2.Effects of interleukin-17A on colonization and infection of respiratory tract bacteria

Interleukin-17A (IL-17A) is one of the proinflammatory cytokines produced by a subset of CD4+T cells named Th17 cells. It plays a crucial role in both host innate and adaptive immune responses against colonization and infection of respiratory tract bacteria. Studies in animal models have demonstrated that IL-17A can induce the expressions of various chemokines, inflammatory factors and adhesion molecules via multiple intracellular signaling pathways and recruit neutrophils to infection sites. Moreover, recent stud-ies have shown that single nucleotide polymorphisms (SNPs) of IL-17A coding gene can regulate the produc-tion of IL-17A in serum, resulting in individual difference in colonization rate by Streptococcus pneumoniae. This review presents recent data related to the structure, functions and signaling pathways of IL-17A, sum-marizes its roles in host defense based on studies in animal models and discusses the effects of genetic varia-tion of IL-17A on colonization, susceptibility, clinical characteristics and disease prognosis of human respira-tory tract bacteria.
3.Effect of Qingyi Lidan Granule on Serum Levels of HMGB1, HSP70, HSP27 and IL-8 of Patients with Severe Acute Pancreatitis

Objective:To study the effect of Qingyi Lidan Granule on the serum levels of high mobility group box 1 (HMGB 1),heat shock protein 70 (HSP70),heat shock protein 27 (HSP27) and interleukin-8 (IL-8) of patients with severe acute pancreatitis.Methods:From August 2015 to July 2016,84 patients with severe acute pancreatitis in our hospital were selected and randomly divided into the observation group and the control group according to random number,42 cases in each group.The control group was given routine treatment,and the observation group was treated by Qingyi Lidan Granule on the basis of control group.The recovery of blood amylase to normal time,white blood cell recovery to normal time,recovery of gastrointestinal function to normal time and relieving time of abdominal pain,serum levels of HMGB1,HSP70,HSP72 and IL-8 in both groups were observed and compared before and after treatment.Results:After treatment,the total clinical effective rate of observation group was significantly higher than that of the control group[92.86% (39/42) vs 71.43% (30/42)] (P<0.05).The recovery of blood amylase to normal time,white blood cell recovery to normal time,recovery of gastrointestinal function to normal time and relieving time of abdominal pain in the observation group were significantly shorter than those of the control group (P <0.05).Before treatment,no significant difference was found in the serum levels of HMGB 1,HSP70,HSP72,IL-8 between groups (P>0.05).After treatment,the serum levels ofHMGB1,HSP70,HSP72 and IL-8 in both groups were significantly lower than those before treatment (P<0.05).The levels ofHMGB1,HSP70,HSP72 and IL-8 in the observation group were lower than those in the control group (P<0.05).Conclusion:Qingyi Lidan Granule could effectively reduce the levels of serum HMGB 1,H SP70,HSP27 and IL-8 and enhance the clinical curative effect of patients with severe acute pancreatitis.
4.Influence of the timing of microsurgical clipping on the prognosis of patients with poor-grade aneurysmal subarachnoid hemorrhage

Objective To investigate the influence of different timing of surgery on patients with poor-grade intracranial aneurysm. Methods The clinical data of 86 patients with aneurysmal subarachnoid hemorrhage (Hunt-Hess gradeⅣ toⅤ)were analyzed retrospectively. The patients were divided into an ultra-early treatment group (the operative time within 24 h after onset;n=40),an early treatment group, (24 to 72 h after onset;n=27),and middle and late treatment group (>72 h after onset;n=19)according to the different timing of surgery. The prognosis of patients at 6 months after procedure was evaluated with the modified Rankin Scale (mRS)scores. The differences of the rate of good prognosis (mRS 0 to 2)and mortality in patients of the 3 groups were compared. Results (1 )The ratios of good prognosis in the ultra-early treatment group,early treatment group,and middle and late treatment group were 55. 0%(n=22),33. 3%(n=9),and 21. 1%(n=4),respectively. There were significant differences (P<0. 05). The mortality rate of the ultra-early treatment group (7. 5%)was lower than that of the early treatment group (25. 9%) or middle and late treatment group (42. 1%). There were significant differences (P<0. 05). (2)There were significant differences in the rate of good prognosis in different timing of surgery in patients with gradeⅣ(P<0. 05);there was no significant difference in the rate of good prognosis in patients with grade Ⅴ(P>0. 05). The mortality ratios in patients of the 3 groups were 2/18,4/11,and 6/11,respectively. There were significant differences (P<0. 05). Conclusion Ultra-early operation may benefit part of the patients with poor-grade aneurysm,particularly in patients with gradeⅣ. As for the patients with grade Ⅴ, ultra-early operation may help to reduce short-term mortality rate,however,the rate of vegetative state is high.
5. Therapeutic effect of unilateral and bilateral pedicle approach percutaneous vertebroplasty in the treatment of osteoporotic vertebral compression fractures

Objective:
To explore the efficacy of percutaneous vertebroplasty (PVP) in the treatment of osteoporotic vertebral compression fractures (OVCF).
Methods:
A retrospective analysis of 76 patients with OVCF treated with single and double pedicle approach PVP from April 2016 to June 2017 in the First Affiliated Hospital of He′nan University was conducted.According to the operation method, the patients were divided into unilateral group (42 patients, unilateral pedicle approach), and bilateral group (34 cases, bilateral pedicle approach). The operation time, number of intraoperative X-rays, amount of bone cement injection, height of the vertebral body and the complications were compared between the two groups.The visual analogue scale (VAS) and the Oswestry dysfunction index(ODI) score were used to assess the recovery of the patients.
Results:
The unilateral group had less operative time[(33.01±3.78)min], intraoperative X-ray number[(22.06±3.85) times]and bone cement injection[(3.53±0.42) mL] compared with the bilateral group (

A target molecule affinity-ultrafiltration liquid chromatography-electrospray ionization tandem mass spectrometric (LC-ESI-MSn) method was established for rapid screening acetylcholinesterase (AChE) inhibitors from total alkaloids of fibraurea recisa Pierre.A total of 12 potential inhibitors were screened from Fibraurea recisa Pierre.and 6 compounds were identified including palmatine,berberine,jatrorrhizine,palmatrubine,7,8-dihydro-8-hydroxyberberine and groenlandicine.The AChE inhibitory activity of these 6 compounds was validated in vitro.Palmatine showed the strongest inhibitory activity for AChE,which was stronger than that of donepezil hydrochloride,demonstrating the potential of palmatine as anti-Alzheimer's drug.This method is simple,rapid,and accurate for directly screening active ingredients which can inhibit AChE from complex extract of traditional Chinese medicines.
7.Therapeutic effect compared between three cannulated screws and proximal femoral locking plate in treatment of femoral neck fracture

Objective To compare the therapeutic effect between three cannulated screws and proximal femoral locking plate in treatment of femoral neck fracture.Methods A total of 75 patients were included,the three cannulated screws group contained 31 patients,and the proximal femoral locking plate group contained 44 patients.The clinical healing time,hip function score at last follow-up and postoperative complication were compared.Results All patients were followed up for 13-18 (15.3 ± 3.2) months.The excellent and good rate of proximal femoral locking plate group was 95.5% (42/44),and three cannulated screws group was 83.9%(26/31),there was no statistical difference between the two groups (P > 0.05).The clinical healing time of fracture and hip function score at last follow-up in proximal femoral locking plate group were significantly better than those in three cannulated screws group [(14.6 ± 1.7) weeks vs.(18.1 ± 4.9) weeks,(90.9 ± 3.5) scores vs.(82.3 ± 8.3) scores],there were statistical differences between the two groups (P < 0.05).Conclusions Proximal femoral locking plate provides short clinical healing time of fracture,stable fixation,good biomechanical properties and a low rate of postoperative complication in treatment of femoral neck fracture.It is one of the ideal methods in treatment of femoral neck fracture.
8.The effect of Dachengqi Decoction and enteral nutrition on the recovery of gastrointestinal function in patients with gastric cancer

Objective:To explore the effect of Dachengqi Decoction and enteral nutrition on the recovery of gastrointestinal function in patients with gastric cancer.Methods:From September 2018 to September 2019, 100 patients with gastric cancer undergoing radical gastrectomy in the First Affiliated Hospital of Henan University of Traditional Chinese Medicine were selected as the research objects, and were divided into observation group and control group according to the random number table method, each group contained 50 cases. Among them, the control group was given routine diet before operation to provide pre adaptation of enteral nutrient solution; the observation group was added with Dachengqi Decoction on the basis of routine diet. The recovery of gastrointestinal function, plasma motilin (MTL), serum gastrin (GAS) and gastrointestinal complications were observed and compared between the two groups.Results:There were 3 cases of abdominal distention and diarrhea in the control group, 1 case of diarrhea and 1 case of nausea and vomiting in the control group. The total incidence of gastrointestinal complications was 16% (8/50). There was 1 case of abdominal distention and 1 case of diarrhea in the observation group, and the total incidence of gastrointestinal complications was 4%(2/50). The difference between the two groups was statistically significant ( χ2 value was 4.000, P<0.05). In the observation group, the time of first bowel sounds, exhaust time and defecation time were (26.12 ± 5.32) h, (25.49 ± 4.49) h, (32.63 ± 4.31) h, respectively, which were significantly shorter than those in the control group (32.24 ± 6.41) h, (46.74 ± 6.14) h, (49.51 ± 6.28) h, and the difference was statistically significant ( t value was 5.195, 19.754, 15.671, P<0.05). The plasma MTL and serum gas in the observation group were (346.54 ± 18.58) ng/L, (123.44 ± 6.19) ng/L. After operation, the results were (247.24 ± 11.40) ng/L, (85.46 ± 5.12) ng/L. The difference was statistically significant ( t value was 97.793, 250.990, P<0.05). The plasma MTL and serum gas of the control group before operation were (344.63±16.65) ng/L and (122.53 ± 6.35) ng/L respectively. After operation, the results were (205.84±10.21)ng/L, (64.79±5.24) ng/L. The difference was statistically significant ( t value was 152.390, 367.823, P<0.05). The levels of plasma MTL and serum gas in the observation group were significantly higher than those in the control group ( t value was 19.129, 19.95, P<0.05). Conclusions:The modified Dachengqi Decoction and the pre adaptation of enteral nutrition solution are helpful to reduce the occurrence of various gastrointestinal complications after gastric cancer operation and promote the rapid recovery of gastrointestinal function, which may be related to the promotion of the secretion of MTL and gas.
9.Astragalus polysaccharide induces apoptosis of human KG-1a cells through inhibiting PTEN-PI3K-Akt signaling pathway

Objective To investigate the effect of astragalus polysaccharide (AP) on proliferation and apoptosis of human acute myelocytic leukemia cell line KG-1a and the underlying mechanisms.Methods Human acute myelocytic leukemia cell line KG-la was cultured under 37 ℃ and 5 % C02,when appropriate cell passage and cryopreservation were performed.After treatment for 48 h with different concentrations of AP,the proliferative activity and apoptosis rate of KG-1a cells were examined by CCK-8 assay and flow cytometry,respectively.The expressions of p-Akt and bcl-2 protein in AP-treated KG-1a cells were evaluated by Western blot.The expression of PTEN mRNA by quantified real-time PCR.Results The proliferative activity of KG-la cell was obviously suppressed by AP treatment,and the inhibition rate increased in a dosedependent manner.The flow cytometry showed that,compared with the control group,the apoptosis rates of KG-1a cells were significantly increased after treatment with AP for 48 h.The early apoptosis rates were (1.98±0.16) %,(12.60±0.48) %,(16.31±0.73) %,the late apoptotic rates were (3.11±0.19) %,(17.17±1.40) %,(21.17 ± 0.81)%,the differences were statistically significant (P < 0.05).Western blot showed that the expressions of p-Akt and bcl-2 protein in KG-1a cells decreased significantly after treatment with AP (P < 0.05).In contrast,the mRNA level of PTEN increased (P <0.05),which was shown by real-time PCR.Conclusion AP could repress cellular proliferation activity of KG-1a cells,which could be attributed to inhibition of PTEN-PI3K-Akt signaling pathway.
10.Technique of retroperitoneoscopic anatomical radical nephrectomy

Objective To introduce the technique and evaluate the clinical effect of retroperito-neoscopic anatomical radical nephrectomy. Methods One hundred and sixty-eight patients under-went retroperitoneoscopic anatomical radical nephrectomies. The average tumor size was 4.7 cm (ran-ging from 2.0-6.9 era) in diameter. There were 87 tumors in the left kidney and 81 tumors in right kidney. Ninety-two eases were in cli.nieal stage T1a. N0 M0 and 76 in T1b N0 M0. Retroperitoneal space was created routinely at lateral decubitus position. Four relatively bloodless planes were orderly entered for exposure and separation of the kidney outside Gerota's fascia. The first dissection plane was be-tween the psoas and posterior Gerota's fascia. The renal pedicle was found in this plane. The following dissections proceeded in the plane between posterior Gerota's fascia and fusion fascia. The third dissec-tion plane was between adrenal gland and the upper pole of kidney or between the adrenal gland and di-aphragma. The fourth dissection plane was in the bottom of Gerota's fascia. Results All operations were successfully completed. The mean operative time was 138:J:46 min and estimated blood loss was 90±30 ml. The average day of resuming oral intake was 1.3 d and time of ambulation was 1.2 d. The mean postoperative hospital stay was 5.8 d. Peritoneum injuries happened in 14 patients. Omalgia oc-curred in 18 patients and disappeared 2 d after operation. One hundred and twenty-three patients were followed up, they all survived during the average follow-up of 8 months (ranging from 6-18 months);, .Conclusions Retroperitoneoscopie anatomical radical nephrectomy is a safe and effective procedure. It can decrease operation time, blood loss and complication rate remarkably. It is a good option for patients needing radical nephrectomy.
