1.A large-stale aero transportation of mass casualties away from 5.12 Wenchuan earthquake region to other provinces

Objective To sum up the experience of a large-scale aero transportation of mass casualties away from the quake-hit region to distant cities,and to provide reference for medical transportation of mass casualties.Method The study analyzed the aero medical transportation of 3 482 casualties from quake-hit region treated in West China Hospital of Siehuan University and other seven hospitals in Sichuan Province to other provinces from 22 May 2008 to 3 June 2008.The influencing factors of aero medical transportation were discussed,including selection standard of patients,preparation of staff and mcterials before transportation,management during transportation.The selection standards were:patients with stable condition after orthopedic operation and craniocerebral operation,and patients with passage of gas by anus after abdominal operation 5 days.Results Total of 3 482 casualties were transported in 99 times to 19 cities including Guangzhou,Shanghai,Shenyang,only 8 victims felt a little bit uncomfortable during flight,and recovered soon after management.Conclusions Total of 3 482 patients were safely transported.Strict selection of patients,comprehensive planning and monitoring,and psychotherapy are important for aero medical transfer.
2.Clinical research of Salmeterol fluticasone propionate and Tiotropium bromide for improvement of pulmona-ry function in patients with chronic obstructive pulmonary disease and bronchial asthma

Objective To observe the improvement of Salmeterol fluticasone propionate and Tiotropium bromide for inhalation in the treatment of chronic obstructive pulmonary disease ( COPD ) with bronchial asthma patients.Methods 89 patients were randomly divided into the observation group with 46 cases and control group with 43 cases,both groups were given conventional therapy.The control group was given Salmeterol fluticasone propionate 50μg/500μg,twice a day;the observation group was given Tiotropium bromide 18μg, twice a day, based on the therapy of control group.The two groups were treated for 14 days,and then curative effect,score of SGRQ,pulmonary function and blood gas analysis index were evaluated, and adverse reactions were recorded.Results The total effective rate of treatment group was 86.1%,higher than that of the control group(68.6%) (Z=2.68,P<0.05);Compared with the control group after treatment,functional parameters[ FEV1 ( L) ,FEV1 (%) and FEV1/FVC(%) ] of the observation group increased significantly,and the observation group were all higher than that of the control group [(2.27 ±0.45) vs (1.92 ±0.36),(64.27 ±16.11) vs (53.64 ±15.55),(54.85 ±14.81) vs (45.33 ± 13.23)],the differences were all statistically significant(t=4.20,3.16,3.19,all P<0.05);PaO2 of the two groups patients were increased and PaCO2 were decreased,scores of SGRQ were decreased obviously,and the change of the observation group was obviously[(47.61 ±3.64) vs (49.34 ±4.23),(67.33 ±5.56) vs (63.66 ±5.21), (45.34 ±3.72) vs (50.65 ±4.16)],the difference was statistically significant(t =2.13,3.32,6.54,all P<0.05) .Conclusion Salmeterol fluticasone propionate and Tiotropium bromide for inhalation can improve lung func-tional parameters and respiratory conditions of the parents with COPD and bronchial asthma obviously, reduce the score of SGRQ,curative effect is distinct,worthy of clinical popularization and application.
3.CT Appearances of Peripheral Pulmonary Carcinoma:A Comparison with Histopathological Findings

4.Prognostic value of combining postprocedural fibrinogen with C-reactive protein levels in acute myocardial infarction patient underwent primary percutaneous coronary intervention

Objective To evaluate the prognostic value of combining postprocedural fibrinogen with high-sensitivity C-reactive protein (hs-CRP) in first time ST elevated myocardial infarction (STEMI) patients who had underwent successful primary percutaneous coronary intervention (PCI).Methods A total of 183 consecutive patients who had their first acute STEMI attack and underwent successful primary PCI were enrolled. Fibrinogen and hs-CRP levels were measured within 12 hours after PCI. All patients were followed up for 2 years. The primary end point was death of any cause. The secondary end point was a combined end point of death, non-fatal MI, heart failure (NYHA Ⅲ~Ⅳ),myocardial ischemia confirmed by stress test and revascularization. 5.Efficacy of rituximab in treatment of children with refractory idiopathic thrombocytopenic purpura

Fifty children with refractory idiopathic thrombocytopenic purpura (RITP) were treated with rituximab (n =26) or vincristine (n =24).The response rate,adverse reaction and recurrence rate in two groups were compared.The CD19 +/CD20 + B cells in peripheral blood were detected by flow cytometry in 26 patients before and after RITP treatment.The response rates of rituximab group was significantly higher than that of vincristine group ( 69.2% vs. 37.5%,x2 =9.74,P < 0.01 ). There was no significant difference in the rate of adverse reaction between two groups ( 11.5% vs.8.3%,x2 =0.62,P > 0.05 ).The recurrence rate of rituximab group ( 22.2% ) was significantly lower than that of vincristine group (55.6%,x2 =7.24,P < 0.05 ).In rituximab group the platelet count after treatment was significantly higher than that before treatment ( t =12.48,P <0.01 ),and the ratio of CD19 +/CD20 + B cells decreased significantly after treatment compare to that before treatment ( t =6.71,P <0.05 ).Rituximab is effective in treatment of refractory idiopathic thrombocytopenic purpura in children,which may be associated with decreased B cells in peripheral bloos.
6.Changes of protein expression profile in vascular tissues of spontaneously hypertensive rats treated by a compound Chinese herbal medicine.

Objective: To investigate the effects of a Chinese herbal formula for calming liver and suppressing yang on the protein expressions of vascular tissues in spontaneously hypertensive rats (SHRs), and to explore the mechanism of efficacy. Methods: Twenty SHRs were randomly divided into model group and treatment group. Another 10 Wistar-Kyoto rats were selected as a normal control. SHRs in the treatment group were administered with the formula for calming liver and suppressing Yang for 4 weeks. During the course of treatment, blood pressure and heart rates were monitored every week and the ethology of rats, including irritability and rotation endurance was also evaluated. After treatment, thoracic aorta was obtained and its proteins were separated by 2-dimensional gel electrophoresis (2-DE). The differentially expressed proteins were identified by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) and database query. Results: The formula for calming liver and suppressing yang not only decreased the systolic blood pressure and heart rate, but also improved irritability degree and rotation endurance time of SHRs. This experiment had established the 2-DE graph of protein expressions of vascular tissues in SHRs. Compared with the normal group, the expressions of 15 proteins were down-regulated, and 12 proteins were up-regulated in vascular tissues of the model group. The formula for calming liver and suppressing yang treatment up-regulated expressions of 10 proteins in the 15 down-regulated proteins, and down-regulated 8 proteins in the 12 up-regulated proteins in vascular tissues of SHRs. After analysis, 16 obviously differentially expressed proteins were found, and 13 of them were identified. Conclusion: The formula for calming liver and suppressing yang can improve the ethology of SHRs. The mechanism is probably concerned with regulating the protein expressions of vascular tissues.

8.Preparation and Identification of Main Ingredients in Effective Parts of Xinjiang Artemisia rupestris

OBJECTIVE:To establish a method for rapid identification and efficient preparation of main ingredients in effective parts of Xinjiang Artemisia rupestris,and provide reference for researching the ethnic medicines. METHODS:LC-HRMS/MS was conducted for the preliminary study of main ingredients in effective parts of A. rupestris. HPLC,UV and MS were used to compare and analyze parts of the compounds and its reference substances,their names were determined. Column separation and preparative liquid chromatography were used for the undetermined compounds to receive monomer rapidly,and the structures were identified. RESULTS:5 compounds were separated from the effective parts,2 of which were identified as artemetin and casticin. A monomer-ic compound was obtained (yield was 0.35 mg/g,the purity was 98.5%),which was confirmed to be 6-demethoxy-4′-O-me-thoxy-capillarisin-7-O-β-D-glucoside by the structure. CONCLUSIONS:The method has achieved rapid separation,identification and preparation of target ingredients,which can be used for the fundamental research of ethnic medicine complex materials.
9.Anticonvulsive action of Pinellia Pedatisecta Schott extract prepared by ethanol-modified supercritical CO2 extraction

AIM To investigate the anticonvulsive action of supercritical CO2 ethanol extract from Pinellia Pedatisecta Schott(SEE-CO2PP). METHODS The rat convulsive model was induced by penicillin localized injected in rat cortex. The effects of SEE-CO2PP on the latency of seizure and changes of convulsive behaviors were investigated. The latency of epileptiform discharge, and frequency and amplitude of highest wave in cortex and hippocampus were recorded by using RM6240C multichannel physiological signal collection and analysis recorder. At the same time, the contents of glutamic acid (Glu), aspartic acid (Asp), glycine (Gly) and γ-aminobutyric acid (GABA) in hippocampus were determined with high performance liquid chromatography. RESULTS SEE-CO2PP 15 and 30 g·kg-1, ig, prolonged the latent period of seizure and weakened the extent. SEE-CO2PP also prolonged the latent period of epileptiform discharge, reduced the frequency and decreased amplitude of the highest wave in both cortex and hippocampus. Moreover, SEE-CO2PP increased the content of GABA in hippocampus, but the levels of Gly,Asp and Glu had no obvious changes. CONCLUSION SEE-CO2PP inhibits the epileptiform discharge and convulsive behaviors of convulsive model rats, which suggests SEE-CO2PP has anticonvulsive action.
10.Effect of HCMV on expressions of K8 and K18 in duct epithelial cells of salivary gland

Objective To study the effect of human cytomegalovirus (HCMV) on expressions of K8 and K18 in duct epithelial cells of salivary gland. Methods The expressions of immediate early antigen of HCMV, K8 and K18 were detected by immunohistochemistry staining in tissues embedded in paraffin of parotid cytomegalic inclusion disease(PCID). Results Cytomegly bearing inclusion appeared in duct epithelium of PCID. DDG9/CCH2 antigen of HCMV was expressed in cytomegly bearing inclusion. K8 was negative in these cytomegly while K18 was intensively positive. Conclusion It is suggested that breaking down of K8 be induced in parotid duct epithelial cells infected by HCMV and that up-regulation of K18 may be a reactive change. Keratin network in simple epithelium functions to impart mechanical integrity to cells.
