Objective To summarize the effect and experience of nursing of patients with rheumatic valvular heart disease with atrial fibrillation treated with Cox Ⅲ labyrinth and simultaneous valve replace-ment operation. Methods A retrospective analysis was carried out in the nursing of 58 patients with rheumatic valvular heart disease with atrial fibrillation treated with Cox Ⅲ labyrinth and simultaneous valve replacement operation in our hospital from April 2006 to July 2008. Results One patient died of periop-erative renal failure,one died of low cardiac output,one died of infectious endoearditis.Tbe mortality rate reached 5.2%.Other patients were discharged from hospital after completely cured.The survived patients maintained sinus rate during hospitalization days after operation and short-term follow-up after being dis-charged.Among 43 patients who completed follow-up of 6 months after operation,3 patients (7.0%) damon-strated atrial fibrillation,while others still kept sinus rate. Conclusions Attention should be paid to psy-chological nursing,respiratory tract preparation,cardiac function monitoring and nursing of anticoagulation treatment for patients treated with Cox Ⅲ labyrinth and simultaneous valve replacement operation.

AIM: To construct the eukaryotic expression vector CD80-IgG by fusing the cDNA encoding extracellular portion of murine CD80 to the 5'-terminus of cDNA encoding Fc fragment of murine immunoglobulin G1 and to express the fusion protein in Chinese hamster ovary (CHO) cells. METHODS: The two cDNAs was amplified by PCR respectively from plasmid pcDNA/B7 containing the full-length cDNA of murine CD80 from murine spleen cells, and cloned to the eukaryotic expression vector pcDNA3.0 by directional cloning. The resultant recombinant plasmid pcDNA/CD80-IgG was transfected into CHO cells with liposome transfection reagent. The stably expressing cells were obtained by G418 screening. Western blot, Dot ELISA, and flow cytometry were used to detect the expression of the fusion protein and its immunological activity. RESULTS: DNA sequencing verified the correction of the construction of recombinant plasmid pcDNA/CD80-IgG. The expressed fusion protein was detected in the supernatant of transfected CHO cells and the molecular weight of the protein was similar to what we expected. Its immunological activity was also established. CONCLUSION: The recombinant plasmid pcDNA/CD80-IgG was successfully constructed and it expressed the fusion protein CD80-IgG.

Objective To analyze the nontubercutous mycobaeterium (NTM) identification data of two groups of sputum sam- ples during the periods of 1986 to 1997 and 2000 to 2005 so as to figure out the identification of NTM.Methods A total of 222 strains of non-tuberculosis mycobacteria were included for strain identification and sensitivity test with traditional methods.Re- suits According to Runyon classification, during the period of 1986-1997 there were 15 strains (15.5%) in GroupⅠ, 4 (4.2%) in GroupⅡ, 23 (24.0%)in GroupⅢand 54 (56.3%) in GroupⅣ;during the period of 2000—2005 there were 30 strains (16.1%) in GroupⅠ, 11(5.9%) in GroupⅡ, 51 (27.4%) in GroupⅢand 94 (50.6%) in GroupⅣ.The number of NTM types increased by 133.3%.The absolute number of NTM isolates in the first five years of this century increased by 93.89% compared with the numbers in the 11 years of last century.Conclusions The number of types and absolute number of i- solates of NTM have increased in the first live years of this century compared with the numbers in the 11 years of last century. We should enhance the epidemiological research on pulmonary NTM in order to provide scientific evidence for comprehensive prevention and treatment.

Objective To study the effects of recombinant human growth hormone(rhGH) on the treatment for valve replacement postoperative in rheumatic heart disease associated cardiac eaehex.Methods Fortytwo patients with rheumatic heart disease associated cardiac eachexia were divided into two groups.Group one(n=20,rhGH group) received standard enteral nutrition (15 kal·kg-1 · d-1)with rhGH 10U injection subeutaneously from postoperation day 7 to day 14 and group two(n = 22,eontrol group) received standard enteral nutrition (15 kal·kg-1· d-1) for the same period.Haemoglobin, serum total protein, serum albumin, blood glucose, handgrip exercise and triceps skin_fold thickness were determined.Meehanieal ventilation, hypostatie pneumonia incidence rate, and length of stay were observed.Results The levels of serum total protein, serum albumin and blood glucose eoneentration in the rhGH group at the 14th day were inereased significantly compared to that in the control group(P <0.01).Haemoglobin, triceps skinfold thiekness and handgrip exercise in rhGH group were significantly different from those in the control group(P <0.05).Postoperative meehanieal ventilation time, intensive care unit time, hospital stay time were signifieantly shorter than those in the eontrol group (P < 0.05), and hypostatie pneumonia was significantly lower than that in the eontroi group(P < 0.01).Conclusions The rhGH can obviously improve anabolie effects of patients with rheumatic heart disease associated cardiac eachexia whieh can reduce hypostatie pneumonia and shorten postoperative hospital stay time.

Objective To evaluate the curative effect of potassium magnesium aspartate on arrhythmia and heart function in valve replacement postoperative patients of rheumatic heart disease. Methods Two hundred and eighty patients with rheumatic heart disease were divided into 2 groups in random,with no statistical significance. Treatment group (n=155) were received i.v. potassium magnesium aspartate 40 ml every day and control group (n=125) were only given conventional therapy. At the same time they were given same treatment. Blood magnesium level,blood potassium levels,arrhythmia incidences and heart function were also observed. Objective To study the effects of the recipe for activating blood circulation and nourishing qi on expression of mitochondrial ribosomal protein L51 (MRPL51) in CVB3 infection model in rat cardiac myocytes, to reveal the pathogenesis of CVB3 myocarditis in the genetic level, to explore the therapeutic mechanism of the recipe for activating blood circulation and nourishing qi on CVB3 myocarditis, and to confirm the validity of the recipe for activating blood circulation and nourishing qi on CVB3 myocarditis. Methods After establishing CVB3 infection model and treatment model with recipe for activating blood circulation and nourishing qi by culturing neonatal rat myocardial cells, a modified suppression subtractive hybridization (SSH) was used to isolate differentially expressed genes between two model groups. These results were further verified by fluorescence RT-PCR. Results The results of SSH showed that gene expression of the treatment group was lower than that of the CVB3 infection group. The results of fluorescent RT-PCR which agreed with that of SSH displayed the threshold cycle number (Ct) in the treatment group was higher than the virus group. Conclusion Up-regulation of MRPL51 might be one of the pathogenesis of CVB3 myocarditis. The recipe for activating blood circulation and nourishing qi could treat viral myocarditis by regulating the expression of MRPL51.

Objective To investigate the effect of azithromycin,erythromycin and roxithromyein at varying concentration on the β-lactamase produced in the biofilm of Klebsiella pneumoniae.Methods Models of Klebsiella pneumoniae were built up with the modified flat-board method and identified with the confocal scanning laser microscopy.Thirty Klebsiella pneumoniae were incubated at 37 ℃for 24 h,and allocated into 7 groups:biofilm (group A),by 1 MIC azithromycin (group B1),by 1/8 MIC azithromycin (group B2),by 1 MIC erythromycin (group C1),by 1/8 MIC erythromycin (group C2),by 1 MIC roxithromycin (group D1),and biofilm induced by 1/8 MIC roxithromycin (group D2),with 200 μ1 each.The β-lactamase activity in each group was quantitated.Results The β-lactamase activity in group A,group B1,group B2,group C1,group C2,group D1 and group D2 was 0.6534,0.3196,0.4260,0.5028,0.5108,0.6524 and 0.6470 U/mg respectively.The β-lactamase activity in group B1,group B2,group C1 and group C2 was lower than that in group A,and statistically significant difference was observed (P＜0.05).The B-lactamase activity in group D1 and group D2 was similar to that in group A,and no statistically significant differences was observed between them (P＞0.05).The β-lactamase activity in group B1 was lower than that in group B2,and statistically significant difference was observed (P <0.05).No statistically significant difference was observed between group C1 and group C2 (P＞0.05).Conclusion Azithromycin at higher concentration exerts better effects on biofilm formation than other macrolides.

Objectives To explore the effects of the blood supply extent on the temperature of target site of tumor and volume of coagulation necrosis (V),and to provide experimental evidence for further precise control of dosage of high intensity focused ultrasound (HIFU) and improve the efficiency of HIFU in the treatment.Methods Twenty-four rabbit liver VX2 tumor models were established and were divided into four groups:10 d group,15 d group,20 d group,no blood supply group (rabbits were put to death in the 10 d after the models were established).The same irradiation parameters of HIFU were used to irradiate the hepatic tumor tissue of every group,the real-time temperature of target site of tumor were measured,the software from temperature controller was used to plot the time-temperature curve (TTC),V was measured after HIFU.Residual tumor tissue was resected for pathological observation and microvascular density (MVD) determination.Results (1) Histopathological analysis showed that the extent of a tumor's blood supply followed the order 10 d group ＞ 15 d group ＞ 20 d group (P ＜ 0.01).(2)Tmax,T1,k1:betweengroups had no statistical difference among 10 d group,15 d group,20 d group (P ＞0.05),T T1,and k1 between three groups with no blood supply group had statistical difference (P ＜ 0.05).(3) k2:10 d group ＞ 15d group ＞ 20d group ＞ no blood supply group;T2:10d group ＜ 15d group ＜ 20d group ＜ no blood supply group;(4)V:10 d group ＜ 15 d group ＜ 20 d group ＜ no blood supply group.Conclusions The extent of a tumor's blood supply had a significant effect on the temperature-decrease phase but not on the temperature-increase phase during HIFU treatment.The more abundant blood supply of the tumor was,the faster heat abstraction after HIFU was;and the easier the tissue return to normal temperature,the smaller the volume of coagulation necrosis tissue were.
