1.Indirect measurement of blood pressure in neonates using an automatic noninvasive oscillometric monitor.

The measurement of blood pressure in neonates is an important diagnostic procedure. But the measurement of blood pressure has not been performed routinely because of difficulty in measuring blood pressure and variable normal range according to measuring apparatus. Recently some accurate and convenient apparatus of measuring blood pressure have been introduced in neonatal care, so the reference values of neonatal blood pressure may be obtainable. The authors measured systolic and diastolic blood pressure using a noninvasive oscillometric monitor instrument on 1,3,6,12,24,48,72 hours of life in 200 neonates born at Eulji General Hospital, Taejon. And we analysed the results according to birth weight, gestational age, delivery type, sex, meconium stain, preeclampsia and hypocalcemia. The following results were obtained: 1) On the 1st day of life, systolic and diastolic blood pressure were 65.611.7 mmHg and 36.7+/-5.8 mmHg in the normal birth weight neonates, and 56.2+/-6.7 mmHg and 34.14.2 mmHg in the low birth weight neonates, respectively. So the blood pressure of normal birth weight neonates were higher than that of low birth weight neonates. 2) On the 1st day of life, systolic and diastolic blood pressure were 65.5+/-11.8 mmHg and 36.6+/-5.8 mmHg in the fullterm neonates, and 57.6+/-5.2 mmHg and 35.6+/-3.8 mmHg in the preterm neonates, respectively. So the blood pressure of full term neonates were higher than that of preterm neonates. 3) The difference of blood pressure in analysis according to birth weight were wider than that according to gestational age. 4) The blood pressure of neonates were lowest on the 3 hours of life and increased gradually during 72 hours of life. 5) The blood pressure of neonates did not show any significant difference in analysis according to sex. delivery type, Meconium stain, preeclampsia, and hypocalcemia.

2.Accompanied Histopathologic Findings and Association of Serum beta-HCG Levels with Myosalpingeal Invasion in Ectopic Tubal Pregnancy.

Most ectopic pregnancies occur in the fallopian tubes. There have been numerous theories to explain the occurrence of ectopic pregnancy in fallopian tubes. The most commonly held view is that the passage of the fertilized ovum through the fallopian tube is delayed or hindered by chronic inflammation and its sequelae. We designed a study to evaluate the details of histopathologic changes and the location of implantation and how they relate to the clinical history. 182 fallopian tube specimens from patients who had undergone total or partial salpingectomy were examined. A high incidence of non-specific inflammation of plicae and wall of tube (31.9%) and salpingitis isthmica nodosa (12.6%) were observed. Other associated findings included acute salpingitis, complex plicae or complex hyperplasia of tubal epithelium, fibrous adhesion with ovary, endometriosis, and calcification. History of previous ectopic tubal pregnancy was found in 8 cases. The cases with serum beta-HCG value above 2,500 I.U./L (group I, n=97) were more frequently noted in those exhibiting myosalpingeal invasion of trophoblast (67 cases) than in those without invasion (30 cases). Of the 182 tubal pregnancies, 117 (64.3%) cases were found in the ampulla and 47 (25.8%) cases in isthmic location. In 117 ampullary pregnancies, the products of conception were found intraluminally in 71 cases (60.7%), and extraluminally in 34 (29.1%) cases, of which the products of conception were found entirely extraluminal. The products of conception, found both within and outside the tubal lumen, were found in 12 cases (10.2%). Of 47 tubes with isthmic pregnancies, 33 cases were intraluminal (70.2%), 12 cases were extraluminal (25.5%), and two cases were mixed (4.3%). In conclusion, significant histopathologic abnormalities accompany a majority of ectopic tubal pregnancy, and myosalpingeal invasion of trophoblast is correlated with high serum beta-HCG. Thus, it is necessary to confirm not only the ectopic placental tissue but also the accompanying details of the other histopathologic findings or the pathologic evaluation of ectopic tubal pregnancy.

4.Factors Influencing Intentions to Care For Emerging Infectious Disease Patients among National and Public Hospitals Nurses

Purpose:
The purpose of this study was to identify the effect of nurses' nursing professionalism, moral sensitivity, and social support on intention to care for patients with emerging infectious diseases.
Methods:
A structured self-report questionnaire was used to measure nursing professionalism, moral sensitivity, social support, and intention to care for patients with emerging infectious diseases. Data were collected from April 9~20, 2019. Participants were 200 nurse nurses working in national and public hospitals. Data were analyzed using Pearson correlation coefficients, and Multiple regression with the SPSS/WIN 24.0 program.
Results:
The perceived behavioral control (β=.48, p<.001), control beliefs (β=-.26, p<.001), moral sensitivity (β=.23, p<.001), normative beliefs (β=.17, p=.002), subjective norms (β=.17, p=.001), and attitude toward behavior (β=.10, p=.036) were a significant predictor of the intention to care for emerging infectious disease patients (Adj. R2=.65).
Conclusion
In order to confidently carry out nursing activities for patients with emerging infectious diseases, sufficient education on the epidemiological characteristics of emerging infectious diseases must be provided and education programs developed and applied with simulation similar to those of actual care for emerging infectious disease patients.
5.Effects of Supplementary Diet on Iron Status and Development in Infants.

This study was carried out to evaluate the effects of supplementary diet in infants. Influence of appropriate dietary habits on infants was also examined by being applicable to diets fortified chewiness as a means of intervention. The iron supplementary diet was supported to the healthy infants twice a day for three months. Measures of hemoglobin, hematocrit, RBC count, serum iron, TIBC, ferritin, development examination, and dietary intake patterns of experimental group (n = 25) and control group (n = 20) were performed before and after the intervention. The amount of iron intake from the supplementary diet in the experimental group was 1.77 +/- 0.80 mg/day. After the intervention period, the experimental group not only had increased intakes of grains also decreased intakes of milk. Outcomes observed in infants receiving iron intervention showed that the improved trend of excessive milk intakes and the possibility as a regular diet by serving the iron supplementary diet which can apply to main dish. All measures in blood did not provide significant differences except TIBC between the experimental and the control group before the intervention. But, after the intervention, the experimental group improved most levels of measures, especially significance in hemoglobin, but serum iron. Development of two groups did not differ significantly and both groups were in the range of normal infants' development. However, the levels of MDI and PDI evaluated by BSID-II in the experimental group were slightly higher than the control. Furthermore, the development of cognitive and languistic function was associated with infant growth in the experimental group. In conclusion, this research demonstrated that the iron supplementary diet could affect the iron status and the development of infants despite low-dose supplementation of iron.

6.Two Cases of Convulsion Associated With Caprolactam.

Two young men were seen with nausea, vomiting, dermatitis, seizure after two-four days of occupational exposure to caprolactam, a nylon fiber precursor. There were no significant results in laboratory test, brain CT, EEG except leukocytosis, hyperglycemia. Caprolactam has been shown to induce convulsive disorder in experimental animal studios and Tuma et al (1981) described that one worker acutely exposed to caprolactam developed generalized tonic-clonic seizure with leukocytosis. The coincidence of typical skin lesion with otherwise unexplained generalized tonic-clonic seizure in those young man strongly suggests that caprolactam was causal agent.

7.Case of Recurrent Diabetic Muscle Infarction Related to Strict Blood Glucose Control.

Diabetic muscle infarction (DMI) is an uncommon complication in patients with diabetes and it tends to be underdiagnosed, or misdiagnosed, clinically. Recently, we experienced a case of recurrent diabetic muscle infarction that was unusual in that the patient was younger than other patients. The patient was a 21-year-old woman with a 9-year history of maturity-onset diabetes of the young (MODY) who was referred to our department complaining of pain and edema in her right thigh. Magnetic resonance imaging (MRI) showed an increased T2-weighted signal and edema in the affected muscle. The final diagnosis was DMI. However, when the patient had partially recovered and was receiving supportive care, she had a second attack on the other side of the same leg during her hospitalization, in spite of her blood glucose level being strictly controlled. We report the clinical characteristics and imaging findings of this patient with recurrent DMI.

9.Effect-Evaluation on Nutrition Education in Related Curriculums for Elementary School Children: Focused on Change of Dietary Habits.

The purpose of this study was to develop of nutritional education activities and effect evaluation in related curriculums for all Dongbu Elementary School children in Jinhae by the teacher in charge. This educational purpose was to build a desirable dietary behavior for optimal growth and health in elementary school children. The educational program contents were developed according to 1) selection of content related curriculums 2) analysis of contents related on dietary habits by grade 3) preparation teaching plans to build desirable dietary habits 4) development of teaching manual by turns. Twelve hours of nutrition education at all school classes were done from March 2 to December 10 2004 in Dongbu Elementary School in Jinhae, Gyeongnam. The education effects were evaluated through the questionnaire surveys before and after the education and their comparative analysis. The results of this study were summarized as follows : Forty percent of the underweight group, 22.8% of the normal group, 42.2% of the obese group had the wrong perceptions on their body figures, but after nutrition education 80.5% of the underweight group, 94.3% of the normal group, 97.9% of the obese group gleaned the correct perceptions on their body figures (p<0.001). As the effects of the education, the subjects who exercise more than 3 times in a week increased in all groups (p<0.01 - 0.001), notably in the obese group. Meal regularity (p<0.001), skipping meals (p<0.001), eating rate and snack selection patterns (p<0.01 - p<0.001) also improved by nutrition education. We could also observe the effects of the nutrition education through the affirmative statements which appeared in their dietary records. These results showed a possibility of nutrition education activities in related curriculums to improve in dietary habits of school children. Next year, we will start with the system of nutrition teachers in elementary school, but nutrition teacher cannot undertake the task of teaching and food service management alone. Because of that, we intend to develop this program as an alternative proposal for the nutrition education in elementary schools.

10.Clinical Characteristics of Methylphenidate Use in Korean Children and Adolescents with Autism Spectrum Disorder : A Retrospective Study.

OBJECTIVES: The purpose of this study was to investigate clinical characteristics of children and adolescents with autism spectrum disorders (ASDs) using methylphenidate (MPH). METHODS: Retrospective review of the charts of 79 children and adolescents with ASDs, who visited the Department of Child and Adolescent Psychiatry of Seoul National Hospital, from July 2010 to July 2011, was conducted. Changes in illness severity and improvement were measured using the Clinical Global Impression-Severity of illness (CGI-S) and Clinical Global Impression-Improvement (CGI-I) Scales. RESULTS: We found that MPH was prescribed in 23 (29.1%) children and adolescents. Of the 23 patients on MPH, 4 patients (17.4%) were on MPH monotherapy and 18 patients (78.3%) were using risperidone concomitantly. MPH was prescribed primarily for symptoms of hyperactivity and impulsivity in ASDs patients. The mean dosage of MPH was 26.2+/-11.1mg/day and mean duration of treatment was 31.9+/-28.7 months. Mean CGI-S score improved significantly from baseline to endpoint (from 5.4+/-0.6 to 4.1+/-0.9 ; p<.01). MPH was reported to be effective in 17 patients (17/23, 73.9%), and 10 patients (10/23, 43.5%) reported side effects. Side effects included decreased appetite (4/23, 17.4%), tic (2/23, 8.6%), sleep disturbances (2/23, 8.6%), headache (1/23, 4.3%) and irritability (1/23, 4.3%). CONCLUSION: The results of this study demonstrate that MPH may be used effectively and safely in children and adolescents with ASDs with hyperactivity and impulsivity. Future controlled trials are needed to confirm these findings.
