 4,800 feet and cabin temperature was 25degrees C. We have investigated the serum viscosity used by viscometer and prothrombin time. These data were analyzed statistically by General linear model (spsswin ver. 10.0). RESULTS: The results showed that there were no differcence between Flight Group and Ground Group statistically. However, the serum viscosity of flight group by position was some higher than ground group but these defference was not statistically significance (P=0.419). Also, the serum viscosity of flight group by time had a decreased trend but these trend was not statistically significance (P=0.061). The prothrombin time of flight group by position was some lower than ground group but these defference was not statistically significance (P=0.472). Also, the prothrombin time of ground group by time had a decreased trend but these trend was not statistically significance (P=0.217). CONCLUSION: This study shows the prediction to venous thrombosis more closely relates to prothrombin time than serum viscosity under immobilization to healthy people on the ground environment. Exercise is more useful method than hydration to prevent venous thrombosis on the ground environment. In the flight environment, we think that hydration is also useful method to prevent venous thrombosis because of more dry environment inside flying aircraft. But, this study was decreased serum viscosity between samples according to time. I think serum viscosity can be reduced by vibration effect inside flying aircraft. Serum viscosity and prothrombin time between samples according to position were not statistically significant. I think that risk for the healthy people is poorly quantified and possibly overestimated that long-distance air-travel is a risk factor for venous thromboembolism.

9.Encapsulating peritoneal sclerosis: case series from a university center.

BACKGROUND/AIMS: Encapsulating peritoneal sclerosis (EPS) is an often-fatal complication of long-term peritoneal dialysis (PD). We here report the clinical features of EPS in Korean PD patients from a single university center. METHODS: The data were collected retrospectively from 606 PD patients at Kyungpook National University Hospital, between August 2001 and August 2011. The diagnosis of EPS was based on clinical signs and symptoms, and confirmed by radiological findings. RESULTS: Eight patients (1.3%, four males) were diagnosed with EPS. The mean age of the patients was 48.5 years (range, 33 to 65). The mean duration of PD was 111.8 months (range, 23 to 186). All patients except for one had three or more episodes of peritonitis. Seven patients were diagnosed with EPS after stopping PD, and only one stayed on PD after initial diagnosis and treatment. Total parenteral nutrition and corticosteroids, in addition to tamoxifen therapy, were used to treat most of the patients, and one patient underwent surgery (adhesiolysis). The overall mortality rate was 50%. CONCLUSIONS: EPS is a serious, life-threatening complication in patients on long-term PD. To reduce the incidence and mortality rate of EPS, careful monitoring and early diagnosis is needed.

10.Comparison on the profiles of a modified Borg scale and the pediatric dyspnea scale during an induced bronchoconstriction in children with clinical asthma.

PURPOSE: Dyspnea is the cardinal symptom of asthma, but it is difficult to quantify clinically. Although modified Borg (mBorg) scale has been successfully used in adult, but there has been some difficulties to apply in children. Recently, Pediatric Dyspnea Scale (PDS) was adequately designed and has been widely used. The aim of this study is to compare 2 evaluating scales of dyspnea provoked by induced-bronchoconstriction in childhood asthma. METHODS: Seventy-three clinically suspected children with asthma were enrolled in this study. Each ‘fractional exhaled nitric oxide (FeNO)’ was documented. Forced expiratory volume in 1 second (FEV₁), mBorg score and PDS score were recorded during methacholine provocation test. RESULTS: Mapping using canonical plot demonstrated global similarity between 2 scales with some distinctive features. Whereas mBorg score showed more diverse categories in low level of dyspnea, PDS score did in medium level of it. A distribution of dyspnea perception score at a 20% decrease in FEV₁ relative to baseline (PS₂₀), a perception score of dyspnea at 20% fall in FEV1 of 2 scales represented similar wide, biphasic feature. Statistical relevance was verified with spearman correlation (R(s)=0.903, P<0.001) and Bland-Altman analysis. PS₂₀ of both scores and FeNO had no statistical relationship. While relationship between PS20 by mBorg score and the concentration of methacholine at 20% fall in FEV₁ (PC₂₀) was not significant (R(s)=0.224, P=0.154), that between PS₂₀ by PDS and PC₂₀ was weak positive (R(s)=0.29, P=0.063). CONCLUSION: PDS had similar pattern to assess the dyspnea with the mBorg scale suggesting adequacy of PDS in evaluating pediatric clinical asthma. We expect these scales to help clinical practice in complementary ways.
