1.Analysis of the procedure for prolapse and hemorrhoids

Objective To assess the clinical value of procedure for prolapse and hemorrhoids(PPH).Methods 32 patients withⅢandⅣhemorrhoids underwent PPH.Pain,bleeding prolapse and edema,1 day,7 days and 30 days after the procedure were investigated.Results Of the 32 patients,the average operating time was 8.20min.On the first day of the procedure,pain occurred in 9.3% and bleeding in 5.7%.But 30 days later,both pain and bleed- ing were relieved.The mean hospital stay after the procedure was(3.4?1.2)days,and the mean hospital cost was (7342.6?486.3)yuan.Retention of urine(68.8%)was the frequent complication,whose percentage was as high as 68.8%.Conclusion PPH is effective,safe,minimally invasive,and is the first choice to deal with severe hemor- rhoids.
2.EFFECT OF DIFFERENT SURFACE CONDITIONING METHODS ON SHEAR BONDING STRENGTH OF GLASS-INFILTRATED CERAMIC I TO DENTINE

To investigate the effects of different surface conditioning methods on the shear bonding strength of glass-infiltrated ceramic to dentine, 24 glass-infiltrated ceramic testing-pieces were divided into four groups randomly. Among them 3 groups were treated with A(sandblasting+silance), B(etching+silace) or C(silance), separately, while group D was set as the control. Bonding the ceramic pieces to dentine with Super-Bond and the shear bonding strengths was measured. The results showed that the shear bonding strength of group D was lower than that of group A, B and C (P0.05). It is suggested that surface conditioning using combined sandblasting with silance can remarkbly improve the shear bonding strength of glass-infiltrated ceramic to dentine.
3.STUDIES ON IN-VIVO PROCESS OF THE ENANTIOMERS OF NICARDIPINE IN RABBIT BY COUPLED CHIRAL AND ACHIRAL HPLC

AIM To study the differences of the pharmacokinetics and tissue distribution of two enantiomers of nicardipine in the rabbit. METHODS Biological samples were diluted by 1 mol*L-1 NaOH solution and extracted with n-hexane - ethyl acetate (1∶1). The concentrations of S-nicardipine and R-nicardipine in samples were determined by coupled achiral C18 column (150 mm×4.6 mm, 5 μm) and chiral OJ column (250 mm×4.6 mm, 10 μm) chromatography. RESULTS The racemic nicardipine and the enantiomers in sample were separated well by the coupled system. The linear range was 55-550 ng*mL-1 for both enantiomers. The within-day and between-days RSD (n=5) were 5.25% and 8.97%, and the relative recoveries were 99.99% and 97.10% for R- and S- enantiomer, respectively. The mean Tmax, Cmax and AUC values were (2.49±0.03) h, (134±2) ng*mL-1 and (1082±32) ng*mL-1*h for S-nicardipine and (1.24±0.05) h, (109±2) ng*mL-1 and (778±22) ng*mL-1*h for R-nicardipine. The concentration of S-nicardipine were generally higher than that of R-nicardipine in main target tissues and cells. CONCLUSION There were significant differences between the two enantiomers of nicardipine in rabbit in pharmacokinetics and tissue distribution.
4.The separation of ?-adrenoceptor antagonist enantiomers on Chiralcel~ OD column

Objective To study the enantioselective characteristics and laws of ?-adrenoceptor antagonist isomers on Chiralcel ○R OD column. Methods The experiments were performed under the following HPLC conditions: a Chiralcel OD column (250 mm?4.6 mm, 10 ?m) as analytical column, a mixture of n-hexane/2-propanol/ triethylamine as the mobile phase, the fluorescence detection wavelengths at 275 nm (? ex) and 310 nm (? em), and the flow rate at 0.5 mL?min -1. The effect of interaction between stationary phase and 2-propanol or triethylamine concentration in mobile phase on the enantiomer resolution were investigated by the stoichiometric displacement model for retention. Results The lgI values of R-enantiomers of 3 out of 5 ?-adrenoceptor antagonist were higher than those of S-enantiomers. The 2-propanol in mobile phase would differently affect the resolution between enantiomers for each ?-adrenoceptor. With the increase of triethylamine concentration in mobile phase, the capacity factor (k′) of the enantiomers decreased but the resolution increased. Conclusion The lgI and Z values of SDM-R could be employed to characterize the resolution efficiency of Chiralcel OD column and the specific effect of enantiomers interacted with the stationary phase. Adding triethylamine in mobile phase will increase the resolution efficacy and changing column temperature can promote enantiomer resolution.
5.Effect of Atorvastatin on Blood Lipid and Uric Acid Levels of Elderly Patients with Coronary Heart Disease and Diabetes Mellitus

Objective:To investigate the effect of atorvastatin on the blood lipid and uric acid levels of elderly patients with coronary heart disease complicated with diabetes mellitus.Methods:116 patients with coronary heart disease and diabetes were randomly divided into the control group and the experimental group,58 cases in each group.Both groups of patients were given blood glucose control,blood pressure and other symptomatic treatment.The control group was treated with Aspirin Enteric-coated Tablets 0.3~0.6 g/times,3 times/d,oral,clopidogrel tablets,2 tablets each time,1 time/d,oral,nitroglycerin,0.25~0.5 g/time,3 times/d,with service;the experimental group was given atorvastatin on the basis of control group,10~20 mg/time,1 time/d,treatment for 4 weeks.During the treatment,the dosage was timely adjusted according to the conditions of patients.The serum low density lipoprotein cholesterol (LDL-C),high density lipoprotein cholesterol (HDL-C),triglyceride (TG),total cholesterol (TC),uric acid (UA),glycosylated hemoglobin (HbA1c) levels before and after treatment and the clinical treatment efficiency were observed and compared between two groups.Results:Compared with before treatment,the serum LDL-C,TG,TC,UA,HbA1c levels were decreased ahter treatment in both groups of patients,the HDL-C level was increased (P<0.05);compared with the control group,the serum LDL-C,TG,TC,UA,HbA1c levels were lower,HDL-C level was higher in the experimental group (P<0.05);compared with the control group,the effective rate of clinical treatment of the experimental group were higher (P<0.05).Conclusion:Atorvastatin could effectively reduce the blood glucose,blood lipid,uric acid levels of elderly patients with coronary heart disease complicated with diabetes.
6.Clinical analysis on the 39 cases of gallstones developed after surgery treatment of esophageal cancer

Objective To investigate the mechanism and prevention of gallstones after esophageal cancer surgery.Methods Clinical data from 297 cases of esophageal cancer patients underwent surgical treatment from 2007 to 2010 inour hospital were retrospectively analyzed for the incidence of post-operative gallstones.ResultsNo cholecystitis and gallstones were detected by B-ultrosound before the operation in the 39 patients.13.1% of the patients developed gallstones from 1 month to 3 years after the operation.The incidence was significantly higher than that in general population.Conclusion There is a high chance of gallstones in patients after radical resection of esophageal cancer.Cutting vagus nerve plays an important role in the incidence of gallstones.Preservation of vagus nerve during the surgery should be performed as possible.
7.Construction of Recombinant Mice Factor Ⅶ-pPIC9K Vector and Expression in Pichia

Objective:To construct the yeast expressive vector of rmFⅦ, in which mFⅦ was mutated to inhibit coagulation without affecting the affinity for TF, and express it in Pichia pastoris. Methods:The full length cDNA encoding mFⅦ was amplified from a mouse liver by RT-PCR method, site-direct mutated and restriction enzyme digested as design. Cloning into pPIC9K, electroporation of Gs115, in vivo screen of multiple inserts by G418 resistance, BMGY/BMMY are used for induction and expression of rmFⅦ in pichia pastoris. These proteins were also screened for functional activity. Results: Three different rmFⅦ-pPIC9K yeast expression vectors and it's aim protein were obtained,two kinds of proteins were found to be functional active as design. Conclusion:rmFⅦ protein can be expressed in pichia pastoris and it might facilitate the development of tumor-target molecule, and novel anti-agiogenesis drug study.
8.Bradycardia as a sign of outcome in patients after cardiac arrest during targeted body temperature management

Objective To evaluate the association between bradycardia and neurological sequel in patients with restoration of spontaneous circulation (ROSC) after cardiopulmonary resuscitation (CPR) during targeted body temperature management (TTM).Methods Forty-three unconscious patients with ROSC after CPR were treated with TTP.The patients were cooled with therapeutic hypothermia to body temperature target range (32-34°C) after cardiac arrest and divided into bradycardia and control groups depending on the lowest heart rate less than 50 beats/min and more than or equal to 50 beats/min respcetively at that time.The bispectral index (BIS) and the neuron-specific enolase (NSE) values were respectively recorded at the following intervals,0h (h0)、24h (h24)、48h (h48)、72h (h72) after ICU admission.Neurological outcome was defined according to the Pittsburgh cerebral performance category (CPC) at 3 months after ICU discharge.Results Compared with the control group,during TTM the bispectral index levels were significantly higher in the bradycardia group at h0,h48,h72 after admission,(h0 bradycardia group 73.0 ± 12.3;control group 58.0 ± 18.6,P <0.01)、(h48 bradycardia group 71.4 ± 21.2;control group 46.3 ± 18.9,P < 0.01)、(h72 bradycardia group 78.6 ± 24.6;control group 51.8 ± 24.1,P =0.01).The neuron-specific enolase level in bradycardia group was significantly lower than that in control group on day3 (118.8 ± 118.8 ng/mL vs.248.3 ± 191.9 ng/mL,P =0.02).The level of CPC in the bradycardia group was significantly higher than that in the control group (P =0.046).Conclusions Patients with bradycardia during TTM had favorable neurological outcome,which could provide evidence for clinical treatment and prognostic evaluation of the patients.
9.The application of a small incision combined with improved percutaneous tracheostomy in difficult tracheostomy

Objective To describe an improved percutaneous tracheostomy combined with conventional tracheostomy technique with result of less trauma and fewer complications, and to explore its application in the patients for whom conventional tracheostomy is difficult to perform.Methods A prospective study was conducted.Fifty-seven hospitalized patients, in whom ordinal tracheostomy was difficult to perform, and admitted to Department of Critical Care Medicine of the First Affiliated Hospital of Bengbu Medical College from January 2013 to December 2014 were enrolled.According to the random digital table method, the patients were divided into small incision combined with percutaneous tracheostomy group (small puncture incision group, n =25) and conventional tracheostomy group (n =32).Amount of blood loss, postoperative bleeding, incision size, operation time and wound healing time were compared between the groups.Results Compared with traditional surgical tracheostomy group, the blood loss and postoperative bleeding were decreased [blood loss (mL): 11.36 ± 4.25 vs.23.72 ± 7.29, t =-7.201, P =0.000;postoperative bleeding (mL):11.60 ± 6.57 vs.26.77 ± 10.77, t =-5.834, P =0.000], incision size was smaller (cm: 2.20 ±0.63 vs.4.06 ± 1.19,t =-6.806, P =0.000), and operation time and wound healing time were shortened [operative time (minutes): 18.16 ±3.61 vs.29.09±6.77, t =-7.001, P =0.000;incision healing time (days): 4.96± 1.59 vs.7.19±2.35, t =-3.975,P =0.000] in small puncture incision group.Conclusion Compared with the traditional method, small incision puncture tracheostomy is less time consuming, with fewer traumas, and fewer intraoperative and postoperative complications.
10.Prognostic value of neuron-specific enolase and bispectral index in patients after cardiopulmonary resuscitation

Objective To evaluate the prognostic value of the neuron-specific enolase ( NSE ) and bispectral index ( BIS) in patient with mild therapeutic hypothermia ( MTH) after cardiopulmonary resuscitation ( CPR ) .Methods Forty-six patients with restoration of spontaneous circulation ( ROSC ) after CPR were treated with MTH.The BIS values were recorded and the serum NSE was measured at the following times:24 h, 48 h, and 72 hours after ICU admission.Neurological outcome was classified according to the Pittsburgh cerebral performance category ( CPC 1 to 5) at 3 months after ICU discharge.Results Fourteen patients had a good neurological outcome with CPC score 1-3, and thirty-two patients had a poor neurological outcome with CPC 4-5 at 3 month.Compared with good outcome group, the NSE values were significantly higher in the poor outcome group on day 2 and day 3 after admission [48 h: (90.1 ±42.7) ng/mL vs.(33.2 ±17.5) ng/mL;72 h: (95.4 ±37.0) ng/mL vs. (29.2 ±17.0) ng/mL, P<0.05].NSE increased markedly in the poor neurological outcome group at 48h and 72h, and decreased significantly in the good group at the same time [△NSE 24 h-48 h: (37.3 ±28.7) ng/mL vs.(-10.7 ±12.1) ng/mL; △NSE 48 h-72h: (5.3 ±13.2) ng/mL vs.(-4.0 ±4.5 ng/mL), P<0.05].Over the 72 h of monitoring, the mean BIS values were lower in the poor outcome group compared to the good outcome group at 48 h [ (39.2 ±24.1) vs.(78.0 ±12.4); 72 h: (45.7 ±26.4) vs.(89.0 ±7.3), P<0.05].Conclusions The values of NSE and BIS were effective prognostic indicators for the neurological outcome of patients with MTH after CPR.
