Objective To investigate the induction mechanism of MT ⅢmRNA in the brain after cerebral ischemia.Methods The forebrain ischemia reperfusion model was established in rats. The changes of the expression of MT Ⅲ mRNA in hippocampus after forebrain ischemia reperfusion were observed by in situ hybridization method. The changes of free Zn 2+ in hippocampus after forebrain ischemia reperfusion was examined using Zn 2+ specific fluorescent probe(TSQ). The Zn 2+ chelator (CaEDTA) was injected into the lateral ventricles for determining influences of Zn 2+ on the MT Ⅲ mRNA expression and the neuronal damage after forebrain ischemia/reperfusion.Results (1) The expression of MT ⅢmRNA in hippocampus increased gradually after cerebral ischemia and reached the peak in 96 hours after reperfusion. Seven days after reperfusion the expression of MT ⅢmRNA reduced to the normal level. (2) Zn 2+ fluorescence in the hilus of dentate gyrus, CA 3 region and the stratum radiatum and stratum oriens of CA 1 decreased slightly at 48 hours after reperfusion. From 72 to 96 hours after reperfusion, the fluorescence returned to normal, but some new fluorescence dots appeared in pyramidal neurons of CA 1 and the hilus of dentate gyrus increased gradually. Seven days after reperfusion, the fluorescence returned to normal. (3) The cell membrane impermeable Zn 2+ chelator could reduce the intracellular concentration of free Zn 2+ and the expression of MT Ⅲ mRNA.Conclusion The expression of MT Ⅲ mRNA can be induced by the increase in the concentration of intracellular free Zn 2+ after forebrain ischemia/reperfusion.

Objective To investigate the value of multi-slice CT in diagnosis of abdominal cocoon.Methods CT findings of six cases with pathologically proved abdominal cocoon were collected and analyzed retrospectively.The three-dimensional imagines were obtained including multi-planar reconstruction(MPR),maximum intensity projection(MIP)and volume rendering(VR)at workstation,the rela-tionships between the lesions and surrounding structure were observed.Results A group of local small intestine were seen gathered in all six cases,the coiled intestine arranged inaccordionshape orbananashape.The fibrous capsule were seen around them with different thickness,which were low density and slightly enhancement.Mesenteric arteries showed abnormal changes,showing ten-sion,aggregation.Conclusion MSCT can provide a wealth of diagnostic information and is the best method of examination abdomi-nal cocoon.

Objective To evaluate of the clinical effect of triple arthrodesis and bone grafting combined with plat screw internal fixation for treatment of necrosis of talus. Methods The clinical data of 24 patients with necrosis of talus who were treated by triple arthrodesis and bone grafting combined with plat screw internal fixation from April 2011 to January 2015 in Xiangya Second Hospital of Central South Univer-sity were retrospectively analyzed. And the clinical effect were measured by AOFAS Ankle Hind-foot Scale System. Results The 24 patients were followed up for 26 months averagely (12~36 months). According to AOFAS Ankle Hind-foot Scale System, there were 4 cases of mod-erate and 20 cases of bad before treatment, and there were 18 cases of excellent, 4 cases of good, and 2 cases of moderate after the treat-ment. Conclusion It can achieve good clinical results in treatment of necrosis of talus through triple arthrodesis and bone grafting combined with plat screw internal fixation.

Objective To compare the esophageal acid and alkaline exposure characteristics of patients with cardia carcinoma after proximal gastrectomy or total gastrectomy.Method A total of 77 patients of cardia carcinoma who underwent radical resection from Sep 2007 to Sep 2011 in our hospital were retrospectively reviewed.24 hour esophageal pH monitoring were performed in all patients.Result Patients were divided into three groups:group TG had total gastrectomy (n =25),group PP had proximal gastrectomy with pyloroplasty (n =33),group NP had proximal gastrectomy (n =19).It revealed that indicators of acid reflux including the overall time length of acid episodes,＞ 5 min times of acid episodes,duration of longest acid episodes,time length of pH ＜ 4.00 and the DeMeester Scores in group NP were significantly higher than in group PP(U =32,P ＜ 0.01 ; U =35,P ＜ 0.01 ; U =23,P ＜ 0.01 ; U =39,P ＜0.01 ;U =49,P ＜0.01 respectively).Only alkline reflux was observed in group TG.The total times of alkaline episodes in PP group was significantly lower than in group TG(U =52,P ＜0.01) and group NP (U =182,P ＜0.01).＞5 min times of alkaline episodes in group TG was larger than in group PP,and that in group PP was larger than in group NP(P ＜0.01).Duration of longest alkaline episodes and total period of pH ＞7.00 in group PP was significantly higher than in group TG(U =125,P ＜ 0.01 ; U =143.5,P ＜ 0.01),and that in group TG was higher than in group NP(U =23.5,P ＜ 0.01 ; U =14,P ＜ 0.01).Conclusions Alkaline reflux deserves more attention in evaluating esophageal reflux in patients with cardia carcinoma after resection.Pyloroplasty is not helpful to relieving esophageal acid episodes while causing severe alkaline reflux.

Objective:To evaluate the accuracy of the application of color Doppler sonography (CDS) and computer tomography angiography (CTA) in preoperative perforator identification and flap design and provide theoretical support for the restoration of oral maxillofacial defect with free superficial circumflex iliac artery perforator flap (SCIAPF). Methods: (1) Preoperative CDS and CTA techniques were performed to map the SCIA perforators of 29 adult patients diagnosed with malignant tumor in the oral maxillofacial head and neck regions. These patients were scheduled for concurrent reconstruction surgery. (2) A diagnostic test was designed to com-pare the CDS and CTA techniques. Results:(1) A total of 18 patients underwent flap preparation. SCIA was not found in one of the pa-tients during surgery, but was observed intra-operatively in the other 17 patients. The average SCIA diameter was 0.69 ± 0.20 mm. (2) The diagnostic test showed a CDS sensitivity of 75.0%, a CDS specificity of 82.4%, and an area under the ROC curve of 0.79. The CTA sensitivity was 75.0%, the specificity was 94.2%, and the area under the ROC curve was 0.85. The diameters measured by CDS and CTA were compared with the diameter measured intra-operatively. Significant differences were observed among the three diame-ters (P<0.05). The average diameter measured by CDS was 0.84 ± 0.14 mm. The average diameter measured by CTA was 1.01 ± 0.19 mm. Conclusion:CDS and CTA are relatively reliable technologies for preoperative detection of perforator vessel. The use of CDS and CTA technology mapping for SCIAPF can provide accurate information about the perforator, including the position of the perforator and the relationship between the peripheral tissues and the caliber of the vessel.
