Objective To explore the relationship between the change of serum ferritin level and acute lung injury.Methods 192 cases living hospital patients with critical factor of acute lung injury were divided into two groups:acute lung injury group(study group,52 cases)and having critical factor of acute lung injury but not devel- oped acute lung injury group(control group,140 cases).50 people at the same time were selected as normal group. 3.Epidemiological features of infectious disease related public health emergencies in Zhejiang Province, 2010-2018

Objective:
To learn the epidemiological characteristics of infectious disease related public health emergencies in Zhejiang Province from 2010 to 2018 for the prevention and control.
Methods:
The surveillance data was extracted from National Public Health Emergency Management Information System. Descriptive epidemiology method was used to analyze main diseases as well as distribution characteristics of time and places.
Results:
A total of 445 events were reported, which caused 14362 cases and 34 deaths, with a attack rate of 0.69% and mortality rate of 0.24%. There were 298 events with less than 30 cases, accounting for 66.97%. The event classification was dominated by general events ( 242 events, 54.38% ) and ungraded events ( 201 events, 45.17% ). The main diseases were chickenpox ( 134 events, 30.11% ), hand foot mouth disease ( 59 events, 13.26% ) and other infectious diarrhea ( 51 events, 11.46% ). The incidence peaked from April to June ( 129 events, 28.99% ) and from November to December ( 131 events, 29.44% ). Ningbo ranked the top in the number of reported events ( 141 events, 31.69% ). Most events ( 322 events, 72.36% ) occurred in schools.
Conclusions
The infectious disease related public health emergencies in Zhejiang Province from 2010 to 2018 were mainly caused by chickenpox, hand-foot-mouth disease and other infectious diarrhea. The two peaks of the emergencies occurred from April to June and from November to December. Ningbo was the main area reporting infectious diseases, and schools were the main places.
4.Effect of Insulin on D_5 Dopamine Receptor Expression and Function in Renal Proximal Tubule Cells

Objective To investigate the effect of insulin on D_5 dopamine receptor expression and function in renal proximal tubule (RPT).Methods Immortalized RPT cells and D_5 receptor transfected HEK293 (HEK-D_5) cells were used in the study to investigate the effect of insulin on D_5 receptor expression and function,and those effects were compared in RPT cells from Wistar-Kyoto (WKY) rats and spontaneously hypertensive rats (SHR). The function of D_5 receptor was determined by measurement of the Na~+-K~+-ATPase activity in HEK-D_5 cells. Results Insulin increased D_5 receptor protein expression in a concentration and time-dependent manner in WKY RPT cells,but not in SHR.The basal level of D_5 receptor expression was higher in WKY cells than that in SHR cells. Stimulation with fenoldopam(D_1-like dopamine receptor agonist) inhibited the Na~+-K~+-ATPase activity;pretreat- ment with insulin increased the inhibitory effect of fenoldopam on Na~+-K~+-ATPase activity in HEK-D_5 cells. Conclusion The abnormal regulation of insulin on D_5 receptor expression and function might be involved in the path- ogenesis of essential hypertension.
5.Urinary interleukin-6 level was associated with urinary tract infection in patients with type 2 diabetes mellitus

The association of urinary cytokine concentration with the incidence of urinary tract infection in patients with diabetes mellitus was explored. Urinary interleakin-6 (IL-6) level in 156 female type 2 diabetes mellitus patients was tested and followed up for 6 months. The incidence of urinary tract infection between the top quartile of baseline urinary cytokine concentration with the bottom quartile was compared. The incidence of top quartile of urinary IL-6 concentration at baseline was significantly lower than that of bottom quartile ( 13.5% vs 37.8%, P<0.05 ). Abnormality of urinary IL-6 concentration may be involved in the mechanism leading to higher prevalence of urinary tract infection in patients with diabetes mellitus.
6.Efficacy observation on navel-warming therapy combined with western medication for yang-deficiency tympanites.

OBJECTIVETo observe the effects of navel-warming therapy on clinical efficacy in patients with yang-deficiency tympanites based on regular treatment of western medication.
METHODSOne hundred and twenty cases of yang-deficiency tympanites were randomly divided into a navel-warming therapy group and a western medication group, sixty cases in each one. The regular treatment of western medicine was applied in the western medication group, including oral administration of antiviral drug and diuretics as well as intravenous drip of hepatic protector. Based on western medicine treatment, the navel-warming therapy was applied in the navel-warming group. A medical cake was laid on Shenque (CV 8), and then a medical cylinder was placed above the medical cake and ignited. The treatment was given once daily. One month was considered as a treatment session in both groups and totally one session was required. The TCM symptom score, B-ultrasound ascites and temporary use of diuretics before and after treatment were observed in both groups; also the efficacy was evaluated.
RESULTSThe total effective rate was 81.7% (49/60) in the navel-warming therapy group, which was superior to 56.7% (34/60) in the western medication group (P < 0.05). After the treatment, the TCM symptom score and ascites were improved in both groups (all P < 0.05), which was more significant in the navel-warming therapy group (all P < 0.05). The temporary use of diuretics was statistically different between the two groups (P < 0.05), indicating the navel-warming therapy group could obviously reduce or stop the use of diuretics.
CONCLUSIONBased on regular treatment of western medication, the navel-warming therapy could significantly improve therapeutic efficacy, effectively relieve clinical symptoms and ease ascites.

7.Long Backbone Fracture Caused by Blunt Force: 99 Cases Analysis.

OBJECTIVE:
To investigate the main point of long backbone fracture caused by blunt force in forensic clinical identification and to provide a reference for the inspection and appraisal practices of such injury.
METHODS:
Ninety-nine cases of adult long backbone fractures were collected from January 2006 to December 2013 in Gutian County of Fujian Province. According to the terms of fracture location, mode of injury, type, the data were summarized.
RESULTS:
In the 99 cases, there were 36 cases caused by hitting, kicking, and falling and 63 cases caused by vehicle collision. The majority of the former was ulna, and those of the latter were tibia and fibula. The types of fracture were transverse one, short oblique one, long oblique one, and spiral one.
CONCLUSION
Different types of long backbone fracture, not only causing stress load of fractures as well as structural differences related to each segment.

8.Clinical trial of benzbromarone tablets versus allopurinol tablets in the treatment of acute cerebral infarction combined with hyperuricemia

Objective To compare the clinical efficacy and safety profile of benzbromarone tablets versus allopurinol tablets in acute cerebral infarction combined with hyperuricemia.Methods A total of 180 patients with acute cerebral infarction combined with hyperuricemia were randomly divided into the control group (n =60 cases),the treatment A group (n =60 cases),the treatment B group (n =60 cases).All three groups were given conventional treatment of cerebral infarction.The control group was given placebo while the treatment A group was given allopurinol 100 mg bid,and the treatment B group was given benzbromarone 50 mg qd.All three groups were treated for 14 days.The clinical efficacy,uric acid (UA),high sensitive C-reactive protein (hs-CRP),interleukin-6 (IL-6),carotid intima-media thickness (IMT),incidence of adverse drug reactions were compared before and after treatment between three groups.Results After treatment,the total effective rates in three groups were 76.67 % (46/60 cases),88.33 % (53/60 cases),95.00% (57/60 cases),with significant differences (P < 0.05).After treatment,the main observation indexes in the treatment A group,the treatment B group and the control group:the levels of UA were (237.31 ± 10.63),(162.67 ± 10.12),(586.52 ± 50.60) μmol · L-1;the levels ofhs-CRP were(4.93 ± 0.68),(3.57 ± 1.03),(12.07 ± 0.54) mg · L-1;the levels of IL-6 were (78.31 ± 10.63),(68.67 ± 10.12),(190.52 ± 50.60) pg · mL-1;the levels of IMT were (1.08 ± 0.25),(0.76 ± 0.29),(1.42 ± 0.54) mm,with statistically significant difference (P < 0.05).Compared with treatment A group,the levels of UA,hs-CRP,IL-6,IMT were significantly lower in treatment B group,with significant difference (P < 0.05).No obvious adverse drug reactions occurred in three groups.Conclusion Both benzbromarone and allopurinol were able to markedly alleviate uric acid levels,inflammatory reaction and atherosclerosis with high safety profile,while benzbromarone exerted higher efficacy.
9.Study on multi-target optimization of prescription dose of Mahuang decoction.

OBJECTIVETo optimize the prescription dose of Mahuang decoction in a multi-target manner, in order to provide reference for the quantitative optimization of the prescription dose of the traditional Chinese medicine compound.
METHODThe number of diaphoretic spots in rats, the tracheal antispasmodic rate in guinea pigs and the writhing times by acetic acid in mice were taken as the indexes for evaluating the diaphoretic, antispasmodic and analgesic effects. According to the experimental results of the 16 orthogonal combination prescriptions, a mathematical dose-effect model was built by support vector regression (SVR) and quadratic response surface regression (RSR) respectively. The multi-target optimization was achieved by elitist non-dominated sorting genetic algorithm (NSGA-II) and entropy weight TOPSIS method.
RESULTThe optimal dose of Mahuang decoction after being optimized by SVR modeling contained 17.71 g of Ephedrae Herba, 9.57 g of Cinnamomi Ramulus, 11.75 g of Armeniacae Semen Amarum and 4.39 g of Glycyrrhizae Radix et Rhizoma Praeparata Cum Melle. The optimized result by RSR modeling contained 13.37 g of Ephedrae Herba, 11.61 g of Cinnamomi Ramulus, 11.98 g of Armeniacae Semen Amarum and 5.67 g of Glycyrrhizae Radix et Rhizoma Praeparate Cum Melle. SVR was superior to RSR in both of the forecast capacity and optimization results.
CONCLUSIONSVR-NSGA-II-TOPSIS method could be adopted for the multi-target optimization for the dose of Mahuang decoction and other traditional Chinese medicine compounds. It is proved to be the optimal prescription with the best efficacy, and could provide scientific quantitative basis for determining the dose of traditional Chinese medicine compound prescriptions and developing new traditional Chinese medicines.

10.Protective mechanism of Danhong injection on brain microvascular endothelial cells injured by hypoxic.

To study the protective mechanism of Danhong injection on brain microvascular endothelial cells (rBMECs) injured by hypoxic. In the experiment, primary suckling mouse's rBMECs cells were collected and identified with factor VIII to establish the 4 h injury model. Meanwhile, rBMECs were given Danhong injection (25, 50, 100 mL . L-1), and the superoxide dismutase (SOD) activity and the malonyldialdehyde (MDA) level were detected by the biochemical method. Cell MMP-9, ICAM-1 and P53 mRNA expression levels were detected by RT-PCR method. Changes in cells' microscopic structure were observed by transmission electron microscope. According to the results, primary rBMECs were notably injured by hypoxia. Compared with model group, Danhong injection (50, 100 mL . L-1) could remarkably resist the injury induced by hypoxic, increase intracellular SOD activity, decrease MDA level and significantly down-regulate ICAM-1, MMP-9 and P53 mRNA expressions. Danhong injection (100 mL . L-1) could protect the cells' normal morphology and microscopic structure, maintain the close intercellular junction, and inhibit the hypoxia-induced cell apoptosis. The results showed that Danhong injection plays a significant role in protecting rBMECs injured by hypoxia. Its mechanism may be related to the enhancement of cells' antioxidant capacity, the inhibition of inflammatory response and the cell apoptosis.
