2.A Recurrence of Ovarian Carcinoma Presenting as Only Axillary Lymphatic Metastasis: A Case Report.

Ovarian carcinoma usually metastasizes to the peritoneal cavity and the regional lymph nodes. While the peritoneal metastases if often widespread, metastases beyond the peritoneal cavity including axillary lymph nodes are uncommon. A recurrence of ovarian carcinoma in only the axillary lymph node is also a rare case. We report here on an axillary lymphatic metastasis as a result of recurrence of ovarian carcinoma in a 43-yr-old woman. Six years after the initial operation for ovarian carcinoma, multiple lymphatic metastases in the right axilla were noted as a palpable axillary masses. There are few previous reports about ovarian carcinoma that metastatized to the axillary lymph nodes. It must be differentiated from breast carcinoma because the treatment and prognosis of metastatic ovarian carcinoma differ from those of primary breast carcinoma.

3.Standardization and Validity Study of Korean Nomenclature of NIC.

The purpose of this study was to dcvclop nd test the validity of standardized Korean nome clature based on Nursing Intervention Classi ications (MC), developed by McCloskey and Bul chek at the University of Iowa, The four phases of the study were 1. Two professors and 15 graduate stude ts, who were taking a nursing intervention cours, translated the 433 nursing interventions of IC into Korean 2. 12 nurses with various clinical bac rounds reviewed the nomenclature, taking into nsider ation the definition and activities for each Korean nursing intervention. The Delphi method was used to determine the best appropriate nomenclature for each intervention ~. 20 academic and clinical experts in nursing were given a questionnaire to rate the validity of each Korean nomenclature using a 5 point Likert scale ranging from very inappropriate to very appropriate 4. five members of the Korean Nurses Association Research Committee reviewed the survey results and determined the most appropriate Korean nomenclature for each nursing intei veiition of NIC system. Most nomenclature had a score of more than 4.0, but seven nursing interventions had a score lower than 4.0: Critical Path Development (2.71), Calming Technique ~3.4l), Grief Work Facilitation (3.44), Order Transcription (3.44), Self Modification Assistance (3.44), and Teaching Psycho-motor Skills (3.47). These research results will be included in the nursing vocabulary dictionary to be published by Korean Nurses Association.

4.Analysis of the Immune Response of a Laparoscopic Surgery between Gas and Gasless Techniques.

PURPOSE: To investigate the immunologic effect of the gasless laparoscopic procedure, we compared it with an immunologically effective, gas insuffulated (pneumoperitoneal) laparoscopic procedure. METHODS: The temporal immune responses in two similar groups of patients randomized to gas vs gasless laparo scopic cholecystectomy were analyzed. The patients were matched by age, weight, and operation time. Immune parameters, including serum white blood cell count and TNF-alpha, INF-gamma, IL-6, IL-8, cortisol and ESR levels, were assessed preoperatively and on postoperative days 1 and 3 between the two groups. Also during the operations, cytokines were checked in cultured peritoneal macrophages. RESULTS: The results were as follows: 1) Serum cortisol and ESR levels were not statistically different between the two groups. White blood cell counts were also not statistically different. 2) The preoperative and postoperative day 1 and days 3 serum TNF-alpha, INF-gamma, IL-6, and IL-8 levels in the two group were not statistically different. However, an immediate decrease cytokine levels was significant in both groups after postoperative day 1. 3) Especially cytokine levels were more increased in cultured peritoneal macro phages than in serum, but difference between the two groups was not statistically significant. CONCLUSION: From our results the beneficial effects of immunology in a gasless laparoscopic procedure were not different from those in a gas laparoscopic procedure. Rather, immediate preservation of the immune function during the postoperative period was detected in both groups.

5.The Occupational Health Status of the Small Scale Industries Participated in the 1997 Subsidiary Program on Health Care Management.

The small scale industries in Korea are important in occupational health care management due to high accident rate and poor working condition. The authors surveyed the 4,811 small scale industries participated in the 1997 subsidiary program on health care management. The contents of survey were the general characteristics of the factory, the results of workers' general and occupational health examination, the results of workplace environmental monitoring, and the number of occupational accident and injured person at 1995 and 1996. We classified all industries by 67 groups. The prevalence of the general disease was higher in 'Manufacture of other fabricated metal productsj. rManufacture of textiles', 'Manufacture of basic metalsj. In occupational disease, that was higher in FManufacture of metal products', 'Manufacture of basic metals', so in environmental monitoring in 'Manufacture of textiles', 'Manufacture of other fabricated metal products' 'Printfrig' in occupational injuries in 'Manufacture of chemical products', 'Manufacture of basic metals'. However the factories of 'manufacture of motor vehicle', 'manufacture of machine and equipment', 'manufacture of electric product' in the program are large in magnitude, but less important in occupational health management. We observed that the prevalence of general disease, occupational disease, workplace environmental monitoring, occupational injuries of subsidized industries in 1996 were increased compared with those in 1995. and were larger than those of nationwide statistics. These phenomenon shows that the subsidiary health program for participated industries is important and to be urgent. We observed that the major groups in district were different and so the approaching method of each district must be various and different. And the health program manager should focus on the C grade in health examination and the action level in workplace monitoring more than the D grade and the permissible level.

6.Comparison of Extended-Spectrum beta-Lactamase-Producing Escherichia coli and Klebsiella pneumoniae Bloodstream Infection Epidemiology.

BACKGROUND: Extended-spectrum beta-lactamase (ESBL)-producing Escherichia coli and Klebsiella pneumoniae are main pathogens of bloodstream infection. We compared the epidemiology of ESBL-producing E. coli and K. pneumoniae. METHODS: From January 2003 through March 2007, we retrospectively investigated the clinical characteristics and comorbidities of patients with bloodstream infection by ESBL-producing E. coli and K. pneumoniae. RESULTS: During the study period, 79 patients (54 with E. coli and 25 with K. pneumoniae) with blood cultures positive for ESBL-producing E. coli or K. pneumoniae were studied. When comparing the source of bloodstream infections, urinary tract infections (UTIs) were more commonly caused by E. coli (24% vs. 0; P=0.007), and respiratory tract and soft tissue infection (36% vs. 15%; P=0.04, 12% vs. 0; P=0.03, respectively) were more commonly associated with K. pneumoniae. Among hospital-acquired bloodstream infection, third-generation cephalosporin was more commonly used in patients with E. coli than patients with K. pneumoniae (81% vs. 24%, P<0.001). Medical devices (central venous catheter and urinary catheter) were more commonly used in patients with K. pneumoniae. CONCLUSION: ESBL-producing E. coli bloodstream infection is more common in urinary tract infections. ESBL-producing K. pneumoniae is more common in respiratory tract infections and in skin and soft tissue infections. In hospital-acquired infections, ESBL-producing E. coli bloodstream infection is more common in biliary tract infection. ESBL-producing E. coli was more commonly associated with prior frequent antibiotics use and K. pneumoniae was more commonly associated with use of medical devices.

7.Body Composition, Food Intake and Clinical Blood Indices of Female College Students.

This study was done to evaluate the health and nutritional status of female college students in Seoul. The subjects were 63 healthy college students aged 20 to 29 years. Their body composition, dietary intakes, clinical blood indices were investigated. Their body composition was determined by means of a multifreqency bioelectrical impedance analysis. Their dietary intake was determined using 3-day record method and their nutrient intake was analyzed by Computer Aided Nutritional analysis program for professional (CAN-pro). Their hemoglobin, hematocrit, red blood cell (RBC), white blood cell (WBC) and mean corpuscular volume (MCV) were determined by semi-automated microcell counter (F-520). Their plasma total cholesterol, TG, and HDL-cholesterol levels were measured using test kits. All data were statistically analyzed by SAS PC package program. Their average consumption of calcium, iron vitamin A, vitamin B2 and niacin were 63.3%, 65.0%, 85.2%, 89.2% and 95.2% of RDA, respectively. The overall mean values of the hematological indices in the female college students were within the normal range. However anemic subjects with hemoglobin (< 12 g/dl) and hematocrit (< 36%) accounted for about 20% of the subjects. The mean levels of total cholesterol, HDL-cholesterol and TG were 188.4 mg/dl, 69.9 mg/dl and 67.4 mg/dl, respectively. The percentages of the subjects with plasma total cholesterol level (> 200 mg/dl) and LDL-cholesterol (> 130 mg/dl) were about 41% and 30.4%, respectively. The data showed a significantly positive correlation between either body fat (%) or BMI and TG. However, there was a significantly negative correlation between either body fat (%) or BMI and HDL-cholesterol. These overall results suggest that it is necessary for college women to be educated regarding consuming more iron and vitamin C and less fat, in order to prevent iron deficiency anemia and/or cardiovascular diseases in later life.

8.Diffusion Metrics as a Potential Prognostic Biomarker in Cervical Myelopathy

Diffusion tensor imaging (DTI) has a diagnostic value in cervical myelopathy. The usefulness of diffusion metrics as a potential prognostic biomarker was assessed in this review. Various diffusion metrics, such as fractional anisotropy, mean diffusivity, apparent diffusion coefficient, and radial diffusivity, were significant in correlation with functional recovery after surgery. Changes in diffusion metrics not only at the compression level, but also above and below the compression level were confirmed. However, it was difficult to confirm the results owing to the diversity of studies. Further efforts to make a consensus on spinal cord DTI are needed.

9.A Case of Cecal Volvulus Presenting with Chronic Constipation in Lissencephaly.

Cecal volvulus is uncommon in pediatric patients and there are few reports of cecal volvulus with cerebral palsy. Here, we report the case of a 19-year-old male patient who presented with abdominal distension, a history of cerebral palsy, refractory epilepsy due to lissencephaly, and chronic constipation. An abdominal x-ray and computed tomography without contrast enhancement showed fixed dilated bowel intensity in the right lower abdomen. Despite decompression with gastric and rectal tube insertion, symptoms did not improve. The patient underwent an exploratory laparotomy that revealed cecal volvulus. Cecal volvulus usually occurs following intestinal malrotation or previous surgery. In this patient, however, intestinal distension accompanying mental disability and chronic constipation resulted in the development of cecal volvulus. We suggest that cecal and proximal large bowel volvulus should be considered in patients presenting with progressive abdominal distension combined with a history of neuro-developmental delay and constipation.

The syndromes of left atrial isomerism and right atrial isomerism, called polysplenia and asplenia syndromes, respectively, consist of congenital heart defects with disturbances in normal left right isometry, and the etiology of atrial isomerism remains unclear. Right atrial isomerism is traditionally associated with severe cardiac defects, especially complete atrioventricular septal defect, transposition of great arteries, pulmonary atresia, and total anomalous pulmonary venous return. Recently, we encountered one case of asplenia diagnosed by prenatal ultrasonography. We report a case with brief review of the literatures.
